Background: Prolonged empiric antibiotics therapy in neonates results in several adverse consequences including widespread antibiotic resistance, late onset sepsis (LOS), necrotizing enterocolitis (NEC), prolonged hospital course (HC) and increase in mortality rates. Objectives: To assess the risk factors and the outcome of prolonged empiric antibiotic therapy in very low birth weight (VLBW) newborns. Materials and Methods: Prospective study in VLBW neonates admitted to NICU and survived > 2 W, from July 2011 - June 2012. All relevant perinatal and postnatal data including duration of antibiotics therapy (Group I < 2W vs Group II > 2W) and outcome up to the time of discharge or death were documented and compared. Results: Out of 145 newborns included in the study, 62 were in group I, and 83 in Group II. Average duration of antibiotic therapy was 14 days (range 3 - 62 days); duration in Group I and Group II was 102.3 vs 25.510.5 days. Hospital stay was 22.311.5 vs 44.3 14.7 days, respectively. Multiple regression analysis revealed following risk factors as significant for prolonged empiric antibiotic therapy: VLBW especially < 1000 g, (P < 0.001), maternal Illness (P = 0.003), chorioamnionitis (P = 0.048), multiple pregnancy (P = 0.03), non-invasive ventilation (P < 0.001) and mechanical ventilation (P < 0.001). Seventy (48.3%) infants developed LOS; 5 with NEC > stage II, 12 (8.3%) newborns died. Infant mortality alone and with LOS/NEC was higher in group II as compared to group I (P < 0.002 and < 0.001 respectively). Conclusions: Prolonged empiric antibiotic therapy caused increasing rates of LOS, NEC, HC and infant mortality

Introduction: La ventilation non invasive (VNI) est un outil utilisé en soins intensifs pédiatriques (SIP) pour soutenir la détresse respiratoire aigüe. Un échec survient dans près de 25% des cas et une mauvaise synchronisation patient-ventilateur est un des facteurs impliqués. Le mode de ventilation NAVA (neurally adjusted ventilatory assist) est asservi à la demande ventilatoire du patient. L’objectif de cette étude est d’évaluer la faisabilité et la tolérance des enfants à la VNI NAVA et l’impact de son usage sur la synchronie et la demande respiratoire. Méthode: Étude prospective, physiologique, croisée incluant 13 patients nécessitant une VNI dans les SIP de l’hôpital Ste-Justine entre octobre 2011 et mai 2013. Les patients ont été ventilés successivement en VNI conventionnelle (30 minutes), en VNI NAVA (60 minutes) et en VNI conventionnelle (30 minutes). L’activité électrique du diaphragme (AEdi) et la pression des voies aériennes supérieures ont été enregistrées pour évaluer la synchronie. Résultats: La VNI NAVA est faisable et bien tolérée chez tous les enfants. Un adolescent a demandé l’arrêt précoce de l’étude en raison d’anxiété reliée au masque sans fuite. Les délais inspiratoires et expiratoires étaient significativement plus courts en VNI NAVA comparativement aux périodes de VNI conventionnelle (p< 0.05). Les efforts inefficaces étaient moindres en VNI NAVA (résultats présentés en médiane et interquartiles) : 0% (0 - 0) en VNI NAVA vs 12% (4 - 20) en VNI conventionnelle initiale et 6% (2 - 22) en VNI conventionnelle finale (p< 0.01). Globalement, le temps passé en asynchronie a été réduit à 8% (6 - 10) en VNI NAVA, versus 27% (19 - 56) et 32% (21 - 38) en périodes de VNI conventionnelle initiale et finale, respectivement (p= 0.05). Aucune différence en termes de demande respiratoire n’a été observée. Conclusion: La VNI NAVA est faisable et bien tolérée chez les enfants avec détresse respiratoire aigüe et permet une meilleure synchronisation patient-ventilateur. De plus larges études sont nécessaires pour évaluer l’impact clinique de ces résultats.

The purpose of this research was to investigate the effects of wetland restoration on the water balance, flushing time, and water chemistry of southern Taylor Slough, a major water way in Everglades National Park. Water balance and flushing time equations were calculated on a monthly time step from 2001 – 2011. Water chemistry of major ions and nutrients were analyzed and correlated with water flushing times. Results showed that evapotranspiration followed by water volume had the greatest influence on flushing time. The flushing times varied between 3 and 78 days, with longer times observed between October and December, and the shorter times between March and May. Ion concentrations at the coastal areas decreased with increased flushing times. Increased surface water inflow that resulted from restoration projects and water management changes were productive in the rainy season and should result in increased flushing times and decreased ion concentrations in Taylor Slough.

