Aims To compare magnetic resonance imaging (MRI) of the pelvic floor musculature (PFM), bladder neck and urethral sphincter morphology under three conditions (rest, PFM maximal voluntary contraction (MVC), and straining) in older women with symptoms of stress (SUI) or mixed urinary incontinence (MUI) or without incontinence. Methods This 2008–2012 exploratory observational cohort study was conducted with community-dwelling women aged 60 and over. Sixty six women (22 per group), mean age of 67.7 ± 5.2 years, participated in the study. A 3 T MRI examination was conducted under three conditions: rest, PFM MVC, and straining. ANOVA or Kruskal–Wallis tests (data not normally distributed) were conducted, with Bonferroni correction, to compare anatomical measurements between groups. Results Women with MUI symptoms had a lower PFM resting position (M-Line P = 0.010 and PC/H-line angle P = 0.026) and lower pelvic organ support (urethrovesical junction height P = 0.013) than both continent and SUI women. Women with SUI symptoms were more likely to exhibit bladder neck funneling and a larger posterior urethrovesical angle at rest than both continent and MUI women (P = 0.026 and P = 0.008, respectively). There were no significant differences between groups on PFM MVC or straining. Conclusions Women with SUI and MUI symptoms present different morphological defects at rest. These observations emphasize the need to tailor UI interventions to specific pelvic floor defects and UI type in older women.

A violência é um problema que perturba a sociedade e a saúde individual e cole-tiva. A violência contra as mulheres é uma forma específica de maus tratos. Tem conse-quências para as mulheres que diretamente a sofrem, com repercussões sociofamiliares, acarretando marcas, mágoas e prejuízo grave nas esferas bio-psico-sociais. O CHBM-EPE tem em curso um programa de atendimento a mulheres vítimas de violência. Tor-na-se necessário realizar a sua revisão e atualização. No projeto atual, através de recolha de dados junto de provedores e beneficiárias de cuidados realizou-se o diagnóstico de situação, descrevem-se ambas as perspetivas e propõem-se medidas de melhoramento. O relatório aqui apresentado consiste na descrição da intervenção realizada. Os resulta-dos serão tidos em conta na revisão do Programa do CHBM-EPE contra a violência doméstica. Pretende-se a melhoria da qualidade dos cuidados; ABSTRAT: Violence Against Women. Review of the Institutional Project of the Hospital Barreiro - Montijo EPE. Violence is a problem that disrupts society and the individual and collective health. Violence against women is a specific form of ill-treatment. It has consequences for the female figures who directly suffer with social-familial repercussions, leading brands, hurts and serious injuries in the bio-psycho-social spheres. The CHBM-EPE has an ongoing program of assistance to women victims of violence. It is necessary to carry out its review and update. The current project, through data collection from providers and recipients of care will describe both perspectives. In the current project, through data collection from providers and recipients of care there was the diagnosis of the sit-uation, describes both perspectives and are proposed improvement measures. The report presented here is the description of the intervention performed. The results will be taken into account in the review of CHBM-EPE Program against domestic violence. It is in-tended to improve the quality of care.

This thesis consists of three papers on gender economics. Chapter 1 studies whether people dislike collaborating with someone who corrects them and whether the dislike is stronger when that person is a woman. Having a good relationship with colleagues is integral in group work, potentially leading to successful collaborations. However, there are occasions when people have to correct their colleagues. Using a quasi-laboratory experiment, I find that people, including those with high productivity, are less willing to collaborate with a person who has corrected them even if the correction improves group performance. In addition, I find suggestive evidence that men respond more negatively to women’s corrections, which is not driven by their beliefs about the difference in women’s and men’s abilities. These findings suggest that there is a behavioral bias in group work that distorts the optimal selection of talents and penalizes those who correct others’ mistakes, and the distortion may be stronger when women correct men. Chapter 2 studies the role of gender and cognitive skills on other peoples’ generosity. Using a novel experimental design where I exogenously vary gender and cognitive skills and sufficiently powered analysis, I find neither the two attributes nor their interactions affect other people’s generosity; if anything, people are more generous to women with high potential. Chapter 3 studies how increased legal tolerance toward domestic violence affects married women’s welfare using the domestic violence decriminalization bill introduced to the Russian national congress in 2016. Using difference-in-differences and flexibly controlling for macroeconomic shocks, I find that the bill decreased married women’s life satisfaction and increased depression, especially among those with a college degree and a highly qualified white-collar occupation supposed to be more sensitive to gender regressive atmosphere. Consistent with this conjecture, people became more tolerant toward general and domestic violence after the bill.

