Nowadays cancer is one of the main causes of death and many efforts worldwide have been driven to find out new treatments and approaches in order to extinguish or reduce this group of disorder. Chemotherapy is the main treatment for cancer, however, conventional schedule based on maximum tolerated dose (MTD) show several side effects and frequently allow the development of drug resistance. In this review we present the evidence that metronomic chemotherapy, based on the frequent administration of low or intermediate doses of chemotherapeutics is as efficient as MTD and works better in some situations. Finally, we present some data indicating that noncytotoxic concentrations of antineoplastic agents are able to both up-regulate the immune system and increase the susceptibility of tumor cells to cytotoxic T lymphocytes. Taken together, data from the literature provide us the evidence that low concentrations of selected chemotherapeutics agents, rather than conventional high doses, should be chosen for combination with immunotherapy

The use of pesticides in Brazil has intensified over the years. And since 2009 it was ranked as the largest market for pesticides worldwide. The consequential diffuse contamination of the environment is therefore due to the increasing number of chemicals of different classes, origins and modes of action. Little is known about the action of pesticides on human health in situations of co-exposure. Possible toxic effects are not restricted to agricultural and industrial workers, but also the general population that may be exposed continuously to its residues in food and water. Although these pesticides are mostly present in the environment at low doses, it must be considered that possible cumulative or synergistic effects may occur when there are concurrent or sustained exposure for two or more of these agents, which can lead to late manifestation of subclinical damages, sometimes irreversible. Thus, the specific objective of this study was to assess the effect of carcinogenesis promotion of a mixture of pesticides at low doses and analyze the phenomena of cell proliferation and apoptosis in rat liver. A total of 50 male Lewis rats was separated into 5 groups for 8 weeks in a medium term hepatocarcinogenesis model. The three different classes of pesticides (dieldrin, dicofol, endosulfan, dichlorvos and permethrin), whose residues were detected by ANVISA during the period from 2001 to 2005 in tomatoes cultures, were added to the feed of rats initiated to hepatocarcinogenesis with diethylnitrosamine (DEN- 200mg/kg ip). We used two different mixtures, one with no toxic effects at doses (MEX1) referring to the NOEL (no-observed-effect level) and another at doses LOEL / LEL / LOAEL (Lowest-observed-effect level / Lowest-effect level / Lowest -observed-adverse-effect level), to the installation of adverse effects (MEX2), derived from chronic studies. Our previous studies have shown that low concentrations (noncytotoxics) of antineoplastic agents modulate positively the dendritic cells, favoring their in vitro maturation and improving their antigen presenting function. The effects on colorectal cancer cells (HCT-116) were also investigated and we have observed an increased immunogenicity and susceptibility to cytotoxic T cells. Thereby, this study aimed to investigate the effect of 5-fluorouracil (5-FU) an azacitidine (AZA), in minimum effective and noncytotoxic concentrations on lymphocytes of healthy donors. In this study we have analyzed the cytotoxic effect of drugs at these concentrations as well as the proliferative ability of lymphocytes. In vitro production of IL-10 and IFN-γ has been also evaluated. We have observed that low concentrations of those chemotherapeutic agents are not cytotoxic for lymphocytes. However, the minimum effective concentrations (5-FU: 0,410±0,088 e AZA: 0,757±0,233; p<0, 05) have reduced the cell number. Proliferative activity of allogeneic lymphocytes in a mixed reaction (MLR) was not affected by the treatment. The cytokine production was not affected by the treatments, either. In conclusion, low concentrations of 5-FU and AZA has no deleterious effects on human peripheral blood lymphocytes and seems to be safe for combinatory administration with DC vaccines

Certain medicines are considered potentially inappropriate (PIM) for elderly people as they increase the risk of adverse drug events (ADE) and because safer alternative therapies are available on the market. In this context, in order to identify the instruments that assess the quality of medical prescriptions for elderly and to determine which drugs are considered PIM, a bibliographic survey was conducted in PUBMED, LILACS and PAHO databases, in February and March/2010. The search strategy included the use of health descriptors and a manual search in the references cited by selected papers. During the period of data collection, 15 instruments were identified. In 2012, with the publication of the update of Beers criteria, this instrument was included in the study. We identified 163 PIM of 25 therapeutic classes, of which 125 (76.7%) are marketed in Brazil. Of these, 31 (24.8%) are essential medicines (RENAME 2012), of which 13 have safer therapeutic equivalents and 19 (15.2%) are over-the-counter drugs. Data suggest the need for inclusion of safer alternatives for the elderly in the national list of essential medicines and the pharmaceutical care for early detection of ADE in this age group, in order to contribute to the safe use of medicines.

Objectives: The use of noninvasive cortical electrical stimulation with weak currents has significantly increased in basic and clinical human studies. Initial, preliminary studies with this technique have shown encouraging results; however, the safety and tolerability of this method of brain stimulation have not been sufficiently explored yet. The purpose of our study was to assess the effects of direct current (DC) and alternating current (AC) stimulation at different intensities in order to measure their effects on cognition, mood, and electroencephalogram. Methods: Eighty-two healthy, right-handed subjects received active and sham stimulation in a randomized order. We conducted 164 ninety-minute sessions of electrical stimulation in 4 different protocols to assess safety of (1) anodal DC of the dorsolateral prefrontal cortex (DLPFC); (2) cathodal DC of the DLPFC; (3) intermittent anodal DC of the DLPFC and; (4) AC on the zygomatic process. We used weak currents of 1 to 2 mA (for DC experiments) or 0.1 to 0.2 mA (for AC experiment). Results: We found no significant changes in electroencephalogram, cognition, mood, and pain between groups and a low prevalence of mild adverse effects (0.11% and 0.08% in the active and sham stimulation groups, respectively), mainly, sleepiness and mild headache that were equally distributed between groups. Conclusions: Here, we show no neurophysiological or behavioral signs that transcranial DC stimulation or AC stimulation with weak currents induce deleterious changes when comparing active and sham groups. This study provides therefore additional information for researchers and ethics committees, adding important results to the safety pool of studies assessing the effects of cortical stimulation using weak electrical currents. Further studies in patients with neuropsychiatric disorders are warranted.
