Neste trabalho descreve-se o aparelho condutor, do testículo até o ductus ejaculatorius, incluindo as glândulas anexas, do macho de Triatoma infestans. O vas deferens compõem-se de três regiões: a) parte proximal do vas deferens; b) vesícula seminalis; c) parte distal do vas deferens com uma região glândular no ponto de saída da vesícula seminalis. As partes finais do vas deferens desembocam nos lados internos de dois ramos terminais do ductus ejaculatorius. O sistema das glândulas anexas consta de 4 mesadênias. Estas são glândulas vesiculares das quais duas são ragiócrinas e duas lipócrinas. A terceira e a quarta glândula possuem a mesma formação e função, enquanto que a primeira se difere profundamente da segunda. As secreções das glândulas misturam-se num hilo de onde o líquido passa ao ductus glandularum que o conduz ao ductus ejeculatorius. Êste possui nos seus ramos terminais uma glândula mesodérmica de natureza ragiócrina (mesadênia modificada em posição extremamente distal) e uma origem ectedérmica (ectadênia modificada em posição extremamente proximal). As secreções são expulsas das vesículas glandulares por contração da musculatura das suas paredes. O transporte dos líquidos misturados, através do ductus glandularum, verifica-se por ondas peristálticas da musculatura da membrana peritoneal do próprio ducto. As glândulas não possuem válvulas. Um refluxo das secreções é evitado pelo turgor das células epiteliais dos canais condutores. O esperma, ao entrar no ductus ejaculatorius, recebe uma mistura de 5 diferentes secreções, na qual o mesmo diluido, formando, finalmente, uma suspensão. El propósito de este papel es describir los principales instrumentos económicos que pueden utilizarse en la política medio ambiental. Se examinan críticamente las soluciones clásicas, sin olvidar las espurias. Se introduce el tiempo en el análisis, se examina la importancia de la tasa de descuento en el cálculo económico y se presenta el concepto de economías de duración, para estudiar el conflicto entre los intereses individuales y el social y generar propuestas de solución. Se presta especial atención al conflicto entre los intereses de las generaciones presentes y las futuras, apuntando algunas vías para solventarlo.

Os autores, continuando com o estudo dos Procamallanus brasileiros, propõem Procamallanus (S.) solaini como espécie nova para o gênero. Redescrevem Procamallanus (S.) iheringi Travassos, Artigas & Pereira, 1928, depois de terem tido acesso ao material tipo e elucidado dúvidas a respeito de sua morfologia. Para Procamallanus (S.) inopinatus Travassos, Artigas & Pereira, 1928, tiveram a descrição de Pinto & Noronha, 1972 adaptada. De Procamallanus (S.) amarali Vas & Pereira, 1934, tornam conhecida a fêmea da espécie e fazem uma breve redescrição do macho. Listam amostras de Procamallanus sp. que não conduziram a um diagnóstico preciso, devido ao seu precário estado de conservação. Foram examinadas amostras de helmintos que fazem parte da Coleção Helmintológica do Instituto Oswaldo Cruz.

Conscientes de las complejidades que presenta la organización administrativa local y la diversidad que caracteriza a los Ayuntamientos de España, y lejos de querer establecer fórmulas mágicas o generalizar soluciones, con esta Guía se pretende aportar ideas útiles mediante propuestas y algunos ejemplos; con ella se quiere ayudar a promover distintas intervenciones a favor de la accesibilidad en los municipios. En este sentido, esta Guía pretende ser soporte para desarrollar una nueva etapa de intervención municipal en accesibilidad, que vaya más allá de la mera supresión de barreras y se prevenga su creación. Las medidas que se desarrollan son posibles vías para que los gobiernos locales puedan gestionar y promocionar de forma eficaz la accesibilidad en el municipio y así establecer una base sobre la cual todos los ciudadanos puedan desarrollar sus libertades individuales plenamente.

