any pregnant women with low back and/or pelvic pain (LBPP) use pain medications to manage this pain, much of which is self-prescribed and potentially harmful. Therefore, there is a need to find effective nonpharmacological treatments for the condition. Reflexology has previously been shown to help nonspecific low back pain. Therefore; a pilot RCT was conducted investigating reflexology in the management of pregnancy-LBPP. 90 primiparous women were randomised to either usual care, a reflexology or footbath intervention. Primary outcome measures were; the Pain Visual Analogue Scale (VAS). 64 women completed the RCT; retention rates for the reflexology group were 80%, usual care group 83.33% and footbath group 50%. The reflexology group demonstrated a Clinically Important Change (CIC) in pain frequency (1.64 cm). Results indicate it is feasible to conduct an RCT in this area, although a footbath is an unsuitable sham treatment. Reflexology may help manage pregnancy-LBPP; however a fully powered trial is needed to confirm this.

Background: The Prenatal Distress Questionnaire (PDQ) is a short measure designed to assess specific worries and concerns related to pregnancy. The aim of this study was to confirm the factor structure of the PDQ in a group of pregnant women with a small for gestational age infant (< 10th centile). Methods: The first PDQ assessment for each of 337 pregnant women participating in the Prospective Observational Trial to Optimise paediatric health (PORTO) study was analysed. All women enrolled in the study were identified as having a small for gestational age foetus (< 10th centile), thus representing an 'elevated risk' group. Data were analysed using confirmatory factor analysis (CFA). Three models of the PDQ were evaluated and compared in the current study: a theoretical uni-dimensional measurement model, a bi-dimensional model, and a three-factor model solution. Results: The three-factor model offered the best fit to the data while maintaining sound theoretical grounds(χ2 (51df) = 128.52; CFI = 0.97; TLI = 0.96; RMSEA = 0.07). Factor 1 contained items reflecting concerns about birth and the baby, factor 2 concerns about physical symptoms and body image and factor 3 concerns about emotions and relationships. Conclusions: CFA confirmed that the three-factor model provided the best fit, with the items in each factor reflecting the findings of an earlier exploratory data analysis. © 2013 Society for Reproductive and Infant Psychology.

Preeclampsia (PE) affects approximately 5% of all pregnancies, but is increased several-fold in women with pre-gestational type 1 diabetes mellitus (T1DM). Increased oxidative stress and altered maternal plasma trace elements that modulate the antioxidant system have been implicated in PE. In non-diabetic women, increased plasma copper and iron and decreased manganese, selenium, and zinc have been associated with PE in cross-sectional studies. In a longitudinal study, we hypothesized that plasma levels of trace elements differ between T1DM women with vs. without subsequent PE. Samples were collected during the first (gestation 12.2 ± 1.9 weeks, [mean ± SD]), second (21.6 ± 1.5 weeks), and third (31.5 ± 1.7 weeks) trimesters of pregnancy, all before the onset of PE. We compared 23 T1DM women who subsequently developed PE with 24 T1DM women who remained normotensive; and we included 19 non-diabetic (non-DM) normotensive pregnant women as reference controls. Trace elements were measured using inductively coupled plasma mass spectroscopy. In T1DM women with subsequent PE vs normotensive, only plasma zinc was significantly higher at the first trimester, while copper:zinc and copper:high-density lipoprotein cholesterol ratios were higher throughout gestation (all P < .05). These findings persisted after adjustment for covariates. Higher copper:zinc ratios may contribute to oxidative stress in T1DM women who develop PE. Ratios of pro- to anti-oxidant factors may predict risk for PE in diabetic pregnancies more effectively than individual trace element levels.

Back pain is a common complaint in pregnancy, affecting approximately two-thirds of pregnant women [Pennick and Lidle, 2013]. This can lead to increased disability, affecting daily activities and cause absence from work. Evidence-based recommendations can be made for the use of exercise as an effective conservative treatment for the relief of back pain in pregnancy [Benten et al, 2014]. This poster explores the background to back pain in pregnancy and the advice women should be offered in relation to exercise to help normalise their pregnancy experience and enhance wellbeing.

Physiologic indicators reflect the functional consequences of vitamin A deficiency and may be particularly useful for detecting early perturbations in vitamin A status. In conjunctival impression cytology (CIC), epithelial morphology and the presence or absence of mucin spots and goblet cells allow samples, obtained by applying filter paper to the temporal conjunctiva, to be characterized as normal or typical of vitamin A-deficient keratinizing metaplasia. The validity of CIC has been established with reference to other indicators of vitamin A status, and a prevalence of > or =20% abnormal results has been suggested as indicative of a public health problem. However, interpretation of specimens requires considerable training, and nonresponsiveness to supplementation is a frequent problem, which limits the utility of CIC as a method for evaluating the impact of intervention programs. Several simplified field protocols for dark adaptation have been developed, including one in which dark adaptation is assessed by the responsiveness of the pupil to light. Night blind subjects have consistently shown abnormal results on this test, and a significant response to placebo-controlled dosing has been demonstrated for children and pregnant women. Scores have correlated significantly with serum retinol and relative dose response. Pupillary dark adaptation testing is acceptable to most children as young as 2 y old. Limitations of this technique include a time course for recovery after dosing as long as 4-6 wk, a testing time of 20 min, and the need for 1-3 d of training. Given its low cost, noninvasive nature, and lack of the need to transport samples, pupillary dark adaptation offers advantages over other techniques for assessing a population's vitamin A status.

