Rheumatoid arthritis (RA) is a chronic illness with great potential to cause comorbidities resulting from cumulative inflammatory activities, and it contributes with the increase of disabilities and death of patients. It affects 1 to 2% of world population and usually occurs between 30 and 50 years of age. Among existing therapeutic options for the disease non-steroidal anti-inflammatory drugs (NSAIDs) still play an important part in the treatment, being widely used by patients to relieve pain and stiffness. However, this class of drugs causes many adverse gastrointestinal effects, such as dyspepsia, heartburn, nausea and vomit, and its use is one of the most common causes of peptic ulcers. Mangiferin (a glicosilated xanthone extracted mainly from the bark of Mangifera indica L.) is the main compound of an aqueous extract made from the bark stem of the mango tree. Previous studies conducted by our research group prove the anti-inflammatory action of mangiferin on an animal model of periodontitis, and its gastroprotective action has been described before. Considering these informations this study evaluated mangiferin’s potential on the treatment of RA and on gastric ulcer healing in animal models, and analyzed toxicity parameters to assure efficacy and safety of the compound as potential new drug for the treatment of the disease. RA was induced in rats by subcutaneous injection of bovine collagen and Freund’s complete adjuvant. This method presented low incidence of RA in rats, but we were able to induce the disease in 60 to 70% of the animals. Due to the wide use of NSAIDs and its potential to cause peptic ulcers, we induced gastric ulcer on arthritic rats to analyze mangiferin’s gastric healing effect. Preeclampsia is a specific disorder of pregnancy, characterized by arterial hypertension and proteinuria detected after 20 weeks of gestation. This pathology is associated with hyperuricemia, higher levels of pro-inflammatory cytokines, enhanced leukocyte activation and oxidative stress. Adenosine deaminase (ADA) is an enzyme present in all human tissues and, it is involved with the maturation of the immune system. Although its function is not fully understood, ADA is considered an indicator of cellular inflammation and, its increased serum concentration is observed in inflammatory diseases, such as tuberculosis and rheumatoid arthritis. This study aimed to assess serum ADA levels in preeclamptic patients (PE) compared with normotensive pregnant (NT) and non-pregnant women (NP), and to correlate these values with TNF-α and IL-1β production. Ninety pregnant women were included: 60 were pre-eclamptic and 30 were normotensive matched for gestational age. As control group 20 healthy non-pregnant women matched with pregnant for age were included. Peripheral blood mononuclear cells (PMMC) obtained from the three groups studied were cultured with or without lipopolysaccharide (LPS) for 18h at 37oC, and TNF-α and IL-1β production was assessed in the supernatant of cultures by enzyme immunoassay (ELISA). ADA plasmatic concentration was determined by colorimetric method. The results show that ADA plasma levels were significantly higher in PE group compared with NT and NP groups. A positive correlation between ADA and uric acid levels was detected in preeclamptic women. There was no significant difference in relation to ADA levels when PE patients were classified in early and late-onset PE. The endogenous production of IL-1β and TNF-α by PBMC was significantly higher in PE group than in NT and NP women, showing the activation state of these cells in PE. The aim of the current review was to investigate the relationship between levels of neuropeptide Substance P in periodontal disease and chronic pain. Substance P is a neuropeptide that is directly related with pain. In periodontal disease, it is expressed during the inflammatory process, and is one of the factors responsible for bone resorption. Studies have shown that Substance P levels are highest in the gingival crevicular fluid from sites with active periodontal disease and bone loss. The persistence of these substances could be sufficient to stimulate neurogenic inflammation in susceptible tissues, and cause pain. The scientific literature shows that Substance P expressed during periodontal disease can be a risk factor for patients with systemic inflammatory pathologies, such as chronic arthritis or rheumatoid arthritis. Additional research is needed to confirm the participation of this substance in the origin of some types of chronic pain.

Periodontitis has been associated with rheumatoid arthritis. In experimental arthritis, concomitant periodontitis caused by oral infection with Porphyromonas gingivalis enhances articular bone loss. The aim of this study was to investigate how lipopolysaccharide (LPS) from P. gingivalis stimulates bone resorption. The effects by LPS P. gingivalis and four other TLR2 ligands on bone resorption, osteoclast formation, and gene expression in wild type and Tlr2-deficient mice were assessed in ex vivo cultures of mouse parietal bones and in an in vivo model in which TLR2 agonists were injected subcutaneously over the skull bones. LPS P. gingivalis stimulated mineral release and matrix degradation in the parietal bone organ cultures by increasing differentiation and formation of mature osteoclasts, a response dependent on increased RANKL (receptor activator of NF-κB ligand). LPS P. gingivalis stimulated RANKL in parietal osteoblasts dependent on the presence of TLR2 and through a MyD88 and NF-κB-mediated mechanism. Similarly, the TLR2 agonists HKLM, FSL1, Pam2, and Pam3 stimulated RANKL in osteoblasts and parietal bone resorption. LPS P. gingivalis and Pam2 robustly enhanced osteoclast formation in periosteal/endosteal cell cultures by increasing RANKL. LPS P. gingivalis and Pam2 also up-regulated RANKL and osteoclastic genes in vivo, resulting in an increased number of periosteal osteoclasts and immense bone loss in wild type mice but not in Tlr2-deficient mice. These data demonstrate that LPS P. gingivalis stimulates periosteal osteoclast formation and bone resorption by stimulating RANKL in osteoblasts via TLR2. This effect might be important for periodontal bone loss and for the enhanced bone loss seen in rheumatoid arthritis patients with concomitant periodontal disease.

Periodontal disease has been increasingly associated as a supporting factor, and even a precursor to some systemic diseases. Just aim of this paper was to analyze by reviewing the literature, the possible mechanisms involved in the etiopathogenesis of periodontal disease that are common to the current systemic diseases such as obesity, atherosclerosis, diabetes and rheumatoid arthritis. The articles were analyzed with in the publication period of the years 2006 to 2011 and were surveyed in the databases PubMed, LILACS and SciELO. Of a total of 907 articles found by searching the indices and their associations, were selected by inclusion criteria 93 articles. For the preparation of this literature review, only 24 articles were selected because they are based on inflammatory mechanisms, metabolic and infectious, where the considerations for the strongest associations in the literature. This literature review reinforces the association of periodontal disease with systemic diseases analyzed due to presence of inflammatory and immunological mechanisms and similar synergism betwen the two diseases, but some hypotheses still need further clarification. The professional should stick to recent discoveries associations to promote periodontal treatment in preventing the development of systemic diseases, as well as in maintaining the systemic health of patients already suffering from chronic systemic diseases.