Nous avons utilisé la télédétection pour examiner comment l’abondance du caribou migrateur pouvait influencer la quantité de ressources alimentaires, et comment ces changements pouvaient affecter la dynamique de population et les patrons d’utilisation de l’espace des caribous. Nous avons évalué les relations entre le caribou et ses ressources alimentaires pour l’aire de mise bas et l’aire d’estivage du troupeau Rivière-George (TRG) du nord du Québec et du Labrador (Canada) entre 1991 et 2011. Nous avons modélisé les relations entre la productivité primaire et des variables climatiques, nous permettant d’isoler les effets d’autres facteurs, comme la pression de broutement des caribous. Nous avons trouvé une relation négative entre la densité de caribous et la productivité primaire à grande échelle, suggérant que la pression de broutement par les caribous pouvait réduire l’abondance des ressources alimentaires et contribuer à la dégradation de l’habitat. Une forte tendance au réchauffement durant la période d’étude, couplée avec un déclin de la taille de population du TRG, a cependant entrainé une productivité primaire plus élevée. Cette hausse de la productivité primaire pourrait représenter un rétablissement de la végétation suite à la réduction de la pression de broutement et/ou un effet du réchauffement climatique.

São Paulo state, Brazil, has been highlighted by the sugarcane crop expansion. The actual scenario of climate and land use changes, bring attention for the large-scale water productivity (WP) analyses. MODIS images were used together with gridded weather data for these analyses. A generalized sugarcane growing cycle inside a crop land mask, from September 2011 to October 2012, was considered in the main growing regions of the state. Actual evapotranspiration (ET) is quantified by the SAFER (Simple Algorithm for Evapotranspiration Retrieving) algorithm, the biomass production (BIO) by the RUE (Radiation Use Efficiency) Monteith?s model and WP is considered as the ratio of BIO to ET. During the four generalized sugarcane crop phases, the mean ET values ranged from 0.6 to 4.0 mm day-1; BIO rates were between 20 and 200 kg ha-1 day-1, resulting in WP ranging from 2.8 to 6.0 kg m-3. Soil moisture indicators are applied, indicating benefits from supplementary irrigation during the grand growth phase, wherever there is water availability for this practice. The quantification of the large-scale water variables may subsidize the rational water resources management under the sugarcane expansion and water scarcity scenarios.

Climate variations over the year and plant density tend to strongly affect the agronomic performance of carrot crops. Thus, the objective of this work was to evaluate the performance of the cultivar Brasilia in crops under mild (winter) and high (summer) temperatures. An experiment was conducted from May 2011 to February 2012, using a randomized block design and treatments arranged in split plot, with three replications. The plots consisted of planting seasons (winter and summer) and the subplots of plant spacing (4, 6, 8 and 10 cm). The height of plants presented a linear decrease, from 53.4 to 51.0 cm, with an increase in spacing in summer planting, while in winter the greatest height (50.7 cm) was obtained with spacing of 8.0 cm between plants. The lowest commercial yields were found in summer crops and with the widest spacing between plants. The smallest spacing between plants (4 cm) had yields of 45.9 Mg ha -1in summer and of 63.1 Mg ha-1 in winter crops. The winter planting had higher fresh root weight (89.9 g root - 1 ) compared to the summer (81.4 g root - 1 ), reaching higher weight with increasing plant spacing. Higher yields are achieved with plant spacing of 4 cm during winter. The carrot can be grown throughout the year in the Submiddle of the São Francisco Valley.

