As melhorias funcionais após uma rotina regular de exercícios físicos nem sempre se traduzem em uma idêntica melhoria da condição aeróbica (AER). O objetivo foi identificar se uma rotina regular de exercícios é capaz de manter ou atenuar a queda do condicionamento aeróbico e funcional, bem como se a diferença nas melhorias nestas variáveis, em indivíduos idosos, pode ser explicada por variações em flexibilidade e força/potência muscular. No primeiro estudo, 176 jogadores profissionais de futebol foram divididos em tercis em relação à idade. Obtivemos o consumo de oxigênio (VO2) e frequência cardíaca (FC), além do perfil de flexibilidade global utilizando o Flexiteste (FLX). Dados de pré-temporada (2005-2011) dos tercis extremos (n=54), mais jovem (17-22 anos) e mais velhos (27-36 anos), foram comparados. Os efeitos do envelhecimento foram avaliados pela comparação do VO2, da FC e de regressões lineares de FLX versus valores previstos para a idade. Os resultados foram semelhantes para VO2max, 62,76,1 vs 63,26,2 mL.(kg.min)-1, (p=0,67), e para FLX, 435,9 vs 416,0, respectivamente (p=0,11), o tercil mais jovem apresentou valores mais altos de FCmáx, 1948,1 vs 1898,8 bpm, (p<0,01). Os jogadores não apresentaram a diminuição prevista no VO2max, enquanto FCmax e FLX diminuiram. No segundo estudo utilizou-se dados de 144 pacientes com idade de 6212 anos submetidos a testes de FLX, força/potência muscular e cardiopulmonar de exercício máximo em cicloergômetro de membros inferiores, após pelo menos 3 meses de participação em um programa de exercício supervisionado (PES). A correlação de Pearson foi calculada para avaliar as associações entre a diferença nas melhorias funcional e aeróbica (DEMFA) e as variações de FLX, força de preensão manual (FPM) e potência muscular (POT) e também entre os valores da primeira avaliação de AER e capacidade funcional (FUN) e as respectivas melhorias e o DEMFA. Após uma média de 32 meses de PES, houve aumento da FLX em 11,6% (p<0,01) e da POT em 14,7% (p<0,01), ajustadas para a idade, com preservação da FPM (p=0,47). Houve uma relação inversa entre os resultados da primeira avaliação e a melhoria AER (r=-0,28; p<0,01). Considerando os valores previstos, a AER aumentou menos do que a FUN - 21% versus 25% (p<0,01). A melhoria na FLX associou-se ao DEMFA (r=0,24; p<0,01). Assim, os estudos mostraram: a) ao manter uma rotina regular de exercícios, principalmente aeróbicos, o VO2 não reduz com a idade em futebolistas entre os 16 e 36 anos, apesar de uma redução na FCmax. b) a participação regular em um PES proporciona melhorias de componentes da aptidão física de pacientes promovendo a restauração dos resultados para equivalentes aos previstos para indivíduos saudáveis. c) uma melhoria da FLX contribui para uma maior melhoria da FUN do que da AER

