OBJETIVO: Determinar a validade de perguntas freqüentemente utilizadas em estudos epidemiológicos sobre higiene bucal de crianças e compará-la segundo renda familiar e escolaridade da mãe. MÉTODOS: Foram analisadas 1.122 crianças participantes do subestudo de saúde bucal de 2009 da Coorte de Nascimentos de Pelotas, RS, 2004. As crianças foram examinadas e suas mães entrevistadas no domicílio. O padrão-ouro da condição de higiene bucal foi avaliado por meio do Índice de Higiene Oral Simplificado e a partir do seu escore total o desfecho foi dicotomizado em: placa dental ausente (escore total = 0) e presente (escore total ≥ 1). As perguntas testadas sobre o padrão de higiene bucal das crianças foram formuladas às mães e incluíram: freqüência diária de escovação, escovação antes de dormir e a combinação dessas duas (higiene bucal), com suas opções de resposta dicotomizadas em regular e irregular. A validade foi determinada por meio do cálculo dos valores percentuais e respectivos intervalos de 95% de confiança de sensibilidade, especificidade, valor preditivo positivo e valor preditivo negativo. RESULTADOS: A prevalência de placa dental foi 37,0%. Os valores de sensibilidade, especificidade, valor preditivo positivo e valor preditivo negativo foram, respectivamente: 29,6%, 82,5%, 49,8% e 66,6%, para freqüência de escovação diária irregular; 41,8%, 64,6%, 40,9% e 65,5%, para escovação antes de dormir irregular; 48,8%, 60,8%, 42,2% e 67,0%, para higiene bucal irregular. A validade do padrão de higiene bucal variou conforme o nível de renda familiar e a escolaridade da mãe, e a sensibilidade e o valor preditivo positivo foram maiores entre os indivíduos com menor renda familiar e com mães menos escolarizadas e o oposto, para a especificidade e o valor preditivo negativo. CONCLUSÕES: Perguntas sobre higiene bucal respondidas pelas mães de crianças não são boas substitutas do padrão real de higiene bucal medido por meio do exame clínico bucal de placa dental.

Para a construção de uma estrutura há diversos factores que influenciam a sua concepção, tais como o vão, a topografia do local, acessibilidades, os possíveis acidentes do terreno, como cursos de água ou a sua função. É de ressalvar ainda que o enquadramento paisagístico cada vez mais é tido em conta, embora não seja determinante para a sua função ou dimensionamento. Na construção de pontes existe uma grande variedade de métodos construtivos a usar variando estes com a dimensão da obra ou com a disponibilidade de equipamento por parte do empreiteiro. Cada vez mais deverá ser bem analisado o sistema a usar, uma vez que o custo do processo construtivo constitui uma percentagem elevada do valor final da obra, podendo ser reduzidos os custos pela escolha acertada do método. Com a presente dissertação pretende-se analisar os diferentes métodos construtivos existentes no mercado, sendo estes distinguidos entre sistemas com aplicação de préfabricação ou com betonagem in-situ, e ainda o processo a usar em função do vão a vencer.

Global warming and the associated climate changes are being the subject of intensive research due to their major impact on social, economic and health aspects of the human life. Surface temperature time-series characterise Earth as a slow dynamics spatiotemporal system, evidencing long memory behaviour, typical of fractional order systems. Such phenomena are difficult to model and analyse, demanding for alternative approaches. This paper studies the complex correlations between global temperature time-series using the Multidimensional scaling (MDS) approach. MDS provides a graphical representation of the pattern of climatic similarities between regions around the globe. The similarities are quantified through two mathematical indices that correlate the monthly average temperatures observed in meteorological stations, over a given period of time. Furthermore, time dynamics is analysed by performing the MDS analysis over slices sampling the time series. MDS generates maps describing the stations’ locus in the perspective that, if they are perceived to be similar to each other, then they are placed on the map forming clusters. We show that MDS provides an intuitive and useful visual representation of the complex relationships that are present among temperature time-series, which are not perceived on traditional geographic maps. Moreover, MDS avoids sensitivity to the irregular distribution density of the meteorological stations.

OBJECTIVE To analyze the access and utilization profile of biological medications for psoriasis provided by the judicial system in Brazil.METHODSThis is a cross-sectional study. We interviewed a total of 203 patients with psoriasis who were on biological medications obtained by the judicial system of the State of Sao Paulo, from 2004 to 2010. Sociodemographics, medical, and political-administrative characteristics were complemented with data obtained from dispensation orders that included biological medications to treat psoriasis and the legal actions involved. The data was analyzed using an electronic data base and shown as simple variable frequencies. The prescriptions contained in the lawsuits were analyzed according to legal provisions.RESULTS A total of 190 lawsuits requesting several biological drugs (adalimumab, efalizumab, etanercept, and infliximab) were analyzed. Patients obtained these medications as a result of injunctions (59.5%) or without having ever demanded biological medication from any health institution (86.2%), i.e., public or private health services. They used the prerogative of free legal aid (72.6%), even though they were represented by private lawyers (91.1%) and treated in private facilities (69.5%). Most of the patients used a biological medication for more than 13 months (66.0%), and some patients were undergoing treatment with this medication when interviewed (44.9%). Approximately one third of the patients discontinued treatment due to worsening of their illness (26.6%), adverse drug reactions (20.5%), lack of efficacy, or because the doctor discontinued this medication (13.8%). None of the analyzed medical prescriptions matched the legal prescribing requirements. Clinical monitoring results showed that 70.3% of the patients had not undergone laboratory examinations (blood work, liver and kidney function tests) for treatment control purposes.CONCLUSIONS The plaintiffs resorted to legal action to get access to biological medications because they were either unaware or had difficulty in accessing them through institutional public health system procedures. Access by means of legal action facilitated long-term use of this type of medication through irregular prescriptions and led to a high rate of adverse drug reactions as well as inappropriate clinical monitoring.