Background Recreational use of 3,4 methylenedioxymethamphetamine (ecstasy, MDMA) is increasing worldwide. Its use by pregnant women causes concern due to potentially harmful effects on the developing fetus. MDMA, an indirect monoaminergic agonist and reuptake inhibitor, affects the serotonin and dopamine systems. Preclinical studies of fetal exposure demonstrate effects on learning, motor behavior, and memory. In the first human studies, we found prenatal MDMA exposure related to poorer motor development in the first year of life. In the present study we assessed the effects of prenatal exposure to MDMA on the trajectory of child development through 2 years of age. We hypothesized that exposure would be associated with poorer mental and motor outcomes. Materials and Methods The DAISY (Drugs and Infancy Study, 2003–2008) employed a prospective longitudinal cohort design to assess recreational drug use during pregnancy and child outcomes in the United Kingdom. Examiners masked to drug exposures followed infants from birth to 4, 12, 18, and 24 months of age. MDMA, cocaine, alcohol, tobacco, cannabis, and other drugs were quantified through a standardized clinical interview. The Bayley Scales (III) of Mental (MDI) and Motor (PDI) Development and the Behavior Rating Scales (BRS) were primary outcome measures. Statistical analyses included a repeated measures mixed model approach controlling for multiple confounders. Results Participants were pregnant women volunteers, primarily white, of middle class socioeconomic status, average IQ, with some college education, in stable partner relationships. Of 96 women enrolled, children of 93 had at least one follow-up assessment and 81 (87%) had ≥ two assessments. Heavier MDMA exposure (M = 1.3 ± 1.4 tablets per week) predicted lower PDI (p < .002), and poorer BRS motor quality from 4 to 24 months of age, but did not affect MDI, orientation, or emotional regulation. Children with heavier exposure were twice as likely to demonstrate poorer motor quality as lighter and non-exposed children (O.R. = 2.2, 95%, CI = 1.02–4.70, p < .05). Discussion Infants whose mothers reported heavier MDMA use during pregnancy had motor delays from 4 months to two years of age that were not attributable to other drug or lifestyle factors. Women of child bearing age should be cautioned about the use of MDMA and MDMA-exposed infants should be screened for motor delays and possible intervention.

Introdução: A atividade física (AF) parece ter um impacto positivo na saúde física e mental durante a gravidez, nascimento e puerpério, sendo que os programas de preparação para a parentalidade (PPP) poderão ser fundamentais para o seu suporte e estimulação. Objetivos: O objetivo do presente estudo foi avaliar a relação entre a preparação para a parentalidade no 3º trimestre de gravidez e os níveis de AF da gestante. Especificamente pretendeu-se verificar a relação entre as caraterísticas sociodemográficas das gestantes, os níveis de ansiedade e o PPP. Métodos: Efetuou-se um estudo transversal analítico onde se utilizaram duas amostras. Uma constituída por 42 gestantes que frequentaram o PPP no CHTS (GPP) e outra por 41 gestantes do HSJ que não frequentou (GNPP). A cada gestante foi pedido que preenchessem 3 questionários (caraterização sociodemográfica e saúde obstétrica, Questionário de Atividade Física para gestantes-PPAQ e Escala de ansiedade de Zung), administrados, individualmente, por um Fisioterapeuta. Resultados: Não se verificaram diferenças entre os grupos relativamente ao score da AF total (p=0,615), contudo, o GPP apresentou um número superior de gestantes que praticava desporto organizado durante a gravidez (p=0,016) comparativamente ao GNPP. Relativamente à intensidade da AF, verificou-se que o GPP apresentava uma prática maior de AF vigorosa (p=0,023). No que diz respeito ao tipo de AF, o GPP apresentou um número superior de gestantes a praticar AF desportiva (p<0,001) enquanto no GNPP se verificou uma maior AF ocupacional (p=0,002). Relativamente às caraterísticas sociodemográficas verificaram-se diferenças estatisticamente significativas entre os dois grupos relativamente à idade (p<0,001), paridade (p<0,001) e nível educacional (p<0,001). No que respeita aos níveis de ansiedade não se verificaram diferenças estatisticamente significativas entre os grupos (GPP vs GNPP p=0,916). Conclusão: No GPP um maior número de gestantes praticava atividade física desportiva e de intensidade vigorosa. Verificaram-se diferenças entre os dois grupos no que diz respeito à idade, paridade e nível educacional. Não se verificou associação entre o PPP e os níveis de ansiedade durante este período.

