This study was designed to investigate the impact of anti-retroviral therapy on both plasma and seminal HIV-1 viral loads and the correlation between viral loads in these compartments after treatment. Viral load, CD4+ and CD8+ T-cell counts were evaluated in paired plasma and semen samples from 36 antiretroviral therapy-naïve patients at baseline and on days 45, 90, and 180 of treatment. Slopes for blood and seminal viral loads in all treated patients were similar (p = 0.21). Median HIV-1 RNA titers in plasma and semen at baseline were 4.95 log10 and 4.48 log10 copies/ml, respectively. After 180 days of therapy, the median viral load declined to 3.15 log10 copies/ml (plasma) and 3.2 log10 copies/ml (semen). At this timepoint 22 patients presented HIV-1 viral load below 400 copies/ml in either plasma or semen, but only 9 had viral loads below 400 copies/ml in both compartments.

We report the findings of abdominal ultrasound and magnetic resonance imaging observed in a patient with advanced schistosomiasis mansoni. A 25-year-old man with hepatosplenic schistosomiasis and variceal bleeding confirmed by upper endoscopy was submitted to abdominal ultrasound and magnetic resonance imaging. During surgery for portal hypertension, a liver biopsy was taken and the diagnosis of Symmers' fibrosis was confirmed. magnetic resonance imaging scans gave more precise information about the gallbladder, periportal thickening and abdominal venous system than did the ultrasound.

Apresenta-se a avaliação clinicoepidemiológica de 95 crianças chagásicas crônicas em idades entre 1 e 14 anos moradoras de Santa Fé, Argentina, não tratadas e tratadas com nifurtimox ou benznidazol, com acompanhamento de até 24 anos. Todas tinham vários antecedentes de risco para transmissão do Trypanosoma cruzi: vetorial, congênito e/ou transfusão sangüínea. O diagnóstico da infecção foi feito através de sorologia convencional. O exame clínico foi complementado por eletrocardiograma, radiografias de tórax e, análise de sangue e urina para avaliação das funções hepáticas. No pós-tratamento, utilizaram-se técnicas idênticas às do diagnóstico, sendo que 33 crianças tiveram, também, avaliação parasitológica. Dentre 24 crianças não tratadas, 14 foram controlados por 8 a 24 anos e mantiveram sorologia positiva e o estado clínico inicial. Das 71 crianças tratadas, 49 tiveram acompanhamento de 4 a 24 anos: 14 mantiveram anticorpos anti-Trypanosoma cruzi; 6 resultados discordantes e 29 negativaram a sorologia. Destas, 9 apresentaram oscilações sorológicas, antes da negativação definitiva. A mediana do tempo de negativação pós-tratamento foi, respectivamente, de 3,5 e 8 anos para crianças de 1 a 6 e 7 a 14 anos. A percentagem de soronegativos diminuiu com a idade em que se medicou, desde 75% em <4 anos até 43% em > 9 anos. A intolerância ao tratamento foi de 3,8%. Nenhuma criança modificou seu estado clínico nesta observação.

Abdominal angiostrongyliasis is a zoonotic infection caused by Angiostrongylus costaricensis, a nematode with an intra-vascular location in the mesentery. Our objective was to address several aspects of the natural history of this parasitosis, in a longitudinal clinical and seroepidemiological study. A total of 179 individuals living in a rural area with active transmission in southern Brazil were followed for five years (1995-1999) resulting in yearly prevalence of 28.2%, 4.2%, 10%, 20.2% and 2.8% and incidences of 0%, 5.9%, 8% and 1.5%, respectively. Both men and woman were affected with higher frequencies at age 30-49 years. In 32 individuals serum samples were collected at all time points and IgG antibody reactivity detected by ELISA was variable and usually persisting not longer than one year. Some individual antibody patterns were suggestive of re-infection. There was no association with occurrence of abdominal pain or of other enteroparasites and there was no individual with a confirmed (histopathologic) diagnosis. Mollusks were found with infective third-stage larvae in some houses with an overall prevalence of 16% and a low parasitic burden. In conclusion, abdominal angiostrongyliasis in southern Brazil may be a frequent infection with low morbidity and a gradually decreasing serological reactivity.

