A autora aborda os diferentes modelos de desenvolvimento humano que ao longo da história têm explicado o desenvolvimento de forma sucessivamente mais aperfeiçoada, transportando conceitos inovadores de grande aplicabilidade em pediatria e repercussão nas estratégias educativas.

Os AA avaliaram dois grupos de crianças oriundas de diferentes estratos socio-familiares, procurando variantes do normal, que confirmem a importância do jardim de infância como factor atenuante de ambiente familiar menos estimulante. Material Métodos: Foram seleccionados aleatoriamente pelo Serviço Social do Hospital, dois jardins de infância, em Lisboa. A avaliação incidiu num grupo de crianças dos dois aos quatro anos, sem patologia. Na caracterização social e familiar foi utilizada a escala de Graffar. A avaliação de desenvolvimento psicomotor foi efectuada por observado único; o teste utilizado foi o "Schedule of Growing Skills in Practice" e a análise estatística foi efectuada pelo teste The Student (significância p=<0,05). Resultados: A população estudad foi constituída por 34 crianças, 14 das quais frequentava Jardim de Infância particular (JIP) e as restantes 20, Insitutição Particular de Solidariedade Social (IPSS), com idades compreendidas entre os 2 e os 4 anos. Na subescala da Locomoção, a pontuação obtida foi de 12,8 e 12,9 respectivamente no IPSS e JIP (=0,824) e na Manipulação foi de 19,3 (IPSS) e 20,7 (JIP) (p=0,006). Os resultados obtidos na área da Visão foram de 16,1 e 17,3 respectivamente na IPSS e JIP (p=0,005). A avaliação da Audição/Linguagem revelou resultados de 13,1 (IPSS) e 15,5 (JIP) (p=0,002) e na subescala da Fala/Linguagem, foram obtidos resultados de 14,5 (IPSS)e 17,3 (JIP) (p=0,008). As áreas de interacção social e autonomia, revelaram ambas pontuações de 18,3 (IPSS) e 19,8 (JIP), (respectivamente p=0,001 e p=0,017). Conclusões: Na avaliação efectuada, não encontrámos diferenças estatísticamente significativas nas subescalas de Locomoção e da Manipulação. Nas áreas da Autonomia, Audição/Linguagem e Fala/linguagem, os resultados foram estatísticamente superioes no grupo de crianças que frequentavam o JIP (oriundas de classes socio-familiares mais favorecidas e de famílias menos numerosas), comparativamente às que frequentavam a IPSS.

As perturbações do neurodesenvolvimento são das patologias crónicas mais frequentes da infância e com tendência a aumentar nas sociedades modernas. Têm na grande maioria dos casos um percurso crónico e com limitação da aprendizagem necessária para a integração na sociedade de um modo autónomo. A Sociedade de Pediatria do Neurodesenvolvimento da Sociedade Portuguesa de Pediatria procedeu em 2008 e 2009 ao levantamento de recursos, movimento e necessidades na área assistencial do neurodesenvolvimento no universo de 49 hospitais portugueses com Pediatria, referente a 31 de Dezembro de 2007. Responderam 42 (85.7%) hospitais. O número total de consultas de desenvolvimento representou 10.7% das de Pediatria, e foi- -lhe imputada uma mediana de tempo de 20 horas por semana. Dedicavam-se ao desenvolvimento 82 pediatras, mas mais de dois terços só o fazia a tempo parcial. Outros profissionais (fisiatras, psicólogos, terapeutas da fala, terapeutas ocupacionais, fisioterapeutas, docentes e técnicos de Serviço Social) faziam parte das equipas do desenvolvimento, mas em menor número que os pediatras, e de igual modo só raramente a tempo completo. Aguardava por consulta de desenvolvimento uma mediana de 185 crianças, e o tempo de espera variou entre um e 18 meses(mediana de seis). No seu conjunto os hospitais a curto prazo recrutariam 34 Pediatras para se dedicarem à área do neurodesenvolvimento,metade em regime de tempo completo. Dos outros profissionais requisitados [psicólogos (21), terapeutas da fala (20), docentes (20), terapeutas ocupacionais (14), fisioterapeutas (8) e técnicos do Serviço Social (6)], solicitavam-nos a tempo inteiro. Concluí-se que o movimento assistencial específico desta área no contexto global da Pediatria representa já um número significativo de consultas. Ainda assim, a resposta na área do neurodesenvolvimento revelou-se insuficiente e as equipas não funcionavam na generalidade em trabalho multidisciplinar. Contudo, os pedidos solicitados de recursos humanos médicos e não médicos e a preferência de que a dedicação ao neurodesenvolvimento fosse a tempo completo reflecte uma evolução positiva a curto prazo, caso estes recrutamentos se venham a concretizar.

