Abusive consumption of alcohol leads to several negative consequences to health and quality of life, as it increases the frequency of diseases that cause death or functional disabilities. The rates of patients admitted to hospital due to physical problems stemming from alcohol abuse are high. This study aimed at identifying patients according to variables age, gender and education level as well as at evaluating the prevalence of CAGE-positive patients and morbidity due to CID-10 (International Classification of Diseases) by the World Health Organization. It is a cross-sectional, observational, exploratory, descriptive and quantitative study. It was conducted at the Botucatu School of Medicine University Hospital (HC) from July to September 2010, at the Internal Medicine ward. This ward aggregates 5 specialties, namely, Cardiology, Hematology, General Medicine, Nephrology and Gastroclinic. Presently, it has 36 beds, of which four are for Intensive Care Therapy (ICT). Three hundred and ten medical charts were analyzed for identification of morbidities and of variables age, gender and schooling. A closed semistructured questionnaire including, among other questions, the CAGE test was applied to each individual. The CAGE test is a questionnaire that comprises four questions and considers that one who answers two or more of such questions affirmatively is a suspected case of alcoholism. The total population studied included 310 patients. Indexes of 60% (186) male and of 40% (124) female patients were found. As regards age range, a population with a higher prevalence of individuals from 51 to 70 years old (41%) was observed. The main education indexes were: incomplete Elementary School: 23.22% (72), complete Elementary School: 21.61% (67), incomplete Secondary School: 18.40% (57) and Illiteracy: 17.74% (55). Acupuncture is one of the oldest forms of treatment, which is based in the grounds of Traditional Chinese Medicine. Despite the ancient history, it’s not completely widespread in Brazil, neither so comprehensive in veterinary medicine as used in humans. The technique of inserting needles into acupuncture points, can also be stimulated by heat, electrical stimulation or implants of metal, is mainly focused on neuromuscular disorders in small animals. The difficulty of comprehension of language and the lack of cientific bases restricted the possible applications of acupuncture. This work will address some of the clinical situations this complementary treatment can act, such as neurological, cardiorespiratory, reproductive, gastrointestinal, immunological, and postoperative intensive care

The evaluation of blood pressure is an essential tool for veterinarian clinical practice and for monitoring anesthetized patients or patients in intensive care, because of its usefulness in diagnostics, treatment and monitoring of several diseases. Apart from the pathological factor, the blood pressure also suffers the influence of different variables, such as age, breed, gender, temperament (anxiety and stress especially during the treatment, “white coat syndrome”), disease state, physical activity and, with lower intensity, animal’s diet. One of the main indications of the evaluation of blood pressure is the observation of clinical changes resulting from hypertension in animals, which one is characterized by lesions in the nervous and cardiovascular systems, kidneys and eyes. Not least important, the evaluation of blood pressure is also essential in hypotensive states, which represent an imminent risk of death. The techniques used in measuring blood pressure correspond to invasive forms (direct) or non invasive (indirect), whose correlation has been the subject of study and improvement within the small animal clinic. Thus, the purpose of this review is emphasize the importance of measuring blood pressure within the veterinary clinic routine, studying the influence of variables associated or not with elevated blood pressure, comparing the different methods used to obtain the blood pressure

The importance of retesting the hearing screening as an indicator of the real early hearing disorder

Early diagnosis of hearing loss minimizes its impact on child development. We studied factors that influence the effectiveness of screening programs. To investigate the relationship between gender, weight at birth, gestational age, risk factors for hearing loss, venue for newborn hearing screening and pass and fail results in the retest. Prospective cohort study was carried out in a tertiary referral hospital. The screening was performed in 565 newborns through transient evoked otoacoustic emissions in three admission units before hospital discharge and retest in the outpatient clinic. Gender, weight at birth, gestational age, presence of risk indicators for hearing loss and venue for newborn hearing screening were considered. Full-term infants comprised 86% of the cases, preterm 14%, and risk factors for hearing loss were identified in 11%. Considering the 165 newborns retested, only the venue for screening, Intermediate Care Unit, was related to fail result in the retest. Gender, weight at birth, gestational age and presence of risk factors for hearing loss were not related to pass and/or fail results in the retest. The screening performed in intermediate care units increases the chance of continued fail result in the Transient Otoacoustic Evoked Emissions test.

Contexto: O manuscrito descreve a reparação cirúrgica de uma úlcera crônica resultante da picada de uma aranhamarrom, um problema de difícil resolução, o qual habitualmente demora meses até a cicatrização. Descrição do caso: A vítima foi picada na coxa direita e desenvolveu fases de envenenamento típicas da picada de Loxosceles, incluindo a placa mármore, a escara e a úlcera profunda e de bordos emoldurados, características do acidente. Devido ao processo cicatricial lento, cuidados intensivos com curativos, dificuldade na deambulação, além do fator estético, optou-se por cirurgia excisional da lesão e sutura direta das bordas. O tratamento cirúrgico proporcionou cicatrização total da lesão dentro de 15 dias. Conclusões: A úlcera loxoscélica apresenta difícil cicatrização, evoluindo para a cronicidade. A cirurgia com excisão completa e síntese primária constitui técnica adequada para a cura da doença.

