Résumé En Suisse, les programmes de désaccoutumance au tabac se réfèrent généralement au modèle de préparation au changement de Prochaska et DiClemente (1983), Les patients atteints de maladies somatiques liées au tabagisme comme les pathologies cardiovasculaires ou pulmonaires accèdent facilement à ces programmes, contrairement aux patients présentant une dépendance à des drogues illicites. La prévalence de fumeurs dans cette population est pourtant élevée et les problèmes engendrés par le tabac sont importants, non seulement d'un point de vue individuel mais aussi en terme de santé publique. Il est par conséquent intéressant d'évaluer la motivation concernant la désaccoutumance au tabac de patients toxicomanes entreprenant un sevrage de drogues illicites. Dans cette étude, nous avons évalué les stades de préparation au changement concernant la dépendance au tabac chez 100 patients toxicomanes hospitalisés sur un mode volontaire dans le cadre d'un programme de sevrage à des drogues illégales. L'évaluation s'est faite à l'aide d'un auto-questionnaire dont les résultats indiquent qu'une minorité de patients sont décidés à interrompre la consommation de tabac. En effet, seul 15% des patients se trouvaient aux stades de contemplation ou de décision. De plus, 93% des sujets considéraient l'arrêt du tabac comme difficile ou très difficile. Ces données montrent qu'il existe un décalage important entre la motivation relative au sevrage de drogues illégales et la motivation liées à l'arrêt du tabac. En effet, malgré leur motivation élevée pour se sevrer de drogues illicites, la proportion de patients restant au stade de précontemplation concernant la désaccoutumance au tabac reste élevée. Diverses hypothèses permettent d'expliquer ces résultats, notamment la perception que la désaccoutumance au tabac est plus difficile à réaliser que le sevrage de substances illicites. Abstract Nicotine cessation programmes in Switzerland, which are commonly based on the stage of change model of Prochaska and DiClemente (1983), are rarely offered to patients with illicit drug dependence. This stands in contrast to the high smoking rates and the heavy burden of tobacco-related problems in these patients. The stage of change was therefore assessed by self-administered questionnaire in 100 inpatients attending an illegal drug withdrawal programme. Only 15% of the patients were in the contemplation or decision stage. 93% considered smoking cessation to be difficult or very difficult. These data show a discrepancy between the motivation to change illegal drug consumption habits and the motivation for smoking cessation. The high pro-portion of patients remaining in the precontemplation stage for smoking cessation, in spite of their motivation for illicit drug detoxification, may be due to the perception that cessation of smoking is more difficult than illicit drug abuse cessation.

En l'àmbit judicial es constata una relació entre drogoaddicció i delicte. Els addictes a drogues han iniciat amb freqüència algun programa de desintoxicació o han sol·licitat suport mèdic o psicològic. L'estudi es proposa avaluar l'associació entre la conducta il·legal i l'abús de substàncies tòxiques, així com interpretar l'eficàcia dels tractaments terapèutics en la disminució d’aquesta associació, i les repercussions de les teràpies tant sobre els pacients com sobre el seu entorn. És un estudi observacional prospectiu d’una mostra de 100 subjectes que tenen relació amb el consum de drogues d'abús, al Jutjat de Guàrdia de Barcelona i en subjectes interns del Centre Penitenciari de Joves de Barcelona. Els resultats de l’estudi diuen que el sexe masculí hi està representat en un 94%, la mitjana d’edat és de 25,5 anys, amb baix nivell d’estudis i sense professió qualificada. Gairebé la meitat de la mostra són estrangers i solters. Els pacients estudiats són politoxicòmans amb un consum majoritari de cocaïna (92%), cannabis (87%), alcohol (71%) i el 54% d’heroïna. Destaca l’alta freqüència dels UDVP i de la positivitat als tres virus (VIH, VHC, VHB), tot i que es demostra el descens de l’ús de la via parenteral. El 73% dels subjectes comenten delictes contra la propietat. El 93% de la mostra han estat en tractament, però en períodes curts (70%). El 72,7% que estan en PMM consumeixen habitualment cocaïna. Els pacients tractats amb psicofàrmacs són detinguts menys vegades que la resta de pacients. Pocs pacients han fet tractaments combinats amb psicoteràpia o lliure de drogues. Entre la població de detinguts, els tractaments rebuts han resultat beneficiosos pel que fa a autoestima, autoagressivitat, heteroagressivitat, conflictivitat penal i violència en els fets delictius, però aquestes variables quasi no es modifiquen en el cas dels joves. Els tractaments produeixen un benefici als pacients toxicòmans i disminueixen la conflictivitat penal. Els tractaments s’han d’iniciar en els primers estadis del consum, així com potenciar l’adhesió i el seguiment. Els centres penitenciaris de justícia juvenil, les comissaries i els jutjats de guàrdia són els llocs idonis per a l’abordatge del tractament del drogodependent.

