Hepatitis B Virus e Antigen Physically Associates With Receptor-Interacting Serine/Threonine Protein Kinase 2 and Regulates IL-6 Gene Expression.

We previously reported that hepatitis B virus (HBV) e antigen (HBeAg) inhibits production of interleukin 6 by suppressing NF-κB activation. NF-κB is known to be activated through receptor-interacting serine/threonine protein kinase 2 (RIPK2), and we examined the mechanisms of interleukin 6 regulation by HBeAg. HBeAg inhibits RIPK2 expression and interacts with RIPK2, which may represent 2 mechanisms through which HBeAg blocks nucleotide-binding oligomerization domain-containing protein 1 ligand-induced NF-κB activation in HepG2 cells. Our findings identified novel molecular mechanisms whereby HBeAg modulates intracellular signaling pathways by targeting RIPK2, supporting the concept that HBeAg could impair both innate and adaptive immune responses to promote chronic HBV infection.

Transcription Factor NFATc2 Controls the Emergence of Colon Cancer Associated with IL-6-Dependent Colitis.

NFAT transcription factors control T-cell activation and function. Specifically, the transcription factor NFATc2 affects the regulation of cell differentiation and growth and plays a critical role in the development of colonic inflammation. Here, we used an experimental model of colitis-associated colorectal carcinoma to investigate the contribution of NFATc2 to the promotion of colonic tumors. Compared with wild-type animals that readily presented with multiple colon tumors, NFATc2-deficient mice were protected from tumor development. This observed decrease in colonic tumor progression was associated with reduced endoscopic inflammation, increased apoptosis of lamina propria T lymphocytes, and significantly reduced levels of the critical proinflammatory cytokines interleukin (IL)-21 and IL-6. Administration of hyper IL-6 abrogated protection from tumor progression in NFATc2-knockout mice and restored tumor incidence to control levels. Taken together, our findings highlight a pivotal role for NFATc2 in the establishment of inflammation-associated colorectal tumors mediated by control of IL-6 expression. Quaternary prevention influences all the activities of family medicine by questioning the utility of primary prevention and early diagnosis, identifying the risks of creating new pathological entities and practicing a maximalist medicine. Family doctors can support quaternary prevention by focusing on their patients' priorities and the local resources of an efficient health system.

Du sens du jeu à la raison d'agirComment est-ce qu'un enfant apprend le sens des mots à partir de mots dont il ne comprend pas le sens ?La thèse « Du sens du jeu à la raison» défend une conception de l'acquisition du langage chez l'enfant. Inspirée de la philosophie de Wittgenstein, elle conteste une représentation empiriste qui voudrait que l'enfant apprenne d'abord la signification des mots dans son expérience prélinguistique du monde pour ensuite associer des noms à des objets - qu'ils soient internes ou qu'ils soient externes - pour revendiquer une représentation profondément anthropologique : celle-ci veut que l'enfant apprenne à parler dans des jeux verbaux dans lesquels il fait usage des mots qu'il hérite d'un mode de vie particulier.«Il ne s'agit pas d'expliquer un jeu de langage par nos expériences vécues, mais de constater un jeu de langage» (Wittgenstein, Recherches philosophiques, Gallimard, Paris, §655).La méthode de recherche est aussi originale. Elle ne se veut pas empirique, mais grammaticale. Il s'agit au moyen d'une série d'exemples - des vidéos d'enfants qui apprennent à parler - de proposer une représentation synoptique de nos usages ordinaires du langage comme «dire maman ou papa», «reconnaître une couleur», «dire aïe!»,. La description de ces jeux de langage n'a pas pour but de nous faire découvrir quelque chose de nouveau, mais de nous faire voir ce que nous avons constamment sous les yeux et qui reste inaperçu. La méthode vise donc une certaine éducation du regard.Ce retour aux jeux de langage de ceux qui apprennent à parler permet de mieux comprendre les possibilités de notre langage et contribue à nous en faire voir les impossibilités. C'est un combat contre les fausses images que nous nous faisons de notre langage, qui nous empêchent de voir le réel usage que nous faisons de nos propres mots.Pour peu que l'on s'accorde avec Wittgenstein pour considérer comme purement mythique la conception de la signification comme quelque chose qui serait associé au mot, on est alors amené à voir autrement - dans l'ordre - nos concepts d'apprentissage, de compréhension et de sujet parlant. En suivant ces étapes, différents types de descriptions (sous forme de texte, de transcription d'interactions, d'images au format de bande dessinée, de vidéos sur DVD en annexe de la thèse) se chevauchent vous invitant à voir autrement comment un enfant apprend le sens des mots à partir de mots dont il ne comprend pas le sens en évoluant progressivement du «sens du jeu à la raison d'agir».

Expansion and tissue infiltration of an allospecific CD4+CD25+CD45RO+IL-7Ralphahigh cell population in solid organ transplant recipients.

It has been recently shown (Seddiki, N., B. Santner-Nanan, J. Martinson, J. Zaunders, S. Sasson, A. Landay, M. Solomon, W. Selby, S.I. Alexander, R. Nanan, et al. 2006. J. Exp. Med. 203:1693-1700.) that the expression of interleukin (IL) 7 receptor (R) alpha discriminates between two distinct CD4 T cell populations, both characterized by the expression of CD25, i.e. CD4 regulatory T (T reg) cells and activated CD4 T cells. T reg cells express low levels of IL-7Ralpha, whereas activated CD4 T cells are characterized by the expression of IL-7Ralpha(high). We have investigated the distribution of these two CD4 T cell populations in 36 subjects after liver and kidney transplantation and in 45 healthy subjects. According to a previous study (Demirkiran, A., A. Kok, J. Kwekkeboom, H.J. Metselaar, H.W. Tilanus, and L.J. van der Laan. 2005. Transplant. Proc. 37:1194-1196.), we observed that the T reg CD25(+)CD45RO(+)IL-7Ralpha(low) cell population was reduced in transplant recipients (P < 0.00001). Interestingly, the CD4(+)CD25(+)CD45RO(+)IL-7Ralpha(high) cell population was significantly increased in stable transplant recipients compared with healthy subjects (P < 0.00001), and the expansion of this cell population was even greater in patients with documented humoral chronic rejection compared with stable transplant recipients (P < 0.0001). The expanded CD4(+)CD25(+)CD45RO(+)IL-7Ralpha(high) cell population contained allospecific CD4 T cells and secreted effector cytokines such as tumor necrosis factor alpha and interferon gamma, thus potentially contributing to the mechanisms of chronic rejection. More importantly, CD4(+)IL-7Ralpha(+)and CD25(+)IL-7Ralpha(+) cells were part of the T cell population infiltrating the allograft of patients with a documented diagnosis of chronic humoral rejection. These results indicate that the CD4(+)CD25(+)IL-7Ralpha(+) cell population may represent a valuable, sensitive, and specific marker to monitor allospecific CD4 T cell responses both in blood and in tissues after organ transplantation.

Rotator cuff disease is the most common pathology causing shoulder pain with an overall prevalence rate of 30%. There is a significant association between increasing age and the presence of rotator cuff tears. Spontaneous healing of clinically relevant tears has not been observed. Although satisfactory pain relief is possible without rotator cuff tendon healing, functional outcome is better for healed repairs. Operative treatment must be considered in the context of the reasonable expectations for success. Repairability and potential of surgically tendon construct to heal are important considerations in surgical indications. There is no difference of outcomes between the arthroscopic and the open techniques.

Après une revue bibliographique commentée sur le sujet, il propose un certain nombre de points critiques concernant la clinique de la psychothérapie individuelle d'orientation systémique.
