Background Support for the adverse effect of high income inequality on population health has come from studies that focus on larger areas, such as the US states, while studies at smaller geographical areas (eg, neighbourhoods) have found mixed results. Methods We used propensity score matching to examine the relationship between income inequality and mortality rates across 96 neighbourhoods (distritos) of the municipality of Sao Paulo, Brazil. Results Prior to matching, higher income inequality distritos (Gini >= 0.25) had slightly lower overall mortality rates (2.23 per 10 000, 95% CI -23.92 to 19.46) compared to lower income inequality areas (Gini <0.25). After propensity score matching, higher inequality was associated with a statistically significant higher mortality rate (41.58 per 10 000, 95% CI 8.85 to 73.3). Conclusion In Sao Paulo, the more egalitarian communities are among some of the poorest, with the worst health profiles. Propensity score matching was used to avoid inappropriate comparisons between the health status of unequal (but wealthy) neighbourhoods versus equal (but poor) neighbourhoods. Our methods suggest that, with proper accounting of heterogeneity between areas, income inequality is associated with worse population health in Sao Paulo.

[EN] Introduction: Candidemia in critically ill patients is usually a severe and life-threatening condition with a high crude mortality. Very few studies have focused on the impact of candidemia on ICU patient outcome and attributable mortality still remains controversial. This study was carried out to determine the attributable mortality of ICU-acquired candidemia in critically ill patients using propensity score matching analysis. Methods: A prospective observational study was conducted of all consecutive non-neutropenic adult patients admitted for at least seven days to 36 ICUs in Spain, France, and Argentina between April 2006 and June 2007. The probability of developing candidemia was estimated using a multivariate logistic regression model. Each patient with ICU-acquired candidemia was matched with two control patients with the nearest available Mahalanobis metric matching within the calipers defined by the propensity score. Standardized differences tests (SDT) for each variable before and after matching were calculated. Attributable mortality was determined by a modified Poisson regression model adjusted by those variables that still presented certain misalignments defined as a SDT > 10%. Results: Thirty-eight candidemias were diagnosed in 1,107 patients (34.3 episodes/1,000 ICU patients). Patients with and without candidemia had an ICU crude mortality of 52.6% versus 20.6% (P < 0.001) and a crude hospital mortality of 55.3% versus 29.6% (P = 0.01), respectively. In the propensity matched analysis, the corresponding figures were 51.4% versus 37.1% (P = 0.222) and 54.3% versus 50% (P = 0.680). After controlling residual confusion by the Poisson regression model, the relative risk (RR) of ICU- and hospital-attributable mortality from candidemia was RR 1.298 (95% confidence interval (CI) 0.88 to 1.98) and RR 1.096 (95% CI 0.68 to 1.69), respectively. Conclusions: ICU-acquired candidemia in critically ill patients is not associated with an increase in either ICU or hospital mortality.

[ES] La aparición de Google Scholar Metrics en abril de 2012 como nuevo sistema de evaluación bibliométrica de revistas científicas a partir del recuento de las citas bibliográficas que estas han recibido en Google Scholar, abre nuevas posibilidades para la medición del impacto de las revistas de humanidades. El objetivo de este trabajo es comprobar el alcance de este producto, a partir del análisis de la cobertura que muestra de las revistas españolas de Historia. Las búsquedas bibliográficas se efectuaron entre el 5 y 6 de diciembre de 2012. Se han identificado 69 revistas, cifra que representa tan solo un 24% de las revistas españolas de Historia. Los valores del índice h alcanzados por las revistas son minúsculos lo que hace que el ranking carezca de poder discriminatorio. Se propone un cambio en el diseño de Google Scholar Metrics para adaptarse a los patrones de producción y citación de la Historia, en particular, y las Humanidades, en general.

