A dengue no Brasil incide tipicamente nos meses mais quentes do ano, sem diferenças qualitativas para as regiões brasileiras, porém, com diferenças quantitativas importantes, dividindo o país em dois grupos distintos quanto ao número de notificações de casos. O primeiro grupo compreende as regiões Nordeste e Sudeste, que deteve cerca de 86% das notificações, enquanto o segundo (regiões Sul, Centro-Oeste e Norte) é responsável por um número significativamente menor. Os índices vetoriais estavam associados primariamente ao tamanho das populações, sendo mais freqüentes os índices abaixo do valor de risco e ainda assim, nesta condição, ocorreram epidemias. Não foi observada correlação positiva entre epidemias e densidades vetoriais.

Evidência de extravasamento vascular decorrente do aumento da permeabilidade capilar caracteriza e diferencia febre hemorrágica da dengue. Este artigo avalia o valor de albumina sérica na detecção de alterações de permeabilidade vascular em casos de dengue. Quatorze pacientes diagnosticados com febre hemorrágica da dengue em dois hospitais privados em Recife, Brasil, entre janeiro e maio de 2002 foram seguidos com dosagens diárias de hematócrito e albumina sérica. Ultra-son e Raio X de tórax foram realizados. Oito (57%) casos apresentaram hemoconcentração de 20% ou mais. Hipoalbuminemia foi detectado em dez (71%) pacientes. Dosagem de albumina sérica aumentou a detecção de alterações de permeabilidade em seis (43%) casos na qual a hemoconcentração foi inferior a 20% e os sintomas foram compatíveis com um a resposta imune exacerbada. Raio-X foi normal em todos os casos. A utilização, portanto, de dosagem de albumina sérica aumenta a sensibilidade de detecção de casos de febre hemorrágica da dengue.

Neurological manifestation is uncommon in dengue infection. The pathogenesis of central nervous system involvement is controversial. We report a rare case of acute disseminated encephalomyelitis in classic dengue, with isolation of serotype 3 in liquor. This condition was associated with significant structural damage detected by magnetic resonance.

The first dengue fever epidemic in the State of Rondônia (western region of Brazil) was recorded in 1997, without laboratory confirmation. Following this, there was an epidemic in Manaus, in the neighboring State of Amazon, in 1998, in which DENV-1 and DENV-2 viruses were isolated from patients. In the present paper, the serotype characterization of the dengue virus isolated from patients with clinically suspected dengue in Porto Velho, Rondônia, between 2001 and 2003 is described. One hundred and fifty blood samples were collected between the first and fifth days of symptoms. Seventy samples of virus isolates were subjected to dengue identification by means of RT-PCR using universal primers for the NS1 gene of DENV, which amplifies a 419 bp fragment. The amplicons obtained were subjected to enzymatic digestion to characterize the viral serotypes. All the samples analyzed were DENV-1. A nucleotide sequence randomly selected from one amplicon, which was also DENV-1, presented 98% similarity to sequences from Southeast Asia that were obtained from GenBank.

This open comparative study for evaluating hypersensitivity to tuberculin among medical students (n=441), nursing students (n=178) and economics students (n=230) in Vitória, State of Espírito Santo, had the aim of assessing whether the prevalence of positive tuberculin tests among healthcare students could be used as a sentinel event for nosocomial transmission in localities with high endemicity. There was a significant change in the frequency of purified protein derivative reactivity in the nursing students (20. 3%) and medical students (18. 4%), in comparison with the economics students (6%) (p <0. 001). As expected, the medical and nursing students had more knowledge about tuberculosis than did the economics students (p <0. 001). Our data suggest that the incidence of Mycobacterium tuberculosis infection among nursing and medical students in our region, which has high endemicity of tuberculosis, does not differ from what is found in low-incidence countries. These data reinforce the need for implementing biosafety programs for this target population.

Considerando a carência de dados clínicos e epidemiológicos da leptospirose humana no Estado de Mato Grosso do Sul e a possibilidade de confusão com outras doenças, soros de pacientes com suspeita clínica inicial de dengue e hepatite viral, porém, sem confirmação laboratorial, foram examinados, através de soroaglutinação microscópica para leptospirose. Os índices de sororreagentes nas amostras com suspeita clínica de dengue e hepatite viral foram, respectivamente, 15,9% e 9%. A maior ocorrência foi para o sorovar hurstbridge (70,4%) e o maior título para o sorovar canicola (1:51. 200). Não se observou associação entre positividade e sexo, idade ou ocupação dos pacientes. O estudo demonstrou que, embora as atuais notificações de casos de leptospirose em Mato Grosso do Sul sejam irrisórias, a prevalência de anticorpos foi elevada nos grupos investigados e, portanto, a hipótese de subnotificação de casos de leptospirose humana em Mato Grosso do Sul e dificuldades no diagnóstico diferencial com dengue e hepatite viral devem ser consideradas.

In Pernambuco, the first dengue cases occurred in 1987. After a seven-year interval without autochthonous cases, a new epidemic occurred in 1995. Important aspects of the dengue epidemics during the period 1995-2006 have been analyzed here, using epidemiological, clinical and laboratory data. A total of 378,374 cases were notified, with 612 confirmed cases of dengue hemorrhagic fever and 33 deaths. The mortality rate was 5.4%. The incidence rate increased from 134 to 1,438/100,000 inhabitants, corresponding to the epidemics due to serotypes 2 and 3, in 1995 and 2002, respectively. Dengue mainly affected adults (20-49 years); 40.7% were male and 59.3% were female. From 2003 onwards, the number of cases among individuals younger than 15 years old increased. Out of 225 dengue hemorrhagic fever cases, 42.7% primary and 57.3% secondary infections were identified (p = 0.0279). Neurological manifestations were also observed. From 2002 onwards, serotypes 1, 2 and 3 were circulating; serotype 3 was predominant.