Breast cancer is the most common type of cancer among women all over the world. An important issue that is not commonly addressed in breast cancer imaging literature is the importance of imaging the underarm region—where up to 80% of breast cancer cells can metastasise to. The first axillary lymph nodes to receive drainage from the primary tumour in the breast are called Sentinel Node. If cancer cells are found in the Sentinel Node, there is an increased risk of metastatic breast cancer which makes this evaluation crucial to decide what follow-up exams and therapy to follow. However, non-invasive detection of cancer cells in the lymph nodes is often inconclusive, leading to the surgical removal of too many nodes which causes adverse side-effects for patients. Microwave Imaging is one of the most promising non-invasive imaging modalities for breast cancer early screening and monitoring. This novel study tests the feasibility of imaging the axilla region by means of the simulation of an Ultra-Wideband Microwave Imaging system. Simulations of such system are completed in several 2D underarm models that mimic the axilla. Initial imaging results are obtained by means of processing the simulated backscattered signals by eliminating artefacts caused by the skin and beamforming the processed signals in order to time-align all the signals recorded at each antenna. In this dissertation several image formation algorithms are implemented and compared by visual inspection of the resulting images and through a range of performance metrics, such as Signal-to-Clutter Ratio and FullWidth Half Maximum calculations. The results in this study showed that Microwave Imaging is a promising technique that might allow to identify the presence and location of metastasised cancer cells in axillary lymph nodes, enabling the non-invasive evaluation of breast cancer staging.

A escassez de recursos humanos da saúde é um problema que afecta especialmente os países em desenvolvimento que recorrem frequentemente a programas baseados no trabalho de Agentes de Saúde Comunitária (ASC). Os ASC são pessoas escolhidas dentro das suas comunidades que são treinadas para dar resposta a pequenas enfermidades. Na Guiné-Bissau, as doenças diarreicas na infância são um dos principais problemas de saúde, tendo os ASC um papel fundamental no diagnóstico e tratamento destas doenças nas zonas mais rurais como a Região Sanitária de Bolama (RSB). O presente estudo focou-se especificamente numa população de ASC da RSB, tendo participado 22 dos 28 ASC existentes na Região. Pretendia-se perceber qual o impacto que a formação realizada sobre doenças diarreicas tinha sobre a efectividade do diagnóstico e tratamento deste tipo de doenças realizado pelos ASC em crianças com menos de 5 anos. Para isso, realizou-se um estudo longitudinal tendo sido efectuadas três avaliações em três momentos distintos – uma avaliação antes da realização de uma formação sobre doenças diarreicas, uma avaliação um mês após a formação e uma última avaliação 3 meses após a formação. Foi aplicada uma grelha de observação da consulta do ASC sempre que havia uma suspeita de uma criança com diarreia, em que se avaliou quais os Sinais e Sintomas, o Diagnóstico e os Tratamentos identificados pelo ASC. Uma grelha igual foi entregue ao médico, que funcionou como padrão de verificação externo. Foi aplicado ainda um questionário de identificação de características sócio-demográficas dos ASC em estudo. Os dados recolhidos foram alvo de tratamento estatístico, tendo sido aplicada a análise de Variância de Friedman e o teste Q-Cochran para comparação dos sucessos obtidos pelos ASC na identificação de itens, nos diferentes momentos de avaliação. Foi ainda aplicado um modelo de regressão logística para averiguar a possível influência de algumas características sócio-demográficas dos ASC sobre a efectividade do diagnóstico. Os resultados obtidos revelam que os ASC melhoram significativamente o seu desempenho imediatamente após a formação mas, 3 meses depois, a efectividade de diagnóstico e tratamento de doenças diarreicas diminui também de forma significativa. Não foi encontrada evidência estatística de que haja influência de alguma característica sócio-demografica sobre a melhoria na efectividade do diagnóstico e tratamento de doenças diarreicas. Os resultados obtidos demonstram que a formação feita aos ASC é, de facto, uma mais-valia para o seu desempenho mas, o impacto da formação acaba por se desvanecer 3 meses após a formação. Este facto pode ser justificável pela perda de competências e pela redução da utilização de algoritmos por parte dos ASC ao longo do tempo, e por factores relacionados com o suporte logístico e material que lhes é dado. A falta de acompanhamento, suporte e formação contínua podem ser também uma razão justificativa da diminuição da efectividade três meses após a formação. É fundamental ter sempre em conta que a selecção, o acompanhamento dos ASC, o suporte logístico e material e a formação continua destes Agentes é tão importante para o sucesso dos programas quanto a formação inicial ministrada.

