Following acute myocardial infarction, necrotic cardiac tissue is replaced by scar leading to ventricular remodeling and pump failure. Transplantation of autologous bone marrow-derived cells into the heart, early post-infarct, aims to prevent ventricular remodeling. This strategy has been evaluated in four controlled, randomized clinical trials, which provided mixed results. A transient improvement in ventricular function was observed in one trial, and a modest improvement (the duration of which remains to be determined) in an additional trial, whereas two trials showed negative results. A modest benefit of bone marrow cell transplantation was also observed in patients with chronic ischemic heart disease. Despite mixed results reported so far, cell therapy of heart disease still is in its infancy and has considerable room for improvement.

This article reviews: 1) some of the results of drug eluting stents (SYNTAX and FAME); 2) the questionnable benefit of physical training in heart failure patients (HF-ACTION); 3) the benefit on cardiac remodelling of cardiac resynchronisation in heart failure patients (REVERSE study) and 4) the role of rate over rhythm control in patients with atrial fibrillation and heart failure (AF-CHF study); this article also reports the encouraging evolution of new technology allowing percutaneous implantation of stents-valves. Finally, this article address the screening of athletes for cardiac diseases.

Heart failure and atrial fibrillation are the main problems in general cardiology. Our therapeutic reflections summarize new ideas in the treatment of theses pathologies but we will not forget importance of PCI. All these therapies have proven now the clinical benefit but also reduction in more morbidity and mortality. Cardiac resynchronisation therapy has shown promising results. The art of medicine to develop will be to better identify the patients benefiting from this therapy. Interventional cardiology is focusing on the acute coronary syndrome. Not only rapid intervention but also the stent technology allow significant modification of endothelial tissue reaction and therefore improve the general benefit for the patient.

O presente trabalho teve como objetivo analisar alguns subprodutos agroindustriais utilizados na alimentação animal e identificar os principais minerais presentes. Amostras de farelos de algodão, arroz, canola, soja e trigo; farinhas de peixe, carne e penas + vísceras; cascas de algodão, arroz, laranja; bagaços de tomate e de laranja foram coletadas em diferentes locais de produção. O método analítico empregado foi a análise por ativação com nêutrons seguida de espectrometria gama. Os níveis de minerais encontrados em todas as amostras, inclusive aqueles considerados tóxicos, tais como As, Cd e Hg, não excederam os limites máximos permitidos em dietas para animais domésticos. Os valores obtidos foram comparados com os comumente encontrados em forragens.

O experimento foi conduzido em área da Universidade Federal de Santa Maria, RS, com o objetivo de verificar os efeitos de três níveis de adubação nitrogenada (0, 150 e 300 kg/ha de N) em pastagem de milheto (Pennisetum americanum (L.) Leeke) cv. Comum, sobre a produção animal. Foram utilizados novilhos de corte e avaliados o desempenho por animal, o número de animais.dia/ha e o ganho de peso vivo por área. O sistema de pastejo adotado foi o contínuo, com ajustes de carga para manter uma pressão de pastejo de 10%, caracterizando um resíduo médio de 3.168 kg de matéria seca/ha. As variáveis dependentes mostraram relação linear positiva com os níveis de adubação nitrogenada, o que denota o alto potencial do milheto.

An increasing number of patients suffering from cardiovascular disease, especially coronary artery disease (CAD), are treated with aspirin and/or clopidogrel for the prevention of major adverse events. Unfortunately, there are no specific, widely accepted recommendations for the perioperative management of patients receiving antiplatelet therapy. Therefore, members of the Perioperative Haemostasis Group of the Society on Thrombosis and Haemostasis Research (GTH), the Perioperative Coagulation Group of the Austrian Society for Anesthesiology, Reanimation and Intensive Care (ÖGARI) and the Working Group Thrombosis of the European Society of Cardiology (ESC) have created this consensus position paper to provide clear recommendations on the perioperative use of anti-platelet agents (specifically with semi-urgent and urgent surgery), strongly supporting a multidisciplinary approach to optimize the treatment of individual patients with coronary artery disease who need major cardiac and non-cardiac surgery. With planned surgery, drug eluting stents (DES) should not be used unless surgery can be delayed for ≥12 months after DES implantation. If surgery cannot be delayed, surgical revascularisation, bare-metal stents or pure balloon angioplasty should be considered. During ongoing antiplatelet therapy, elective surgery should be delayed for the recommended duration of treatment. In patients with semi-urgent surgery, the decision to prematurely stop one or both antiplatelet agents (at least 5 days pre-operatively) has to be taken after multidisciplinary consultation, evaluating the individual thrombotic and bleeding risk. Urgently needed surgery has to take place under full antiplatelet therapy despite the increased bleeding risk. A multidisciplinary approach for optimal antithrombotic and haemostatic patient management is thus mandatory.

Interventional paediatric and congenital cardiology is expanding at a rapid pace. Validated techniques (such as aortic or pulmonary valve dilatations and occlusion of persistent ductus arteriosus and atrial septal defects) are improving thanks to the use of smaller introducers and sheaths, low-profile balloons and novel devices. Moreover, catheter-based interventions have emerged as an attractive alternative to surgery in other fields: pulmonary valve replacement, balloon and stent implantation for native and recurrent coarctation, and percutaneous closure of ventricular septal defects. On the other hand, percutaneous interventions in the paediatric population may be limited by patient size or the anatomy of the defect. Hybrid approaches involving both cardiac interventionists and surgeons are being developed to overcome these limitations. Based on a better understanding of cardiac development, fetal cardiac interventions are being attempted in order to alter the history of severe obstructive lesions. Finally, some interventional procedures still carry a low success rate-for example, pulmonary vein stenosis, even with the use of conventional stents. Biodegradable stents and devices are being developed and may find an application in this setting as well as in others. The purpose of this review is to highlight the advances in paediatric interventional cardiology since the beginning of the third millennium.

With the aim of improving human health, scientists have been using an approach referred to as translational research, in which they aim to convey their laboratory discoveries into clinical applications to help prevent and cure disease. Such discoveries often arise from cellular, molecular, and physiological studies that progress to the clinical level. Most of the translational work is done using animal models that share common genes, molecular pathways, or phenotypes with humans. In this article, we discuss how translational work is carried out in various animal models and illustrate its relevance for human sleep research and sleep-related disorders.