L’estudi de la saliva com a matriu d’anàlisis de drogues d’abús en conductors es fa des dels anys 80. Els investigadors trobaven dificultats però el major inconvenient va ser l’existència de tècniques analítiques no prou sensibles per a la detecció de les drogues. A finals de les anys 90 la saliva es va considerar un fluid molt adequat i es van anar publicant treballs que investigaven aquesta temàtica. No obstant això encara no n ’hi ha molts treballs, especialment al nostre país. A Catalunya no s’han fet estudis encara que s’ha contribuït en alguns projectes realitats fora de la nostra comunitat. Objectius: Obtenir dades del consum de drogues d’abús en una mostra de conductors de vehicles a motor. Obtenir dades dels tòxics més freqüents trobats en les mostres analitzades. Relacionar el consum de tòxics amb alteracions en la conducta o conduccions temeràries en la població que es sotmet a l’estudi. Metodologia: Estudi d’una mostra de subjectes que són aturats per la Guàrdia Urbana de Barcelona, Girona ,Tarragona i policia local de el Prat de Llobregat conduint vehicles a motor. Obtenció d’una mostra de saliva als subjectes que es consideren que poden trobar-se sota la influència de les drogues. Anàlisis de les mostres per immunoassaig mitjançant el kit Cozart DDS com a test de camp i confirmació dels resultats per cromatografia de gasos i espectrometria de masses. Resultats: Es descriuen en els resultats obtinguts, les freqüències de consum, les principals drogues d’abús trobades i es valoren els símptomes clínics que presentaven els subjectes. Conclusions: Les drogues d’abús estan presents en conductors de vehicles a motor. El kit utilitzat per la determinació de drogues a peu de carretera és una bona eina atès que es confirmen els resultats especialment per a la cocaïna i opiacis.

There has been an ardent interest in herbivore saliva due to its roles in inducing plant defenses and its impact on herbivore fitness. Two techniques are described that inhibit the secretion of labial saliva from the caterpillar, Helicoverpa zea, during feeding. The methods rely on cauterizing the caterpillar's spinneret, the principal secretory structure of the labial glands, or surgically removing the labial salivary gland. Both methods successfully inhibit secretion of saliva and the principal salivary enzyme glucose oxidase. Caterpillars with inhibited saliva production feed at similar rates as the untreated caterpillars, pupate, and emerge as adults. Glucose oxidase has been suggested to increase the caterpillar's survival through the suppression of inducible anti-herbivore defenses in plants. Tobacco (Nicotiana tabacum) leaves fed on by caterpillars with ablated salivary glands had significantly higher levels of nicotine, an inducible anti-herbivore defense compound of tobacco, than leaves fed upon by caterpillars with intact labial salivary glands. Tomato (Lycopersicon esculentum) leaves fed upon by caterpillars with suppressed salivary secretions showed greatly reduced evidence of hydrogen peroxide formation compared to leaves fed upon by intact caterpillars. These two methods are useful techniques for determining the role that saliva plays in manipulating plant anti-herbivore defenses.

