O Helicobacter pylori, é tido como o principal fator de risco para o carcinoma gástrico. Diferentes estudos experimentais em animais procuram relacionar essa carcinogênese a outros fatores carcinógenos sem sucesso. Neste estudo, procurou-se avaliar-se em ratos, se há correlação entre o refluxo duodenogástrico, o Helicobacter pylori e o desenvolvimento do câncer gástrico ou de seus precursores. Para tal, realizou-se nos três grupos de ratos (n de dez por grupo) as técnicas de: piloroplastia precedida de infecção, gastrectomia subtotal precedida de infecção e um grupo no qual foi praticada apenas a infecção. Apois seis meses, analisou-se as alterações da mucosa, comparando-se os três grupos. As alterações da mucosa pesquisadas foram as seguintes: gastrites, metaplasias, displasias e neoplasias epiteliais. Ao término do estudo, foi encontrado, no grupo submetido a piloroplastia precedida de infecção um alto percentual de alterações epiteliais. Conclui-se que, no rato, a operação de piloroplastia, levou ao maior desenvolvimento da população do Helicobacter pylori, que se relaciona com as lesões pré- malignas e o adenocarcinoma gástrico.

Background Dipeptidyl-peptidase IV (EC 3.4.14.5) (DPPIV) is a serine peptidase involved in cell differentiation, adhesion, immune modulation and apoptosis, functions that control neoplastic transformation. Previous studies have demonstrated altered expression and activity of tissue and circulating DPPIV in several cancers and proposed its potential usefulness for early diagnosis in colorectal cancer (CRC). Methods and principal findings The activity and mRNA and protein expression of DPPIV was prospectively analyzed in adenocarcinomas, adenomas, uninvolved colorectal mucosa and plasma from 116 CRC patients by fluorimetric, quantitative RT-PCR and immunohistochemical methods. Results were correlated with the most important classic pathological data related to aggressiveness and with 5-year survival rates. Results showed that: 1) mRNA levels and activity of DPPIV increased in colorectal neoplasms (Kruskal-Wallis test, p<0.01); 2) Both adenomas and CRCs displayed positive cytoplasmic immunostaining with luminal membrane reinforcement; 3) Plasmatic DPPIV activity was lower in CRC patients than in healthy subjects (Mann-U test, p<0.01); 4) Plasmatic DPPIV activity was associated with worse overall and disease-free survivals (log-rank p<0.01, Cox analysis p<0.01). Conclusion/significance 1) Up-regulation of DPPIV in colorectal tumors suggests a role for this enzyme in the neoplastic transformation of colorectal tissues. This finding opens the possibility for new therapeutic targets in these patients. 2) Plasmatic DPPIV is an independent prognostic factor in survival of CRC patients. The determination of DPPIV activity levels in the plasma may be a safe, minimally invasive and inexpensive way to define the aggressiveness of CRC in daily practice.

O câncer do colo do útero corresponde a cerca de 15% de todos os tipos de câncer em mulheres, no mundo. No Brasil, em 2002, o câncer de colo do útero foi responsável por 7,1% de todas as mortes por câncer em mulheres, ocupando a quarta posição entre os demais. Para o mesmo ano, a taxa de mortalidade por câncer do colo do útero ajustada por idade, pela população padrão mundial, foi de 5,03/100.000. Já as taxas de incidência ajustadas por idade variaram entre 14,3 por 100.000 mulheres em Salvador e 50,7 por 100.000 mulheres no Distrito Federal, para o período compreendido entre 1991 e 2001. O câncer do colo do útero inicia-se a partir de uma lesão pré-invasiva, curável em até 100% dos casos, que geralmente progride lentamente, por anos, antes de atingir o estágio invasor da doença, quando a cura se torna mais difícil, quando não impossível. A abordagem mais efetiva para o controle do câncer do colo do útero continua sendo o rastreamento por meio do exame preventivo de Papanicolaou. O objetivo deste estudo foi identificar fatores associados à não realização do exame de Papanicolaou em mulheres de 25 a 59 anos nos três anos anteriores à pesquisa, nos municípios de Fortaleza e Rio de Janeiro. Para cada localidade foi utilizado o delineamento transversal, de base populacional com amostragem por conglomerados com dois estágios de seleção e auto-ponderada. Os dados foram analisados por regressão de Poisson obedecendo a um modelo hierárquico previamente determinado. O percentual de mulheres não submetidas ao exame de Papanicolaou nos três anos anteriores à pesquisa, em Fortaleza e no Rio de Janeiro, foi de 19,1% (IC95%: 16,1-22,1) e 16,5% (IC95%: 14,1-18,9), respectivamente. Mulheres de baixa escolaridade, de menor renda per capita, de maior idade, não casadas, não submetidas a mamografia, ao exame clínico das mamas, aos exames de glicemia e colesterolemia foram as que apresentaram as maiores de razões de prevalências para a não realização do exame de Papanicolaou, em ambas as localidades. As fumantes foram menos submetidas ao exame em relação às demais mulheres, sendo essa diferença estatisticamente significativa somente no Rio de Janeiro. Finalmente, as informações aqui apresentadas apontam para a necessidade de intervenção em um grupo específico de mulheres. Deve-se priorizar atividades de educação para o diagnóstico precoce e rastreamento em mulheres sintomáticas e assintomáticas, respectivamente, além da garantia de acesso aos métodos de diagnóstico e tratamento adequados.

