A doença de Charcot-Marie-Tooth (CMT) é a mais freqüente das neuropatias hereditárias, mas a sua história natural é pouco conhecida. Apesar da maioria dos pacientes apresentarem uma evolução com características mais “benignas”, é associado à doença com um comprometimento funcional, o que pode ser acompanhado de limitações e incapacitações. O presente estudo teve como objetivo avaliar a qualidade de vida de pacientes afetados pela CMT1A. Considerando que a qualidade de vida pode sofrer influência de variáveis sócio-culturais, um estudo neste contexto mostra-se oportuno. Nenhum estudo de nosso conhecimento realizou tal avaliação no contexto brasileiro. Fizeram parte deste estudo pacientes com CMT decorrente da duplicação 17p11.2-p12 (CMT1A) que foram avaliados quanto ao comprometimento funcional por meio da aplicação do SF-36. Esse questionário é utilizado internacionalmente para aferir a qualidade de vida dos indivíduos. Os resultados obtidos foram submetidos à análise estatística. Como resultados não se observaram associações entre o gênero e os domínios avaliados para a qualidade de vida. Os dados obtidos revelaram que a qualidade de vida dos pacientes com CMT1A se mostrou comprometida principalmente quanto aos seus Aspectos Sociais (p = 0,02) e Aspectos emocionais (p = 0,04). Adicionalmente, houve uma tendência ao comprometimento da Capacidade Funcional (p = 0,08), sendo que os outros índices de avaliação de comprometimento físico se mostraram preservados. A presença ou ausência de ocupação parece influenciar os Aspectos Sociais apresentando uma tendência de diferença significativa (p = 0,07), possuindo o grupo com ocupação valores superiores. Apesar da CMT1A apresentar piora clínica ao longo dos anos, a idade mais avançada não implicou em uma pior percepção da qualidade de vida no grupo de pacientes estudados no presente trabalho... The involvement of glutamatergic neurotransmission in the rostral ventrolateral medulla/Bötzinger/pre-Bötzinger complexes (RVLM/BötC/pre-BötC) on the respiratory modulation of sympathoexcitatory response to peripheral chemoreflex activation (chemoreflex) was evaluated in the working heart-brain stem preparation of juvenile rats. We identified different types of baro- and chemosensitive presympathetic and respiratory neurons intermingled within the RVLM/BötC/pre-BötC. Bilateral microinjections of kynurenic acid (KYN) into the rostral aspect of RVLM (RVLM/BötC) produced an additional increase in frequency of the phrenic nerve (PN: 0.38 ± 0.02 vs. 1 ± 0.08 Hz; P < 0.05; n = 18) and hypoglossal (HN) inspiratory response (41 ± 2 vs. 82 ± 2%; P < 0.05; n = 8), but decreased postinspiratory (35 ± 3 vs. 12 ± 2%; P < 0.05) and late-expiratory (24 ± 4 vs. 2 ±1%; P < 0.05; n = 5) abdominal (AbN) responses to chemoreflex. Likewise, expiratory vagal (cVN; 67 ± 6 vs. 40 ± 2%; P < 0.05; n = 5) and expiratory component of sympathoexcitatory (77 ± 8 vs. 26 ± 5%; P < 0.05; n = 18) responses to chemoreflex were reduced after KYN microinjections into RVLM/BötC. KYN microinjected into the caudal aspect of the RVLM (RVLM/pre-BötC; n = 16) abolished inspiratory responses [PN (n = 16) and HN (n = 6)], and no changes in magnitude of sympathoexcitatory (n = 16) and expiratory (AbN and cVN; n = 10) responses to chemoreflex, producing similar and phase-locked vagal, abdominal, and sympathetic responses. We conclude that in relation to chemoreflex activation 1) ionotropic glutamate receptors in RVLM/BötC and RVLM/pre-BötC are pivotal to expiratory and inspiratory responses, respectively; and 2) activation of ionotropic glutamate receptors in RVLM/BötC is essential to the coupling of active expiration and sympathoexcitatory response.

The aim of this study was to evaluate the influence of specimen size, in comparison with the ISO Standard, on the three point flexural strength of resin composite restorative materials Filtek Supreme and Filtek Z-250. Forty specimens were fabricated for each material with the following length, width and thickness measurements (n = 10): 1) 20 × 2 × 2 mm (ISO 4049); 2) 10 × 2 × 1 mm; 3) 10 × 1 × 1 mm; 4) 8 × 0.8 × 0.8 mm. The composites were inserted in a single increment into two-piece metal device and light-polymerized. The specimens were dry stored at 37 ± 1 °C and protected from light for 7 days. After this period, flexural strength was measured by three-point flexure test using MTS 810 equipment, with a load cell of 10 kN at a speed of 0.5 mm/min. For the evaluated sizes, the results showed significant variability (p = 0.00) with values when compared with the ISO Standard (116.700 MPa), being statistically higher for the test specimens measuring 10 × 1 × 1 mm (142.530 MPa), similar for those of 10  ×  2  ×  1  mm (115.815 MPa) and lower for those of 8 × 0.8 × 0.8 mm (86.650 MPa). There was statistical equality (p = 0.08) for the studied composites (Filtek Supreme, 125.270 MPa; Filtek Z-250, 108.130 MPa). Specimens measuring 10 × 2 × 1 mm provided flexural strength values equivalent to those obtained in the sizes recommended by the ISO 4049 standard, with lower consumption of material, energy and time.