Insulin and leptin are peripheral metabolic factors signaling the body needs in energy to the central nervous system. Because energy homeostasis and reproductive function are closely related phenomena, we investigated the respective roles played by insulin and leptin in the hypothalamic control of GnRH secretion. We observed that increasing circulating insulin levels, by performing hyperinsulinemic clamp studies in male mice, was associated with a significant rise in LH secretion. This effect of insulin is likely mediated at the hypothalamic level, because it was also found to stimulate the secretion and the expression of GnRH by hypothalamic neurons in culture. Leptin was found to potentiate the effect of insulin on GnRH secretion in vitro but was devoid of any effect on its own. These data represent the first evidence of direct insulin sensing by hypothalamic neurons involved in activating the neuroendocrine gonadotrope axis. They also demonstrate that these neurons can integrate different hormonal signals to modulate net hypothalamic GnRH output. We propose that such integration is an essential mechanism for the adaptation of reproductive function to changes in the metabolic status of an individual.

The aim of the present work was to find out whether NPY synthesized in human adrenal chromaffin cells controls in an autocrine/paracrine fashion the release of catecholamines by these cells. Accordingly, the constitutive and regulated release of both NPY and catecholamines was measured simultaneously in cultured human chromaffin cells. In addition, by using both RT-PCR and a combination of specific agonists and antagonists, we characterized the expression of NPY receptors on these cells as well as their pharmacology. Our results were as follows. 1) Human chromaffin cells constitutively secrete NPY. 2) Nicotine elicits a rapid increase in the release of both catecholamines and NPY; this release of NPY is more sustained than that of catecholamines. 3) RT-PCR shows expression of Y1, Y2, Y4, and Y5 receptor mRNA by chromaffin cells; these receptors are functional, as various receptor specific agonists elicit an increase in intracellular calcium. 4) Peptide YY, in contrast to NPY, is not able to stimulate the release of catecholamines. This finding was corroborated by the observation that no receptor-specific antagonists were able to reduce constitutive catecholamine release, whereas an NPY-immunoneutralizing antibody markedly attenuated the secretion. Taken together, these data suggest that NPY originating from the adrenal medulla locally enhances the secretion of catecholamines, presumably by acting via the putative y3 receptor.

Although exposure to secondhand smoke (SHS) is reportedly high in prison, few studies have measured this in the prison environment, and none have done so in Europe. We measured two indicators of SHS exposure (particulate matter PM10 and nicotine) in fixed locations before (2009) and after (2010) introduction of a partial smoking ban in a Swiss prison. Access to smoking cessation support was available to detainees throughout the study. Objectives To measure SHS before and after the introduction of a partial smoking ban. Methods Assessment of particulate matter PM10 (suspended microparticles of 10 μm) and nicotine in ambient air, collected by real-time aerosol monitor and nicotine monitoring devices. Results The authors observed a significant improvement of nicotine concentrations in the air after the introduction of the smoking ban (before: 7.0 μg/m(3), after: 2.1 μg/m(3), difference 4.9 μg/m(3), 95% CI for difference: 0.52 to 9.8, p=0.03) but not in particulate matter PM10 (before: 0.11 mg/m(3), after: 0.06 mg/m(3), difference 0.06 mg/m(3), 95% CI for difference of means: -0.07 to 0.19, p=0.30). Conclusions The partial smoking ban was followed by a decrease in nicotine concentrations in ambient air. These improvements can be attributed to the introduction of the smoking ban since no other policy change occurred during this period. Although this shows that concentrations of SHS decreased significantly, protection was still incomplete and further action is necessary to improve indoor air quality.

This bulletin presents key findings regarding polydrug use (the use of more than one substance within a specific time period) in Ireland. These are based on the drug prevalence survey of households in Ireland and Northern Ireland. A representative sample of adults aged between 15 and 64 years was sampled during late 2010 and early 2011. The bulletin presents prevalence rates for combinations of both legal and illegal drug use for the Republic of Ireland and also examines gender and age differences and the relationship between the use of a particular substance and the use of another substance. The survey was carried out according to standards set by the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA). View the report here.

