Introduction: Isocyanates are sensitizing chemicals used in various industries such as polyurethane foam production or paint-related purposes. Acting as haptens recognized by T-lymphocytes, they can cause allergic asthma and rarely hypersensitivity pneumonitis (HP). We aim to present a case report of acute HP due to hexamethylene diisocyanate (HDI) in a paint quality controller, a profession not generally considered at a high risk for work-related Isocyanates exposure. Case report: A 30-yr-old otherwise healthy female, light smoker working as a paint quality controller developed shortness of breath, malaise, sweating and chills at workplace six hours after handling a HDI-based hardener. Upon admission to emergency department, symptoms had progressed to severe respiratory failure. HR computer tomography (HRCT) showed bilateral ground-glass attenuation without pleural effusion. Rapid clinical and radiological improvement occurred under facial oxygen supply and systemic steroid therapy. Occupational medicine investigations revealed regular handling of HDI using latex gloves without respiratory protection. Assessment at workplace showed insufficient air renewal (1.5 times per hour), inadequate local aspiration and HDI exposure at levels of 1-4.25 ppb/m3 (Swiss Occupation Exposure Limit 5 ppb/m3). Biological monitoring after identical work procedure executed by a co-worker showed HDI exposure (5.1 micrograms hexamethylene diamine/g creatinine). Resumption of work was disadvised because of the life-threatening event. Discussion: The diagnosis of occupational HP is highly supported by classical findings on imagery and typical symptoms occurring within approved latency interval, associated with rapid clinical improvement. Although neither broncho-alveolar lavage nor specific IgG diagnosis (en route) were performed during the acute episode, various blood tests managed to rule out evidence of an infection or autoimmune disease. Other causes of HP seem unlikely as the patient did not have any recurrence of symptoms since absence from work. Workplace evaluation provided significant information on HDI exposure and allowed substantial recommendations to diminish Isocyanate exposure for the 20 still healthy laboratory co-workers. Although the entryways (air or skin) and precise mechanism of toxicity remain unclear, the present case clearly shows that Isocyanates may trigger acute HP in susceptible workers in a profession not generally considered at a high risk.

L'exposition aux bioaérosols (endotoxines, bactéries et spores de champignons en suspension dans l'air) et les problèmes de santé qui en découlent sont bien connus dans certains milieux professionnels (station d'épuration des eaux usées, élevages d'animaux, traitements des déchets organiques, travailleurs du bois, récolte et manutention des céréales, agriculture...). Cependant, les études avec investigations des concentrations aéroportées d'endotoxines et de micro-organismes se font très rares dans d'autres milieux professionnels à risque. Cette note d'actualité scientifique présente la synthèse de deux publications visant à quantifier les bioaérosols dans deux milieux professionnels rarement étudiés : les cabinets dentaires et les cultures maraîchères de concombres et tomates. Les dentistes ainsi que leurs assistants sont souvent bien informés sur les risques chimiques, les risques liés aux postures et les risques d'accidents avec exposition au sang. En revanche, le risque infectieux lié à une exposition aux bioaérosols est la plupart du temps méconnu. La flore bactérienne buccale est très riche et l'utilisation d'instruments tels que la fraise, le détartreur à ultrasons et le pistolet air-eau entraîne la dissémination aéroportée d'une grande quantité de bactéries. De plus, la conception des instruments générant un jet d'eau (diamètre des tubulures) favorise la formation de biofilm propice à l'adhérence et à la multiplication de micro-organismes à l'intérieur même des tuyaux. Ces micro-organismes se retrouvent alors en suspension dans l'air lors de l'utilisation de ces pistolets.L'inhalation de grandes quantités de ces micro-organismes pourrait alors engendrer des problèmes respiratoires (hypersensibilisation, asthme). De plus la présence de pathogènes, tels que les légionelles, les pseudomonas et les mycobactéries à croissance rapide, dans l'eau de ces unités dentaires peut aussi entraîner des risques infectieux pour les patients et pour les soignants. La production de tomates et concombres en Europe en 2008, était respectivement de 17 et 2 millions de tonnes dont 850 000 et 140 000 tonnes pour la France. La récolte, le tri et la mise en cageots ou en barquette individuelle de ces légumes génèrent de la poussière riche en matières organiques. Très peu d'études ont investigué l'exposition à ces poussières et aux endotoxines dans les serres de cultures intensives. Notamment, les données concernant les cultures de tomates sont inexistantes bien que ce légume soit un des plus cultivés en Europe. [Auteur]