OBJECTIVE To test whether the occupational conditions of professional truck drivers are associated with amphetamine use after demographic characteristics and ones regarding mental health and drug use are controlled for.METHODS Cross-sectional study, with a non-probabilistic sample of 684 male truck drivers, which was collected in three highways in Sao Paulo between years 2012 and 2013. Demographic and occupational information was collected, as well as data on drug use and mental health (sleep quality, emotional stress, and psychiatric disorders). A logistic regression model was developed to identify factors associated with amphetamine use. Odds ratio (OR; 95%CI) was defined as the measure for association. The significance level was established as p < 0.05.RESULTS The studied sample was found to have an average age of 36.7 (SD = 7.8) years, as well as low education (8.6 [SD = 2.3] years); 29.0% of drivers reported having used amphetamines within the twelve months prior to their interviews. After demographic and occupational variables had been controlled for, the factors which indicated amphetamine use among truck drivers were the following: being younger than 38 years (OR = 3.69), having spent less than nine years at school (OR = 1.76), being autonomous (OR = 1.65), working night shifts or irregular schedules (OR = 2.05), working over 12 hours daily (OR = 2.14), and drinking alcohol (OR = 1.74).CONCLUSIONS Occupational aspects are closely related to amphetamine use among truck drivers, which reinforces the importance of closely following the application of law (Resting Act (“Lei do Descanso”); Law 12,619/2012) which regulates the workload and hours of those professionals. Our results show the need for increased strictness on the trade and prescription of amphetamines in Brazil.

The activity of growing living bacteria was investigated using real-time and in situ rheology-in stationary and oscillatory shear. Two different strains of the human pathogen Staphylococcus aureus-strain COL and its isogenic cell wall autolysis mutant, RUSAL9-were considered in this work. For low bacteria density, strain COL forms small clusters, while the mutant, presenting deficient cell separation, forms irregular larger aggregates. In the early stages of growth, when subjected to a stationary shear, the viscosity of the cultures of both strains increases with the population of cells. As the bacteria reach the exponential phase of growth, the viscosity of the cultures of the two strains follows different and rich behaviors, with no counterpart in the optical density or in the population's colony-forming units measurements. While the viscosity of strain COL culture keeps increasing during the exponential phase and returns close to its initial value for the late phase of growth, where the population stabilizes, the viscosity of the mutant strain culture decreases steeply, still in the exponential phase, remains constant for some time, and increases again, reaching a constant plateau at a maximum value for the late phase of growth. These complex viscoelastic behaviors, which were observed to be shear-stress-dependent, are a consequence of two coupled effects: the cell density continuous increase and its changing interacting properties. The viscous and elastic moduli of strain COL culture, obtained with oscillatory shear, exhibit power-law behaviors whose exponents are dependent on the bacteria growth stage. The viscous and elastic moduli of the mutant culture have complex behaviors, emerging from the different relaxation times that are associated with the large molecules of the medium and the self-organized structures of bacteria. Nevertheless, these behaviors reflect the bacteria growth stage.

O estudo epidemiológico e neuropatológico concernente a trinta autópsias de encefalite rábica foi realizado, tendo sido os dados obtidos comparados com aqueles da literatura. Não houve relação estatística entre a topografia lesional e o local da mordida, entre o período de incubação ou a duração da doença e a presença ou ausência de inclusões virais (IV) assim como entre a intensidade do processo inflamatorio (PI) e a presença de IV. A dispersão do PI e das IV através o SNC foi diretamente proporcional à sua intensidade. A estrutura mais freqüentemente comprometida pelo PI foi o mesencéfalo, seguido pelo bulbo, ponte e medula espinhal. A estrutura mais intensamente afetada foi o bulbo. As IV foram particularmente proeminentes no hipocampo e cerebelo.

Defective interfering (DI) viruses are thought to cause oscillations in virus levels, known as the ‘Von Magnus effect’. Interference by DI viruses has been proposed to underlie these dynamics, although experimental tests of this idea have not been forthcoming. For the baculoviruses, insect viruses commonly used for the expression of heterologous proteins in insect cells, the molecular mechanisms underlying DI generation have been investigated. However, the dynamics of baculovirus populations harboring DIs have not been studied in detail. In order to address this issue, we used quantitative real-time PCR to determine the levels of helper and DI viruses during 50 serial passages of Autographa californica multiple nucleopolyhedrovirus (AcMNPV) in Sf21 cells. Unexpectedly, the helper and DI viruses changed levels largely in phase, and oscillations were highly irregular, suggesting the presence of chaos. We therefore developed a simple mathematical model of baculovirus-DI dynamics. This theoretical model reproduced patterns qualitatively similar to the experimental data. Although we cannot exclude that experimental variation (noise) plays an important role in generating the observed patterns, the presence of chaos in the model dynamics was confirmed with the computation of the maximal Lyapunov exponent, and a Ruelle-Takens-Newhouse route to chaos was identified at decreasing production of DI viruses, using mutation as a control parameter. Our results contribute to a better understanding of the dynamics of DI baculoviruses, and suggest that changes in virus levels over passages may exhibit chaos.