Os autores fizeram um estudo retrospectivo de 16 casos de hydrops fetalis observados durante um período de 4 anos na Maternidade Dr. Alfredo da Costa. Em 5 dos recém-nascidos (RN) a hydrops era devida a isoimunização Rh. Nos 11 casos de natureza não imune, uma causa ou a presença de anomalias associadas foram detectadas em 4 doentes: taquicardia supraventricular (2 casos), doença renal multiquística (1 caso) e mielomeningocelo (1 caso). Os restantes casos eram de origem idiopática. Dois doentes faleceram in útero e quatro no período neonatal – mortalidade perinatal de 37%. A mortalidade foi mais elevada nos RN com idade gestacional mais baixa, no grupo com hydrops não imune, e nos RN com complicações orgânicas ou metabólicas graves. São revistos os problemas clínicos mais frequentes nesta situação, em particular o diagnóstico e actuação pré-natal, a asfixia perinatal e as complicações no período neonatal.

A determinação dos níveis séricos de transferrina deficiente em carbo-hidratos (CDT) e do rácio de transferrina no consumidor abusivo persistente de álcool aparece com promissora utilidade na área da alcoologia. Na série actual demonstra-se excelente especificidade (970 o) no entanto com fraca sensibilidade (52° o) para o CDT. Contudo o rácio CDT Tft adiciona-lhe uma sensibilidade acrescida atingindo 740 o mantendo a elevada especificidade. Nos consumidores abusivos 70g dia) persistentes, este índice que se correlaciona positivamente com a transferrina sérica, é capaz de escurtinar em elevada frequência estas capitações de álcool e mostra uma informação independente pois não se correlaciona significativamente com os níveis dos marcadores biológicos tradicionais (AST, ALT, GGT, VGM). Embora com limitações metodológicas definidas, estes índices prenunciam, com melhoria da acessibilidade técnica, uma aplicabilidade prática no rastreio de consumidores abusivos crónicos. Na área da hepatologia o comportamento do CDT e ácio da transferrina é capaz de afirmar em elevada frequência o envolvimento do etanol no estudo da natureza de uma doença hepática crónica. No entanto, o grau de lesão do figado tomado pelo índice hepático PGA, não influencia apreciavelmente os níveis séricos do CDT e rácio de transferrina. Na presente série, as circunstancias e condições do consumo de álcool parecem ser o determinante independente do carácter informativo que estes índices revelam.