This study compared the serum levels of IL-6, TNF-alpha and IFN-gamma, in children under 1 year of age with and without dengue. Sera were collected from a total of 41 children living in the Department of Antioquia, Colombia (27 patients with dengue and 14 controls). The results showed higher cytokine levels in children with dengue than without dengue, with statistically significant differences for IL-6 and IFN-gamma. No statistically significant differences were found between clinical forms, although IL-6 and IFN-gamma levels were higher in dengue fever cases than in dengue hemorrhagic fever cases. On the other hand, TNF-alpha levels were higher in dengue hemorrhagic fever than in dengue fever. The levels of IL-6 and TNF-alpha were higher in secondary infection than in primary infection, although IFN-gamma levels were higher in primary infection. These results suggest that IL-6, TNF-alpha and IFN-gamma are involved in dengue infection independently of the clinical form.

Seven individuals living in a town in the Southwest of Bahia developed sudden signs of cardiac and systemic impairment, with lethality of 28.6%. Serological tests were positive at least in one test in the five patients examined. Forty percent of the Triatoma sordida mynphs found inside or around Trypanosoma cruzi were found by blood culturig in there out five cases the homes of these cases were positive for Trypanosoma cruzi. Transmission probably occurred through consumption of water contaminated with triatomine feces. These findings emphasize the necessity to evaluation the importance of vectors like Triatoma sordida in maintaining the endemicity of this disease.

Aspectos da dinâmica populacional de Aedes aegypti foram investigados a partir de coletas no decorrer de dois anos na área urbana de Uberlândia, MG. A dinâmica populacional do mosquito foi influenciada por fatores físicos como temperatura e pluviosidade. Altas densidades larvais também influenciaram no desenvolvimento do mosquito.

O estudo teve como objetivo verificar a freqüência de sarampo, rubéola, dengue e eritema infeccioso entre casos suspeitos de sarampo e rubéola, no estado de Pernambuco, Brasil. Foram testadas 1.161 amostras de soro coletadas no período de 2001 a 2004, para as quatro viroses, utilizando-se ensaios imunoenzimáticos para detecção de anticorpos IgM. Desse total, 276 (23,8%) amostras foram positivas para uma das quatro viroses analisadas. Foram detectados 196 (16,9%) casos positivos para dengue, 38 (3,3%) para eritema infeccioso (parvovírus B19), 32 (2,8%) para rubéola e 10 (0,9%) para sarampo. Entre os casos suspeitos de sarampo e rubéola, a infecção pelo vírus dengue foi a mais freqüente, seguida pelo parvovírus B19. A semelhança de manifestações clínicas entre as doenças exantemáticas contribui para dificultar o diagnóstico de sarampo, rubéola, dengue e eritema infeccioso, quando observados apenas os critérios clínicos. Deve-se salientar que os quatro testes utilizados foram insuficientes para diagnosticar 76,2% das doenças febris exantemáticas notificadas. Este é o primeiro estudo que evidencia a circulação de parvovírus B19 humano em Pernambuco.

O objetivo deste trabalho é apresentar os aspectos clínicos e epidemiológicos das crianças acometidas por dengue, em Manaus, AM, no período de 2006 a 2007. Foram analisados 482 casos de dengue confirmados laboratorialmente, em 2006, sendo 46,9% nos menores de 15 anos, e 1.538 casos, em 2007, sendo 57,7% nos menores de 15 anos. Os dados foram obtidos do Sistema Nacional de Agravos de Notificação (SINAN) de Manaus. As variáveis analisadas foram: idade, gênero, bairro de residência, mês do inicio dos sintomas, sorotipo e forma clínica da doença. Verificou-se um aumento estatisticamente significativo da proporção de casos em menores de 15 anos, bem como das formas graves entre 2006 e 2007. Apesar de a forma hemorrágica ter sido quase três vezes mais freqüente em 2007, houve uma redução do coeficiente de letalidade. Os sorotipos identificados foram DEN-3 em 2006 e DEN-1, DEN-2 e DEN-3 em 2007. Em 2007, houve registro de casos em todos os meses do ano.

A study was conducted on all newborns from mothers with Chagas disease who were attended at Hospital Donación F. Santojanni between January 1, 2001, and August 31, 2007. Each child was investigated for the presence of Trypanosoma cruzi parasitemia through direct examination of blood under the microscope using the buffy coat method on three occasions during the first six months of life. Serological tests were then performed. Ninety-four children born to mothers infected with Trypanosoma cruzi were attended over the study period. Three of these children were born to mothers coinfected with the human immunodeficiency virus. Vertical transmission of Chagas disease was diagnosed in 13 children, in all cases by identifying parasitemia. The overall Chagas disease transmission rate was 13.8% (13/94). It was 100% (3/3) among the children born to mothers with HIV infection and 10.9% (10/91) among children born to mothers without HIV [Difference = 0.89; CI95 = 0.82-0.95; p = 0.0021]. We concluded that coinfection with HIV could increase the risk of vertical transmission of Chagas disease.

Future broadband wireless systems are expected to cope with severely time dispersive channels, due to multi-path signal propagation between the transmitter and the receiver while having high power and spectral efficiency. Thus, advanced Frequency Domain Equalization techniques are required. The implementation complexity in mobile terminals should be as low as possible to achieve highest possible efficiency. Therefore, most of the signal processing requirements will be shifted to the base station and we will employ signals compatible with an efficient, grossly nonlinear power amplification. For this reason, we will consider offset modulation signals with quasi-constant envelope and develop receivers that will obtain good BER performance. However, these signals require a bandwidth significantly above the Nyquist rate, which can be reduced by an overlap of different frequency channels.
