A total of 73 isolates (57 Enterobacter cloacae and 16 Enterobacter agglomerans), recovered during an outbreak of bacteremia in the Campinas area, São Paulo, Brazil, were studied. Of these isolates, 61 were from parenteral nutrition solutions, 9 from blood cultures, 2 from a sealed bottle of parenteral nutrition solution, and one was of unknown origin. Of the 57 E. cloacae isolates, 54 were biotype 26, two were biotype 66 and one was non-typable. Of 39 E. cloacae isolates submitted to ribotyping, 87.2% showed the same banding pattern after cleavage with EcoRI and BamHI. No important differences were observed in the antimicrobial susceptibility patterns among E. cloacae isolates exhibiting the same biotype, serotype and ribotype. All E. agglomerans isolates, irrespective of their origin, showed same patterns when cleaved with EcoRI and BamHI. The results of this investigation suggest an intrinsic contamination of parenteral nutrition solutions and incriminate these products as a vehicle of infection in this outbreak.

The prevalence of TT virus (TTV) infection was investigated by Polymerase Chain Reaction (PCR) in low- (blood donors and healthy children/adolescents) and high-risk (hemophiliacs) groups from São Paulo, Brazil. Primers based on the untranslated region (UTR) of the viral genome proved to be much more ubiquitous, leading to much higher frequencies for both groups ( > or = 81%) than the earlier N22-PCR directed to the open reading frame 1 (blood donors, 5.5%, and hemophiliacs, 42.3%). The UTR-PCR also revealed an interesting profile for healthy children/adolescents: very high prevalence at the early years and significant decrease in male teenagers. The N22-PCR, in turn, demonstrated higher frequency in hemophiliacs treated with fresh blood products (58%), than in those treated with virus-inactivated clotting factors (9.4%) and blood donors (5.5%).

A two year-old female child was admitted at the Pediatric Intensive Care Unit in a septic shock associated with a lymphoproliferative syndrome, with history of fever, adynamia and weight loss during the last two months. On admission, the main clinical and laboratory manifestations were: pallor, jaundice, disseminated enlarged lymph nodes, hepatosplenomegaly, crusted warts on face, anemia, eosinophilia, thrombocytopenia, increased direct and indirect bilirubin, alkaline phosphatase, and gammaglutamyl transpeptidase. A parenteral administration of fluids, dobutamine and mechanical ventilation was started, without improvement of the clinical conditions. A direct examination of exsudate collected from cervical lymph node revealed numerous oval-to-around cells with multiple budding, like a "pilot wheel" cell, suggesting Paracoccidioides brasiliensis. Even though treatment with intravenous sulfamethoxazole-trimethoprine was soon started, the child died 36 hours after hospital admission. Disseminated paracoccidioidomycosis was confirmed in the autopsy. This is the youngest case of paracoccidioidomycosis in children reported in the literature.

Fungal infections are an important cause of morbidity and mortality in patients with acute leukemia (AL). Candidemia, once rare, is now a common nosocomial infection because of the intensity of chemotherapy, prolonged neutropenia, administration of broad-spectrum antibiotics and use of central venous catheters (CVC). We retrospectively identified patients treated for AL from 6/86 to 6/95 who also had candidemia. We describe 28 patients (incidence 6.3%) with a median age of 39 years, 24 of whom were on remission induction and 4 on postremission chemotherapy. All patients had CVC and empiric antimicrobial therapy, 4 had been given prophylactic antifungal drugs, and 2 had parenteral nutrition. Neutropenia was profound (median leukocyte nadir 200/microliters, median duration 19 days). Candida was isolated in blood cultures 10 days (median) after the start of neutropenia. The clinical presentation included fever (100%), respiratory symptoms (71.4%), skin lesions (39.2%) and septic shock (17.8%). Amphotericin B was given to 17 patients and liposomal amphotericin to 5 patients. Infection resolved in 18 patients (64.2%). 10 of whom were in complete remission. Mortality from candidemia was 17.8% (5/28). In conclusion, fungal infections are responsible for death in a significant number of patients. In our series treatment success was related to its rapid onset and to the recovery of neutropenia.

A previously healthy 75-year-old white male dentist presented with a 6-month history of low-back pain treated with chronic steroid therapy had a Nocardia farcinica infection diagnosed by aspirate of thyroid abscess and six blood cultures. Despite the treatment with parenteral combination of trimethoprim/sulfamethoxazole, the patient failed to respond and died after two days of therapy. Autopsy revealed disseminated nocardiosis, involving lungs with pleural purulent exudate in both sides, heart, thyroid, kidneys, brain, bones, and lumbosacral soft tissue with destruction of L2-L4.