Suele darse por sentado que la valoración de los impactos que recaen sobre las generaciones presentes no comporta dificultad alguna. Sin embargo esto no es cierto. Es más, no se dispone de una metodología ampliamente aceptada para contabilizar dichos impactos. Algunos de los aspectos más controvertidos son: la elección del valor apropiado para la tasa de descuento, las unidades -físicas o monetarias- en que deben expresarse los flujos y la valoración de los bienes tangibles e intangibles. Cuando existen impactos a muy largo plazo surge el problema de valorar los costes y beneficios que afectan a las generaciones futuras mediante, por ejemplo, una tasa de descuento intergeneracional. Incluso en este último caso estaría dominando la perspectiva de las generaciones presentes, que actuarían como si poseyeran la totalidad de los derechos de propiedad sobre todos los recursos. Por lo tanto es necesario incorporar el requisito de sostenibilidad en el análisis. En este trabajo se examinan algunos de estos aspectos y se proponen vías de solución.

En la actualidad, la gran cantidad de aplicaciones que surgen dentro del ámbito de la radiofrecuencia hacen que el desarrollo de dispositivos dentro de este campo sea constante. Estos dispositivos cada vez requieren mayor potencia para frecuencias de trabajo elevadas, lo que sugiere abrir vías de investigación sobre dispositivos de potencia que ofrezcan los resultados deseados para altas frecuencias de operación (GHz). Dentro de este ámbito, el objetivo principal de este proyecto es el de realizar un estudio sobre este tipo de dispositivos, siendo el transistor LDMOS el candidato elegido para tal efecto, debido a su buen comportamiento en frecuencia para tensiones elevadas de funcionamiento.

A description of Physa cubensis Pfeiffer, 1839, based on 15 speciments collected in Havana, Cuba, is presented. The shell, measuring 9.0 x 4,8mm to 12.3 x 6.4mm, is ovate-oblong, thin, diaphanous, horncolored, shining. Spire elevated, broadly conical; protoconch distinct, roundish, reddish-brown. About five moderately shouldered, roundly convex whorls, penultimate whorl expanded; spiral striation subobsolete; growth line faint on the intermediate whorls, clearly visible on the body whorl, crowded here and there. Suture well impressed. Aperture elongated 2.05 - 2.67 (mean 2.27) times as long as the remaining length of the shell, narrow obovulate-lunate; upper half acute-angled, lower half oval, narrowly rounded at the base; outer lip sharp, inner lip completely closing the umbilical region; a thick callus on the parietal wall; columellar plait well marked. Ratios: shell width/shell length - 0.52-0.61 (mean 0.55); spire length/shell length = 0.27 - 0.33 (mean 0.31); aperture length/shell length = 0.67 - 0.73 (mean 0.69). Oral lappets laterally mucronate; foot spatulate with acuminate tail. Mantle relection with 6 - 8 short triangular dentations in the right lobe (columellar side) and 4 - 6 in the left lobe (near the pneumostome). Renal tube tightly folded into a zigzag course. Ovotestis, ovispermiduct, seminal vesicle, oviduct, nidamental gland, uterus and vagina as in Physa marmorata (see Paraense, 1986, Mem. Inst. Oswaldo Cruz, 81: 459-469). Spermathecal body egg-shaped or pear-shaped; spermathecal ducta uniformly narrow with expanded base, a little longer than the body. Spermiduct, prostate and vas deferens as in P. marmorata (Paraense, loc. cit.). Penis wide proximally, narrowing gradually apicad; penial canal with subterminal outlet. Penial sheath following the width of the penis and ending up by a bulbous expansion somewhat narrower than the proximal portion. Penaial sheath/prepuce ration = 1,25 - 1,83 (mean 1.49). Prepuce much wider than the bulb of the penial shealth, moderately shouldered owing to the intromission of the bulb, and with a large gland in one side of its proximal half occupating about a third of its length. Extrinsic muscles of the penial complex as in P. marmorata. Jaw a simple obtusely V-shaped plate. Radula to be described separetely.

