This study contributes to research examining how professional autonomy and hierarchy impacts upon the implementation of policy designed to improve the quality of public services delivery through the introduction of new managerial roles. It is based on an empirical examination of a new role for nurses – modern matrons – who are expected by policy-makers to drive organizational change aimed at tackling health care acquired infections (HCAI) in the National Health Service (NHS) within England. First, we show that the changing role of nurses associated with their ongoing professionalization limits the influence of modern matrons over their own ranks in tackling HCAI. Second, the influence of modern matrons over doctors is limited. Third, government policy itself appears inconsistent in its support for the role of modern matrons. The attempts of modern matrons to tackle HCAI appear more effective where infection control activity is situated in professional practice and where modern matrons integrate aspirations for improved infection control within mainstream audit mechanisms in a health care organization.

As organizações em geral procuram alcançar objetivos, adotando estratégias que lhes permitam cumprir aquele desiderato com a máxima eficiência e eficácia. Os ramos das Forças Armadas procuram implementar a estratégia ao longo de um período que, face ao impacto das alterações entretanto verificadas, exige a capacidade para fazer ajustamentos àquela estratégia inicial. Nesse sentido, recorrem a ferramentas para avaliar o desempenho organizacional, que lhes permita saber qual o desempenho real e quais os ajustamentos necessários a fazer para alcançar esses objetivos. O objeto de estudo são as ferramentas de gestão estratégica utilizadas pelos ramos das Forças Armadas com o objetivo de analisar a sua pertinência, a sua eficiência na avaliação do desempenho organizacional e as diferenças relativamente ao Sistema Integrado de Gestão e Avaliação do Desempenho na Administração Pública (SIADAP). Seguiu-se uma estratégia de investigação qualitativa e um desenho de pesquisa comparativo, verificando-se que as ferramentas de gestão estratégica são pertinentes para a formulação e implementação da estratégia, são eficientes na avaliação do desempenho organizacional e diferenciam-se do SIADAP por integrarem a formulação, a implementação e a avaliação da estratégia segundo quatro perspetivas relevantes para o cumprimento da missão dos ramos das Forças Armadas. Abstract: The general organizations seek to achieve goals, adopting strategies that will enable them to fulfil that desideratum with maximum efficiency and effectiveness. The branches of the armed forces seek to implement the strategy over a period that address the impact of changes however checked, and it requires the ability to make adjustments to that initial strategy. In that sense, they use tools to assess organizational performance, enabling them to know what the actual performance and what adjustments needed to do to achieve those goals. The object of study are the strategic management tools used in the branches of the armed forces, with the aim of analysing their relevance, their effectiveness in assessing organizational performance and the differences from the Integrated Management and Performance Evaluation System in Public Administration (SIADAP). There was a qualitative research strategy and a comparative research design, verifying that the strategic management tools are relevant to the formulation and implementation of strategy, are effective in assessing organizational performance and differentiate SIADAP by integrating the formulation, implementation and evaluation of the strategy in four relevant perspectives to fulfil the mission of the branches of the armed forces.