O teste de caminhada em seis minutos (TC6M) avalia a capacidade respiratória durante o exercício. Recentemente, o teste do degrau em seis minutos (TD6M) está sendo estudado como uma proposta para essa mesma avaliação. Diante do exposto, o desenvolvimento de uma equação de referência se torna importante. O objetivo desse estudo foi desenvolver uma equação de referência padrão para o Teste do Degrau em Seis minutos. Esse estudo foi do tipo transversal, em que foram selecionados 452 indivíduos. Após a aplicação dos critérios de inclusão/exclusão, foram selecionados 326 sujeitos saudáveis e sedentários com idade entre 20 e 80 anos. Para serem considerados saudáveis, os participantes não podiam ter história de doenças (exceto hipertensão arterial sistêmica ou diabetes mellitus em tratamento) e foram submetidos à radiografia de tórax, espirometria e eletrocardiograma, que deveriam ser normais. O nível de sedentarismo foi avaliado através do International Physical Activity Questionnaire (IPAQ). Foram coletados os seguintes dados demográficos: idade, peso e altura. Todos os indivíduos realizaram TD6M na sua própria cadência (autocadenciado) em um degrau de 16,5 cm de altura, 65 de largura e 30 cm de comprimento. O número de subidas e descidas foi contado por um pedômetro digital. Foram mensuradas a pressão arterial, a frequência cardíaca e a saturação de oxigênio, antes e depois do TD6M. A análise estatística foi realizada pelo software STATA 12.0, e as equações foram desenvolvidas pelo modelo estatístico de regressão linear múltipla. Como resultado, observou-se que os participantes apresentaram exames normais, sendo 135 homens e 191 mulheres. O IPAQ demonstrou 157 ativos, 114 irregularmente ativos e 40 inativos, sendo que 14 indivíduos não responderam. A análise da distância alcançada em relação à idade e à diferença da frequência cardíaca tanto para homens quanto para mulheres mostrou significância estatística, demonstrando a sua importância para o desenvolvimento das equações para cada gênero. Para mulheres: Distância (m) = 88,83 - [(FC final- FC inicial)* 0,23] (Idade *0,37); para homens: Distância (m)= 110,20 - [(FC final- FC inicial)*0,18] (Idade * 0,59). O estudo concluiu que as equações de referência desenvolvidas nesse estudo foram realizadas em uma população de indivíduos saudáveis e sedentários e pode ser usada como padrão de referência para o TD6M.

Circulatory responses of crucian carp injected intraperitoneally with extracted micro-cystins (MCs) were studied at sublethal and lethal doses (150 and 600 mu g MC kg(-1) body mass, respectively). Mean arterial blood pressure (MAP), heart rate, hematocrit (Hct), red blood cell (RBC) counts, and circulating blood volume (BV) were assayed at 0, 1, 3, 12, 24, and 48 h post-toxin administration. MAP decreased significantly in a dose-dependent manner over time. Within the 48-h test period, the lethal dose as well as the sublethal dose resulted in a steady decline of MAP without recovery. Heart rate significantly increased within 24 h post-injection as blood pressure significantly dropped, then showed a terminal decline to the control level. The dose-dependent decreases in BV and Hct were directly related to the drop in MAP. Intraperitoneal injection of a lethal dose of MCs led to hepatic and gill hemorrhage. Consequently, crucian carp given MCs suffered from hypovolemic hypotensive shock. (C) 2009 Elsevier Ltd. All rights reserved.