The aim of this retrospective study was to compare the clinical and radiographic results after TKA (PFC, DePuy), performed either by computer assisted navigation (CAS, Brainlab, Johnson&Johnson) or by conventional means. Material and methods: Between May and December 2006 we reviewed 36 conventional TKA performed between 2002 and 2003 (group A) and 37 navigated TKA performed between 2005 and 2006 (group B) by the same experienced surgeon. The mean age in group A was 74 years (range 62-90) and 73 (range 58-85) in group B with a similar age distribution. The preoperative mechanical axes in group A ranged from -13° varus to +13° valgus (mean absolute deviation 6.83°, SD 3.86), in group B from -13° to +16° (mean absolute deviation 5.35, SD 4.29). Patients with a previous tibial osteotomy or revision arthroplasty were excluded from the study. Examination was done by an experienced orthopedic resident independent of the surgeon. All patients had pre- and postoperative long standing radiographs. The IKSS and the WOMAC were utilized to determine the clinical outcome. Patient's degree of satisfaction was assessed on a visual analogous scale (VAS). Results: 32 of the 37 navigated TKAs (86,5%) showed a postoperative mechanical axis within the limits of 3 degrees of valgus or varus deviation compared to only 24 (66%) of the 36 standard TKAs. This difference was significant (p = 0.045). The mean absolute deviation from neutral axis was 3.00° (range -5° to +9°, SD: 1.75) in group A in comparison to 1.54° (range -5° to +4°, SD: 1.41) in group B with a highly significant difference (p = 0.000). Furthermore, both groups showed a significant postoperative improvement of their mean IKSS-values (group A: 89 preoperative to 169 postoperative, group B 88 to 176) without a significant difference between the two groups. Neither the WOMAC nor the patient's degree of satisfaction - as assessed by VAS - showed significant differences. Operation time was significantly higher in group B (mean 119.9 min.) than in group A (mean 99.6 min., p <0.000). Conclusion: Our study showed consistent significant improvement of postoperative frontal alignment in TKA by computer assisted navigation (CAS) compared to standard methods, even in the hands of a surgeon well experienced in standard TKA implantation. However, the follow-up time of this study was not long enough to judge differences in clinical outcome. Thus, the relevance of computer navigation for clinical outcome and survival of TKA remains to be proved in long term studies to justify the longer operation time. References 1 Stulberg SD. STUDY DESIGN: Prospective, controlled, observational outcome study using clinical, radiographic, and patient/physician-based questionnaire data, with patient outcomes at 12 months follow-up. OBJECTIVE: To validate appropriateness criteria for low back surgery. SUMMARY OF BACKGROUND DATA: Most surgical treatment failures are attributed to poor patient selection, but no widely accepted consensus exists on detailed indications for appropriate surgery. METHODS: Appropriateness criteria for low back surgery have been developed by a multispecialty panel using the RAND appropriateness method. Based on panel criteria, a prospective study compared outcomes of patients appropriately and inappropriately treated at a single institution with 12 months follow-up assessment. Included were patients with low back pain and/or sciatica referred to the neurosurgical department. Information about symptoms, neurologic signs, the health-related quality of life (SF-36), disability status (Roland-Morris), and pain intensity (VAS) was assessed at baseline, at 6 months, and at 12 months follow-up. The appropriateness criteria were administered prospectively to each clinical situation and outside of the clinical setting, with the surgeon and patients blinded to the results of the panel decision. The patients were further stratified into 2 groups: appropriate treatment group (ATG) and inappropriate treatment group (ITG). RESULTS: Overall, 398 patients completed all forms at 12 months. Treatment was considered appropriate for 365 participants and inappropriate for 33 participants. The mean improvement in the SF-36 physical component score at 12 months was significantly higher in the ATG (mean: 12.3 points) than in the ITG (mean: 6.8 points) (P = 0.01), as well as the mean improvement in the SF-36 mental component score (ATG mean: 5.0 points; ITG mean: -0.5 points) (P = 0.02). Improvement was also significantly higher in the ATG for the mean VAS back pain (ATG mean: 2.3 points; ITG mean: 0.8 points; P = 0.02) and Roland-Morris disability score (ATG mean: 7.7 points; ITG mean: 4.2 points; P = 0.004). The ATG also had a higher improvement in mean VAS for sciatica (4.0 points) than the ITG (2.8 points), but the difference was not significant (P = 0.08). The SF-36 General Health score declined in both groups after 12 months, however, the decline was worse in the ITG (mean decline: 8.2 points) than in the ATG (mean decline: 1.2 points) (P = 0.04). Overall, in comparison to ITG patients, ATG patients had significantly higher improvement at 12 months, both statistically and clinically. CONCLUSION: In comparison to previously reported literature, our study is the first to assess the utility of appropriateness criteria for low back surgery at 1-year follow-up with multiple outcome dimensions. Our results confirm the hypothesis that application of appropriateness criteria can significantly improve patient outcomes.