Diffusion Kurtosis Imaging (DKI) is a fairly new magnetic resonance imag-ing (MRI) technique that tackles the non-gaussian motion of water in biological tissues by taking into account the restrictions imposed by tissue microstructure, which are not considered in Diffusion Tensor Imaging (DTI), where the water diffusion is considered purely gaussian. As a result DKI provides more accurate information on biological structures and is able to detect important abnormalities which are not visible in standard DTI analysis. This work regards the development of a tool for DKI computation to be implemented as an OsiriX plugin. Thus, as OsiriX runs under Mac OS X, the pro-gram is written in Objective-C and also makes use of Apple’s Cocoa framework. The whole program is developed in the Xcode integrated development environ-ment (IDE). The plugin implements a fast heuristic constrained linear least squares al-gorithm (CLLS-H) for estimating the diffusion and kurtosis tensors, and offers the user the possibility to choose which maps are to be generated for not only standard DTI quantities such as Mean Diffusion (MD), Radial Diffusion (RD), Axial Diffusion (AD) and Fractional Anisotropy (FA), but also DKI metrics, Mean Kurtosis (MK), Radial Kurtosis (RK) and Axial Kurtosis (AK).The plugin was subjected to both a qualitative and a semi-quantitative analysis which yielded convincing results. A more accurate validation pro-cess is still being developed, after which, and with some few minor adjust-ments the plugin shall become a valid option for DKI computation

Rupture of aortic aneurysms (AA) is a major cause of death in the Western world. Currently, clinical decision upon surgical intervention is based on the diameter of the aneurysm. However, this method is not fully adequate. Noninvasive assessment of the elastic properties of the arterial wall can be a better predictor for AA growth and rupture risk. The purpose of this study is to estimate mechanical properties of the aortic wall using in vitro inflation testing and 2D ultrasound (US) elastography, and investigate the performance of the proposed methodology for physiological conditions. Two different inflation experiments were performed on twelve porcine aortas: 1) a static experiment for a large pressure range (0 – 140 mmHg); 2) a dynamic experiment closely mimicking the in vivo hemodynamics at physiological pressures (70 – 130 mmHg). 2D raw radiofrequency (RF) US datasets were acquired for one longitudinal and two cross-sectional imaging planes, for both experiments. The RF-data were manually segmented and a 2D vessel wall displacement tracking algorithm was applied to obtain the aortic diameter–time behavior. The shear modulus G was estimated assuming a Neo-Hookean material model. In addition, an incremental study based on the static data was performed to: 1) investigate the changes in G for increasing mean arterial pressure (MAP), for a certain pressure difference (30, 40, 50 and 60 mmHg); 2) compare the results with those from the dynamic experiment, for the same pressure range. The resulting shear modulus G was 94 ± 16 kPa for the static experiment, which is in agreement with literature. A linear dependency on MAP was found for G, yet the effect of the pressure difference was negligible. The dynamic data revealed a G of 250 ± 20 kPa. For the same pressure range, the incremental shear modulus (Ginc) was 240 ± 39 kPa, which is in agreement with the former. In general, for all experiments, no significant differences in the values of G were found between different image planes. This study shows that 2D US elastography of aortas during inflation testing is feasible under controlled and physiological circumstances. In future studies, the in vivo, dynamic experiment should be repeated for a range of MAPs and pathological vessels should be examined. Furthermore, the use of more complex material models needs to be considered to describe the non-linear behavior of the vascular tissue.