As experiências traumáticas precoces são um fator de risco preditivo de problemas psicopatológicos futuros. O Early Trauma Inventory (ETI) é um instrumento que avalia em indivíduos adultos experiências traumáticas ocorridas antes dos 18 anos de idade. Tal instrumento foi traduzido, transculturalmente adaptado e sua consistência interna foi avaliada. Vítimas de violência que preencheram os critérios de inclusão e exclusão foram submetidas a uma entrevista diagnóstica (SCID-I) e ao ETI. Foram incluídos 91 pacientes com o transtorno do estresse pós-traumático (TEPT). O alfa de Cronbach nos diferentes domínios variou de 0,595-0,793, e o escore total foi de 0,878. A maior parte dos itens nos vários domínios, com exceção do abuso emocional, apresentou índices de correlação interitem entre 0,51-0,99. A versão adaptada foi útil tanto na clínica quanto na pesquisa. Apresentou boa consistência interna e na correlação interitem. O ETI é um instrumento válido, com boa consistência para se avaliar a presença de história de traumas precoces em indivíduos adultos.

The literature examining purported relationships between ownership of companion animals and health is extremely heterogeneous. While much of the descriptive literature tends to support benefits of animal companionship, large scale, controlled research yields inconsistent and even contradictory findings on several issues, including associations with cardiovascular disease, mood and wellbeing. In an analysis of a large longitudinal data-set from the Australian Longitudinal Study on Women's Health, a prospective study of a nationally representative sample of more than 12,000 older women, difficulties with disentangling the effects of powerful demographic variables and age-related factors from the specific effects of pet ownership became apparent. Both cross-sectional and longitudinal analyses demonstrated that associations between mental and physical health and pet ownership as well as changes in pet ownership over time were weak and inconsistent compared to the large effects of living arrangements and other demographic variables. As sociodemographic variables relate strongly to both health and opportunities for pet ownership, this high level of confounding means it is unlikely that the impact of the specific variable of pet ownership on health can be ascertained from such studies. Rather, well-designed experimental studies, wherein the majority of such confounding variables can be held constant or at least somewhat controlled, are needed.

Eccentric exercise commonly results in muscle damage. The primary sequence of events leading to exercise-induced muscle damage is believed to involve initial mechanical disruption of sarcomeres, followed by impaired excitation-contraction coupling and calcium signaling, and finally, activation of calcium-sensitive degradation pathways. Muscle damage is characterized by ultrastructural changes to muscle architecture, increased muscle proteins and enzymes in the bloodstream, loss of muscular strength and range of motion and muscle soreness. The inflammatory response to exercise-induced muscle damage is characterized by leukocyte infiltration and production of pro-inflammatory cytokines within damaged muscle tissue, systemic release of leukocytes and cytokines, in addition to alterations in leukocyte receptor expression and functional activity. Current evidence suggests that inflammatory responses to muscle damage are dependent on the type of eccentric exercise, previous eccentric loading (repeated bouts), age and gender. Circulating neutrophil counts and systemic cytokine responses are greater after eccentric exercise using a large muscle mass (e.g. downhill running, eccentric cycling) than after other types of eccentric exercise involving a smaller muscle mass. After an initial bout of eccentric exercise, circulating leukocyte counts and cell surface receptor expression are attenuated. Leukocyte and cytokine responses to eccentric exercise are impaired in elderly individuals, while cellular infiltration into skeletal muscle is greater in human females than males after eccentric exercise. Whether alterations in intracellular calcium homeostasis influence inflammatory responses to muscle damage is uncertain. Furthermore, the effects of antioxidant supplements are variable, and the limited data available indicates that anti-inflammatory drugs largely have no influence on inflammatory responses to eccentric exercise. In this review, we compare local versus systemic inflammatory responses, and discuss some of the possible mechanisms regulating the inflammatory responses to exercise-induced muscle damage in humans.