Perineural and intraneural fibrosis is thought to be the main cause of failure of the many surgical treatments of neuropathic pain. We have used Adcon-T/N carbohydrate polymer gel for prevention of perineural fibrosis in several parts of the body. In this retrospective study, 54 patients who presented with postoperative neuropathic pain had microsurgical epineural neurolysis and relocation of a terminal neuroma. In 19 of them, the carbohydrate gel was applied at the same time. The mean follow-up was four years and the nerve distribution varied. Postoperative improvement in pain scores (visual analogue scale (VAS) and neuropathic pain scale inventory (NPSI)), sensitivity, overall improvement and satisfaction were equivalent in the two groups, with pain relief in about 80% of the patients. There was no significant beneficial effect in the carbohydrate gel group. Patients treated with this device had a higher infection rate (21 compared with 0, p = 0.01) and delayed wound healing (31.6 compared with 11.8, p = 0.2). We conclude that good long-term pain relief is obtained postoperatively independently of the addition of carbohydrate gel. There was a slight but not significant trend towards profound pain relief with the gel.

We studied whether readiness to change predicts alcohol consumption (drinks per day) 3 months later in 267 medical inpatients with unhealthy alcohol use. We used 3 readiness to change measures: a 1 to 10 visual analog scale (VAS) and two factors of the Stages of Change Readiness and Treatment Eagerness Scale: Perception of Problems (PP) and Taking Action (TA). Subjects with the highest level of VAS-measured readiness consumed significantly fewer drinks 3 months later [Incidence rate ratio (IRR) and 95% confidence interval (CI): 0.57 (0.36, 0.91) highest vs. lowest tertile]. Greater PP was associated with more drinking [IRR (95%CI): 1.94 (1.02, 3.68) third vs. lowest quartile]. Greater TA scores were associated with less drinking [IRR (95%CI): 0.42 (0.23, 0.78) highest vs. lowest quartile]. Perception of Problems' association with more drinking may reflect severity rather than an aspect of readiness associated with ability to change; high levels of Taking Action appear to predict less drinking. Although assessing readiness to change may have clinical utility, assessing the patient's planned actions may have more predictive value for future improvement in alcohol consumption.

ABSTRACT Objectives: Patients with failed back surgery syndrome (FBSS) and chronic neuropathic pain experience levels of health-related quality of life (HRQoL) that are considerably lower than those reported in other areas of chronic pain. The aim of this article was to quantify the extent to which reductions in (leg and back) pain and disability over time translate into improvements in generic HRQoL as measured by the EuroQoL-5D and SF-36 instruments. Methods: Using data from the multinational Prospective, Randomized, Controlled, Multicenter Study of Patients with Failed Back Surgery Syndrome trial, we explore the relationship between generic HRQoL-assessed using two instruments often used in clinical trials (i.e., the SF-36 and EuroQol-5D)-and disease-specific outcome measures (i.e., Oswestry disability index [ODI], leg and back pain visual analog scale [VAS]) in neuropathic patients with FBSS. Results: In our sample of 100 FBSS patients, generic HRQoL was moderately associated with ODI (correlation coefficient: -0.462 to -0.638) and mildly associated with leg pain VAS (correlation coefficient: -0.165 to -0.436). The multilevel regression analysis results indicate that functional ability (as measured by the ODI) is significantly associated with HRQoL, regardless of the generic HRQoL instrument used. On the other hand, changes over time in leg pain were significantly associated with changes in the EuroQoL-5D and physical component summary scores, but not with the mental component summary score. Conclusions: Reduction in leg pain and functional disability is statistically significantly associated with improvements in generic HRQoL. This is the first study to investigate the longitudinal relationship between generic and disease-specific HRQoL of neuropathic pain patients with FBSS, using multinational data.

