Background: In Angola, malaria is an endemic disease having a major impact on the economy. The WHO recommends testing for all suspected malaria cases, to avoid the presumptive treatment of this disease. In malaria endemic regions laboratory technicians must be very comfortable with microscopy, the golden standard for malaria diagnosis, to avoid the incorrect diagnosis. The improper use of medication promotes drug resistance and undesirable side effects. The present study aims to assess the impact of a three-day refresher course on the knowledge of technicians, quality of blood smears preparation and accuracy of microscopy malaria diagnosis, using qPCR as reference method. Methods: This study was implemented in laboratories from three hospitals in different provinces of Angola: Bengo, Benguela and Luanda. In each laboratory samples were collected before and after the training course (slide with thin and thick blood smears, a dried blood spot and a form). The impact of the intervention was evaluated through a written test, the quality of slide preparation and the performance of microscopy. Results: It was found a significant increase on the written test median score, from 52.5% to 65.0%. A total of 973 slides were analysed to evaluate the quality of thick and thin blood smears. Considering all laboratories there was a significant increase in quality of thick and thin blood smears. To determine the performance of microscopy using qPCR as the reference method we used 1,028 samples. Benguela presented the highest values for specificity, 92.9% and 98.8% pre and post-course, respectively and for sensitivity the best pre-course was Benguela (75.9%) and post-course Luanda (75.0%). However, no significant increase in sensitivity and specificity after the training course was registered in any laboratory analysed. Discussion: The findings of this study support the need of continuous refresher training for microscopists and other laboratory staff. The laboratories should have a quality control programme to supervise the diagnosis and also to assess the periodicity of new training. However, other variables needed to be considered to have a correct malaria diagnosis, such as adequate equipment and reagents for staining and visualization, good working conditions, motivated and qualified personnel.

Purpose - The education and training of a nuclear medicine technologist (NMT) is not homogeneous among European countries, which leads to different scope of practices and, therefore, different technical skills are assigned. The goal of this research was to characterize the education and training of NMT in Europe. Materials and methods - This study was based on a literature research to characterize the education and training of NMT and support the historical evolution of this profession. It was divided into two different phases: the first phase included analysis of scientific articles and the second phase included research of curricula that allow health professionals to work as NMT in Europe. Results - The majority of the countries [N=31 (89%)] offer the NMT curriculum integrated into the high education system and only in four (11%) countries the education is provided by professional schools. The duration in each education system is not equal, varying in professional schools (2-3 years) and high education level system (2-4 years), which means that different European Credit Transfer and Accumulation System, such as 240, 230, 222, 210 or 180 European Credit Transfer and Accumulation System, are attributed to the graduates. The professional title and scope of the practice of NMT are different in different countries in Europe. In most countries of Europe, nuclear medicine training is not specific and curriculum does not demonstrate the Nuclear Medicine competencies performed in clinical practice. Conclusion - The heterogeneity in education and training for NMT is an issue prevalent among European countries. For NMT professional development, there is a huge need to formalize and unify educational and training programmes in Europe.

Worldwide competitiveness poses enormous challenges on managers, demanding a continuous quest to increase rationality in the use of resources. As a management philosophy, Lean Manufacturing focuses on the elimination of activities that do not create any type of value and therefore are considered waste. For companies to successfully implement the Lean Manufacturing philosophy it is crucial that the human resources of the organization have the necessary training, for which proper tools are required. At the same time, higher education institutions need innovative tools to increase the attractiveness of engineering curricula and develop a higher level of knowledge among students, improving their employability. This paper describes how Lean Learning Academy, an international collaboration project between five EU universities and five companies, from SME to Multinational/Global companies, developed and applied an innovative training programme for Engineers on Lean Manufacturing, a successful alternative to the traditional teaching methods in engineering courses.

In an increasingly competitive and globalized world, companies need effective training methodologies and tools for their employees. However, selecting the most suitable ones is not an easy task. It depends on the requirements of the target group (namely time restrictions), on the specificities of the contents, etc. This is typically the case for training in Lean, the waste elimination manufacturing philosophy. This paper presents and compares two different approaches to lean training methodologies and tools: a simulation game based on a single realistic manufacturing platform, involving production and assembly operations that allows learning by playing; and a digital game that helps understand lean tools. This paper shows that both tools have advantages in terms of trainee motivation and knowledge acquisition. Furthermore, they can be used in a complementary way, reinforcing the acquired knowledge.