The aim of this study was to perform a retrospective analysis of cases of candidemia in a Brazilian hospital in the city of Fortaleza, Ceará. A total of 50 blood cultures were analyzed from 40 candidemic patients. The mycological diagnosis was based on the phenotypical analysis and the patients' data were recorded in appropriate files. The most frequent species were Candida parapsilosis (n = 18), followed by C. albicans (n = 14), C. tropicalis (n = 8), C. guillermondii (n = 6), C. glabrata (n = 2), and Candida spp. (n = 2). A detailed descriptive study was undertaken with 21 patients whose medical records were complete. The candidemia episodes occurred in eight male patients and 13 female patients. The most representative risk factors implicated in candidemia were prior antibiotic therapy, central venous catheters, parenteral nutrition, gastric probes and mechanical ventilation. Death occurred in 13 of the 21-candidemic patients. This study demonstrated the emergence of candidemia caused by C. parapsilosis in a Brazilian hospital in the city of Fortaleza, Ceará.

Subcutaneous candidal abscess is a very rare infection even in immunocompromised patients. Some cases are reported when breakdown in the skin occurs, as bacterial cellulites or abscess, iatrogenic procedures, trauma and parenteral substance abuse. We describe a case of Candida albicans subcutaneous abscess without fungemia, which can be associated with central venous catheter.

The complex interaction between hepatitis C virus infection, iron homeostasis and the response to antiviral treatment remains controversial. The aim of this study was to evaluate the influence of hepatic iron concentration (HIC) on the sustained virological response (SVR) to antiviral therapy in patients with chronic hepatitis C. A total of 50 patients who underwent pretreatment liver biopsy with assessment of HIC by graphite furnace atomic absorption spectroscopy and were subsequently submitted to antiviral treatment with interferon/peginterferon and ribavirin were included in the study. Patients with alcoholism, history of multiple blood transfusion, chronic kidney disease, hemolytic anemia and parenteral iron therapy were excluded. The iron related markers and HIC were compared between those who achieved an SVR and non-responders (NR) patients. The mean age was 45.7 years and the proportion of patients' gender was not different between SVR and NR patients. The median serum iron was 138 and 134 µg/dL (p = 0.9), the median serum ferritin was 152.5 and 179.5 ng/mL (p = 0.87) and the median HIC was 9.9 and 8.2 µmol/g dry tissue (p = 0.51), for SVR and NR patients, respectively. Thus, hepatic iron concentration, determined by a reliable quantitative method, was not a negative predictive factor of SVR in patients with chronic hepatitis C presenting mild to moderate hepatic iron accumulation.

A propósito do estudo “Early high calcium and phosphorus intake by parenteral nutrition prevents short-term bone strength decline in preterm infants”, de colaboração entre a UCIN do HDE, os Serviços Farmacêuticos do HDE e estudantes da Licenciatura de Dietética e Nutrição da ESTeSL, foi realçada a vantagem da sinergia multiprofissional multicêntrica em investigação clínica: a investigação foi possível sem depender de financiamento, dado que o dispendioso equipamento de ultrassonografia quantitativa pertence à ESTeSL; a UCIN beneficiou de investigação com a colaboração de investigadores externos (estudantes ESTeSL) dedicados em exclusividade ao projecto; estes, por sua vez, beneficiaram da aprendizagem em investigação de qualidade; o estudo ao ser publicado em revista internacional indexada (J Pediatr Gastroenterol Nutr.), proporcionou o enriquecimento curricular às instituições (HDE e ESTeSL) e aos investigadores (estudantes, médicos e farmacêutica) envolvidos.

SUMMARYConsidered to be an emerging endemic mycosis in Latin America, paracoccidioidomycosis is characterized by a chronic course and involvement of multiple organs in immunocompromised hosts. Infection sequelae are mainly related to pulmonary and adrenal insufficiency. The host-parasite interaction results in different expressions of the immune response depending on parasite pathogenicity, fungal load and genetic characteristics of the host. A few controlled and case series reports have shown that azoles and fast-acting sulfa derivatives are useful treatment alternatives in milder forms of the disease. For moderate/severe cases, more prolonged treatments or even parenteral routes are required especially when there is involvement of the digestive tract mucosa, resulting in poor drug absorption. Although comparative studies have reported that shorter treatment regimens with itraconazole are able to induce cure in chronically-infected patients, there are still treatment challenges such as the need for more controlled studies involving acute cases, the search for new drugs and combinations, and the search for compounds capable of modulating the immune response in severe cases as well as the paradoxical reactions.