A maioria dos pacientes que apresentam metástases hepáticas colorretais (MHCR) não são elegíveis para ressecção. Por isso, outras técnicas para se alcançar o controle locorregional da doença têm sido utilizadas. A Ablação por Radiofrequência (ARF) hepática tem sido empregada frequentemente para o tratamento desta condição devido às boas taxas de resposta, principalmente quando associada ao emprego de quimioterápicos modernos. Apesar da caracterização das MHCR no pré-operatório estar bem estabelecida, os parâmetros de ressonância magnética (RM) após ARF no período pós-cirúrgico requerem maior padronização objetiva. O coeficiente de difusão aparente de água (CDA) tem sido usado na RM como um parâmetro de isquemia e necrose. Entretanto, não está ainda bem definido seu papel e das imagens ponderadas de difusão (DWI) na avaliação da necrose coagulativa gerada pela ARF, especificamente em pacientes com MHCR. O objetivo deste estudo consiste em avaliar o espectro de mudança em RM funcional após a ARF de MHCR. Foi realizado estudo retrospectivo entre 2001 e 2006, avaliando 51 pacientes que foram submetidos à ARF por MHCR no Hospital Johns Hopkins (Baltimore, EUA) dos quais 16 preencheram os critérios de inclusão. Os critérios de inclusão foram: (1) apresentar MHCR tratada cirurgicamente com intenção curativa por ARF guiados por ultrassom per-operatório, e (2) ter uma RM anterior e imediatamente após a cirurgia (até 10 dias). As imagens de RMs antes e após a ablação hepática para MCHR foram identificadas e revistas. As imagens de RM de difusão e captação de contraste foram feitas numa unidade de 1.5 T. Imagens em T2 e T1 foram realizadas na presença e ausência de contraste venoso. Todos os exames tiveram a espessura de 4 a 6 mm e um intervalo de 2 mm, apresentando um angulo de rotação de 150. O índice de difusão foi determinado com um b-valor (intensidade do gradiente de difusão) de 500 seg/mm2. As análises de parâmetros para avaliar e comparar o pré e pós ARF através da RM funcional incluiram: (1) valores do mapa de CDA, (2) captação de contraste, (3) difusão e (4) mudança no tamanho da lesão. Em adição, a sobrevida mediana global e tempo para recorrência local foram calculados. As imagens foram avaliadas por um consenso de dois radiologistas/cirurgiões. Foram avaliados no total 65 lesões, com tamanho médio pré-ablação de 1,7 cm. Após ablação o tamanho médio da lesão aumentou para 3,5 cm (p<0,001). A captação de contraste nas fases arterial e venosa diminuíram significativamente após ARF (diminuição média 10% e 17,5%, respectivamente, p = 0,002 e <0,001). O valor médio do mapa de CDA da lesão foi de 2.79 x 10-3 mm2/seg pré-ARF, e após ARF diminui em média para 1.75 x 10-3 mm2/seg (p<0.001). A sobrevida mediana global foi de 34,7 meses. A ressonância magnética funcional através da captação de contraste e difusão pode prover uma estimativa subjetiva e objetiva da necrose coagulativa tissular e da desidratação celular na área ablada por radiofrequência. Quando combinada ao aumento no tamanho do tumor, podem atuar como um marcador adicional de resposta tumoral.