A Administração Pública tem sido alvo de uma reflexão sistemática, que pretende encontrar políticas de gestão que consigam solucionar os problemas de performance dos serviços públicos, evidenciando as suas potencialidades e procurando responder aos desafios com que se confrontam na atualidade. O fenómeno da globalização está na origem de acelerados ciclos de mudança que exigem das organizações urna gestão mais cativa e objetiva, alicerçada em métodos de gestão moderna que vão de encontro às exigências do mundo atual. Eficiência e eficácia dos serviços públicos são, mais do que nunca, duas imposições de uma sociedade crítica e carente de qualidade. A melhoria da performance do serviço público, mais do que urna necessidade, é uma exigência. Como responder a este desafio? Regra geral, os serviços públicos possuem urna fraca cultura de gestão estratégica, o que justifica parte da dificuldade que sentem em transformar a estratégia em objetivos operacionais, de forma a estabelecer ações que visem a obtenção de resultados, definindo medidas para a sua monitorização. O Balanced Scorecard (BSC) poderá contribuir para a implementação de uma cultura de gestão estratégica no Sector Público. A formulação dos objetivos estratégicos, das metas e dos indicadores, para cada uma das perspetivas subjacentes a esta metodologia, permite às organizações efetuarem uma gestão mais adequada dos sistemas que são objeto da sua acão. Este trabalho, partindo da identificação dos ajustamentos que é necessário efetuar ao modelo original de BSC de Kaplan e Norton, propõe um BSC adequado às Instituições de Ensino Superior Público (IESP) em geral e à Universidade de Évora em particular, assumindo-se que esse BSC poderá constituir uma importante ferramenta de gestão estratégica, essencial na avaliação e gestão da performance organizacional. ABSTRACT: The Public Administration has been the target of a systematic reflexion, who wish to find management policies that can resolve the performance problems on public services, showing their potential and aim to respond to the challenges they are facing today. The phenomenon of globalization is the cause of accelerated cycles of change that require organizations a more active and objective management, based on modern management methods that meet the demands of today’s world. Efficiency and effectiveness of public services are more than ever, two charges of a society criticism and lacking in quality. Improving the performance of public service, rather than a necessity is a requirement. How to respond to this challenge? Generally, the public services have a weak culture of strategic management, which explains part of the difficulty they feel in turning the strategy into operational objectives in order to establish measures aimed at achieving results, defining measures for its monitoring. The Balanced Scorecard can contribute to the implementation of a culture of strategic management in the Public Sector. The formulation of strategic objectives, targets and indicators for each of the perspectives that underlie this methodology enables organizations to make better management systems that are the subject of its action. This work, leaving from the identification of the adjustments that is necessary to effectuate to an original model of BSC of Kaplan and Norton, proposes a BSC appropriate to the Institutions of Superior Public Teaching (IESP) in general and to the University of Évora in individual, when are assumed that this BSC will be able to constitute an important tool of strategic, essential management in the evaluation and management of the organizational performance.

Despite the central role hospitals have in the health care system, relatively few health promotion activities are conducted in Australian public hospitals. This study investigated the types of obstacles that were perceived to inhibit health promotion activities in hospitals. A questionnaire for self-completion was sent to medical superintendents in all public hospitals in Queensland and 112 questionnaires were returned (92.6 per cent response rate). The results indicated that lack of finance, lack of interest by relevant others, and needs (for appropriate programs, training and patient receptivity) were the barriers reported by superintendents. The barriers of 'interest' and 'needs' were related to a lack of written policies in some areas, but not directly to levels of other health promotion activities being conducted in the hospitals. Success in facilitating health promotion programs in hospitals will need to include a change in the environment, in particular the views of medical superintendents. The combination of attitude change and the availability of a motivated person (such as a health promotion officer) to lead the activities may be needed in order to produce an increase in the level of health promotion in public hospitals. The work motivation construct is central to the theory and practice of many social science disciplines. Yet, due to the novelty of validated measures appropriate for a deep cross-national comparison, studies that contrast different administrative regimes remain scarce. This study represents an initial empirical effort to validate the Public Service Motivation (PSM) instrument proposed by Kim and colleagues (2013) in a previously unstudied context. The two former communist countries analyzed in this dissertation—Belarus and Poland—followed diametrically opposite development strategies: a fully decentralized administrative regime in Poland and a highly centralized regime in Belarus. The employees (n = 677) of public and nonprofit organizations in the border regions of Podlaskie Wojewodstwo (Poland) and Hrodna Voblasc (Belarus) are the subjects of study. ^ Confirmatory factor analysis revealed three dimensions of public service motivation in the two regions: compassion, self-sacrifice, and attraction to public service. The statistical models tested in this dissertation suggest that nonprofit sector employees exhibit higher levels of PSM than their public sector counterparts. Nonprofit sector employees also reveal a similar set of values and work attitudes across the countries. Thus, the study concludes that in terms of PSM, employees of nonprofit organizations constitute a homogenous group that exists atop the administrative regimes. ^ However, the findings propose significant differences between public sector agencies across the two countries. Contrary to expectations, data suggest that organization centralization in Poland is equal to—or for some items even higher than—that of Belarus. We can conclude that the absence of administrative decentralization of service provision in a country does not necessarily undermine decentralized practices within organizations. Further analysis reveals strong correlations between organization centralization and PSM for the Polish sample. Meanwhile, in Belarus, correlations between organization centralization items and PSM are weak and mostly insignificant. ^ The analysis indicates other factors beyond organization centralization that significantly impact PSM in both sectors. PSM of the employees in the studied region is highly correlated with their participation in religious practices, political parties, or labor unions as well as location of their organization in a capital and type of social service provided.^