Actualmente a sepsis continua a ser uma das principais causas de morte durante o internamento em Unidades de Cuidados Intensivos (UCI). Apesar dos recentes avanços a nível terapêutico existem ainda lacunas por preencher nomeadamente ao nível da identificação de biomarcadores que permitam um melhoramento do prognóstico tendo em conta a necessidade de identificação de doentes de maior risco. Este estudo foi realizado com o objectivo de contribuir para uma melhor compreensão da influência de alterações na microcirculação na sepsis. Para tal foram analisados diversos parâmetros hemorreológicos, como a deformabilidade e agregação eritrocitárias, NO e GSNO eritrocitário. Foi também avaliada a relação de tais parâmetros com a inflamação e trombose através da avaliação da concentração sérica de sCD40L. O estudo incidiu sobre doentes com choque séptico (n=22) e indivíduos saudáveis (n=34). Os doentes com sepsis foram monitorizados na admissão na UCI, 24 e 72 horas depois. Verificaram-se variações longitudinais nos valores de agregação eritrocitária, cujos valores aumentam ao longo do tempo de internamento, até atingirem às 72h valores significativamente diferentes do grupo controlo. Uma tendência inversa apresenta os valores de GSNO eritrocitário, os quais estão aumentados nos doentes em choque séptico aquando da admissão à UCI, diminuindo para valores semelhantes ao grupo controlo. Observaram-se também variações nas concentrações de sCD40L, tendo sido registada uma diminuição ao longo do tempo tornando-se evidente às 72 horas após o internamento. Apesar de preliminares, a comparação entre doentes sobreviventes e não-sobreviventes permitiu inferir uma possível relação do aumento de sCD40L e diminuição de NO e GSNO eritrocitários na admissão com um pior prognóstico. Estes resultados vêm fundamentar a ideia de que existe realmente uma relação entre as alterações a nível da microcirculação e a sepsis.

INTRODUCTION: There is no study relating magnetic resonance imaging (MRI) to ultrasound (US) findings in patients with Schistosomiasis mansoni. Our aim was to describe MRI findings inpatients with schistosomal liver disease identified by US. METHODS: Fifty-four patients (mean age 41.6±13.5years) from an area endemic for Schistosomiasis mansoni were selected for this study.All had US indicating liver schistosomal fibrosis and were evaluated with MRI performed witha 1.5-T superconducting magnet unit (Sigma). RESULTS: Forty-seven (87%) of the 54 patientsshowing signs of periportal fibrosis identified through US investigation had confirmed diagnosesby MRI. In the seven discordant cases (13%), MRI revealed fat tissue filling in the hilar periportalspace where US indicated isolated thickening around the main portal vein at its point of entryto the liver. We named this the fatty hilum sign. One of the 47 patients with MRI evidence ofperiportal fibrosis had had his gallbladder removed previously. Thirty-five (76.1%) of the other46 patients had an expanded gallbladder fossa filled with fat tissue, whereas MRI of the remainingeleven showed pericholecystic signs of fibrosis. CONCLUSIONS: Echogenic thickening of thegallbladder wall and of the main portal vein wall heretofore attributed to fibrosis were frequentlyidentified as fat tissue in MRI. However, the gallbladder wall thickening shown in US (expandedgallbladder fossa in MRI) is probably secondary to combined hepatic morphologic changes inschistosomiasis, representing severe liver involvement.