El canvi climàtic és una realitat i té efectes, tant directes com indirectes, sobre la salut. Són necessàries actuacions concertades per abordar aquestes qüestions de salut pública plantejades pel canvi climàtic, així com també disposar de mecanismes per predir quina serà la seva evolució. En aquest projecte es discuteixen les malalties infeccioses adquirides a través de diferents vies (artròpodes vectors, rosegadors, aigua, aliments i aire) en referència al canvi climàtic al món i també a Catalunya. Basat en una extensa revisió dels treballs i articles publicats, i de comentaris d’experts, es presenta una avaluació dels canvis de les malalties infeccioses: incidència, prevalença i distribució dels patògens i vehicles transmissors en un entorn canviant. En el present estudi es detallen alguns dels casos més estudiats i demostrats d’emergència i reemergència de brots infecciosos a diferents zones del planeta associats a certes variacions en les variables climàtiques. Degut a l’alt nivell d’incertesa sobre el ritme del canvi climàtic i el seu impacte sobre les malalties infeccioses, es proposa un seguit de línies de futur. Una de les propostes és crear una xarxa integrada de dades ambientals i epidemiològiques, amb capacitat de connectar aquestes dades amb la vigilància dels patògens, vectors i de la qualitat de l’aigua, entre d’altres factors. Aquestes anàlisis podrien orientar les estratègies d’actuació en un futur per la protecció de la salut de la població.

BACKGROUND: The course of alcohol consumption and cognitive dimensions of behavior change (readiness to change, importance of changing and confidence in ability to change) in primary care patients are not well described. The objective of the study was to determine changes in readiness, importance and confidence after a primary care visit, and 6-month improvements in both drinking and cognitive dimensions of behavior change, in patients with unhealthy alcohol use. METHODS: Prospective cohort study of patients with unhealthy alcohol use visiting primary care physicians, with repeated assessments of readiness, importance, and confidence (visual analogue scale (VAS), score range 1-10 points). Improvements 6 months later were defined as no unhealthy alcohol use or any increase in readiness, importance, or confidence. Regression models accounted for clustering by physician and adjusted for demographics, alcohol consumption and related problems, and discussion with the physician about alcohol. RESULTS: From before to immediately after the primary care physician visit, patients (n = 173) had increases in readiness (mean +1.0 point), importance (+0.2), and confidence (+0.5) (all p < 0.002). In adjusted models, discussion with the physician about alcohol was associated with increased readiness (+0.8, p = 0.04). At 6 months, many participants had improvements in drinking or readiness (62%), drinking or importance (58%), or drinking or confidence (56%). CONCLUSION: Readiness, importance and confidence improve in many patients with unhealthy alcohol use immediately after a primary care visit. Six months after a visit, most patients have improvements in either drinking or these cognitive dimensions of behavior change.

AIM: Although acute pain is frequently reported by patients admitted to the emergency room, it is often insufficiently evaluated by physicians and is thus undertreated. With the aim of improving the care of adult patients with acute pain, we developed and implemented abbreviated clinical practice guidelines (CG) for the staff of nurses and physicians in our hospital's emergency room. METHODS: Our algorithm is based upon the practices described in the international literature and uses a simultaneous approach of treating acute pain in a rapid and efficacious manner along with diagnostic and therapeutic procedures. RESULTS: Pain was assessed using either a visual analogue scale (VAS) or a numerical rating scale (NRS) at ER admission and again during the hospital stay. Patients were treated with paracetamol and/or NSAID (VAS/NRS <4) or intravenous morphine (VAS/NRS > or =04). The algorithm also outlines a specific approach for patients with headaches to minimise the risks inherent to a non-specific treatment. In addition, our algorithm addresses the treatment of paroxysmal pain in patients with chronic pain as well as acute pain in drug addicts. It also outlines measures for pain prevention prior to minor diagnostic or therapeutic procedures. CONCLUSIONS: Based on published guidelines, an abbreviated clinical algorithm (AA) was developed and its simple format permitted a widespread implementation. In contrast to international guidelines, our algorithm favours giving nursing staff responsibility for decision making aspects of pain assessment and treatment in emergency room patients.