ARC Healthy Living Centre provides a range of services for a range of groups eg older people, young, unemployed etc. This is their 3rd application and is a Level 3 project. Previous BCPP projects have focussed on for example, peer education programmes for young people, working with Surestart and addiction services. As a result of the project the pharmacist has integrated well into the community. This project will build upon previous work to increase health awareness, promote interagency understanding and deliver a wide range of interactive sessions with groups such as SureStart and alcohol projects. It will also seek to take this out into other areas and encourage other pharmacists to become involved in this way of working.

The organisation helps those with mental ill health and those suffering addiction. This is their second community-pharmacy project. The first focussed on men and this project will move on to work with women. Women who have been referred to Cloona Oasis have been through hardship. The Pharmacist will work with them to develop their confidence, explain their medication and break down barriers so that they can access other services.

On World Heart Day, 29 September 2011, the Public Health Agency is urging all smokers to stop smoking and reduce their risk of developing heart disease (cardiovascular disease - CVD) or suffering a stroke or a heart attack.Heart disease is one of the leading causes of death and illness in the UK. Research shows that smoking is one of the main contributors of the disease, causing around 25,000 deaths a year in the UK. Cigarette smokers are two times more likely than non-smokers to suffer a heart attack.The majority of people who suffer a heart attack before the age of 50 are smokers. Cigarette smoke causes heart disease by:· reducing oxygen to the heart;· increasing blood pressure and heart rate;· increasing blood clotting;· damaging cells that line coronary arteries and other blood vessels, causing narrowing of the arteries.From the moment smoke reaches your lungs, your heart is forced to work harder. Your pulse quickens, forcing your heart to beat an extra 10 to 25 times per minute, as many as 36,000 additional times per day. Because of the irritating effect of nicotine and other components of tobacco smoke, your heartbeat is more likely to be irregular. This can contribute to cardiac arrhythmia and many other serious coronary conditions, such as heart attack.For smokers who already suffer from heart problems, quitting will dramatically help. Many heart patients notice an almost immediate improvement when they stop smoking. Often, they need less medication and can cope better with physical exertion.Gerry Bleakney, Head of Health and Social Wellbeing Improvement, PHA, said: "Smoking is one of the major causes of cardiovascular disease and smokers are almost twice as likely to have a heart attackas someone who has never smoked. INTRODUCTION: Common variation in the CHRNA5-CHRNA3-CHRNB4 gene region is robustly associated with smoking quantity. Conversely, the association between one of the most significant single nucleotide polymorphisms (SNPs; rs1051730 within the CHRNA3 gene) with perceived difficulty or willingness to quit smoking among current smokers is unknown. METHODS: Cross-sectional study including current smokers, 502 women, and 552 men. Heaviness of smoking index (HSI), difficulty, attempting, and intention to quit smoking were assessed by questionnaire. RESULTS: The rs1051730 SNP was associated with increased HSI (age, gender, and education-adjusted mean ± SE: 2.6 ± 0.1, 2.2 ± 0.1, and 2.0 ± 0.1 for AA, AG, and GG genotypes, respectively, p < .01). Multivariate logistic regression adjusting for gender, age, education, leisure-time physical activity, and personal history of cardiovascular or lung disease showed rs1051730 to be associated with higher smoking dependence (odds ratio [OR] and 95% CI for each additional A-allele: 1.38 [1.11-1.72] for smoking more than 20 cigarette equivalents/day; 1.31 [1.00-1.71] for an HSI ≥5 and 1.32 [1.05-1.65] for smoking 5 min after waking up) and borderline associated with difficulty to quit (OR = 1.29 [0.98-1.70]), but this relationship was no longer significant after adjusting for nicotine dependence. Also, no relationship was found with willingness (OR = 1.03 [0.85-1.26]), attempt (OR = 1.00 [0.83-1.20]), or preparation (OR = 0.95 [0.38-2.38]) to quit. Similar findings were obtained for other SNPs, but their effect on nicotine dependence was no longer significant after adjusting for rs1051730. Conclusions: These data confirm the effect of rs1051730 on nicotine dependence but failed to find any relationship with difficulty, willingness, and motivation to quit.