Objective: High levels of domestic violence, mental illness, and alienation from authorities are associated with high incidence of children/adolescents living on the streets in low and middle income countries. The Equilibrium Project (Programa Equilibrio) was created to facilitate social reintegration through a virtual partnership between an academic psychiatric institute and highly vulnerable children and adolescents living on the streets, in group shelter with supervision, and in other high risk situations. Methods: Descriptive presentation of qualitative data and analysis of preliminary empirical data collected over a 24-month period. Results: Dialogue between academic professionals, street children, and city officials shaped The Equilibrium Project over the last 2 years. The program has progressively moved from a professional clinic setting to a community-based but protected activity center with recreational and professional services and an emphasis on linkage with social service agencies, city government and law enforcement officials in an academic research context. A total of 351 patients have been served of whom virtually all were neglected by their parents, 58.4% report physical or sexual abuse, 88.89% have been diagnosed with a psychiatric disorder, 40.4% drug use. After 2 years of operation, 63.5% (n = 223) successfully completed or continue in treatment and 34.8% (n = 122) were reunited with their families. Conclusions and Practice implications: Program development guided by consumer input led to a successful program offering professional services in a protected community setting that facilitates social reintegration by providing ""go between"" services integrating relationships between alienated consumers and formal psychiatric, pediatric, social service, and criminal justice systems. Apolipoprotein E (APOE) has been extensively studied as a risk factor for sporadic and late onset Alzheimer`s Disease (AD). APOE allele *3, the most frequent variant, is not associated to cognitive dysfunction (CD) or to increased AD risk. Differently, the *4 allele is a well-established risk factor for CD, while the *2 allele is associated with survival and longevity. CD is an important feature of Bipolar Disorder (BD) and recent data suggest that CD may be one of its endophenotypes, although controversial results exist. The aim of this research is to study the association of APOE genotype (APOE) and neurocognitive function in a sample of drug free young BD-type I patients. Sample consisted of 25 symptomatic BD (type I) patients (age 18-35 years old). They were submitted to an extensive neuropsychological evaluation and genotyped for APOE. Subjects with allele *2 presented better cognitive performance. The presence of allele *4 was associated with worse performance in a few executive tasks. APOE *3*3 was associated with overall severe dysfunction on cognitive performance. In young individuals with nontreated BD-type I, APOE may predict cognitive performance. Further and larger studies on APOE and cognition in BD are required to clarify whether APOE is a BD cognitive endophenotype.

Objective: To assess consent to record linkage, describe the characteristics of consenters and compare self-report versus Medicare records of general practitioner use. Method. Almost 40,000 women in the Australian Longitudinal Study on Women's Health were sent a request by mail for permission to link their Medicare records and survey data. Results: 19,700 women consented: 37% of young (18-23 years), 59% of mid-age (4550 years) and 53% of older women (70-75 years). Consenters tended to have higher levels of education and, among the older cohort, were in better health than nonconsenters. Women tended to under-report the number of visits to general practitioners. Conclusions: Record linkage of survey and Medicare data on a large scale is feasible. The linked data provide information on health and socio-economic status which are valuable for understanding health service utilisation. Implications: Linked records provide a powerful tool for health care research, particularly in longitudinal studies.

Osteoporosis is a major public health problem for older women and men. Parathyroid hormone (PTH) (1-34), which produces similar biological activity to the parent hormone, was tested in postmenopausal women with prior vertebral fractures. In 18 months, PTH (1-34) caused a dramatic 65% decrease in the risk of new vertebral fractures with a 10% increase in bone mineral density with few side effects. PTH (1-34) represents an exciting new therapy for this high risk group.

Background: Promoting physical activity is a public health priority, and changes in the environmental Contexts of adults' activity choices are believed to be crucial. However, of the factors associated with physical activity, environmental influences are among the least understood. Method: Using journal scans and computerized literature database searches, we identified 19 quantitative studies that assessed the relationships With physical activity behavior of perceived and objectively determined physical environment attributes. Findings were categorized into those examining five categories: accessibility of facilities, opportunities for activity, weather, safety, and aesthetic attributes. Results: Accessibility, opportunities, and aesthetic attributes had significant associations with physical activity, Weather and safety showed less-strong relationships. Where Studies pooled different categories to create composite variables, the associations were less likely to be statistically significant. Conclusions: Physical environment factors have consistent associations with physical activity behavior. Further development of ecologic and environmental models, together with behavior-specific and context-specific measurement strategies, should help in further understanding of these associations. Prospective Studies are required to identify possible causal relationships.

In the 1996 baseline surveys of the Australian Longitudinal Study of Women's Health (ALSWH), 36.1% of mid-age women (45-50) and 35% of older women (70-75) reported leaking urine. This study aimed to investigate (a) the range of self-management strategies used to deal with urinary incontinence (UI); (b) the reasons why many women who report leaking urine do not seek help for UI; and (c) the types of health professionals consulted and treatment provided, and perceptions of satisfaction with these, among a sample of women in each age group who reported leaking urine often' at baseline. Five hundred participants were randomly selected from women in each of the mid-age and older cohorts of the ALSWH who had reported leaking urine often in a previous survey. Details about UI (frequency, severity, and situations), self-management behaviors and help-seeking for UI, types of health professional consulted, recommended treatment for the problem, and satisfaction with the service provided by health care professionals and the outcomes of recommended treatments were sought through a self-report mailed follow-up survey. Most respondents had leaked urine in the last month (94% and 91% of mid-age and older women, respectively), and 72.2% and 73. 1% of mid-aged and older women, respectively, had sought help or advice about their UI. In both age groups, the likelihood of having sought help significantly increased with severity of incontinence. The most common reasons for not seeking help were that the women felt they could manage the problem themselves or they did not consider it to be a problem.. Many women in both cohorts had employed avoidance techniques in an attempt to prevent leaking urine, including reducing their liquid consumption, going to the toilet just in case, and rushing to the toilet the minute they felt the need to. Strategies are needed to inform women who experience UI of more effective management techniques and the possible health risks associated with commonly used avoidance behaviors. There may be a need to better publicize existing incontinence services and improve access to these services for women of all ages.