L’elaborato approfondisce il diritto alla salute nell’ordinamento dell’Unione europea, con la consapevolezza che il settore della sanità, nella complessità di una tensione perdurante tra la sua matrice sociale e l’applicazione di logiche europee di mercato, rappresenta un ambito da sempre sottratto all’intervento diretto e vincolante delle istituzioni. Contemporaneamente, prende spunto dall’osservazione della transizione digitale dei sistemi sanitari nazionali per proporre una rilettura dei tradizionali equilibri istituzionali tra ordinamenti e constatare il grado di influenza dell’Unione oltre la dimensione transfrontaliera. Infatti, per le attuali esigenze di sostenibilità dei sistemi di tutela della salute e per il valore aggiunto riconosciuto alle azioni europee nel corso della gestione della pandemia da Covid-19, l’eHealth ha rappresentato l’occasione per una vigorosa intrusione delle istituzioni europee entro prerogative tipicamente statuali, fino all’emersione di una eGovernance sanitaria europea. Pertanto, la trattazione compie un percorso evolutivo che muove dalla Direttiva 2011/24 sull’assistenza transfrontaliera e l’assistenza sanitaria online, in combinato disposto con il complesso degli atti di soft law connessi, per verificarne l’esiguo impatto sui sistemi sanitari degli Stati membri e, alla luce dei recenti investimenti strategici ed interventi normativi rilevanti in tema di tecnologie applicate alla sanità, riconoscerne il sostanziale superamento. In particolare, il confronto tra l’insufficiente livello di digitalizzazione raggiunto finora nei sistemi sanitari degli Stati membri ed il tenore della Proposta di regolamento sullo European Health Data Space suggerisce l’evoluzione dell’impianto di governo dei dati sanitari stabilito nella Direttiva, a partire dalla previsione di una disciplina comune sulla cartella sanitaria. A questo proposito, l’interoperabilità tra tecnologie diviene un presupposto operativo indefettibile, che corrobora la natura ‘tecnologicamente condizionata’ del diritto alla salute e propone l’idea che la sanità digitale rappresenti un passo in avanti verso un’assistenza europea uniforme.

The conference promoting the objectives of the international European Landscape Convention (ELC) was held on 7-9 September 2011 in Inari. The primary aim of the conference was to increase awareness of the ELC and the prospects and requirements it brings to practical planning work, especially at the municipal level. The conference speakers included top experts from Sweden, Norway, Finland, France, Holland and Catalonia. This report is a collection of articles written by the experts in the ELC conference. The report is available in five languages: Finnish, Swedish, Norwegian, Northern Sámi and English. The common characteristics of the North Calotte area, such as the magnificent riverside scenery and the beautiful fell landscapes formed by the Sámi culture and reindeer management are broadly reflected in the environment. Alongside the traditional forms of land use, many other livelihoods and forms of land use now need to be accommodated in the area. Tourism, energy production, mining industry and new infrastructure create new and manifold challenges to the authorities in charge of land use in these areas. Municipalities need information and support for versatile planning in the future, so that the unique and valuable characteristics of the North can be preserved. Landscape protection and management in the landscape areas is executed through areal and land use planning, mainly through town planning and the creation of landscape management plans. The municipalities in the North Calotte region have their own partly divergent methods and practices, based on the various land use processes are executed. The municipalities and inhabitants have a great interest in preserving and protecting their living environment and maintaining and increasing the appeal of the area. Landscape is viewed as one of the most significant appealing factors that cannot be lost. The increasing land use in municipalities, for example due to energy production, mining industry and increasing tourism, create a need for more multidimensional planning. The reconciliation of the wishes of different interest groups, and traditional and new livelihoods will not be easy. Conflict is inevitable. This will lead to a greater need to engage local bodies to the planning processes right from the start of the projects. Close cooperation between different administrative branches, operators, local bodies and landscape research promote the objectives of the convention. To ensure that the work continues, a landscape co-operation group is planned to be established in the North Calotte area. It´s main task is to promote and develop landscape policy in the area.

