BACKGROUND: The workplace is affected by poor lifestyle habits that can cause alterations in the quality of life (QOL) of the employees. OBJECTIVE: To evaluate the quality of life and the cardiovascular risk factors in employees of a business. Relate the quality of life with the variables of cardiovascular risk, gender and work sector. METHOD: 41 employees were evaluated (30 females and 11 males) aging between 18 and 54 years (mean 27.4 ± 8.9 years). A protocol was used with: personal data, personal background, physical examination (anthropometry) and the WHOQOL-bref questionnaire to evaluate the QOL. Statistical analysis was completed by the Chi-squared test and Sperman’s rank correlation, with a significance of 5%. RESULTS: We found 31(75.6%) sedentary; 16(39.0%) excess weight; 2(4.9%) smokers; 9(22.0%) alcohol consumers and 25(61.0%) family history. We also found 13(31.7%) overweight/low risk and 3(7.3%) obese/moderate risk. The best score on the WHOQOL-bref was on physical health (72.9), and the worst on environment (61.3) and mean score of 69.5 for total QOL. Male employees presented greater scores on all domains, but this association had a non-significant result. There was positive correlation between the physical and psychological domains with the environment and total QOL. CONCLUSION: The employees presented important cardiovascular risk factors like sedentary lifestyle, family history, excess weight and alcohol consumption. The employee’s QOL is considered satisfactory in the perception of health; the best QOL being in the physical domain and the worst in the environment. Change measures are suggested to the company for the cardiovascular risk factors detected.

BackgroundThe success of epidural anaesthesia depends on correct identification of the epidural space. For several decades, the decision of whether to use air or physiological saline during the loss of resistance technique for identification of the epidural space has been governed by the personal experience of the anaesthesiologist. Epidural block remains one of the main regional anaesthesia techniques. It is used for surgical anaesthesia, obstetrical analgesia, postoperative analgesia and treatment of chronic pain and as a complement to general anaesthesia. The sensation felt by the anaesthesiologist from the syringe plunger with loss of resistance is different when air is compared with saline (fluid). Frequently fluid allows a rapid change from resistance to non-resistance and increased movement of the plunger. However, the ideal technique for identification of the epidural space remains unclear.ObjectivesTo evaluate the efficacy and safety of both air and saline in the loss of resistance technique for identification of the epidural space.To evaluate complications related to the air or saline injected.Search methodsWe searched the Cochrane Central Register of Controlled Trials (CENTRAL) (2013, Issue 9), MEDLINE, EMBASE and the Latin American and Caribbean Health Science Information Database (LILACS) (from inception to September 2013). We applied no language restrictions. The date of the most recent search was 7 September 2013.Selection criteriaWe included randomized controlled trials (RCTs) and quasi-randomized controlled trials (quasi-RCTs) on air and saline in the loss of resistance technique for identification of the epidural space.Data collection and analysisTwo review authors independently assessed trial quality and extracted data.Main resultsWe included in the review seven studies with a total of 852 participants. The methodological quality of the included studies was generally ranked as showing low risk of bias inmost domains, with the exception of one study, which did not mask participants. We were able to include data from 838 participants in the meta-analysis. We found no statistically significant differences between participants receiving air and those given saline in any of the outcomes evaluated: inability to locate the epidural space (three trials, 619 participants) (risk ratio (RR) 0.88, 95% confidence interval (CI) 0.33 to 2.31, low-quality evidence); accidental intravascular catheter placement (two trials, 223 participants) (RR 0.90, 95% CI 0.33 to 2.45, low-quality evidence); accidental subarachnoid catheter placement (four trials, 682 participants) (RR 2.95, 95% CI 0.12 to 71.90, low-quality evidence); combined spinal epidural failure (two trials, 400 participants) (RR 0.98, 95% CI 0.44 to 2.18, low-quality evidence); unblocked segments (five studies, 423 participants) (RR 1.66, 95% CI 0.72 to 3.85); and pain measured by VAS (two studies, 395 participants) (mean difference (MD) -0.09, 95% CI -0.37 to 0.18). With regard to adverse effects, we found no statistically significant differences between participants receiving air and those given saline in the occurrence of paraesthesias (three trials, 572 participants) (RR 0.89, 95% CI 0.69 to 1.15); difficulty in advancing the catheter (two trials, 227 participants) (RR 0.91, 95% CI 0.32 to 2.56); catheter replacement (two trials, 501 participants) (RR 0.69, 95% CI 0.26 to 1.83); and postdural puncture headache (one trial, 110 participants) (RR 0.83, 95% CI 0.12 to 5.71).Authors' conclusionsLow-quality evidence shows that results do not differ between air and saline in terms of the loss of resistance technique for identification of the epidural space and reduction of complications. Applicability might be compromised, as most of the results described in this review were obtained from parturient patients. This review underlines the need to conduct well-designed trials in this field.