Feeding habits of many animals have been used widely in animal classifications. This is so, because the type of diet an organism requires demands structural specialisation which will utilise the available resource. Many animals may however have many structural modifications to enable them to be described as omnivourous or generalised feeders such as H. empodisma and H. riponianus (GREENWOOD 1960) which may show varying degrees of structural and adaptational intermediacy between two trophic groups. Generally, however, the diet of many animals including fish changes as the animal grow larger. The change in structural modifications is usually correlated with changes in the diet. In fishes the change may involve change from tricuspid to biscuspid and finally to unicuspid type of teeth. The degree of modification in the structure depends on the diet, thus Haplochromis that feeds on soft tissues of snails only requires modifications in oral dentition while Haplochromis that feeds on both soft tissues and shells of snails require modification in the lower pharyngeal bone for grinding purposes. Other modifications connected with food utilisation may be located in the alimentary canal. (I) The fish species that are commercially exploited are Protopterus aethiopicus, Clarias mossambicus, Tilapia esculenta, Tilapia amphimelas and Tilapia hybrids. The other fish species present in the lake but not commercially exploited are: Gnathonemus sp. Alestes sp. Labeo sp., Barbus paludinoses, Barbus jacksoni, Barbus lineomaculatus, Barbus regersi, Leptogrlanis sp., Schilbe sp., Haplochromis spp. and Hemihaplochromis sp. (2) Protopterus sp. and Clarias sp. are mostly caught with hooks on long lines. There has been a steady increase in number of hooks on the lake. Since the stocks of Protopterus and C/arias in the lake have a limit, we should control the number of hooks used by each of the fishermen in order to avoid overharvesting. (3) All the previous studies on Lake Kitangiri fisheries suggested the use of gill nets with mesh size greater than 88.9 mm in order to avoid the capture of immature Ti/apia spp. But if the fishermen are to obtain economic gains from the fishery, the optimum mesh size for use is 88.9 -101.6 mm. (4) The gillnet is a passive gear with very beneficial selective characteristics. Unfortunately the drive-in fishery which exists on Lake Kitangiri more or less destroys the gillnet selectivity characteristics. It is therefore recommended that the beating of water with poles be discouraged and stopped. (5) There is need for provision of stable fishing canoes to replace the unstable bottle palm dug-out canoes which are currently being used and which are very risky to operate. (6) The fish processing facilities on Lake Kitangiri are still inadequate. Most of the fish is sun dried, Since sun drying is very difficult during the rainy season, most fishermen carry out intensive fishing during the dry season, Concentrating most of the fishing effort in anyone season instead of spreading evenly this effort over the whole year could damage the age structure of the exploitable stocks. (7) There are considerable fluctuations in the volume of water of the lake. The feasibility of regulating the water loss through the effluent Sibiti river should be investigated by the Water Development Department. (8) Damming the Sibiti river is an expensive undertaking and therefore, the Rural Development Bank of Tanzania should be asked to assess the economic feasibility of such a project.

Predatory behaviour of Nandus nandus was studied by offering Cyprinus carpio as prey. The study was conducted with six N. namdus (8.2 ±0.2 cm and 7.60 ±0.3g) represented as P 1, P 2, P 3, P 4, P 5 and P 6. Three size categories of prey (C. carpio) such as small (2.0 ±0.1 cm and 0.23 ±0.01g), large (3.6 ±0.1 cm and 0.57 ±O.O.lg) and mixed group consisting of both small and large prey were used for 14 days of trial. Predatory behavior was classified as targeting, driving, catching, handling, resting and next attempt of catching prey. After introduction of prey into the aquarium predators followed the movement of preys by eye movements and tried to target smaller one first. The predator grasped the head of the prey by its jaws by a drive and engulfed it wholly into the mouth. The average handling time (time taken to manipulate and swallow prey from capture to ceasation of pharyngeal movement) was 42±2 sec and 47±2 sec for small and large prey respectively. N. nandus were ingested more small prey than large prey though the size classes were equally available in case of mixed prey used. Although the prey consumption was higher in number when small prey were ingested but in weight the consumption was higher when ingested large size of prey. The study indicated that N. nandus, ingested more small prey and grasped the headfirst.

A new genus and two new species are described from the Pearl River drainage in Guangxi Province, South China. Hongshuia, new genus, can be distinguished from all other Asian genera of the Labeonini by having a lower lip with its median lobe modified into a round, fleshy plate peripherally greatly protruded so as to form a ring-like fold that is posteromedially continuous with the mental region, and centrally sunken so as to form a round, flat, fleshy pad. This genus is distinct from all other Asian labeonine genera of the Garrina except for one newly described species of Parasinilabeo ( P. longibarbus), Pseudocrossocheilus, and Sinocrossocheilus, in the presence of well-developed maxillary barbels. Hongshuia differs from the above three genera in the lower lip morphology, and further from both Pseudocrossocheilus and Qianlabeo in the number of pharyngeal tooth rows and from Sinocrossocheilus in the colour pattern. Two new species, H. banmo and H. paoli, differ in the distribution density and degree of development of papillae on the rostral fold, depth of indentations on the distal edge of the rostral fold, presence or absence of papillae on the lower lip, size and shape of tubercles on the tip of the snout and anterior portion of the lachrymal, length, position and colour pattern of the dorsal fin, and snout length.