Occupational exposures to wood dust have been associated with an elevated risk of sinonasal cancer (SNC). Wood dust is recognized as a human carcinogen but the specific cancer causative agent remains unknown. One possible explanation is a co-exposure to; wood dust and polycyclic aromatic hydrocarbons (PAHs). PAHs could be generated during incomplete combustion of wood due to heat created by use of power tools. To determine if PAHs are generated from wood during common wood working operations, PAH concentrations in wood dust samples collected in an experimental chamber operated under controlled conditions were analyzed. In addition, personal air samples from workers exposed to wood dust (n = 30) were collected. Wood dust was generated using three different power tools: vibrating sander, belt sander, and saw; and six wood materials: fir, Medium Density Fiberboard (MDF), beech, mahogany, oak and wood melamine. Monitoring of wood workers was carried out by means of personal sampler device during wood working operations. We measured 21 PAH concentrations in wood dust samples by capillary gas chromatography-ion trap mass spectrometry (GC-MS). Total PAH concentrations in wood dust varied greatly (0.24-7.95 ppm) with the lowest being in MDF dust and the highest in wood melamine dust. Personal PAH exposures were between 37.5-119.8 ng m(-3) during wood working operations. Our results suggest that PAH exposures are present during woodworking operations and hence could play a role in the mechanism of cancer induction related to wood dust exposure.

NanoImpactNet (NIN) is a multidisciplinary European Commission funded network on the environmental, health and safety (EHS) impact of nanomaterials. The 24 founding scientific institutes are leading European research groups active in the fields of nanosafety, nanorisk assessment and nanotoxicology. This 4−year project is the new focal point for information exchange within the research community. Contact with other stakeholders is vital and their needs are being surveyed. NIN is communicating with 100s of stakeholders: businesses; internet platforms; industry associations; regulators; policy makers; national ministries; international agencies; standard−setting bodies and NGOs concerned by labour rights, EHS or animal welfare. To improve this communication, internet research, a questionnaire distributed via partners and targeted phone calls were used to identify stakeholders' interests and needs. Knowledge gaps and the necessity for further data mentioned by representatives of all stakeholder groups in the targeted phone calls concerned: potential toxic and safety hazards of nanomaterials throughout their lifecycles; fate and persistence of nanoparticles in humans, animals and the environment; risks associated to nanoparticle exposure; participation in the preparation of nomenclature, standards, methodologies, protocols and benchmarks; development of best practice guidelines; voluntary schemes on responsibility; databases of materials, research topics and themes. Findings show that stakeholders and NIN researchers share very similar knowledge needs, and that open communication and free movement of knowledge will benefit both researchers and industry. Consequently NIN will encourage stakeholders to be active members. These survey findings will be used to improve NIN's communication tools to further build on interdisciplinary relationships towards a healthy future with nanotechnology.

Les sports équestres sont en plein développement en France où l'on compte plus d'un million de chevaux. Le nombre de cavaliers pratiquant en club ou chez eux est estimé à 600 000 et l'on dénombre plus de 68 000 emplois en France dans tous les domaines : agriculture, environnement, courses, loisirs, sports (www.haras-nationaux.fr). On trouve également dans les métiers du cheval les vétérinaires spécialisés ainsi que les maréchaux-ferrants. De plus, la France est un pays reconnu pour son activité d'élevage avec une production de chevaux de concours de très grande qualité.Récemment, plusieurs études relatant les expositions professionnelles à la poussière des écuries ont paru dans les journaux scientifiques. Les résultats montrent que la poussière issue des écuries contient, entre autres, des concentrations relativement élevées d'endotoxines (1) et de ?(1-3)-glucan (2) pouvant avoir des conséquences sur la santé respiratoire des travailleurs. En effet, des études précédentes ont montré que le fait de travailler ou de côtoyer des chevaux était associé avec une augmentation des risques de problèmes des voies respiratoires supérieures tels que des irritations du nez, de la toux (sèche et productive), de l'asthme, des bronchites chroniques et des épisodes d'" organic dust toxic syndrome (3) " (Gallagher et al., 2007 ; Mazan et al., 2009). OBJECTIVES: Skin notations are used as a hazard identification tool to flag chemicals associated with a potential risk related to transdermal penetration. The transparency and rigorousness of the skin notation assignment process have recently been questioned. We compared different approaches proposed as criteria for these notations as a starting point for improving and systematizing current practice. METHODS: In this study, skin notations, dermal acute lethal dose 50 in mammals (LD(50)s) and two dermal risk indices derived from previously published work were compared using the lists of Swiss maximum allowable concentrations (MACs) and threshold limit values (TLVs) from the American Conference of Governmental Industrial Hygienists (ACGIH). The indices were both based on quantitative structure-activity relationship (QSAR) estimation of transdermal fluxes. One index compared the cumulative dose received through skin given specific exposure surface and duration to that received through lungs following inhalation 8 h at the MAC or TLV. The other index estimated the blood level increase caused by adding skin exposure to the inhalation route at kinetic steady state. Dermal-to-other route ratios of LD(50) were calculated as secondary indices of dermal penetrability. RESULTS: The working data set included 364 substances. Depending on the subdataset, agreement between the Swiss and ACGIH skin notations varied between 82 and 87%. Chemicals with a skin notation were more likely to have higher dermal risk indices and lower dermal LD(50) than chemicals without a notation (probabilities between 60 and 70%). The risk indices, based on cumulative dose and kinetic steady state, respectively, appeared proportional up to a constant independent of chemical-specific properties. They agreed well with dermal LD(50)s (Spearman correlation coefficients -0.42 to -0.43). Dermal-to-other routes LD(50) ratios were moderately associated with QSAR-based transdermal fluxes (Spearman correlation coefficients -0.2 to -0.3). CONCLUSIONS: The plausible but variable relationship between current skin notations and the different approaches tested confirm the need to improve current skin notations. QSAR-based risk indices and dermal toxicity data might be successfully integrated in a systematic alternative to current skin notations for detecting chemicals associated with potential dermal risk in the workplace. Late-onset asthma is a common disease, with characteristics that differentiate it from childhood asthma persisting in adults, including a heterogeneity of underlying mechanism, a correlation with occupational exposure and the tendency to chronicize. Inadequate perception of the disease importance and difficulties in diagnosis, due to the association to COPD, the broad differential diagnosis spectrum and an atypical clinical manifestation, make this disease entity underdiagnosed and under-treated, with consequences for morbidity and mortality. The treatment of adults with asthma follows the usual guidelines, but must take into account an occasional poor therapeutic response and the risk of side effects in polymorbid patients.