OBJECTIVE To compare the effectiveness of two speech therapy interventions, vocal warm-up and breathing training, focusing on teachers’ voice quality.METHODS A single-blind, randomized, parallel clinical trial was conducted. The research included 31 20 to 60-year old teachers from a public school in Salvador, BA, Northeasatern Brazil, with minimum workloads of 20 hours a week, who have or have not reported having vocal alterations. The exclusion criteria were the following: being a smoker, excessive alcohol consumption, receiving additional speech therapy assistance while taking part in the study, being affected by upper respiratory tract infections, professional use of the voice in another activity, neurological disorders, and history of cardiopulmonary pathologies. The subjects were distributed through simple randomization in groups vocal warm-up (n = 14) and breathing training (n = 17). The teachers’ voice quality was subjectively evaluated through the Voice Handicap Index (Índice de Desvantagem Vocal, in the Brazilian version) and computerized voice analysis (average fundamental frequency, jitter, shimmer, noise, and glottal-to-noise excitation ratio) by speech therapists.RESULTS Before the interventions, the groups were similar regarding sociodemographic characteristics, teaching activities, and vocal quality. The variations before and after the intervention in self-assessment and acoustic voice indicators have not significantly differed between the groups. In the comparison between groups before and after the six-week interventions, significant reductions in the Voice Handicap Index of subjects in both groups were observed, as wells as reduced average fundamental frequencies in the vocal warm-up group and increased shimmer in the breathing training group. Subjects from the vocal warm-up group reported speaking more easily and having their voices more improved in a general way as compared to the breathing training group.CONCLUSIONS Both interventions were similar regarding their effects on the teachers’ voice quality. O presente projeto enquadra-se na área das redes de sensores sem fios, onde o seu crescente desenvolvimento permite aplicar esta tecnologia em diversas áreas, como por exemplo, na monitorização ambiental, utilização militar, domótica, saúde, entre outras. Uma rede de sensores sem fios consiste em diversos nós dispersos num campo de aplicação onde procedem à recolha de dados do ambiente em que estão inseridos, como o valor de uma temperatura, humidade ou outra grandeza física, e os transmitem para uma estação base onde podem ser monitorizados. Tendo a área da saúde uma importância significativa, este projeto focalizou-se na mesma. O projeto apresentado nesta dissertação teve como principal objetivo o desenvolvimento de uma rede de sensores sem fios, em que dois nós procedam à aquisição da temperatura corporal de uma pessoa em dois locais distintos, para posterior envio da mesma para os restantes nós da rede, onde será apresentada em estações de monitorização. Este projeto foi desenvolvido baseado num recente protocolo de redes sem fios, nomeadamente o protocolo ANTTM. Assim sendo, em primeiro lugar, serão abordados neste relatório os objetivos e a contextualização deste projeto. Em seguida, será apresentada uma comparação sobre alguns aspetos de algumas tecnologias de comunicações sem fios, nomeadamente o ZigBee, Bluetooth e ANT. Devido ao fato da tecnologia ANT ser a escolhida para o desenvolvimento deste projeto, será também apresentado um estudo mais detalhado sobre o mesmo. Depois, será apresentado o desenvolvimento e implementações efetuadas, e por último serão apresentadas as conclusões técnicas e pessoais que este projeto permitiu obter.

This chapter examines the cross-cultural influence of training on the adjustment of international assignees. We focus on the pre-departure training (PDT) before an international assignment. It is an important topic because in the globalized world of today more and more expatriations are needed. The absence of PDT may generate the failure of the expatriation experience. Companies may neglect PDT due to cost reduction practices and ignorance of the need for it. Data were collected through semi-structured interviews to 42 Portuguese international assignees and 18 organizational representatives from nine Portuguese companies. The results suggest that companies should develop PDT programs, particularly when the cultural distance to the host country is bigger and when there is no previous experience of expatriation to that country in the company. The study is original because it details in depth the methods of PDT, its problems, and consequences. Some limitations linked to the research design and detailed in the conclusion should be overcome in future studies.