The mitochondrial 16S ribosomal RNA gene is sequenced from 24 ingroups taxa, including 18 species from Labeoninae grouped in 13 genera. Phylogenetic analyses are subjected to neighbor joining, maximum parsimony, maximum likelihood and Bayesian analyses. Phylogenetic analysis indicates that Labeoninae is basically a monophyletic assemblage and can be divided into 2 major clades: one comprising the genera Cirrhinus, Crossocheilus and Garra; and the other consisting of the genera Labeo, Sinilabeo, Osteochilus, Pseudoorossocheilus, Parasinilabeo. Ptychidio, Semilabeo, Pseudogyricheilus, Rectori and Discogobio. According to our present analysis, the features such as the presence of the adhesive disc on the chin and the pharyngeal teeth in 2 rows used in the traditional taxonomy of Labeoninae provide scarce information for phylogeny of labeonine fishes.

yLinichthys, new genus, is proposed for Barbodes laticeps Lin and Zhang, 1986. It can be separated from all other genera of Asian Barbinae in having a shallow depression extending along the median portion of the upper lip. Linichthys is superficially very similar to the type species of a monotypic Chinese cyprinid cavefish genus, Typhlobarbus nudiventris, in having an elongate and somewhat compressed body form, a truncate distal margin of the dorsal fin, and a soft and smooth last unbranched dorsal-fin ray, all of them being atypical for most of Asian barbins. The two genera differ in size of the eye and the number of rows of pharyngeal tooth. Although the same body form and/or structure of the dorsal fin are also shared with some species currently placed in other genera of Asian barbins, the recognition of Linichthys is warranted because of their marked differences in other characters. A redescription is also provided for the type species of the monotypic genus, Linichthys laticeps. It is known only from the upper and middle Zhu Jiang basin and the upper Chang Jiang basin in the city of Guiyang, Guizhou Province, South China.

Bighead carp is one of the most important freshwater filter-feeding fish of Chinese aquaculture. In recent decades, there have been a number of contradictory conclusions on the digestibility of algae by bighead carp based on the results from gut contents and digestive enzyme analysis or radiolabelled isotope techniques. Phytoplankton in the gut contents of bighead carp (cultured in a large net cage in Lake Donghu) were studied during March-May. In biomass, the dominant phytoplankters in the fore-gut contents were the centric diatom Cyclotella (average 54.5%, range 33.8-74.3%) and the dinoflagellate Cryptomonas (average 22.8%, range 6.8-55.8%). Phytoplankton in water samples were generally present in proportionate amounts in samples from the fore-guts of bighead carp. The size of most phytoplankton present in the intestine of bighead carp was between 8 and 20 mum in length. Bighead carp was also able to collect particles (as small as 5-6 mum) much smaller than their filtering net meshes, suggesting the importance of mucus in collecting small particles, Examination of the change in the integrity of Cyclotella on passage through the esophagus of bighead carp indicated that disruption of the algal cell walls is principally by the pharyngeal teeth, explaining the previous contradictory conclusions. The digestibility of algae by stomachless filter-feeding fish has been debated for decades. Results from gut contents and digestive enzyme analysis suggest poor utilization, while the measurement of food assimilation using radiolabeled isotope techniques indicate reasonable assimilation efficiency. Phytoplankton in the gut contents of the planktivorous filter-feeding silver carp were studied during March-May. The fish were cultured in a large net cage in a shallow, hypertrophic subtropical Chinese lake, Lake Donghu. In terms of biomass, the dominant phytoplankton in the fore-gut contents were Cyclotella (average 77.8%, range 69.7-93.5%) and Cryptomonas (average 9.57%, range 0-20.4%). The Ivlev's electivity index E of silver carp was much higher for Cyclotella (1.54) than for Cryptomonas (0.56). The majority of the phytoplankton found in the intestines of silver carp were 8-20 mu m, but they were also able to collect particles as small as 4.5-10 mu m, smaller than their filtering net meshes, suggesting that the secretion of mucus may play an important role in collecting such small particles. We conclude that disruption of cell walls is principally by the pharyngeal teeth, explaining previous contradictory conclusions. (C) 1999 Elsevier Science B.V. All rights reserved.