Deaths from microcystin toxication have widely been attributed to hypovolemic shock due to hepatic interstitial hemorrhage, while some recent studies suggest that cardiogenic complication is also involved. So far, information on cardiotoxic effects of MC has been rare and the underlying mechanism is still puzzling. The present study examined toxic effects of microcystins on heart muscle of rats intravenously injected with extracted MC at two doses, 0.16LD(50) (14 mu g MC-LReq kg(-1) body weight) and 1LD(50) (87 mu g MC-LReq kg(-1) body weight). In the dead rats, both TTC staining and maximum elevations of troponin I levels confirmed myocardial infarction after MC exposure, besides a serious interstitial hemorrhage in liver. In the 1LD(50) dose group, the coincident falls in heart rate and blood pressure were related to mitochondria dysfunction in heart, while increases in creatine kinase and troponin I levels indicated cardiac cell injury. The corresponding pathological alterations were mainly characterized as loss of adherence between cardiac myocytes and swollen or ruptured mitochondria at the ultrastructural level. MC administration at a dose of 1LD(50) not only enhanced activities and up-regulated mRNA transcription levels of antioxidant enzymes, but also increased GSH content. At both doses, level of lipid peroxides increased obviously, suggesting serious oxidative stress in mitochondria. Simultaneously. complex I and III were significantly inhibited, indicating blocks in electron flow along the mitochondrial respiratory chain in heart. In conclusion, the findings of this study implicate a role for MC-induced cardiotoxicity as a potential factor that should be considered when evaluating the mechanisms of death associated with microcystin intoxication in Brazil. The aim of this research is to explore heart rate variability frequency field characteristics and personality influential factors of internet addicts by experimentation and questionnaire. Two studies were carried out: study 1 was to explore the pathological and mental mechanism of internet addicts by heart rate variability physiological index and EPQ and internet addiction scale. Study 2 was to compare the personality and mental characteristics between internet addicts and Non-addicts. The testees were 30 internet addict schoolboys who were in-patient and 43 schoolboys who accorded with qualification from grade 2 in senior high school and sophomore. It is found that: 1、 Internet addicts have obviously lower HFNU than Non-addicts, but have obviously higher LFNU and LF/HF ratio than Non-addicts. Internet addicts have dysfunction in their sympathetic and parasympathetic system; 2、 Internet addicts and Non-addicts have no significant difference in their EPQ except their lying point, but the nervous characteristics of EPQ of internet addicts have influence on their equipoise of parasympathetic system, that is to say when the score of nervous characteristics of EPQ become higher, equipoise of parasympathetic system become worse and worse. However the EPQ personality characteristics of non-addicts have no influence on their sympathetic and parasympathetic system. 3、 The extent of internet addiction of the internet addicts is independent of their equipoise of parasympathetic system, but non-addicts use internet more time, their equipoise of parasympathetic system become lower, there is the significant difference in the neurophysiology between internet addicts and non-addicts. That is to say internet environment is safe to most adolescent, they can make use of internet environment accurately, but internet addicts, their physiological and psychological level has changed to a certain, need to be treated.; 4、 Serious internet game addicts have the metal characteristics of low social support, low purpose in life and low adventure; 5、 The objective support、support utilization of social support questionnaire and sensation seeking characteristics have prognosticative function for internet addiction degree; 6、 Serious internet game addicts have the metal characteristics of low social support, low lying particularity, they are inclined to self abandonment. Serious internet game addicts of low N characteristic have high sensation seeking characteristic, and at the same E personality foundation, they also seek new stimulus with higher intension. There is a prompt that we should pay attention to internet addicts’ personality so that obtain better curative effect for internet addiction therapy.

Previous studies have witnessed some psychological or behavioral deviation (such as aggressive behavior) might have an association with cerebral hemisphere cooperative dysfunction, however, it is still unclear whether there is an association between individuals with social cognitive bias and their hemispheric cooperative functions especially while the interhemisphere cooperative processing is under the conditions of emotional interferences. The purpose of this study is to explore the differences between the social cognitive bias group and the normal group’s interhemispheric cooperative functional activity under the conditions of with or without interferences. Methods: According to Dodge’s (1993) model of “social-cognitive mechanisms in the development of conduct disorder and depression”, a 51 items of “social cognitive bias scale” was created and was used to screen the high score group. 20 male subjects was composed of high score group and other 23 matched the control group. Stimulus tachistoscopically presented to the bilateral visual field and compared with the central. Both group’s interhemispheric cooperative functional activity were observed and compared under the conditions of without interference- i.e. base level and with the emotional interferences of white noise level and negative evaluative feedback speech level while finishing: experiment one: Chinese word-figure Stroop analogue task; experiment two: two single Chinese Characters combination task. Heart rate and respiratory rate were simultaneously recorded as index of emotional changes. Results: ① The high score group showed a decrease in processing accuracy compared with the normal group under the condition of white noise interference level in experiment one. ② Still under the condition of white noise interference level, there were more reaction time and more errors were observed in high score group than normal in experiment two. ③ Both groups showed speed up effect and the strategic processing tendency of speed-accuracy trade-off effect under the condition of white noise interference level in both experiments. ④ Between group differences of interhemipheric cooperative function were not observed under the conditions of base level and the negative evaluative feedback speech level within both experiments. Conclusion: The results suggested that interhemispheric cooperative functional differences exists between the two groups, characterized as ① differences existed in interhemispheric cooperative processing strategy between the two groups, with the high score group presented “hierarchic” deficiency strategy. ② the appearance of the differences between the two groups were condition specified , and in this research it was only under the white noise interference condition. ③ the features of the differences between the two groups were the differences on multidimensional performances and with a deficit orientation in high score group. ④ the varieties of the differences were changing with cooperative tasks, as in this research the high score group performed worse in complementary cooperative task. In addition, both group adjusted the processing strategy respectively under the condition of white noise evoked emotional interference implied that the interaction between the interhemisphere cooperative processing and emotion might exist.