Background: Stress and anxiety during pregnancy have been associated with premature and low birth weight babies, presumably through fetus over exposion to glucocorticoids. Antenatal stress also seems to have long-term effects upon infant development and adult health. However, medication for stress may carry risks to the expectant mother, therefore the efficacy of nonpharmacological interventions should be investigated. Methods: Pregnant women (n = 154) awaiting amniocentesis, were randomly assigned in the morning and the afternoon to three groups for 30 min: (1) listening to relaxing music, (2) sitting and reading magazines, and (3) sitting in the waiting-room. Before and after that period, they completed the Spielberger’s State and Trait anxiety inventory and provided blood samples for cortisol. The groups were then compared regarding change in cortisol levels and anxiety. Results: Maternal cortisol and state anxiety were correlated (r = 0.25, p = 0.04) in the afternoon, but not in the morning. The larger decreases in cortisol occurred in the music group ( 61.8 nmol/L, ANOVA: p = 0.01), followed by magazine, being differences among groups more pronounced in the morning. Women in the music group also exhibited the greater decreases in state anxiety ( p < 0.001). Younger mothers with less gestational age were on average the most anxious, and also the ones with greater decreases in cortisol and anxiety levels after relaxation. Conclusion: A relaxing intervention as short as 30 min, especially listening to music, decreases plasma cortisol and self-reported state anxiety score. Pregnant women might benefit from the routine practice of relaxation in the imminence of clinical stressful events.

A highly Al-resistant dissimilatory sulphatereducing bacteria community was isolated from sludge of the wetland of Urgeiriça mine (community W). This community showed excellent sulphate removal at the presence of Al3+. After 27 days of incubation, 73,86 and 81% of sulphate was removed in the presence of 0.48, 0.90 and 1.30 mM of Al3+, respectively. Moreover,Al3+ was simultaneously removed: 55,85 and 78% of metal was removed in the presence of 0.48, 0.90 and 1.30 mM of Al3+, respectively. The dissociation of aluminiumlactate soluble complexes due to lactate consumption by dissimilatory sulphate-reducing bacteria can be responsible for aluminum removal, which probably precipitates as insoluble aluminium hydroxide. Phylogenetic analysis of 16S rRNA gene showed that this community was mainly composed by bacteria closely related to Desulfovibrio desulfuricans. However, bacteria affiliated to Proteus and Ralstonia were also present in the community.

To contribute to the validation of the sensory and behavioral criteria for Regulation Disorders of Sensory Processing (RDSP) (DC:0-3R, 2005), this study examined a sample of toddlers in a clinical setting to analyze: (1) the severity of sensory modulation deficits and the behavioral symptoms of RDSP; (2) the associations between sensory and behavioral symptoms; and (3) the specific role of sensory modulation deficits in an RDSP diagnosis. Based on clinical observations, 78 toddlers were classified into two groups: toddlers with RDSP (N = 18) and those with‘‘other diagnoses in Axis I/II of the DC:0-3R’’ (OD3R; N = 60). The parents completed the Infant Toddler Sensory Profile and the Achenbach Checklist. The results revealed that the RDSP group had more severe sensory modulation deficits and specific behavioral symptoms; stronger, although not significant, associations between most sensory and behavioral symptoms; and a significant sensory modulation deficit effect. These findings support the validity of RDSP.

The term “mastocytosis” denotes a heterogeneous group of disorders characterised by abnormal growth and accumulation of mast cells (MC) in one or more organ systems. Symptoms result from MC chemical mediator’s release, pathologic infiltration of neoplastic MC in tissues or both. Multiple molecular, genetic and chromosomal defects seem to contribute to an autonomous growth, but somatic c-kit D816V mutation is more frequently encountered, especially in systemic disease. We present a literature review of mastocytosis and a rare case report of an 18 month-old-girl with a bullous dermatosis, respiratory distress and anaphylaxis, as clinical manifestations of mastocytosis. The developments of accepted classification systems and novel useful markers allowed a re-evaluation and updating of the classification of mastocytosis. In paediatric age cutaneous forms of disease prevail and may regress spontaneously. SM is more frequently diagnosed in adults and is a persistent(clonal) disease of bone marrow. The clinical course in these patients is variable.Today diagnostic criteria for each disease variant are reasonably well defined. There are, however, peculiarities, namely in paediatric age, that makes the diagnostic approach difficult. Systemic disease may pose differential diagnostic problems resulting from multiple organ systems involvement. Coversly, the “unexplained” appearance of those symptoms with no skin lesions should raise the suspicion of MC disease. This case is reported in order to stress the clinical severity and difficult diagnostic approach that paediatric mastocytosis may assume.