Objectivo: Avaliar o impacto de diferentes estratégias de suporte nutricional na composição corporal de recém-nascidos de baixo peso (RNBP), por intermédio não só do peso, mas também das áreas muscular braquial (AMB) e adiposa braquial (AAB). Indivíduos e métodos: Comparou-se a média do dia de recuperação do peso, da AMB e da AAB entre dois grupos de RNBP assistidos em períodos distintos, com suportes nutricionais diferentes — grupo I (n=10): 1991 versus grupo II (n=24): 1995/96. Os recém-nascidos foram incluidos de modo consecutivo e avaliados prospectivamente sob o ponto de vista antropométrico. Resultados: Não houve diferenças significativas da média das idades gestacionais e dos pesos de nascimento entre os dois grupos. Os recém-nascidos do grupo II, comparativamente com os do grupo I, receberam provisões mais precoces de energia, lípidos e proteínas através da alimentação parentérica. Não se observaram diferenças significativas no dia de recuperação do peso e da AMB entre os grupos. No entanto, os RN do grupo II recuperaram mais precocemente a AAB (dia 6,9 ± 2,5 vs dia 12,6 ± 5,1) (p<0,01), após um período inicial de declínio dos respectivos valores. Conclusões: O atraso na adequada provisão de energia e proteínas pode ter um impacto negativo na reserva lipídica, não detectada pela observação isolada da evolução ponderai. A medição das áreas braquiais representa um método simples e não invasivo de avaliação indirecta da composição corporal dos RNBP.

Forty patients with a diagnosis of snake bite were studied at the Infectious and Parasitic Disease Service of the Faculty of Medicine of Botucatu. Thirty were males and 10 females, ranging in age from 16 to 70 years. All were farm laborers and 35 of them were bitten in the lower limbs. Two of the 9 patients seen more than 6 hours after the bite died. The low mortality rate (5%) observed could be explained by the early care provided, by the use of appropriate doses of anti-crotalus serum, parenteral hydration, urine alkalinization with sodium bicarbonate and induction of osmotic diuresis with a mannitol solution. Anatomopathological examination of one of the patients who died revealed extensive hepatic necrosis. The authors discuss the possibility of the effect of a factor of snake venom in the genesis of hepatic necrosis and in the increased transaminase levels.

La modification del biotopo perihabitacional consiste en crear alrededor de la vivienda humana, rural, selvática o suburbuna, un espado perimetral limpio, totalmente libre de malezas y chaparral, despojado de nidos, madrigueras o refúgios de animales silvestres y de habitáculos de animales domésticos; de suficiente magnitud para evitar que tras desinsectación, queden focos peridomiciliarios de proliferation de triatomineos, vectores del Trypanosoma cruzi, focos inmediatos y habituales de reinfestación domiciliaria por estos redúvidos hematófagos. Los trabajos de rociado entomicida y aún el mejoramiento de la vivienda se reducen extremadamente si no son complementadas con la modificación del biotopo perihabitacional. Se expone una experiencia piloto sobre un barrio de la ciudad Villa Carlos Paz, provinda Córdoba, Argentina, que en 2 anos de aplicación permitió verificar la desaparición de vinchucas en un area de baja desindad de vectores y hospedadores.

Se llevó a cabo un estúdio epidemiológicopara obtener unpanorama general de la transmisión doméstica del Trypanosoma cruzi en áreas rurales del Departamento San Miguel, Conientes, Argentina. Se investigaron 100 viviendas precarias, 50,0% de las mismas resultó infestado por Triatoma infestans y 1,0% por T. sórdida, citãndose por primera vez para la provinda de Comentes la colonización domiciliaria de esta especie. El 23,1% de T. infestans estaba infectado por T. cruzi. La seroprevalencia de 388 pacientes analizados por hemaglutinacíôn indirecta e inmunofluorescencia indirecta fue 23,4%, destacándose el alto índice (12,9%) constatado en menores de 10 anos. Losporcentajes de seropositividad aumentaron con la edad, alcanzándose 50,0% de infectados entre los 31-40 anos. La infestación doméstica por T. infestans, la prevalencia humana de seropositivos al T. cruzi y las condiciones deprecariedad en que se desenvuelve la vida de los pobladores revelan la vigência de la endemia chagãsica en el área estudiada.