This study sought predictors of mortality in patients aged >or=75 years with a first ST-segment elevation myocardial infarction (STEMI) and evaluated the validity of the GUSTO-I and TIMI risk models. Clinical variables, treatment and mortality data from 433 consecutive patients were collected. Univariable and multivariable logistic regression analyses were applied to identify baseline factors associated with 30-day mortality. Subsequently a model predicting 30-day mortality was created and compared with the performance of the GUSTO-I and TIMI models. After adjustment, a higher Killip class was the most important predictor (OR 16.1; 95% CI 5.7-45.6). Elevated heart rate, longer time delay to admission, hyperglycemia and older age were also associated with increased risk. Patients with hypercholesterolemia had a significantly lower risk (OR 0.46; 95% CI 0.24-0.86). Discrimination (c-statistic 0.79, 95% CI 0.75-0.84) and calibration (Hosmer-Lemeshow 6, p = 0.5) of our model were good. The GUSTO-I and TIMI risk scores produced adequate discrimination within our dataset (c-statistic 0.76, 95% CI 0.71-0.81, and c-statistic 0.77, 95% CI 0.72-0.82, respectively), but calibration was not satisfactory (HL 21.8, p = 0.005 for GUSTO-I, and HL 20.6, p = 0.008 for TIMI). In conclusion, short-term mortality in elderly patients with a first STEMI depends most importantly on initial clinical and hemodynamic status. The GUSTO-I and TIMI models are insufficiently adequate for providing an exact estimate of 30-day mortality risk.

The aim of this study was to assess the appearance of cardiac troponins (cTnI and/or cTnT) after a short bout (30 s) of ‘all-out’ intense exercise and to determine the stability of any exercise-related cTnI release in response to repeated bouts of high intensity exercise separated by 7 days recovery. Eighteen apparently healthy, physically active, male university students completed two all-out 30 s cycle sprint, separated by 7 days. cTnI, blood lactate and catecholamine concentrations were measured before, immediately after and 24 h after each bout. Cycle performance, heart rate and blood pressure responses to exercise were also recorded. Cycle performance was modestly elevated in the second trial [6·5% increase in peak power output (PPO)]; there was no difference in the cardiovascular, lactate or catecholamine response to the two cycle trials. cTnI was not significantly elevated from baseline through recovery (Trial 1: 0·06 ± 0·04 ng ml−1, 0·05 ± 0·04 ng ml−1, 0·03 ± 0·02 ng ml−1; Trial 2: 0·02 ± 0·04 ng ml−1, 0·04 ± 0·03 ng ml−1, 0·05 ± 0·06 ng ml−1) in either trial. Very small within subject changes were not significantly correlated between the two trials (r = 0·06; P>0·05). Subsequently, short duration, high intensity exercise does not elicit a clinically relevant response in cTnI and any small alterations likely reflect the underlying biological variability of cTnI measurement within the participants.