Objective. To assess the value of vaginal screening cytology after hysterectomy for benign disease.Methods. This cross-sectional study used cytology audit data from 2,512,039 screening tests in the metropolitan region of Campinas from 2000 to 2012; the object was to compare the prevalence of abnormal tests in women who had undergone a hysterectomy for benign diseases (n = 53,891) to that of women who had had no hysterectomy. Prevalence ratios (95% confidence intervals, 95% Cl) were determined, and chi-square analysis, modified by the Cochrane-Armitage test for trend, was used to investigate the effects of age.Results. The prevalence of atypical squamous cells (ASC), low-grade squamous intraepithelial lesion (LSIL), and high-grade squamous intraepithelial lesion or squamous-cell carcinoma (HSIL/SCC) was 0.13%, 0.04% and 0.03%, respectively, in women who had undergone hysterectomy, and 0.93%, 0.51% and 0.26% in women who had not undergone hysterectomy. The prevalence ratios for ASC, LSIL and HSIL/SCC were 0.14(0.11-0.17), 0.08 (0.06-0.13) and 0.13 (0.08-020), respectively, in women with a hysterectomy versus those without. For HSIL/SCC, the prevalence ratios were 0.09 and 029, respectively, for women <50 or >= 50 years. The prevalence rates in women with a previous hysterectomy showed no significant variation with age.Conclusion. The prevalence rates of ASC, LSIL and HSIL/SCC were significantly lower in women with a previous hysterectomy for benign disease compared with those observed in women with an intact uterine cervix. This study reinforces the view that there is no evidence that cytological screening is beneficial for women who have had a hysterectomy for benign disease. (C) 2015 Elsevier Inc. ObjectiveFindings on weight training and waist circumference (WC) change are controversial. This study examined prospectively whether weight training, moderate to vigorous aerobic activity (MVAA), and replacement of one activity for another were associated with favorable changes in WC and body weight (BW).MethodsPhysical activity, WC, and BW were reported in 1996 and 2008 in a cohort of 10,500 healthy U.S. men in the Health Professionals Follow-up Study. Multiple linear regression models (partition/substitution) to assess these associations were used.ResultsAfter adjusting for potential confounders, a significant inverse dose-response relationship between weight training and WC change (P-trend<0.001) was observed. Less age-associated WC increase was seen with a 20-min/day activity increase; this benefit was significantly stronger for weight training (-0.67 cm, 95% CI -0.93, -0.41) than for MVAA (-0.33 cm, 95% CI -0.40, -0.27), other activities (-0.16 cm, 95% CI -0.28, -0.03), or TV watching (0.08 cm, 95% CI 0.05, 0.12). Substituting 20 min/day of weight training for any other discretionary activity had the strongest inverse association with WC change. MVAA had the strongest inverse association with BW change (-0.23 kg, 95% CI -0.29, -0.17).ConclusionsAmong various activities, weight training had the strongest association with less WC increase. Studies on frequency/volume of weight training and WC change are warranted.

Priestley and Taylor provided a practical formulation of the partitioning of net radiation between heat flux and evaporation contained within a parameter α. Their model (PTM) needs verification under a range of environmental conditions. Micrometeorological data sets collected over the Amazon forest at the Ducke Reserve site (2°57′S; 59°57′W) gave an opportunity to evaluate α. Evidence presented here and by others shows that there is pronounced diurnal variation in α, with minimum values around midday and maximum values in the morning and evening hours. During unstable and stable conditions in the daylight hours, the Bowen ratio (B) varied from 0.10 to 0.57 and -0.71 to -0.08, respectively, whereas α varied from 0.67 to 1.16 and 1.28 to 3.12, respectively. A mean value of α = 1.16±0.56 was obtained from daytime hourly values for two days. The daily data sets from three expeditions gave a mean of α = 1.03±0.13. This work confirms that α is a function of atmospheric stability over the Amazon forest. Thus the PTM should be applied with caution over time-intervals of one day or less because of the sensitivity to variation in α. The calculated values of α are in general agreement with those reported in literature. © 1991.