Introdução: A gravidez e a prática da atividade física (AF) são temas ainda pouco abordados na nossa sociedade. Muitas mulheres sofrem de ansiedade no período pré-natal e não conhecem os benefícios que a prática da AF tem sobre a mesma. Estudos demonstram que a AF está associada à diminuição da ansiedade, contudo poucos avaliam este efeito na gravidez. Objetivo(s): Os objetivos deste estudo foram classificar o nível de AF das grávidas; avaliar os sintomas de ansiedade e analisar a associação entre os níveis de ansiedade e a AF das gestantes. Métodos: Trata-se de um estudo longitudinal prospetivo, realizado numa amostra de 56 grávidas com idades entre os 18 e os 37 anos (29,14 ±4,641 anos). As participantes foram avaliadas em dois momentos, T1 (10-12 semanas gestacionais) e T2 (20-22 semanas gestacionais), tendo sido mensurada a AF com acelerómetros GTX3 e a ansiedade com a escala de Zung. Resultados: A média da AF leve e moderada não apresentou diferenças entre os dois momentos de avaliação; contudo, verificou-se uma tendência para a AF leve diminuir do primeiro para o segundo trimestre (1283,88 ± 530,37 vs 1098,16 ± 489,72 counts/min total semana; p=0,459) e a AF moderada aumentar (2470,62 ± 404,50 vs 2528,55 ± 493,05 counts/min total semana; p=0,459). A maioria das grávidas não cumpriu as recomendações da ACSM nos dois momentos de avaliação (T1: 72,1% e T2: 73,2%) (p=0,06). Não se verificaram diferenças no estado de ansiedade das grávidas de T1 para T2. 10,7% das grávidas apresentaram ansiedade em T1 e 7,1% em T2 (p=0,063). Cumprir ou não as recomendações da ACSM não esteve associado ao estado de ansiedade. Conclusão: Neste estudo verificámos que a maioria das gravidas não cumpre as recomendações da AF segundo ACSM, os níveis de ansiedade não diferiram ao longo da gravidez, verificámos ainda que o cumprimento das recomendações da AF não está associado aos níveis de ansiedade.

INTRODUCTION: Pregnant women with mechanical prosthetic heart valves are at increased risk for valve thrombosis. Management decisions for this life-threatening complication are complex. Open-heart surgery has a very high risk of maternal mortality and fetal loss. Bleeding and embolic risks associated with thrombolytic agents, the limited efficacy of thrombolysis in certain subgroups, and a lack of experience in the setting of pregnancy raise important concerns. CASE REPORT: We report a case of mitral prosthetic valve thrombosis in early pregnancy, which was successfully treated with streptokinase. Ten years later, the same patient had an uneventful pregnancy, throughout which acenocoumarol was maintained. CONCLUSION: With this case we review the prevention (with oral anticoagulant therapy) and treatment of prosthetic valve thrombosis during pregnancy, which is important for both obstetrician and cardiologist.

RESUMO - O género Listeria contém oito espécies (L. monocytogenes, L. ivanovii, L. innocua, L. seeligeri, L. welshimeri, L. grayi, L. marthii, e a L. rocoutiae), das quais duas são patogénicas. L. monocytogenes é patogénica para humanos e animais; L. ivanovii primeiramente infecta animais e raramente causa doenças em humanos. A Listeria monocytogenes é uma bactéria patogénica Gram-positiva facultativa intracelular, ubíqua na natureza. Nos últimos anos o número de casos de listeriose tem vindo a aumentar. Pode causar uma doença rara e grave chamada listeriose, especialmente nas mulheres grávidas, nos idosos ou em indivíduos com o sistema imunitário debilitado, de maneira esporádica ou em forma surtos. Realizou-se um estudo observacional, descritivo com o objectivo de se fazer a descrição e a caracterização do surto de listeriose ocorrido na região de Lisboa e Vale do Tejo entre 2009-2011. O período de maior número de casos diagnosticados de listeriose ocorreu entre o mês de Abril e Agosto de 2010. Mas a janela temporal em que ocorreu o surto estendeu-se de Março de 2009 a Janeiro de 2012. Ocorreram 51 casos de internamento com diagnóstico de listeriose, dos quais 25 casos foram confirmados, pela técnica de PFGE, pertencer à mesma estirpe I, sorotipo 4b e pulsotipo 070 e 0101. Na maioria dos casos eram do sexo feminino, com uma média de 57,14 anos de idade e com residência na região de Lisboa e Vale do Tejo. Em 96,08% dos doentes internados com listeriose apresentavam factores de predisposição, comorbilidade e /ou imunossupressão. A bacteriemia foi a manifestação clínica mais frequente, seguida da meningite. A letalidade da listeriose foi de 15,69%.