Les tumeurs malignes des cavités naso-sinusiennes (CNS) représentent environ 3% des cancers ORL. La poussière de bois est reconnue comme cancérigène pour l'homme (IARC, classe 1A) et la maladie professionnelle formellement identifiée est le CNS ou le cancer de l'ethmoïde. L'adénocarcinome des CNS est une maladie professionnelle admise chez certains travailleurs spécialisés tels les menuisiers et les ébénistes. Selon les enquêtes WOODEX dans les 25 états membres de UE en 2006, environ 3.6 millions de travailleurs sont exposés par voie respiratoire à la poussière de bois, soit 2% de la population active. En France, 307 000 travailleurs sont exposés, et il y a environ 200 cas de cancers sino-nasaux reconnus comme maladie professionnelle par année. La forte proportion de travailleurs du bois développant un adénocarcinome des CNS a suggéré diverses pistes, tel que le tanin pour les bois durs, le formaldéhyde pour les contre-plaqués et le benzo(a)pyrène produit par le bois surchauffé. Il est reconnu que le tanin ne provoque pas de cancer dans l'exposition à la poussière de thé. Le formaldéhyde est un irritant mais il est aussi classé cancérogène. La piste des hydrocarbures aromatiques polycycliques (HAP) cancérogènes provoqués par le bois surchauffé est séduisante. Nous avons étudié leur teneur en fonction des opérations pratiquées sur le bois en cabine d'expérimentation. Les matériaux testés sont du sapin brut, du chêne et chêne imprégné de polyuréthane. La poussière de bois contient des HAP au niveau du µg/g ou ppm. Le bois imprégné de vernis PU produit 100 fois plus de HAP que le bois brut lors des opérations de ponçage.

BACKGROUND: carbon nanotubes (CNT) can have adverse effects on health. Therefore, minimizing the risk associated with CNT exposure is of crucial importance. The aim of this work was to evaluate if coating multi-walled CNT (MWCNT) with polymers could modify their toxicity, thus representing a useful strategy to decrease adverse health effects of CNT. We used industrially-produced MWCNT uncoated (NT1) or coated (50/50 wt%) with acid-based (NT2) or polystyrene-based (NT3) polymer, and exposed murine macrophages (RAW 264.7 cell line) or Balb/c mice by intratracheal administration. Biological experiments were performed both in vitro and in vivo, examining time- and dose-dependent effects of CNT, in terms of cytotoxicity, expression of genes and proteins related to oxidative stress, inflammation and tissue remodeling, cell and lung tissue morphology (optical and transmission electron microscopy), and bronchoalveolar lavage fluid content analysis.RESULTS: extensive physico-chemical characterization of MWCNT was performed, and showed, although similar dimensions for the 3 MWCNT, a much smaller specific surface area for NT2 and NT3 as compared to NT1 (54.1, 34 and 227.54 m(2)/g respectively), along with different surface characteristics. MWCNT-induced cytotoxicity, oxidative stress, and inflammation were increased by acid-based and decreased by polystyrene-based polymer coating both in vitro in murine macrophages and in vivo in lung of mice monitored for 6 months.CONCLUSIONS: these results demonstrate that coating CNT with polymers, without affecting their intrinsic structure, may constitute a useful strategy for decreasing CNT toxicity, and may hold promise for improving occupational safety and that of general the user.