The aim of this cephalometric study was to evaluate the influence of the sagittal skeletal pattern on the 'Y-axis of growth' measurement in patients with different malocclusions. Lateral head films from 59 patients (mean age 16y 7m, ranging from 11 to 25 years) were selected after a subjective analysis of 1630 cases. Sample was grouped as follows: Group 1 - class I facial pattern; group 2 - class II facial pattern; and Group 3 - class III facial pattern. Two angular measurements, SNGoGn and SNGn, were taken in order to determine skeletal vertical facial pattern. A logistic regression with errors distributed according to a binomial distribution was used to test the influence of the sagittal relationship (Class I, II, III facial patterns) on vertical diagnostic measurement congruence (SNGoGn and SNGn). RESULTS show that the probability of congruence between the patterns SNGn and SNGoGn was relatively high (70%) for group 1, but for groups II (46%) and III (37%) this congruence was relatively low. The use of SNGn appears to be inappropriate to determine the vertical facial skeletal pattern of patients, due to Gn point shifting throughout sagittal discrepancies. Clinical Significance: Facial pattern determined by SNGn must be considered carefully, especially when severe sagittal discrepancies are present.

Mining activities pose severe environmental risks worldwide, generating extreme pH conditions and high concentrations of heavy metals, which can have major impacts on the survival of organisms. In this work, pyrosequencing of the V3 region of the 16S rDNA was used to analyze the bacterial communities in soil samples from a Brazilian copper mine. For the analysis, soil samples were collected from the slopes (geotechnical structures) and the surrounding drainage of the Sossego mine (comprising the Sossego and Sequeirinho deposits). The results revealed complex bacterial diversity, and there was no influence of deposit geographic location on the composition of the communities. However, the environment type played an important role in bacterial community divergence; the composition and frequency of OTUs in the slope samples were different from those of the surrounding drainage samples, and Acidobacteria, Chloroflexi, Firmicutes, and Gammaproteobacteria were responsible for the observed difference. Chemical analysis indicated that both types of sample presented a high metal content, while the amounts of organic matter and water were higher in the surrounding drainage samples. Non-metric multidimensional scaling (N-MDS) analysis identified organic matter and water as important distinguishing factors between the bacterial communities from the two types of mine environment. Although habitat-specific OTUs were found in both environments, they were more abundant in the surrounding drainage samples (around 50 %), and contributed to the higher bacterial diversity found in this habitat. The slope samples were dominated by a smaller number of phyla, especially Firmicutes. The bacterial communities from the slope and surrounding drainage samples were different in structure and composition, and the organic matter and water present in these environments contributed to the observed differences.

Polymorphisms of Rh, Kell, Duffy, Kidd and Diego blood group systems were studied in 209 unrelated Brazilian Japanese descendants from South of Brazil. The methods used were multiplex-PCR, AS-PCR and RFLP-PCR. The differences in frequencies among the populations were evaluated using chi-square test. The frequencies for Rh, Kell, Kidd and Diego system were similar to those of the Japanese. RHCE(*)CC, RHCE(*)EE genotypes and FY(*)01 allele were lower and FY(*)01N.01 was higher than Japanese. These differences in the frequencies between Brazilian Japanese descendants and Japanese could indicate a gene flow in Brazilian population and reinforce the importance of this knowledge to achieve safe red blood cells.

Objective To assess depression and anxiety symptoms of adolescents with epilepsy compared with adolescents without epilepsy. Method The study sample consisted of: case participants (50 subjects) attending the pediatric epilepsy clinic of a tertiary hospital and control participants (51 subjects) from public schools. The instruments utilized were: identification card with demographic and epilepsy data, Beck Depression Inventory and State-Trait Anxiety Inventory. Results No significant differences were founded between the groups regarding scores for depression and anxiety symptoms but both groups presented moderate scores of anxiety. A correlation was found between low scores anxiety and not frequent seizures, low scores anxiety and perception of seizure control, high scores of anxiety and depression and occurrence of seizures in public places. Conclusion Low scores of anxiety are associated with not frequent seizures; high scores of anxiety and depression are associated with occurrence of seizures in public places.