Kansainvälinen Eurooppalaisen maisemayleissopimuksen tavoitteiden saavuttamista edistävä konferenssi järjestettiin Inarissa 7.-9.9.2011. Konferenssissa käsiteltiin maisemasopimuksen tavoitteita, tarpeita ja mahdollisuuksia Pohjois-Kalotin alueella. Tähän raporttiin on koottu konferenssissa esiintyneiden asiantuntijoiden laatimat artikkelit aiheesta. Raportin toivotaan helpottavan alueiden käytön suunnittelijoita käytännön työssään ja rohkaisevan kuntia toteuttamaan tulevaisuuden suunnittelua Eurooppalaisen maisemayleissopimuksen tavoitteiden mukaisesti. Raportti on saatavissa viidellä eri kielellä: suomeksi, ruotsiksi, norjaksi, pohjoissaameksi ja englanniksi. Pohjois-Kalotin aluetta yhdistää yhteinen tunnusomainen maisema. Saamelaiskulttuurin sekä poronhoidon muovaamat tunturimaisemat ja upeat jokivarsimaisemat heijastuvat ympäristössä laaja-alaisesti. Alueen maisemalle haasteita tuovat sen moninaiset maankäyttömuodot ja elinkeinot, kuten matkailu, kaivosteollisuus ja energiantuotanto. Arvokkaiden maisemapiirteiden säilyttäminen vaatii erityistä tuntemusta ja tietämystä alueen kunnissa. Maisemansuojelua ja -hoitoa toteutetaan alueiden- ja maankäytön suunnittelun kautta, pääasiallisesti kaavoituksella ja maisemanhoitosuunnitelmilla. Pohjois-Kalotin alueen kunnissa on erilaiset osin toisistaan poikkeavat toimintatavat ja käytännöt maisemanhoidossa. Kuitenkin alueen asukkailla on yhtäläisiä intressejä elinympäristönsä suojeluun ja säilyttämiseen. Maisema nähdään vetovoimatekijänä, jota ei haluta menettää. Lisääntyvät maankäyttötarpeet kunnissa luovat tarpeen entistä moniulotteisemmalle suunnittelulle. Paikallisten tahojen osallistaminen suunnitteluprosesseihin aikaisessa vaiheessa ja monimuotoinen vuorovaikutus, niin toimijoiden, kansalaisten kuin viranomaisten välillä vähentää konfliktien muodostumista. Tutkimustiedon tehokkaampi hyödyntäminen ja valtiorajat ylittävä yhteistyö helpottaa päätöksentekoa ja luo yhteisiä linjoja maisemansuojelutyöhön. Yhteiset kokemukset, käytännöt ja tietotaidon jakaminen sekä laajemman maisemakokonaisuuden hahmottaminen parantaa eurooppalaisen maisemayleissopimuksen tavoitteiden toteutumista. Työn jatkuvuuden varmistamiseksi Pohjois-Kalotin alueelle suunnitellaan perustettavaksi maisemayhteistyöryhmä, jonka tehtävä on maisemapolitiikan kehittäminen ja eteenpäin vieminen.

För att kunna nå de uppsatta målen för den europeiska landskapskonventionen anordnades en internationell europeisk konferens den 7–9 september 2011 i Enare. Konferensen var främst inriktad på att öka kunskapen om den europeiska landskapskonventionen och de möjligheter och behov den ger vad gäller det praktiska planeringsarbetet, särkilt på kommunnivå. Den här rapport är samlat artikel skrev av ledande expertes som var föreläsare i konferensen. Vi hoppas att den här rapport underlättar det praktiska arbetet för planerarna av de här områdena och inspirerar kommunerna att genomföra sina framtidsplaner enligt den europeiska landskapskonventionen. Publikationen finns tillgänglig på fem olika språk: finska, svenska, norska, nordsamiska och engelska. Naturen på Nordkalotten återspeglar på bred front de gemensamma karaktäristiska dragen, såsom de fantastiska älvlandskapen och de vackra fjälltrakterna formade av samekulturen och renskötseln. Turism, energiproduktion, gruvnäring och utbyggnad av infrastruktur leder till nya och mångfacetterade utmaningar för dem som är ansvariga för områdena. Kommunerna behöver kunskap och stöd för att kunna förverkliga de allt mer komplexa planerna i framtiden, så att de unika och värdefulla särdragen kan bevaras. Landskapsskyddet och -vården genomförs i landskapsområdena via områdesplanering och planering av markanvändning, i huvudsak genom planläggning och utarbetande av landskapsvårdsplaner. Kommunerna i Nordkalotten har egna, delvis olikartade metoder och praxis, på basis av vilka olika processer i anslutning till områdesanvändningen sköts. Kommunerna och kommuninvånarna har ett stort intresse att bevara och skydda den egna livsmiljön och att upprätthålla och öka områdets attraktionskraft. Landskapet ses ju i kommunerna som en av de viktigaste attraktionsfaktorerna som man inte vill förlora. Kommunernas ökade markanvändningsbehov med avseende på bland annat energiproduktionen, gruvindustrin och den växande turistnäringen skapar ett välgrundat behov av en planering med ännu fler dimensioner. Det är viktigt att lokala intressegrupper deltar i planeringen redan i början av processen. Mångsidigt samarbete mellan medborgare, aktörer och myndigheter kan minska eller hindra konflikter. En dialog mellan medborgare, organisationer och myndigheter har en stor inverkan på många projekts effekter på landskapet. Med hjälp av forskning får vi mer information om landskapen, deras betydelse och hur olika markanvändningsmetoder påverkar landskapen. Gemensamma erfarenheter och praktik, uppdelningen av kunnande (know-how) samt uppfattningen av bredare landkapshelheten dörbättrar tilsammans de möjligheterna att förverkliga målen som definierats i Europeiska landskapkonventionen.För att säkerställa att arbetet fortsätter upprättas en landskapssamarbetsgrupp på Nordkalottens område, som huvuduppgift är att utveckla landskapspolitiken och driva den framåt.