The purpose of the study was to outline the profile of patients hospitalized at the Pediatric Intensive Care Unit of the Hospital das Clínicas de Botucatu - UNESP. This is a descriptive, cross-sectional and quantitative study. The data were extracted from the “Discharge, Admission and Death Register” of the unit of the patients hospitalized between January and December 2011. There was predominance of male children (54.4%) under one year of age (40,7%) were, with a mean length of stay of 5.46 days. Most of these children came from cities included in the DIR XI/SP in Botucatu (78.2%). The discharge contributed with 91.1% of the total discharges from the unit. Most admissions happened during the fall and winter, with the Immediate Post-Operative (IPO - 32.3%) and respiratory diseases (24.2%) as the most frequent causes. The profile of patients at the PICU makes it possible to elucidate individual aspects, family, social, demographic, seasonal, climatic, and, also, the clinical conditions, and, thus, comprehend the context of hospitalizations, in order to propose improvements on assistance, in an individualized and integral manner, for patients and their families.

This study aimed to evaluate the parameters established in COFEN Resolution 293/04 concerning nursing staff dimensioning in adult intensive care units (AICU). The research was conducted in six hospitals in São Paulo City. The daily quantitative average of professionals needed for patient care was calculated according to the parameters established by COFEN. The obtained results were compared with the existing number of daily staff members in these units. It was observed that the proportions recommended by COFEN for the nurse category are superior to those used in the hospitals studied, which represents a challenge for Brazilian nursing. Mean care time values were found appropriate and represent important standards for dimensioning the minimum number of professionals in AICU. This study contributed to the validation of the parameters indicated in Resolution 293/04 for nursing staff dimensioning in the AICU.

Estudo transversal que avaliou a qualidade dos registros de enfermagem sobre ressuscitação cardiopulmonar. Foram revisados 42 prontuários de pacientes em uma unidade de terapia intensiva, utilizando o protocolo Utstein. Houve predomínio de homens (54,8%), idade de 21 a 70 anos (38,1%), correção de cardiopatias adquiridas (42,7%), com mais de um dispositivo pré-existente (147). Como causa imediata de parada cardiorrespiratória, predominou hipotensão (48,3%) e como ritmo inicial, bradicardia (37,5%). Apenas a hora do óbito e hora da parada foram registradas em 100% da amostra. Não foi registrado treinamento dos profissionais em Suporte Avançado de Vida. As causas da parada e ritmo inicial foram registrados em 69% e 76,2% da amostra. Compressões torácicas, obtenção de vias aéreas pérvias e desfibrilação foram registradas em menos de 16%. Os registros foram considerados de baixa qualidade, podendo incorrer em sanções legais aos profissionais e não permitindo a comparação da efetividade das manobras com outros centros.

[ES]Objetivo principal. Analizar las características epidemiológicas y los factores pronósticos de los pacientes críticos según la duración de la ventilación mecánica. Material y Método. Estudio retrospectivo observacional con los pacientes adultos ingresados en una Unidad de Cuidados intensivos (UCI) de tercer nivel, durante 7 años y que precisaron de ventilación mecánica. Se analizaron variables demográficas como la edad, sexo, tipo de paciente, APACHE II, la estancia (UCI, pre-UCI y hospitalaria) y la necesidad de depuración extrarrenal. Se estudiaron las diferencias según su supervivencia y según la duración de la VM. Se llevaron a cabo modelos de previsión de la supervivencia mediante una regresión logística. Por último se estudió el impacto económico que ocasionaban en el servicio.

El presente trabajo está orientado a identificar qué signos y síntomas inciden en el desgaste psicofísico del personal de enfermería que se desempeña en los servicios de Unidad de Terapia Intensiva Pediátrica, Unidad de Cuidados Intensivos Neonatales y la Unidad de Recuperación de Cirugía Cardiovascular, del Hospital Pediátrico Dr. Humberto Notti ubicado en el departamento de Guaymallen, Mendoza. La investigación se realizará sobre una población de noventa y seis enfermeros universitarios y licenciados en enfermería que trabajan en dichas áreas. La finalidad de este trabajo de investigación es dar a conocer un problema cada vez mas frecuente que tiene un fuerte impacto en cuanto a la optimización del desempeño laboral e insatisfacción personal. El agotamiento psicofísico está determinado por un conjunto de signos y síntomas, que son respuesta a diferentes situaciones de estrés, característicos de los servicios cerrados o de escasa circulación y además conllevan el gradiente del cuidado de pacientes pediátricos de alto riesgo.