The objective was to validate Regulatory Sensory Processing Disorders’ criteria (DC:0-3R, 2005) using empirical data on the presence and severity of sensory modulation deficits and specific psychiatric symptoms in clinical samples. Sixty toddlers who attended a child mental health unit were diagnosed by a clinical team. The following two groups were created: toddlers with RSPD(N = 14) and those with ‘‘other diagnoses in Axis I/II of the DC:0-3R00(OD3R) (N = 46). Independently of the clinical process, parents completed the Infant Toddler Sensory Profile (as a checklist for sensory symptoms) and the Achenbach Behavior Checklist for ages 1/2–5 (CBCL 1/2–5). The scores from the two groups were compared. The results showed the following for the RSPD group: a higher number of affected sensory areas and patterns than in the OD3R group; a higher percentage of sensory deficits in specific sensory categories; and a higher severity of behavioral symptoms such as withdrawal, inattention, other externalizing problems and pervasive developmental problems in CBCL 1/2–5. The results confirmed our hypotheses by indicating a higher severity of sensory symptoms and identifying specific behavioral problems in children with RSPD. The results revealed convergent validity between the instruments and the diagnostic criteria for RSPD and supported the validity of RSPD as a unique diagnosis. The findings also suggested the importance of identifying sensory modulation deficits in order to develop an early intervention to enhance the sensory capacities of children who do not fully satisfy the criteria for some DSM-IV-TR disorders.

The aim of our study was to access office hysteroscopy results in postmenopausal patients with thickened endometrium. A retrospective descriptive study was carried out on 245 postmenopausal patients submitted to office hysteroscopy after sonographic diagnosis of thickened endometriumin 20 consecutive months.Women were evaluated for age, hormonal therapy, hysteroscopic findings, procedure duration, complications and associated pain, and histological diagnosis. Patients with and without uterine bleeding were considered separately. Symptomatic patients were older and had longer procedure duration. The most frequent hysteroscopic finding was endometrial polyp in both groups. Pain was subjectively assessed in a numeric scale from 0 to 10 and median value was 4. There were no complications reported. Global neoplasia rate was 2.9% for asymptomatic patients and 16.4% for symptomatic ones (p<0.05). Thickened endometrium with postmenopausal metrorrhagia gave patients a significantly higher risk for neoplasia and hyperplasia.

Despite its efficacy, including in the prevention of vertical transmission, the antiretroviral nevirapine is associated with severe idiosyncratic hepatotoxicity and skin rash. The mechanisms underlying nevirapine toxicity are not fully understood, but drug bioactivation to reactive metabolites capable of forming stable protein adducts is thought to be involved. This hypothesis is based on the paradigm that drug reactive metabolites have the potential to bind to self-proteins, which results in drug-modified proteins being perceived as foreign by the immune system. The aim of the present work was to identify hemoglobin adducts in HIV patients as biomarkers of nevirapine haptenation upon bioactivation. The ultimate goal is to develop diagnostic methods for predicting the onset of nevirapine-induced toxic reactions. All included subjects were adults on nevirapine-containing antiretroviral therapy for at least 1month. The protocol received prior approval from the Hospital Ethics Committees and patients gave their written informed consent. Nevirapine-derived adducts with the N-terminal valine of hemoglobin were analyzed by an established liquid chromatography-electrospray ionization-tandem mass spectrometry method and characterized on the basis of retention time and mass spectrometric fragmentation pattern by comparison with adduct standards prepared synthetically. The nevirapine adducts were detected in 12/13 patient samples, and quantified in 11/12 samples (2.58±0.8 fmol/g of hemoglobin). This work represents the first evidence of nevirapine-protein adduct formation in man and confirms the ability of nevirapine to modify self-proteins, thus providing clues to the molecular mechanisms underlying nevirapine toxicity. Moreover, the possibility of assessing nevirapine-protein adduct levels has the potential to become useful for predicting the onset of nevirapine-induced adverse reactions.