Background: Immunity plays an important role in controlling human papillomavirus (HPV) infection and associated lesions. Unlike infections caused by other viruses, natural HPV infection does not always result in a protective antibody response. Therefore, HPV antibodies are also considered markers of cumulative exposure. The aim of this study was to identify determinants of HPV16 seroreactivity at enrollment among women from the Ludwig-McGill cohort, a natural history study of HPV infection and risk of cervical neoplasia.Methods: HPV16 serology was assessed by ELISA for L1 and L2 capsid antigens, while HPV typing and viral load measurements were performed by PCR-based methods. The associations were analyzed by unconditional logistic regression.Results: Of 2049 subjects, 425 (20.7%) were strongly seropositive for HPV16. In multivariate analysis, seroreactivity was positively correlated with age, lifetime number of sexual partners, frequency of sex, and HPV16 viral load, and negatively associated with duration of smoking.Conclusions: HPV16 seroreactivity is determined by factors that reflect viral exposure.

In the last few decades, Brazil has experienced an accelerated urbanization process in which many cities have grown in a disorderly way occupying environmentally fragile areas unsuitable for habitation. Anthropogenic actions such as high levels of impermeable soil, structural changes in watercourses, lack of riparian vegetation, illegal presence of trash and rubbish along the river banks added to irregular settlements in floodplains result in the rise of high risk areas. When accompanied by intense and prolonged rainfall phenomena, those areas have been the scenery of serious accidents such as floods. This study aims to classify the level of the risk of floods in the neighborhood of Jardim Inocoop, in the town of Rio Claro, São Paulo countryside, Brazil. One of the main technical support to tackle this issue is the identification and classification of the risks. In order to classify the risk level of flood in this case study, the methodology adopted was developed by the Ministry of Cities and Technology Research Institute, and take into account the arrangement of the hydrological scenario, vulnerability of households and dangerous process according to the distance of the houses from the axis of drainage. Therefore, the risk levels adopted to classify are listed below: very high (MA), high risk (A), moderate (M) and low risk (B). In conclusion, it is imperative to develop prevention plans in order to avoid or to minimize the damages caused by natural disasters. Therefore, the zoning of the risk sceneries remains as an important issue once it helps to identify the areas with high level risk of flood. Consequently, the occupation must be regulated where there is low or absent risk and it must be often forbidden where the high risk of flood is detected. Objective: To evaluate if the total bacterial count of vaginal samples with bacterial vaginosis assessed by flow cytometry influences the response to treatment with metronidazol. Methods: In this cross-sectional study, 273 low-risk reproductive aged women were enrolled. Vaginal samples were taken to evaluate the pattern of vaginal flora according to Nugent’s criteria, as well as the presence of trichomoniasis and candidosis. Cases identified of bacterial vaginosis were treated with metronidazole and controlled after 45 days. Cervical infection by Chlamydia trachomatis and Neisseria gonorrhoeae were also assessed. Flow cytometry for total bacterial counting was performed in propidium iodide stained cervicovaginal samples, using fluorescent beads at a known concentration. Non-parametric Mann-Whitney test was used to compare total bacterial count between groups of interest, at p<0.05. Results: From the total of 273 women enrolled, 50 were excluded as they presented at least one of the infections investigated. Bacterial vaginosis was detected in 79 women (35.4%), of which 33 (41.8%) returned for re-evaluation after treatment, being 21 cases successfully treated and 12 with persistent abnormal vaginal flora. Flow cytometric data showed that total bacterial counting does not differ between normal flora and bacterial vaginosis samples (p=0.14). Also, no difference was found between the cases of treated and persistent bacterial vaginosis (p=0.48). Conclusion: Total bacterial counting does not influence the response to metronidazole treatment of bacterial vaginosis

The paper approaches the U.S. perceptions about the sources of insecurity from Latin America, considered a low risk area and priority of its foreign policy. El análisis se concentra en la evaluación de las amenazas y desafíos a sus intereses estratégicos y las modalidades de actuación privilegiadas en la esfera regional, en que se verifican coincidencias entre las administraciones Demócratas y Republicanas en la adopción de un liderazgo con prerrogativas clasificatorias sobre los tipos de régimen político, con una agenda concebida como promoción de la convergencia hemisférica de democracias y economías liberales. The analysis focuses on assessing the threats and challenges to its strategic interests and privileged modes of action at the regional level, which verifies similarities between Republican and Democratic administrations in taking a leadership prerogatives qualifiers on the types political regime, with an agenda designed as promoting hemispheric convergence of liberal democracies and economies. En ese contexto, cabe destacar la prioridad atribuida a los países de la Alternativa Bolivariana para las Américas (ALBA) como eje opositor de sus políticas. In this context, include the priority given to the countries of the Bolivarian Alternative for the Americas (ALBA) as the linchpin of his political opponent.