Aims: The objectives of the current study were (1) to measure type and severity of urinary leakage and (2) to investigate the association between these factors and age-related life events and conditions in three groups of Australian women with a history of urinary leakage. Methods: Five hundred participants were randomly selected from women in the young (aged 18-22 in 1996), mid-age (4550),and older (70-75) cohorts of the Australian Longitudinal Study of Women's Health (ALSWH) who had reported leaking urine in the 1996 baseline survey. Details about leaking urine (frequency, severity, situations) and associated factors (pregnancy, childbirth, body mass index [BMI]) were sought through self-report mailed follow-up surveys in 1999. Results & Conclusions: Response rates were 50, 83, and 80% in the young, mid-age, and older women, respectively. Most women confirmed that they had, leaked urine in the past month, and the majority of these were cases of mixed incontinence. Incontinence severity tended to increase with BMI for women of all ages, and increased severity scores were associated with having urine that burns or stings. Additional independent risk factors for increasing incontinence severity were heavy smoking in young women, past or present use of hormone replacement therapy in older women, and BMI and history of hysterectomy in mid-age women. A violência está presente a nível nacional e mundial no cotidiano de muitas famílias e se configura como um fenômeno de múltiplas determinações, podendo estar presente no cenário público e privado. Diante dos diversos tipos de violência presentes no espaço social, encontra-se a violência praticada pelo homem contra sua companheira. O presente estudo, com enfoque qualitativo, objetivou de maneira geral investigar de que forma o Centro de Referência Especializado da Assistência Social – Proteção e Atendimento Especializado a Famílias e Indivíduos (CREAS PAEFI “Adulto”) do município de Colatina, compõe a rede de apoio social e afetiva de mulheres/mães vítimas de violência física e/ou psicológica por parte de seus maridos/companheiros na percepção dos profissionais atuantes no serviço e, também, na percepção das próprias mulheres vítimas de violência usuárias do serviço. Para tanto, participaram da pesquisa 10 mulheres/mães usuárias do CREAS PAEFI “Adulto” do município de Colatina/ES, com faixa etária variando de 34 a 45 anos, que sofreram violência física e/ou psicológica por parte de seus maridos/companheiros e que possuíam pelo menos um filho. Também participaram da pesquisa 6 profissionais, com faixa etária variando de 25 a 38 anos, que faziam parte da equipe técnica do serviço. Os dados foram coletados por meio de entrevistas semi estruturadas realizadas individualmente em sala cedida pelo CREAS PAEFI “Adulto”. Para a organização dos dados utilizou-se a Análise de Conteúdo, sendo estes organizados em eixos temáticos e discutidos com base na Teoria Bioecológica do Desenvolvimento Humano de Urie Bronfenbrenner. Como resultados principais, pode-se notar que as interações estabelecidas entre os profissionais e as mulheres vítimas de violência usuárias do CREAS favoreceram processos proximais que promoveram mudanças positivas nas características pessoais das mulheres e reforçaram as relações que elas possuíam com suas famílias, principalmente com os filhos. Também foi verificado no macrossistema, que os papéis sociais estabelecidos pela sociedade para o homem e para a mulher estavam presentes nos discursos das usuárias do serviço, os quais naturalizavam a mulher como responsável pelos filhos, casa e marido, e o homem como o provedor da casa. A maternidade foi considerada por todas as usuárias como um fator importante em suas vidas e causou mudanças na dinâmica familiar com o companheiro.Verificou-se o comprometimento e envolvimento dos profissionais no atendimento às mulheres que se mostraram interessadas e engajadas com o atendimento e que tinham o objetivo de reconstruir suas vidas. A articulação do CREAS com a rede de atendimento à mulher vítima de violência no município de Colatina foi considerada satisfatória, principalmente levando em consideração o fato de que a cidade é pequena, o que facilita o contato entre os serviços. Algumas mulheres expuseram outras fontes de apoio que atuaram concomitantemente ao CREAS como, os amigos e a religião (Deus). Conclui-se que o CREAS PAEFI foi um serviço integrante da rede de apoio sócio afetiva das mulheres vítimas de violência que participaram do estudo, atuando como um microssistema significativo que favoreceu o desenvolvimento das mulheres frente à situação de violência vivenciada
