The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.

The Internet of Things (IoT) has emerged as a paradigm over the last few years as a result of the tight integration of the computing and the physical world. The requirement of remote sensing makes low-power wireless sensor networks one of the key enabling technologies of IoT. These networks encompass several challenges, especially in communication and networking, due to their inherent constraints of low-power features, deployment in harsh and lossy environments, and limited computing and storage resources. The IPv6 Routing Protocol for Low Power and Lossy Networks (RPL) [1] was proposed by the IETF ROLL (Routing Over Low-power Lossy links) working group and is currently adopted as an IETF standard in the RFC 6550 since March 2012. Although RPL greatly satisfied the requirements of low-power and lossy sensor networks, several issues remain open for improvement and specification, in particular with respect to Quality of Service (QoS) guarantees and support for mobility. In this paper, we focus mainly on the RPL routing protocol. We propose some enhancements to the standard specification in order to provide QoS guarantees for static as well as mobile LLNs. For this purpose, we propose OF-FL (Objective Function based on Fuzzy Logic), a new objective function that overcomes the limitations of the standardized objective functions that were designed for RPL by considering important link and node metrics, namely end-to-end delay, number of hops, ETX (Expected transmission count) and LQL (Link Quality Level). In addition, we present the design of Co-RPL, an extension to RPL based on the corona mechanism that supports mobility in order to overcome the problem of slow reactivity to frequent topology changes and thus providing a better quality of service mainly in dynamic networks application. Performance evaluation results show that both OF-FL and Co-RPL allow a great improvement when compared to the standard specification, mainly in terms of packet loss ratio and average network latency. 2015 Elsevier B.V. TB is currently considered to be the most important infectious disease among HIV-1-infected subjects in developing countries, such as Brazil. A retrospective analysis of TB cases was performed, occurring from January 1995 to December 2010 in our cohort of 599 HIV positive patients. The primary outcome was the occurrence of active TB. Forty-one TB cases were diagnosed over this period of 16 years, among 599 HIV positive patients in an open cohort setting in the city of Sao Paulo, Brazil. All-time lowest mean CD4 T cell count at the time of TB diagnosis was 146 and 186 cells/mm³, respectively. The mean HIV viral load was 5.19 log10 copies/mL, and 59% of the patients were on HAART. TB incidence was 1.47 per 100 person-years, for a total follow-up time of 2775 person-years. The probability of surviving up to 10 years after diagnosis was 75% for TB patients as opposed to 96% for patients with other, non-TB opportunistic diseases (p = 0.03). TB can be considered a public health problem among people living with HIV in Brazil despite of the widespread use of antiretrovirals for the treatment of HIV infection/AIDS.

The enzyme-linked immunosorbent assay (ELISA) is the most widely used tool to detect anti-Toxocara IgG antibodies for both serodiagnostic and seroepidemiological surveys on human toxocariasis. In the last eight years a high prevalence of toxocariasis (32.2-56.0%) has been reported in children attending public health units from municipalities in the state of Paraná, Brazil. Therefore, the aim of this work was to compare the frequency found among the general child population with that of children attending a public pneumology service in Maringá, Paraná, Brazil and describe the laboratorial, clinical and epidemiological findings. The research was conducted at the Consórcio Público Intermunicipal de Saúde do Setentrião Paranaense (CISAMUSEP) from July 2009 to July 2010 among children aged between one and 15 years. From a total of 167 children studied, only 4.2% (7/167) tested positive for anti-Toxocara spp. IgG antibodies and presented mild eosinophilia (2/7), increased serum IgE levels (6/7) and a positive allergy test for mites (5/7). The presence of pets (dogs or cats) at home did not correlate with the seroprevalence. In conclusion, cases of toxocariasis involving the respiratory tract are rare in children attending a public health pneumology unit in the northwestern region of Paraná State, despite the high prevalence of this type of toxocariasis among the infantile population attending Basic Health Units in the same geographical area.