Hidden caries is a type of lesion visualized in dentin, mainly by means of bitewing radiographs, in which, clinically, the occlusal enamel appears healthy or minimally demineralized and radiographically presents progressive demineralization in dentin, which may progress and compromise the pulp-dentin complex. Although the etiology of hidden caries is unknown, many theories have been studied, including structural and anatomical defects of enamel, their specific microbiota and the use of fluoride, which is most accepted theory. Considering the clinical concern, since these lesions have a silent progression and often are not detected on routine clinical examinations, this study aims to conduct a critical discussion of the etiology, prevalence, diagnostic methods and treatment decision for lesions of hidden caries, alerting professionals about the importance of routine use of bitewing radiographs even in patients considered “cariesfree” or low risk of caries, for more effective diagnosis.

Introduction The most commonly used method in neonatal hearing screening programs is transient evoked otoacoustic emissions in the first stage of the process. There are few studies comparing transient evoked otoacoustic emissions with distortion product, but some authors have investigated the issue. Objective To correlate the results of transient evoked and distortion product otoacoustic emissions in a Brazilian maternity hospital. Methods This is a cross-sectional, comparative, and prospective study. The study included 579 newborns, ranging from 6 to 54 days of age, born in a low-risk maternity hospital and assessed for hearing loss. All neonates underwent hearing screening by transient evoked and distortion product otoacoustic emissions. The results were analyzed using the Spearman correlation test to relate the two procedures. Results The pass index on transient evoked otoacoustic emissions was 95% and on distortion product otoacoustic emissions was 91%. The comparison of the two procedures showed that 91% of neonates passed on both procedures, 4.5% passed only on transient evoked otoacoustic emissions, 0.5% passed only on distortion product otoacoustic emissions, and 4% failed on both procedures. The inferential analysis showed a significant strong positive relationship between the two procedures. Conclusion The failure rate was higher in distortion product otoacoustic emissions when compared with transient evoked; however, there was correlation between the results of the procedures.

The aim of this study was to study the seroepidemiological profile of brucellosis and leptospirosis in horses traction Island Maiandeua, state of Para. In two distinct periods, blood samples were collected from 52 animals of both sexes and different ages (2 to 17 years), totaling 104 samples. For the research of antibodies anti-Brucella abortus were used in rapid agglutination test in plate. In the first harvest, none animal was positive, however in the second harvest there were three animals reactive serum. The research of antibodies anti-Leptospira spp. was performed with the use of the microscopic agglutination test (MAT), the first harvest was 23.07% reacting animals and 15.38% at the second harvest, for one or more Leptospira spp. with titers ranging from 100 to 200. The predominant serotype at first and second harvest was the Autumnalis 40% and 37.5% respectively. According to age, it was observed in group 1 (2 to 7 years) 27.78% and 13.89% respectively in the two samples and the second group (> 7 years) was found 12.50% and 18 75% of reactive serum. The results observed in this study demonstrated that the island of Maiandeua, state of Para, there is the presence of Leptospira spp, with the most frequent serovar autumnalis and possible exposure of animals to brucella smooth, suggesting low risk of infection in the population of horses examined.

The aims of this research were to evaluate the acute toxicity (LC/EC50) and the environmental risk of toltrazuril (TOL) and florfenicol (FFC) for plant Lemna minor, snail Pomacea canalicuta, fish Piaractus mesopotamicus and Hyphessobrycon eques and the microcrustacean Daphnia magna. The organisms were acclimated in room bioassay with controlled temperature according to standard to each one. They were exposed at nominal concentrations in static system. For environmental risk (RQ) was used the estimated environmental concentration (EEC) that is the dosage indicate to treatment and the lethal or effective concentration (LC/CE50) from each drug in acute exposure. FFC showed LC50;7d of 97.03 mg L-1 for L. minor; >100.0 mg L-1 for P. mesopotamicus and H. eques and EC50;48h > 100.0 mg L-1 for P. canaliculata and D. magna, and it was classified low risk (RQ = 0.01) for all bioindicators. TOL howed LC50;7d >100.0 mg L-1 for L. minor, 3.72 mg L-1 for P. mesopotamicus; 6.22 mg L-1 for H. eques and CE50;48h of 7.59 mg L-1 for P. canaliculata and 18.57 mg L-1 for D. magna, and it was classified low risk (RQ = 0.01) for L. minor and high risk for P. mesopotamicus (RQ = 2.68), H. eques (RQ = 6.22), P. canaliculata (RQ = 1.31) and D. magna (RQ = 0.53). Lemna minor was the bioindicator indicating of FFC toxicity and H. eques, bioindicator of the TOL. FFC is safety however and the use of TOL necessaries cautions to treat the wastewater before discard on the environment.