RESUMO: Contexto: Indicadores fidedignos da composição corporal são importantes na orientação das estratégias nutricionais de recém-nascidos e pequenos lactentes submetidos a cuidados intensivos. O braço é uma região acessível para avaliar a composição corporal regional, pela medida dos seus compartimentos. A antropometria e a ultrassonografia (US) são métodos não invasivos, relativamente económicos, que podem ser usados à cabeceira do paciente na medição desses compartimentos, embora esses métodos não tenham ainda sido validados neste subgrupo etário. A ressonância magnética (RM) pode ser usada como método de referência na validação da medição dos compartimentos do braço. Objectivo: Validar em lactentes pré-termo, as medidas do braço por antropometria e por US. Métodos: Foi estudada uma coorte de recém-nascidos admitidos consecutivamente na unidade de cuidados intensivos neonatais, com 33 semanas de idade de gestação e peso adequado para a mesma, sem anomalias congénitas major e não submetidas a diuréticos ou oxigenoterapia no momento da avaliação. Nas vésperas da alta, foram efectuadas medições do braço, com ocultação, pelos métodos antropométrico, ultrassonográfico e RM. As medidas antropométricas directas foram: peso (P), comprimento (C), perímetro cefálico (PC), perímetro braquial (PB) e prega cutânea tricipital (PT). As área braquial total, área muscular (AM) e área adiposa foram calculadas pelos métodos de Jeliffee & Jeliffee e de Rolland-Cachera. Utilizando uma sonda PSH-7DLT de 7 Hz no ecógrafo Toshiba SSH 140A foram medidos os perímetros braquial e muscular e calculadas automaticamente as áreas braquial e muscular, sendo a área adiposa obtida por subtracção. Como método de referência foi utilizada a RM – Philips Gyroscan ACS-NT, Power-Track 1000 ®, 1.5 Tesla com uma antena de quadratura do joelho. Na análise estatística foram utilizados os métodos paramétricos e não paramétricos, conforme adequado. Resultados: Foram incluídas 30 crianças, nascidas com ( ±DP) 30.7 ±1.9 semanas de gestação, pesando 1380 ±325g, as quais foram avaliadas às 35.4 ±1.1 semanas de idade corrigida, quando pesavam 1786 ±93g. Nenhuma das medidas antropométricas, individualmente, constitui um indicador aceitável (r2 <0.5) das medições por RM. A melhor e mais simples equação alternativa encontrada é a que estima a AM (r2 = 0.56), derivada dos resultados da análise de regressão múltipla: AMRM = (P x 0.17) + (PB x 5.2) – (C x 6) – 150, sendo o P expresso em g, o C e o PB em cm. Nenhuma das medidas ultrassonográficas constitui um indicador aceitável (r2 <0.4) das medições por RM. Conclusões: A antropometria e as medidas ultrassonográficas do braço não são indicadores fidedignos da composição corporal regional em lactentes pré-termo, adequados para a idade de gestação.----------ABSTRACT: Background: Accurate predictors for body composition are valuable tools guiding nutritional strategies in infants needing intensive care. The upper-arm is a part of the body that is easily accessible and convenient for assessing the regional body composition, throughout the assessment of their compartments. Anthropometry and by ultrasonography (US) are noninvasive and relatively nonexpensive methods for bedside assessment of the upper-arm compartments. However, these methods have not yet been validated in infants. Magnetic resonance imaging (MRI) may be used as gold standard to validate the measurements of the upper-arm compartments. Objective: To validate the upper-arm measurements by anthropometry and by US in preterm infants. Methods: A cohort of neonates consecutively admitted at the neonatal intensive care unit, appropriate for gestational age, with 33 weeks, without major congenital abnormalities and not subjected to diuretics or oxygen therapy, was assessed. Before the discharge, the upper-arm was blindly measured by anthropometry, US and MRI. The direct anthropometric parameters measured were: weight (W), length (L), head circumference (HC), mid-arm circumference (MAC), and tricipital skinfold thickness. The arm area (AA), arm muscle area (AMA) and arm fat area were calculated applying the methods proposed by Jeliffee & Jeliffee and by Rolland-Cachera. Using the sonolayer Toshiba SSH 140A and the probe PSH-7DLT 7Hz, the arm and muscle perimeters were measured by US, the arm and muscle areas included were automatically calculated, and the fat area was calculated by subtraction. The MR images were acquired on a 1.5-T Philips Gyroscan ACS-NT, Power-Track 1000 scanner, and a knee coil was chosen for the upper-arm measurements. For statistical analysis parametric and nonparametric methods were used as appropriate. Results: Thirty infants born with ( ±SD) 30.7 ±1.9 weeks of gestational age and weighing 1380 ±325g were included in the study; they were assessed at 35.4 ±1.1 weeks of corrected age, weighing 1786 ±93g. None of the anthropometric measurements are individually acceptable (r2 <0.5) for prediction of the measurements obtained by MRI. The best and simple alternative equation found is the equation for prediction of the AMA (r2 = 0.56), derived from the results of multiple regression analysis: AMARM = (W x 0.17) + (MAC x 5.2) – (L x 6) – 150, being the W expressed in g, and L and MAC in cm. None of the ultrasonographic measurements are acceptable (r2 <0.5) predictors for the measurements obtained by MRI. Conclusions: The measurements of the upper-arm by anthropometry and by US are not accurate predictors for the regional body composition in preterm appropriate for gestational age infants.

The non-technical loss is not a problem with trivial solution or regional character and its minimization represents the guarantee of investments in product quality and maintenance of power systems, introduced by a competitive environment after the period of privatization in the national scene. In this paper, we show how to improve the training phase of a neural network-based classifier using a recently proposed meta-heuristic technique called Charged System Search, which is based on the interactions between electrically charged particles. The experiments were carried out in the context of non-technical loss in power distribution systems in a dataset obtained from a Brazilian electrical power company, and have demonstrated the robustness of the proposed technique against with several others natureinspired optimization techniques for training neural networks. Thus, it is possible to improve some applications on Smart Grids.