RESUMO: Enthesitis is the hallmark of spondyloarthritis (SpA), and is observed in all subtypes. Wide information on SpA abnormalities, including synovitis, tendinitis and enthesitis, can be efficiently perceived by Doppler ultrasound. Furthermore, several studies on imaging of enthesis showed that imaging techniques are better than clinical examination to detect enthesis alterations; and vascularized enthesitis detected by Doppler ultrasound appears to be a valuable diagnostic tool to confirm SpA diagnosis. However, data published until now concerning entheseal elementary alterations that characterize SpA enthesitis (enthesis inflammatory activity) or enthesopathy (permanent structural changes) reflect rather the authors’ empiric opinion than a methodological validation process. In this sense it seems crucial to identify elementary entheseal lesions associated with activity or damage, in order to improve monitoring and treatment response in SpA patients. The development of better assessment tools is today a challenge and a need in SpA. The first study of this thesis focused on the analysis of the reliability of inter-lector and inter-ultrasonography equipment of Madrid sonography enthesitis index (MASEI). Fundamental data for the remaining unrolling project validity. In the second and third studies we concerned about two entheseal elemental lesions: erosions and bursa. In literature erosions represent a permanent structural damage, being useful for monitoring joint injury, disease activity and therapeutic response in many rheumatic diseases; and to date, this concept has been mostly applied in rheumatoid arthritis (RA). Unquestionably, erosion is a tissue-related damage and a structural change. However, the hypothesis that we decided to test was if erosions represent a permanent structural change that can only grow and worsen over time, as occurs in RA, or a transitory alteration. A longitudinal study of early SpA patients was undertaken, and the Achilles enthesis was used as a model. Our results strongly suggested that previously detected erosions could disappear during the course of the disease, being consistent with the dynamic behavior of erosion over time. Based on these striking results it seems reasonable to suggest that the new-bone formation process in SpA could be associated with the resolution of cortical entheseal erosion over time. These results could also be in agreement with the apparent failure of anti-tumor necrosis factor (TNF) therapies to control bone proliferation in SpA; and with the relation of TNF-α, Dickkopf-related protein 1 (Dkk-1) and the regulatory molecule of the Wnt signaling pathway in the bone proliferation in SpA. In the same model, we then proceeded to study the enthesis bursa. Interestingly, the Outcome Measures in Rheumatology Clinical Trials (OMERACT) enthesopathy definition does not include bursa as an elementary entheseal lesion. Nonetheless, bursa was included in 46% of the enthesis studies in a recently systematic literature review, being in agreement with the concept of “synovio-entheseal complex” that includes the link between enthesitis and osteitis in SpA. It has been clarified in recent data that there is not only a close functional integration of the enthesis with the neighboring bone, but also a connection between enthesitis and synovitis. Therefore, we tried to assess the prevalence and relevance of the bursa-synovial lesion in SpA. Our findings showed a significant increase of Achilles bursa presence and thickness in SpA patients compared to controls (healthy/mechanical controls and RA controls). These results raise awareness to the need to improve the enthesopathy ultrasonographic definition. In the final work of this thesis, we have explored new perspectives, not previously reported, about construct validity of enthesis ultrasound as a possible activity outcome in SpA. We performed a longitudinal Achilles enthesis ultrasound study in patients with early SpA. Achilles ultrasound examinations were performed at baseline, six- and twelve-month time periods and compared with clinical outcome measures collected at basal visit. Our results showed that basal erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are higher in patients with Doppler signal in enthesis, and even that higher basal ESR, CRP and Ankylosing Spondylitis Disease Activity Score (ASDAS) predicted a higher Doppler signal (an ultrasound alteration accepted as representative of inflammation) six months later. Patients with very high disease activity assessed by ASDAS (>3.5) at baseline had significantly higher Achilles total ultrasound score verified at the same time; and ASDAS <1.3 predicted no Doppler signal at six and twelve months. This seems to represent a connection between classical biomarkers and clinical outcomes associated with SpA activity and Doppler signal, not only at the same time, but also for the following months. Remarkably, patients with inactive disease (ASDAS < 1.3) at baseline had no Doppler signal at six and twelve months. These findings reinforce the potential use of ultrasound related techniques for disease progression assessment and prognosis purposes. Intriguingly, Ankylosing Spondylitis Disease Activity Index (BASDAI) didn’t show significant differences between different cut-offs concerning ultrasound lesions or Doppler signal, while verified with ASDAS. These results seem to indicate that ASDAS reflects better than BASDAI what happens in the enthesis. The work herein discussed clearly shows the potential utility of ultrasound in enthesis assessment in SpA patients, and can be important for the development of ultrasound activity and structural damage scores for diagnosis and monitoring purposes. Therefore, local promotion of this technique constitutes a medical intervention that is worth being tested in SpA patients for diagnosis, monitoring and prognosis purposes.

Although cryptococcal infections begin in the lungs, meningoencephalitis is the most frequently encountered manifestation of cryptococcosis among individuals with advanced immunosuppression. As the infection progresses along the Virchow-Robin spaces, these structures may become dilated with mucoid material produced by the capsule of the organism. We report a case of a 24-year-old man with cryptococcal meningoencephalitis in which magnetic resonance imaging showed clusters of gelatinous pseudocysts in the periventricular white matter, basal ganglia, mammillary bodies, midbrain peduncles and nucleus dentatus with a soap bubble appearance.