In this thesis, we present our work about some generalisations of ideas, techniques and physical interpretations typical for integrable models to one of the most outstanding advances in theoretical physics of nowadays: the AdS/CFT correspondences. We have undertaken the problem of testing this conjectured duality under various points of view, but with a clear starting point - the integrability - and with a clear ambitious task in mind: to study the finite-size effects in the energy spectrum of certain string solutions on a side and in the anomalous dimensions of the gauge theory on the other. Of course, the final desire woul be the exact comparison between these two faces of the gauge/string duality. In few words, the original part of this work consists in application of well known integrability technologies, in large parte borrowed by the study of relativistic (1+1)-dimensional integrable quantum field theories, to the highly non-relativisic and much complicated case of the thoeries involved in the recent conjectures of AdS5/CFT4 and AdS4/CFT3 corrspondences. In details, exploiting the spin chain nature of the dilatation operator of N = 4 Super-Yang-Mills theory, we concentrated our attention on one of the most important sector, namely the SL(2) sector - which is also very intersting for the QCD understanding - by formulating a new type of nonlinear integral equation (NLIE) based on a previously guessed asymptotic Bethe Ansatz. The solutions of this Bethe Ansatz are characterised by the length L of the correspondent spin chain and by the number s of its excitations. A NLIE allows one, at least in principle, to make analytical and numerical calculations for arbitrary values of these parameters. The results have been rather exciting. In the important regime of high Lorentz spin, the NLIE clarifies how it reduces to a linear integral equations which governs the subleading order in s, o(s0). This also holds in the regime with L ! 1, L/ ln s finite (long operators case). This region of parameters has been particularly investigated in literature especially because of an intriguing limit into the O(6) sigma model defined on the string side. One of the most powerful methods to keep under control the finite-size spectrum of an integrable relativistic theory is the so called thermodynamic Bethe Ansatz (TBA). We proposed a highly non-trivial generalisation of this technique to the non-relativistic case of AdS5/CFT4 and made the first steps in order to determine its full spectrum - of energies for the AdS side, of anomalous dimensions for the CFT one - at any values of the coupling constant and of the size. At the leading order in the size parameter, the calculation of the finite-size corrections is much simpler and does not necessitate the TBA. It consists in deriving for a nonrelativistc case a method, invented for the first time by L¨uscher to compute the finite-size effects on the mass spectrum of relativisic theories. So, we have formulated a new version of this approach to adapt it to the case of recently found classical string solutions on AdS4 × CP3, inside the new conjecture of an AdS4/CFT3 correspondence. Our results in part confirm the string and algebraic curve calculations, in part are completely new and then could be better understood by the rapidly evolving developments of this extremely exciting research field.

Il Cloud Storage è un modello di conservazione dati su computer in rete, dove i dati stessi sono memorizzati su molteplici server, reali e/o virtuali, generalmente ospitati presso strutture di terze parti o su server dedicati. Tramite questo modello è possibile accedere alle informazioni personali o aziendali, siano essi video, fotografie, musica, database o file in maniera “smaterializzata”, senza conoscere l’ubicazione fisica dei dati, da qualsiasi parte del mondo, con un qualsiasi dispositivo adeguato. I vantaggi di questa metodologia sono molteplici: infinita capacita’ di spazio di memoria, pagamento solo dell’effettiva quantità di memoria utilizzata, file accessibili da qualunque parte del mondo, manutenzione estremamente ridotta e maggiore sicurezza in quanto i file sono protetti da furto, fuoco o danni che potrebbero avvenire su computer locali. Google Cloud Storage cade in questa categoria: è un servizio per sviluppatori fornito da Google che permette di salvare e manipolare dati direttamente sull’infrastruttura di Google. In maggior dettaglio, Google Cloud Storage fornisce un’interfaccia di programmazione che fa uso di semplici richieste HTTP per eseguire operazioni sulla propria infrastruttura. Esempi di operazioni ammissibili sono: upload di un file, download di un file, eliminazione di un file, ottenere la lista dei file oppure la dimensione di un dato file. Ogniuna di queste richieste HTTP incapsula l’informazione sul metodo utilizzato (il tipo di richista, come GET, PUT, ...) e un’informazione di “portata” (la risorsa su cui effettuare la richiesta). Ne segue che diventa possibile la creazione di un’applicazione che, facendo uso di queste richieste HTTP, fornisce un servizio di Cloud Storage (in cui le applicazioni salvano dati in remoto generalmene attraverso dei server di terze parti). In questa tesi, dopo aver analizzato tutti i dettagli del servizio Google Cloud Storage, è stata implementata un’applicazione, chiamata iHD, che fa uso di quest’ultimo servizio per salvare, manipolare e condividere dati in remoto (nel “cloud”). Operazioni comuni di questa applicazione permettono di condividere cartelle tra più utenti iscritti al servizio, eseguire operazioni di upload e download di file, eliminare cartelle o file ed infine creare cartelle. L’esigenza di un’appliazione di questo tipo è nata da un forte incremento, sul merato della telefonia mobile, di dispositivi con tecnologie e con funzioni sempre più legate ad Internet ed alla connettività che esso offre. La tesi presenta anche una descrizione delle fasi di progettazione e implementazione riguardanti l’applicazione iHD. Nella fase di progettazione si sono analizzati tutti i requisiti funzionali e non funzionali dell’applicazione ed infine tutti i moduli da cui è composta quest’ultima. Infine, per quanto riguarda la fase di implementazione, la tesi presenta tutte le classi ed i rispettivi metodi presenti per ogni modulo, ed in alcuni casi anche come queste classi sono state effettivamente implementate nel linguaggio di programmazione utilizzato.
