In this paper we propose a parsimonious regime-switching approach to model the correlations between assets, the threshold conditional correlation (TCC) model. This method allows the dynamics of the correlations to change from one state (or regime) to another as a function of observable transition variables. Our model is similar in spirit to Silvennoinen and Teräsvirta (2009) and Pelletier (2006) but with the appealing feature that it does not suffer from the course of dimensionality. In particular, estimation of the parameters of the TCC involves a simple grid search procedure. In addition, it is easy to guarantee a positive definite correlation matrix because the TCC estimator is given by the sample correlation matrix, which is positive definite by construction. The methodology is illustrated by evaluating the behaviour of international equities, govenrment bonds and major exchange rates, first separately and then jointly. We also test and allow for different parts in the correlation matrix to be governed by different transition variables. For this, we estimate a multi-threshold TCC specification. Further, we evaluate the economic performance of the TCC model against a constant conditional correlation (CCC) estimator using a Diebold-Mariano type test. We conclude that threshold correlation modelling gives rise to a significant reduction in portfolio´s variance.

BACKGROUND: Patients requiring surgical skin excision after massive weight loss are challenging and require an individualized approach. The characteristic abdominal deformity includes a draping apron of panniculus, occasionally associated with previous transverse surgical scars from open gastric bypass surgery in the upper abdomen, which compromise blood supply of the abdominal skin. METHODS: We propose four different surgical techniques for safe abdominal body contouring in the presence of such scars: (1) a limited abdominoplasty of the lower abdomen is performed, and then contouring is completed by a reversed abdominoplasty with scar positioning in the submammary folds; (2) a one-stage procedure characterized by skin resection in the upper and lower abdomen, in which blood supply of the skin island between the submammary and suprapubic incisions is ensured by periumbilical perforators; (3) a perforator-sparing abdominoplasty with selective dissection of periumbilical abdominal wall perforators to secure flap blood supply and allow complete flap undermining up to the xyphoid process; (4) for patients with extensive excess skin, a modified Fleur-de-Lys abdominoplasty performed in such a way that the old transverse scar is transformed into a vertical scar. RESULTS: The treatment of four exemplary patients is described. All techniques yielded good esthetic and functional results through preservation of abdominal blood supply. CONCLUSION: Through an individualized approach, adequate abdominal body contouring can be performed safely, even in the presence of transverse surgical scars in the upper abdomen.

INTRODUCTION: Fabry disease is an X-linked recessive abnormality of glycosphingolipid metabolism that is due to deficiency of the lysosomal enzyme alpha-galactosidase A. CURRENT KNOWLEDGE AND KEY POINTS: A majority of hemizygous men develop severe multisystemic disease (classic form), dominated by renal failure, progressive neurological and cardiac involvement. Nevertheless, some affected men retain sufficient enzyme activity and long remain asymptomatic (atypical form); their main manifestation is hypertrophic cardiomyopathy. Female heterozygous carriers are usually asymptomatic; 15% of them, however, have severe involvement of one or several organs. Laboratory, histologic and molecular diagnosis identifies 100% of hemizygous and over 80% of heterozygous subjects. FUTURE PROSPECTS AND PROJECTS: With developments in molecular genetics, it is now possible to produce the human recombinant enzyme alpha-galactosidase A. Two recent studies had proven that this therapeutic approach was able to be clinically and histologically effective in men. In addition, the results of a trial of gene therapy in a Fabry gene knocked-out mouse appear promising.

La relación educativa entraña, inevitablemente, el debate entorno de la verticalidad y del ejercicio del poder. De qué modo nos relacionamos y cómo gestionamos la comunicación entre educadores y educandos, así como entre iguales. Esa tensión se ve amplificada en la medida que incorporamos lógicas evaluativas en nuestro quehacer profesional. La siguiente comunicación describe la experiencia de la metodología de aprendizaje basada en la evaluación entre iguales aplicada a la profesionalización de los y las educadores/as sociales. Tanto a nivel escolar como en el universitario hay numerosas experiencias publicadas en relación a la metodología de evaluación entre iguales. Además, existen numerosos apartados de los “Documentos profesionalizadores” donde se justifica el hecho de que se entrenen algunas habilidades que deberían darse en la práctica profesional (como por ejemplo en la definición, en el código ético, y en el catálogo de funciones y competencias del/la educador/a social). En esta comunicación pretendemos dar a conocer la experiencia de entrenamiento de futuros educadores en aspectos que tienen una relación directa con la evaluación entre educadores/as sociales. Así pues, utilizaremos una justificación académica y metodológica de la actividad, enlazándola con los documentos profesionalizadores con un fin relacionado directamente con el ejercicio de la profesión.

