Background. Malaria is one of the most significant infectious diseases in the world and is responsible for a large proportion of infant deaths. Toll-like receptors (TLRs), key components of innate immunity, are central to countering infection. Variants in the TLR-signaling pathway are associated with susceptibility to infectious diseases. Methods. We genotyped single nucleotide polymorphisms ( SNPs) of the genes associated with the TLR-signaling pathway in patients with mild malaria and individuals with asymptomatic Plasmodium infections by means of polymerase chain reaction. Results. Genotype distributions for the TLR-1 I602S differed significantly between patients with mild malaria and persons with asymptomatic infection. The TLR-1 602S allele was associated with an odds ratio ( OR) of 2.2 ( P = .003; P(corrected) = .015) for malaria among patients with mild malaria due to any Plasmodium species and 2.1 ( P = .015; P(corrected) = .75) among patients with mild malaria due to Plasmodium falciparum only. The TLR-6 S249P SNP showed an excess of homozygotes for the TLR-6 249P allele in asymptomatic persons, compared with patients with mild malaria due to any Plasmodium species (OR 2.1; 95% confidence interval [CI], 1.1-4.2; P = .01; P(corrected) = .05), suggesting that the TLR-6 249S allele may be a risk factor for malaria ( OR, 2.0; 95% CI, 1.1-3.7; P = 0.01; P(corrected) = .05). The TLR-9-1486C allele showed a strong association with high parasitemia ( P < .001). Conclusions. Our findings indicate that the TLR-1 and TLR- 6 variants are significantly associated with mild malaria, whereas the TLR-9-1486C/T variants are associated with high parasitemia. These discoveries may bring additional understanding to the pathogenesis of malaria.

BACKGROUND: This study evaluated demographic profiles and prevalence of serologic markers among donors who used confidential unit exclusion (CUE) to assess the effectiveness of CUE and guide public policies regarding the use of CUE for enhancing safety versus jeopardizing the blood supply by dropping CUE. STUDY DESIGN AND METHODS: We conducted a cross-sectional analysis of whole blood donations at a large public blood center in Sao Paulo from July 2007 through June 2009, compared demographic data, and confirmed serologic results among donors who used and who have never used CUE (CUE never). RESULTS: There were 265,550 whole blood units collected from 181,418 donors from July 2007 through June 2009. A total of 9658 (3.6%) units were discarded, 2973 (1.1%) because CUE was used at the current donation (CUE now) and 6685 (2.5%) because CUE was used in the past (CUE past). The CUE rate was highest among donors with less than 8 years of education (odds ratio [OR], 2.78; 95% confidence interval [CI], 2.51-3.08). CUE now donations were associated with higher positive infectious disease marker rates than CUE never donations (OR, 1.41; CI, 1.13-1.77), whereas CUE past donations were not (OR, 1.04; CI, 0.75-1.45). CONCLUSION: The CUE process results in a high rate of unit discard. CUE use on an individual donation appears predictive of a high-risk marker-positive donation and, thus, appears to contribute modestly to blood safety. The policy of discarding units from donors who have previously CUE-positive donations does not improve safety and should be discontinued.

BACKGROUND: A major problem in Chagas disease donor screening is the high frequency of samples with inconclusive results. The objective of this study was to describe patterns of serologic results among donors to the three Brazilian REDS-II blood centers and correlate with epidemiologic characteristics. STUDY DESIGN AND METHODS: The centers screened donor samples with one Trypanosoma cruzi lysate enzyme immunoassay (EIA). EIA-reactive samples were tested with a second lysate EIA, a recombinant-antigen based EIA, and an immunfluorescence assay. Based on the serologic results, samples were classified as confirmed positive (CP), probable positive (PP), possible other parasitic infection (POPI), and false positive (FP). RESULTS: In 2007 to 2008, a total of 877 of 615,433 donations were discarded due to Chagas assay reactivity. The prevalences (95% confidence intervals [CIs]) among first-time donors for CP, PP, POPI, and FP patterns were 114 (99-129), 26 (19-34), 10 (5-14), and 96 (82-110) per 100,000 donations, respectively. CP and PP had similar patterns of prevalence when analyzed by age, sex, education, and location, suggesting that PP cases represent true T. cruzi infections; in contrast the demographics of donors with POPI were distinct and likely unrelated to Chagas disease. No CP cases were detected among 218,514 repeat donors followed for a total of 718,187 person-years. CONCLUSION: We have proposed a classification algorithm that may have practical importance for donor counseling and epidemiologic analyses of T. cruzi-seroreactive donors. The absence of incident T. cruzi infections is reassuring with respect to risk of window phase infections within Brazil and travel-related infections in nonendemic countries such as the United States.