These findings suggested that stakeholders and NIN researchers share very similar knowledge needs, and that open communication and free movement of knowledge will benefit both researchers and industry. Subsequently a workshop was organised by NIN focused on building a sustainable multi-stakeholder dialogue. Specific questions were asked to different stakeholder groups to encourage discussions and open communication. 1. What information do stakeholders need from researchers and why? The discussions about this question confirmed the needs identified in the targeted phone calls. 2. How to communicate information? While it was agreed that reporting should be enhanced, commercial confidentiality and economic competition were identified as major obstacles. It was recognised that expertise was needed in the areas of commercial law and economics for a wellinformed treatment of this communication issue. 3. Can engineered nanomaterials be used safely? The idea that nanomaterials are probably safe because some of them have been produced 'for a long time', was questioned, since many materials in common use have been proved to be unsafe. The question of safety is also about whether the public has confidence. New legislation like REACH could help with this issue. Hazards do not materialise if exposure can be avoided or at least significantly reduced. Thus, there is a need for information on what can be regarded as acceptable levels of exposure. Finally, it was noted that there is no such thing as a perfectly safe material but only boundaries. At this moment we do not know where these boundaries lie. The matter of labelling of products containing nanomaterials was raised, as in the public mind safety and labelling are connected. This may need to be addressed since the issue of nanomaterials in food, drink and food packaging may be the first safety issue to attract public and media attention, and this may have an impact on 'nanotechnology as a whole. 4. Do we need more or other regulation? Any decision making process should accommodate the changing level of uncertainty. To address the uncertainties, adaptations of frameworks such as REACH may be indicated for nanomaterials. Regulation is often needed even if voluntary measures are welcome because it mitigates the effects of competition between industries. Data cannot be collected on voluntary bases for example. NIN will continue with an active stakeholder dialogue to further build on interdisciplinary relationships towards a healthy future with nanotechnology.

L'exposition aux bactéries résistantes aux antibiotiques, en particulier à Staphylococcus aureus résistant à la méthicilline (SARM (1)) sur le lieu de travail, a été montrée comme étant un facteur de risque pour la santé des opérateurs, la fréquence des contacts avec cette bactérie augmentant la probabilité d'en devenir porteur. En plus du fait que les SARM augmentent d'un facteur 4 le risque d'infection chez le porteur, le choix du traitement antibiotique en cas d'infection est fortement limité. C'est pourquoi il est important d'identifier les environnements de travail et les conditions qui favorisent la transmission de cette bactérie de l'animal à l'Homme. La résistance à la méthicilline est conférée au S. aureus par un élément génétique mobile, appelé « staphylococcal cassette chromosome » mec (SCCmec), qui contient le gène de résistance à la méthicilline, mecA. SCCmec a cinq formes (I, II, III, IV and V) qui ont été acquises et intégrées dans le génome de S. aureus lors d'événements indépendants de transfert horizontal. Certaines de ces lignées spécifiquement associées au bétail traité aux antibiotiques (tel que le complexe clonal 398, CC398 (2)), peuvent également coloniser le nez humain. Ainsi, la colonisation nasale ou contamination a été constatée chez 23 à 86 % des agriculteurs et vétérinaires ayant un contact direct avec des porcs, ainsi que chez un à cinq pour cent des personnes ayant une exposition indirecte (par exemple les membres de la famille d'agriculteurs, les visiteurs de la ferme). La pathogénicité du SARM CC398 pour l'Homme a été documentée dans une série de rapports décrivant des cas d'endocardite, d'otomastoïdite et de pneumonie. En outre, le SARM CC398 a été introduit dans des structures de santé (hôpitaux, cliniques, etc.) situées principalement dans les zones d'élevage à forte densité. Si les porcs sont des vecteurs bien connus de transmission de CC398 à l'Homme, d'autres animaux peuvent l'être également, tels que les dindes en Allemagne, comme illustré par le premier article cité dans cette note. Par ailleurs, la propagation de ces souches résistantes aux antibiotiques est inquiétante. Le deuxième article de cette note révèle l'apparition de souches de CC398 dans le lait de vache au Royaume-Uni pays où, jusqu'alors, la surveillance n'en avait pas détecté.