Germany is one of the eight EU member states which participate in the EU Strategy for the Baltic Sea Region along with Denmark, Estonia, Finland, Latvia, Lithuania, Poland and Sweden. Germany had a positive approach to the EUSBSR strategy (see Appendix 1) right from planning stage. This project contributed to the continuation of Germany’s co-operation with the countries in this region, which has been conducted since the mid 1980s mainly by German federal states. Germany is playing a major role as part of this strategy because it is the coordinator of its three priority areas.However, the German federal government sees the EUSBSR as a project to be implemented at the level of federal states. This has been proven by the great activity of three German federal states participating in the strategy (Hamburg, Mecklenburg-Vorpommern and Schleswig-Holstein) and at the same time the low level of engagement from the Bundestag, the federal government and expert circles. Furthermore, federal states more often formulate evaluations of the effects of co-operation achieved so far as part of the EUSBSR. Still, the relatively low level of Berlin’s engagement does not mean that it is not interested in co-operation in the Baltic region as such. Germany actively participates in the work of such bodies as the Council of the Baltic Sea States or the Baltic Marine Environment Protection Commission (HELCOM). All German entities engaged in the strategy make its future attractiveness and the success of individual projects as part of it dependent on including Russia in the EUSBSR. As long as Germany has the opportunity of regional co-operation with Russia at other forums (for example, the Council of the Baltic Sea States), it is unlikely to become more engaged in developing the strategy and enhancing co-operation as part of this project.
THE COSTS OF RAISING EQUITY RATIO FOR BANKS Evidence from publicly listed banks operating in Finland

The solvency rate of banks differs from the other corporations. The equity rate of a bank is lower than it is in corporations of other field of business. However, functional banking industry has huge impact on the whole society. The equity rate of a bank needs to be higher because that makes the banking industry more stable as the probability of the banks going under will decrease. If a bank goes belly up, the government will be compensating the deposits since it has granted the bank’s depositors a deposit insurance. This means that the payment comes from the tax payers in the last resort. Economic conversation has long concentrated on the costs of raising equity ratio. It has been a common belief that raising equity ratio also increases the banks’ funding costs in the same phase and these costs will be redistributed to the banks customers as higher service charges. Regardless of the common belief, the actual reaction of the funding costs to the higher equity ratio has been studied only a little in Europe and no study has been constructed in Finland. Before it can be calculated whether the higher stability of the banking industry that is caused by the raise in equity levels compensates the extra costs in funding costs, it must be calculated how much the actual increase in the funding costs is. Currently the banking industry is controlled by complex and heavy regulation. To maintain such a complex system inflicts major costs in itself. This research leans on the Modigliani and Miller theory, which shows that the finance structure of a firm is irrelevant to their funding costs. In addition, this research follows the calculations of Miller, Yang ja Marcheggianon (2012) and Vale (2011) where they calculate the funding costs after the doubling of specific banks’ equity ratios. The Finnish banks studied in this research are Nordea and Danske Bank because they are the two largest banks operating in Finland and they both also have the right company form to able the calculations. To calculate the costs of halving their leverages this study used the Capital Asset Pricing Model. The halving of the leverage of Danske Bank raised its funding costs for 16—257 basis points depending on the method of assessment. For Nordea the increase in funding costs was 11—186 basis points when its leverage was halved. On the behalf of the results found in this study it can be said that the doubling of an equity ratio does not increase the funding costs of a bank one by one. Actually the increase is quite modest. More solvent banks would increase the stability of the banking industry enormously while the increase in funding costs is low. If the costs of bank regulation exceeds the increase in funding costs after the higher equity ratio, it can be thought that this is the better way of stabilizing the banking industry rather than heavy regulation.