As a by-product of the ‘information revolution’ which is currently unfolding, lifetimes of man (and indeed computer) hours are being allocated for the automated and intelligent interpretation of data. This is particularly true in medical and clinical settings, where research into machine-assisted diagnosis of physiological conditions gains momentum daily. Of the conditions which have been addressed, however, automated classification of allergy has not been investigated, even though the numbers of allergic persons are rising, and undiagnosed allergies are most likely to elicit fatal consequences. On the basis of the observations of allergists who conduct oral food challenges (OFCs), activity-based analyses of allergy tests were performed. Algorithms were investigated and validated by a pilot study which verified that accelerometer-based inquiry of human movements is particularly well-suited for objective appraisal of activity. However, when these analyses were applied to OFCs, accelerometer-based investigations were found to provide very poor separation between allergic and non-allergic persons, and it was concluded that the avenues explored in this thesis are inadequate for the classification of allergy. Heart rate variability (HRV) analysis is known to provide very significant diagnostic information for many conditions. Owing to this, electrocardiograms (ECGs) were recorded during OFCs for the purpose of assessing the effect that allergy induces on HRV features. It was found that with appropriate analysis, excellent separation between allergic and nonallergic subjects can be obtained. These results were, however, obtained with manual QRS annotations, and these are not a viable methodology for real-time diagnostic applications. Even so, this was the first work which has categorically correlated changes in HRV features to the onset of allergic events, and manual annotations yield undeniable affirmation of this. Fostered by the successful results which were obtained with manual classifications, automatic QRS detection algorithms were investigated to facilitate the fully automated classification of allergy. The results which were obtained by this process are very promising. Most importantly, the work that is presented in this thesis did not obtain any false positive classifications. This is a most desirable result for OFC classification, as it allows complete confidence to be attributed to classifications of allergy. Furthermore, these results could be particularly advantageous in clinical settings, as machine-based classification can detect the onset of allergy which can allow for early termination of OFCs. Consequently, machine-based monitoring of OFCs has in this work been shown to possess the capacity to significantly and safely advance the current state of clinical art of allergy diagnosis

BACKGROUND: Coronary artery bypass grafting (CABG) is often used to treat patients with significant coronary heart disease (CHD). To date, multiple longitudinal and cross-sectional studies have examined the association between depression and CABG outcomes. Although this relationship is well established, the mechanism underlying this relationship remains unclear. The purpose of this study was twofold. First, we compared three markers of autonomic nervous system (ANS) function in four groups of patients: 1) Patients with coronary heart disease and depression (CHD/Dep), 2) Patients without CHD but with depression (NonCHD/Dep), 3) Patients with CHD but without depression (CHD/NonDep), and 4) Patients without CHD and depression (NonCHD/NonDep). Second, we investigated the impact of depression and autonomic nervous system activity on CABG outcomes. METHODS: Patients were screened to determine whether they met some of the study's inclusion or exclusion criteria. ANS function (i.e., heart rate, heart rate variability, and plasma norepinephrine levels) were measured. Chi-square and one-way analysis of variance were performed to evaluate group differences across demographic, medical variables, and indicators of ANS function. Logistic regression and multiple regression analyses were used to assess impact of depression and autonomic nervous system activity on CABG outcomes. RESULTS: The results of the study provide some support to suggest that depressed patients with CHD have greater ANS dysregulation compared to those with only CHD or depression. Furthermore, independent predictors of in-hospital length of stay and non-routine discharge included having a diagnosis of depression and CHD, elevated heart rate, and low heart rate variability. CONCLUSIONS: The current study presents evidence to support the hypothesis that ANS dysregulation might be one of the underlying mechanisms that links depression to cardiovascular CABG surgery outcomes. Thus, future studies should focus on developing and testing interventions that targets modifying ANS dysregulation, which may lead to improved patient outcomes.

G protein-coupled receptors (GPCRs) play an integral role in the signal transduction of an enormous array of biological phenomena, thereby serving to modulate at a molecular level almost all components of human biology. This role is nowhere more evident than in cardiovascular biology, where GPCRs regulate such core measures of cardiovascular function as heart rate, contractility, and vascular tone. GPCR/ligand interaction initiates signal transduction cascades, and requires the presence of the receptor at the plasma membrane. Plasma membrane localization is in turn a function of the delivery of a receptor to and removal from the cell surface, a concept defined most broadly as receptor trafficking. This review illuminates our current view of GPCR trafficking, particularly within the cardiovascular system, as well as highlights the recent and provocative finding that components of the GPCR trafficking machinery can facilitate GPCR signaling independent of G protein activation.