The Condemned es un juego de lucha en dos dimensiones desarrollado en Flash CS4 y ActionScript 3. El juego consta de cuatro pantallas, en cada una de ellas el jugador se enfrenta a un enemigo controlado por el ordenador a través de una inteligencia artificial. En la creación de este videojuego se ha pasado por todas las fases de desarrollo: diseño gráfico de personajes y escenarios, programación y control de errores.

We report a 14-year-old boy who presented with vision loss secondary to peripapillary neovascular membrane (PPNVM) as the initial and only symptom of papilledema secondary to idiopathic intracranial hypertension. After one lumbar puncture, visual acuity progressively recovered during the course of 1 week and further improved with the administration of oral acetazolamide. One year after the onset of vision loss, the patient's visual acuity had recovered to baseline measurements. The previously active PPNVM had involuted into a residual peripapillary fibrotic scar. To our knowledge, this is the first report of PPNVM complicating idiopathic intracranial hypertension in a child.

Hereditary non-structural diseases such as catecholaminergic polymorphic ventricular tachycardia (CPVT), long QT, and the Brugada syndrome as well as structural disease such as hypertrophic cardiomyopathy (HCM) and arrhythmogenic right ventricular cardiomyopathy (ARVC) cause a significant percentage of sudden cardiac deaths in the young. In these cases, genetic testing can be useful and does not require proxy consent if it is carried out at the request of judicial authorities as part of a forensic death investigation. Mutations in several genes are implicated in arrhythmic syndromes, including SCN5A, KCNQ1, KCNH2, RyR2, and genes causing HCM. If the victim's test is positive, this information is important for relatives who might be themselves at risk of carrying the disease-causing mutation. There is no consensus about how professionals should proceed in this context. This article discusses the ethical and legal arguments in favour of and against three options: genetic testing of the deceased victim only; counselling of relatives before testing the victim; counselling restricted to relatives of victims who tested positive for mutations of serious and preventable diseases. Legal cases are mentioned that pertain to the duty of geneticists and other physicians to warn relatives. Although the claim for a legal duty is tenuous, recent publications and guidelines suggest that geneticists and others involved in the multidisciplinary approach of sudden death (SD) cases may, nevertheless, have an ethical duty to inform relatives of SD victims. Several practical problems remain pertaining to the costs of testing, the counselling and to the need to obtain permission of judicial authorities.

In the present review, microvascular remodelling refers to alterations in the structure of resistance vessels contributing to elevated systemic vascular resistance in hypertension. We start with some historical aspects, underscoring the importance of Folkow's contribution made half a century ago. We then move to some basic concepts on the biomechanics of blood vessels, and explicit the definitions proposed by Mulvany for specific forms of remodelling, especially inward eutrophic and inward hypertrophic. The available evidence for the existence of remodelled resistance vessels in hypertension comes next, with relatively more weight given to human, in comparison with animal data. Mechanisms are discussed. The impact of antihypertensive drug treatment on remodelling is described, again with emphasis on human data. Some details are given on the three studies to date which point to remodelling of subcutaneous resistance arteries as an independent predictor of cardiovascular risk in hypertensive patients. We terminate by considering the potential role of remodelling in the pathogenesis of end-organ damage and in the perpetuation of hypertension.

Con el título “El trabajo en equipo, la planificación y concertación de servicios en la práctica profesional” se presenta la experiencia formativa que se desarrolla en el segundo curso de la Diplomatura de Educación Social con la intención de que los estudiantes investiguen y reflexionen sobre la práctica de la planificación socioeducativa en un contexto de concurso público para la concertación de un servicio.

La propuesta de innovación metodológica se sitúa en el marco del Grado de Educación Social (asignatura de Diversidad Funcional). Esta carrera universitaria pretende formar profesionales que generen cambio social especialmente entre los colectivos más vulnerables de nuestra sociedad. La propuesta, on li-ne mayoritariamente, consiste en utilizar Twitter y otras plataformas de almacenamiento gráfico para poner en común situaciones de vulnerabilidad de personas con discapacidad que se dan en las diferentes ciudades de los estudiantes. Además de difundirlas mediante Twitter, en caso necesario, se utiliza la plataforma ziudad.com para ejercer denuncia de la situación que se esté produciendo. Mediante este sistema de geolocalización las denuncias se situaran públicamente (en abierto a otros ciudadanos y con su participación) en los diferentes puntos del territorio. A partir de lo que se ha ido estudiando los estudiantes, si lo creen necesario, además realizan quejas o sugerencias fuera del aula mediante cartas a diferentes medios de comunicación, a instituciones públicas o privadas y otras acciones donde el contenido de la asignatura trasciende fuera del aula. En tres meses se han conseguido poner en común más de 500 fotografías, se han publicado 30 cartas en diferentes medios de comunicación y hay presentadas denuncias/sugerencias a 10 municipios.