A presente tese pretende fazer uma abordagem ao crescimento dos consumos de energia elétrica, que se tem verificado, nos últimos anos, no setor das telecomunicações e das tecnologias de informação; devido ao constante crescimento das redes, dos equipamentos a ela ligados e do tráfego que nelas transita. Num contexto de globalização da economia, no qual, as redes de telecomunicações e de energia elétrica são dois dos maiores contribuintes, a presente tese procura encontrar enquadramentos e soluções para um dos maiores desafios que a humanidade enfrenta atualmente, e que em parte, é consequente dessa globalização: encontrar novas fontes e formas de utilização da energia, -particularmente da energia elétrica - para que a humanidade continue a usufruir, de uma forma sustentável, dos benefícios que a mesma proporciona. Na primeira parte, procura-se fazer uma abordagem que utiliza fontes de informação e conhecimento, do mercado global, nomeadamente, entidades reguladoras e normalizadoras, operadores, fornecedores de tecnologias e consumidores, que abrangessem os três maiores mercados mundiais – União Europeia, Estados Unidos da América e Ásia-Pacífico. Considerou-se fundamental fazê-lo, por se tratar de uma dissertação no âmbito de um Mestrado com o selo de garantia EUR-ACE. Ao longo da dissertação analisou-se a temática da eficiência energética nas redes de telecomunicações e das tecnologias de informação e comunicação, um tema cada vez mais pertinente, já que o número de pessoas com ligações à Internet, já supera os 3 mil milhões, e as redes passaram a ser o meio por onde são transmitidos, a cada segundo, terabytes de sinais de voz, dados e vídeo. Procurou-se encontrar as linhas de orientação que estão a ser traçadas, para otimizar os consumos energéticos, de um complexo sistema convergente de redes e serviços, formado por entidades reguladoras e normalizadoras, operadores, fornecedores de tecnologias e consumidores, onde nem sempre as fronteiras estão perfeitamente definidas. Perante a constatação da realidade exposta, analisou-se as políticas energéticas desenvolvidas nos últimos anos, pelos vários players do mercado das telecomunicações, das tecnologias de informação e dos sistemas elétricos de energia bem como algumas métricas e objetivos comumente aceites. viii São analisados os contributos das partes interessadas, para o desenvolvimento de políticas energéticas eficazes, por forma a permitirem uma implementação, que considere o funcionamento dos equipamentos como um todo, e não de uma forma isolada como tradicionalmente o assunto era abordado. As especificidades na forma como funcionam as redes de telecomunicações e respetivos equipamentos, são expostas sobre várias óticas, comprovando-se que a temática da eficiência energética é uma das áreas mais difíceis lidar, de todas as consideradas nas políticas energéticas. Demonstrou-se que muitos dos equipamentos não estão otimizados em termos de gestão de energia, procurou-se evidenciar as consequências dessa realidade, uma vez que os equipamentos referidos, têm a necessidade de estar permanentemente a ser alimentados pela rede de energia elétrica, para garantir as funções para que foram projetados. Da pesquisa efetuada e descrita ao longo da dissertação, constatamos o empenho de toda a comunidade científica, operadores e agências de energia e de telecomunicações, em resolver o problema, já que há a consciencialização de que o ritmo de crescimento da rede e equipamentos terminais, é superior ao registado na melhoria da eficiência energética dos vários componentes e equipamentos terminais. Na segunda parte do relatório da tese, procurou-se testar a aplicabilidade das normas e recomendações dos organismos que tutelam a atividade a nível global - algumas publicadas nos últimos 2 anos - a um caso prático. Um edifício hospitalar de média dimensão. Foi elaborada uma aplicação informática, que suportada numa metodologia padronizada, seja capaz de fazer a avaliação da eficiência energética dos equipamentos serviços de telecomunicações de informação e comunicação em funcionamento do hospital. Por dificuldades de disponibilidade dos responsáveis do edifício, os resultados ficaram aquém do esperado. Conseguiu-se desenhar a aplicação, inventariar-se apenas parte dos equipamentos. Demonstrou-se que, a forma como alguns equipamentos estão a ser utilizados, não cumprem regras de utilização racional e eficiente. Procurou-se sensibilizar alguns dos responsáveis, para a necessidade de alterar comportamentos e prosseguir o processo de inventariação, por forma, a que o trabalho iniciado atinja os objetivos propostos.

By controlling the transmission of Chagas disease, the challenge of providing assistance to millions of infected patients that reach old age arises. In this study, the socioeconomic, demographic and comorbidity records of all elderly chagasic patients followed at the Pharmaceutical Care Service of the Chagas Disease Research Laboratory were assessed. The information related to the clinical form of the disease was obtained from medical records provided by the Walter Cantídio University Hospital. The profile of the studied population was: women (50.5%); mean age of 67 years; retired (54.6%); married (51.6 %); high illiteracy rate (40.2%); and family income equal to the minimum wage (51.5%). The predominant clinical forms of Chagas disease were cardiac (65.3%) and indeterminate (14.7%). The main electrocardiographic changes were the right bundle branch block (41.0%), associated or not with the anterosuperior left bundle branch block (27.4%). The average number of comorbidities per patient was 2.23 ± 1.54, with systemic arterial hypertension being the main one found (67.0%). It was found that the elderly comprise a vulnerable group of patients that associate aging with cardiac and/or digestive disorders resulting from the evolution of Chagas disease and other comorbidities, which requires special attention from health services to ensure more appropriate medical and social care.

ABSTRACT: Financing is a critical factor in ensuring the optimal development and delivery of a mental health system. The primary method of financing worldwide is tax-based. However many low income countries depend on out-of-pocket payments. There is a report on Irish Health Care funding but none that deals exclusively with mental health care. This paper analyses the various financial models that exist globally with respect to financing the mental health sector, examines the impact of various models on service users, especially in terms of relative ‘financial burden’ and provides a more detailed examination of the current mental health funding situation in Ireland After extensive internet and hardcopy research on the above topics, the findings were analysed and a number of recommendations were reached. Mental health service should be free at the point of delivery to achieve universal coverage. Government tax-based funding or mandatory social insurance with government top-ups, as required, appears the optimal option, although there is no one funding system applicable everywhere. Out-of-pocket funding can create a crippling financial burden for service users. It is important to employ improved revenue collection systems, eliminate waste, provide equitable resource distribution, ring fence mental health funding and cap the number of visits, where necessary. Political, economic, social and cultural factors play a role in funding decisions and this can be clearly seen in the context of the current economic recession in Ireland. Only 33% of the Irish population has access to free public health care and the number health insurance policy holders has dramatically declined, resulting in increased out-of-pocket payments. This approach risks negatively impacting on the social determinants of health, increasing health inequalities and negatively affecting economic productivity. It is therefore important the Irish government examines other options to provide funding for mental health services.

The course of infection with a Peru strain of T. cruzi was studied in 3 rhesus monkeys. An unsuccessful attempt to vaccinate one monkey is described. The IaM leves rise during the course of infection in ali 3 animais. A lumnhocytosis and mild anaemia was noted in these acute infections. Orbital oedema occurred in all 3 animais following subcutaneous inoculation of the infection dose into the eyelid.