Esta dissertação aborda a trajetória da Educação Infantil no MST – Movimento dos Trabalhadores Rurais Sem Terra, enfocando a Ciranda Infantil como espaço próprio da criança de zero a seis anos. O texto parte de uma contextualização sobre o movimento social em questão, após enfocamos a história da Educação Infantil no MST, no que se refere ao processo desencadeado para a realização da escolha do nome Ciranda Infantil, bem como, sua organização nas diferentes instâncias do Movimento, enquanto as grandes atividades/ações desenvolvidas e a sua organização nas áreas de acampamentos e assentamentos. Considerando essas diversas realidades/necessidades do Movimento, destacamos as diferentes formas de organização das Cirandas Infantis, como: - Ciranda Infantil Itinerante, para as crianças que acompanham as ações do MST, tanto à nível nacional, como estaduais; - Ciranda Infantil Permanente, quando está organizada para atender um público mais fixo e com encontros freqüentes ; Ciranda Infantil Eventual, quando organizada para atender um público mais fixo, porém que a busca esporadicamente. A pesquisa de campo aconteceu em dois momentos. A primeira pesquisa sobre as crianças de zero a seis anos, aconteceu em um acampamento com um grupo de vinte e seis crianças, entre um mês a seis anos. Este trabalho consistiu em inúmeras entrevistas com as mães e com as crianças, em muitas visitas nos barracos para acompanhar as suas atividades de rotina na cidade de lonas pretas. Nesse acampamento, não havia nenhum processo constituído no campo da Ciranda Infantil, mas havia enorme necessidade de atendimento às crianças dessa faixa etária, como também, necessitava um acompanhamento especial para as mães grávidas e com bebês recém nascidos. O segundo trabalho foi um estudo de caso de uma Ciranda Infantil da Cooperativa de Produção Agropecuária Nova Santa Rita. O mesmo realizou-se em diversas visitas à cooperativa e aos seus dirigentes, às famílias das treze crianças pesquisadas e que freqüentavam a Ciranda Infantil e também em muitas visitas para registrar a rotina das crianças na Ciranda Infantil. A conquista da Ciranda Infantil, enquanto espaço dentro da cooperativa, é inquestionável. Todos afirmam a sua importância para deixar as crianças, seja para os pais trabalharem ou, em outras ocasiões, quando a família necessita. É um espaço de educação, onde se aprende a cuidar e a organizar os ambientes, a ter contato com diferentes materiais, como tesoura, cola, canetas, livros, folhas, interagindo com diferentes pessoas de diferentes faixas etárias, crianças e adultos.

Acompanhou-se o desenvolvimento dos processos da salga em salmoura saturada (salga úmida) e salga seca de filés de tilápia-do-nilo (Oreochromis niloticus) e avaliaram-se algumas características indicativas de qualidade do produto durante a estocagem. Os processos foram acompanhados por 156 horas na salga úmida e por 96 horas na salga seca, e os filés salgados foram estocados, respectivamente, por 60 e 45 dias à temperatura ambiente. Os teores máximos de cloreto nos filés (14%) foram atingidos com 72 horas na salga úmida e com 36 horas na salga seca. Os filés de tilápia salgados em salmoura mantiveram as características próprias do produto por um período de 45 dias, e os submetidos à salga seca apresentaram baixo teor de umidade (6%) e alta concentração de extrato etéreo (4,6%). Recomenda-se somente o processo de salga em salmoura saturada como forma de conservação dos filés de tilápia-do-nilo.

Considering that counting the percentage of CD4 T lymphocytes can add prognostic information regarding patients infected with HIV, the aim of this study was to evaluate the percentage values of CD4+ T lymphocytes from 81 patients determined by flow cytometry and estimated by flow cytometry in conjunction with a hematology counter. Means were compared through the Student's t-test. Pearson's correlation was determined, and the agreement between results was tested by Bland-Altman. The level of significance was P < 0.05. It was found a significantly higher mean difference between the relative values of CD4+ T lymphocytes to the hematologic counter (P < 0.05), for all strata studied. Positive and significant correlations (P < 0.01) were found between the strata CD4 < 200 cells/mL (r = 0.93), between 200 and 500 cells/mL (r = 0.65), and >500 cells/mL (r = 0.81). The limits of agreement were 1.0 +/- 3.8% for the stratum of CD4 < 200 cells/mL, approximately 2.2 +/- 13.5% for the stratum of CD4 between 200 and 500 cells/mL, and approximately 6.2 +/- 20.4% for the stratum > 500 cells/mL. The differences in the percentages of CD4+ T lymphocytes obtained by different methodologies could lead to conflict when used in clinical decisions related to the treatment and care of people infected with HIV.

In this paper, a thermoeconomic analysis method based on the first and second law of thermodynamics and applied to an evaporative cooling system coupled to an adsorption dehumidifier, is presented. The main objective is the use of a method called exergetic manufacturing cost (EMC) applied to a system that operates in three different conditions to minimize the operation costs. Basic parameters are the RIP ratio (reactivation air/process air) and the reactivation air temperature. Results of this work show that the minimum reactivation temperature and the minimum RIP ratio corresponds to the smaller EMC. This result can be corroborated through an energetic analysis. It is noted that this case is also the one corresponding to smaller energy loss. (C) 2003 Elsevier B.V. Ltd. All rights reserved.