Noncarious cervical lesions (NCCLs) are considered to be of multifactorial origin, normally associated with inadequate brushing. This study assessed the influence in vitro of simulated brushing on NCCL formation. Fifteen human premolars were submitted to brushing in the cementoenamel junction region, using hard-, medium- and soft-bristled brushes associated with a toothpaste of medium abrasiveness under a 200 g load, at a speed of 356 rpm for 100 minutes. The surface topography of the region was analyzed before and after brushing, by means of a laser interferometer, using "cut-off" values of 0.25 and considering roughness values in mm. The initial roughness (mm) results for dentin (D / bristle consistency: 1 - soft, 2 - medium and 3 - hard) were as follows: (D1) 1.25 ± 0.45; (D2) 1.12 ± 0.44; (D3) 1.05 ± 0.41. For enamel (E / bristle consistency: 1 - soft, 2 - medium and 3 - hard), the initial results were: (E1) 1.18 ± 0.35; (E2) 1.32 ± 0.25; (E3) 1.50 ± 0.38. After brushing, the following were the values for dentin: (D1) 2.32 ± 1.99; (D2) 3.30 ± 0.96; (D3) Over 500. For enamel, the values after brushing were: (E1) 1.37 ± 0.31; (E2) 2.15 ± 0.90; (E3) 1.22 ± 0.47. Based on the results of the ANOVA and Tukey statistical analyses (a = .05) it was concluded that soft, medium and hard brushes are not capable of abrading enamel, whereas dentin showed changes in surface roughness by the action of medium- and hard-bristled brushes.

A multi-pumping flow system exploiting prior assay is proposed for sequential turbidimetric determination of sulphate and chloride in natural waters. Both methods are implemented in the same manifold that provides facilities for: in-line sample clean-up with a Bio-Rex 70 mini-column with fluidized beads: addition of low amounts of sulphate or chloride ions to the reaction medium for improving supersaturation; analyte precipitation with Ba(2+) or Ag(+); real-time decision on the need for next assay. The sample is initially run for chloride determination, and the analytical signal is compared with a preset value. If higher, the sample is run again, now for sulphate determination. The strategy may lead to all increased sample throughput. The proposed system is computer-controlled and presents enhanced figures of merit. About 10 samples are run per hour (about 60 measurements) and results are reproducible and Unaffected by the presence of potential interfering ions at concentration levels usually found in natural waters. A study was designed to determine how the degree programs in Information and library science available in 2000-2005 at the public universities of Madrid fit the tabour market needs of their students. The methodology used was the development of a questionnaire addressed to graduates. Although the number of surveys completed is not high (118), the authors believe that the results obtained permit a series of conclusions that may be extrapolated to the entire cohort.

The purpose of this study was to test the hypotheses that in obese children: 1) hypocaloric diet (D) improves both heart rate recovery at 1 min (Delta HRR1) cfter an exercise test, and cardiac autonomic nervous system activity (CANSA) in obese children; 2) Diet and exercise training (DET) combined leads to greater improvement in both Delta HRR1 after an exercise test and in CANSA, than D alone. Moreover, we examined the relationships among Delta HRR1, CANSA, cardiorespiratory fitness and anthropometric variables (AV) in obese children submitted to D and to DET. 33 obese children (10 +/- 0.2 years; body mass index (BMI) >95(th) percentile) were divided into 2 groups: D (n = 15; BMI = 31 +/- 1 kg/m(2)) and DET (n = 18; 29 +/- 1 kg/m(2)). All children performed a maximal cardiopulmonary exercise test on a treadmill. The Delta HRR1 was defined as the difference between heart rate at peak and at 1-min post-exercise. CANSA was assessed using power spectral analysis of heart rate variability at rest. The sympathovagal balance (low frequency and high frequency ratio, LF/HF) was measured. After interventions, all obese children showed reduced body weight (P < 0.05). The D group did not improve in terms of peak VO(2), Delta HRR1 or LF/HF ratio (P > 0.05). In contrast, the DET group showed increased peak VO(2) (P = 0.01) and improved Delta HRR1 (Delta HRR1 = 37.3 +/- 2.6; P = 0.01) and LF/HF ratio (P = 0.001). The DET group demonstrated significant relationships among Delta HRR1, peak VO(2) and CANSA (P < 0.05). In conclusion, DET, in contrast to D, promoted improved Delta HRR1 and CANSA in obese children, suggesting a positive influence of increased levels of cardiorespiratory fitness by exercise training on cardiac autonomic activity.