The endostyle of invertebrate chordates is a pharyngeal organ that is thought to be homologous with the follicular thyroid of vertebrates. Although thyroid-like features such as iodine-concentrating and peroxidase activities are located in the dorsolateral part of both ascidian and amphioxus endostyles, the structural organization and numbers of functional units are different. To estimate phylogenetic relationships of each functional zone with special reference to the evolution of the thyroid, we have investigated, in ascidian and amphioxus, the expression patterns of thyroid-related transcription factors such as TTF-2/MoxE4 and Pax2/5/8, as well as the forkhead transcription factors FoxQ1 and FoxA. Comparative gene expression analyses depicted an overall similarity between ascidians and amphioxus endostyles, while differences in expression patterns of these genes might be specifically related to the addition or elimination of a pair of glandular zones. Expressions of Ci-FoxE and BbFoxE4 suggest that the ancestral FoxE class might have been recruited for the formation of thyroid-like region in a possible common ancestor of chordates. Furthermore, coexpression of FoxE4, Pax2/5/8, and TPO in the dorsolateral part of both ascidian and amphioxus endostyles suggests that genetic basis of the thyroid function was already in place before the vertebrate lineage. PURPOSE: To compare the efficacy of paclitaxel versus doxorubicin given as single agents in first-line therapy of advanced breast cancer (primary end point, progression-free survival ¿PFS) and to explore the degree of cross-resistance between the two agents. PATIENTS AND METHODS: Three hundred thirty-one patients were randomized to receive either paclitaxel 200 mg/m(2), 3-hour infusion every 3 weeks, or doxorubicin 75 mg/m(2), intravenous bolus every 3 weeks. Seven courses were planned unless progression or unacceptable toxicity occurred before the seven courses were finished. Patients who progressed within the seven courses underwent early cross-over to the alternative drug, while a delayed cross-over was optional for the remainder of patients at the time of disease progression. RESULTS: Objective response in first-line therapy was significantly better (P =.003) for doxorubicin (response rate ¿RR, 41%) than for paclitaxel (RR, 25%), with doxorubicin achieving a longer median PFS (7.5 months for doxorubicin v 3.9 months for paclitaxel, P <.001). In second-line therapy, cross-over to doxorubicin (91 patients) and to paclitaxel (77 patients) gave response rates of 30% and 16%, respectively. The median survival durations of 18.3 months for doxorubicin and 15.6 months for paclitaxel were not significantly different (P =.38). The doxorubicin arm had greater toxicity, but this was counterbalanced by better symptom control. CONCLUSION: At the dosages and schedules used in the present study, doxorubicin achieves better disease and symptom control than paclitaxel in first-line treatment. Doxorubicin and paclitaxel are not totally cross-resistant, which supports further investigation of these drugs in combination or in sequence, both in advanced disease and in the adjuvant setting.

The aim of this study was to determine the maximum tolerated dose (MTD), dose-limiting toxicities (DLT), and potential activity of combined gemcitabine and continuous infusion 5-fluorouracil (5-FU) in metastatic breast cancer (MBC) patients that are resistant to anthracyclines or have been pretreated with both anthracyclines and taxanes. 15 patients with MBC were studied at three European Organization for Research and Treatment of Cancer centres. 13 patients had received both anthracylines and taxanes. Gemcitabine was given intravenously (i.v.) on days 1 and 8, and 5-FU as a continuous i.v. infusion on days 1 through to 14, both drugs given in a 21-day schedule at four different dose levels. Both were given at doses commonly used for the single agents for the last dose level (dose level 4). One of 6 patients at level 4 (gemcitabine 1200 mg/m2 and 5-FU 250 mg/m2/day) had a DLT, a grade 3 stomatitis and skin toxicity. One DLT, a grade 3 transaminase rise and thrombosis, occurred in a patient at level 2 (gemcitabine 1000 mg/m2 and 5-FU 200 mg/m2/day). Thus, the MTD was not reached. One partial response and four disease stabilisations were observed. Only 1 patient withdrew from the treatment due to toxicity. The MTD was not reached in the phase I study. The combination of gemcitabine and 5-FU is well tolerated at doses up to 1200 mg/m2 given on days 1 and 8 and 250 mg/m2/day given on days 1 through to 14, respectively, every 21 days. The clinical benefit rate (responses plus no change of at least 6 months) was 33% with one partial response, suggesting that MBC patients with prior anthracycline and taxane therapy may derive significant benefit from this combination with minimal toxicity.
