OBJECTIVE: To report a case of bilateral giant renal angiomyolipoma associated with tuberous sclerosis, with successful treatment, and to review the literature concerning angiomyolipoma treatment. CASE REPORT: Patient with tuberous sclerosis and angiomyolipoma diagnosed by ultrasonography during her pregnancy. At that time, the angiomyolipoma on the right side was 9 cm in diameter. Conservative management was selected during her pregnancy. The patient returned 7 years later, with a 24.7 x 19.2 x 10.7 cm tumor on the right side and another of 13 x 11.5 x 6.5 cm on the left side, in addition to multiple small angiomyolipomas. A nephron-sparing surgery with tumoral enucleation was performed on the right side, and after 3 months, the tumor on the left side was removed. Renal function in the post-operative period was preserved, and contrast medium progression was uniform and adequate in both kidneys. CONCLUSION: We conclude that an angiomyolipoma larger than 4 cm should be removed surgically, since they have a greater growth rate and pose a risk of hemorrhage. Resection of smaller tumors is safe and has decreased morbidity. Tumoral enucleation is an effective treatment method that preserves kidney function.

The objective of this study was to differentiate benign ovarian tumors from malignant ones before surgery using color and pulsed Doppler sonography, and to compare results obtained before and after use of contrast medium, thereby verifying whether contrast results in an improvement in the diagnostic sensitivity. METHODS: Sixty two women (mean age 49.9 years) with ovarian tumors were studied, 45 with benign and 17 with malignant tumors. All women underwent a transvaginal color Doppler ultrasonographic exam. A study of the arterial vascular flow was made in all tumor areas, as well as an impedance evaluation of arterial vascular flow using the resistance index. RESULT: Localization of the vessels in the tumor revealed a greater proportion of malignant tumors with detectable internal vascular flows (64%) than benign tumors with such flows (22%). There was a considerable overlap of these findings. The use of contrast identified a greater number of vessels with confirmation in the totality of tumors, but did not improve the Doppler capacity in tumoral differentiation. Malignant tumors presented lower values of resistance index than the benign ones, whether or not contrast was used. The cutoff value for resistance index that better maximized the Doppler sensitivity and specificity was 0.55. Through this value, an increase of the sensitivity after contrast use was obtained, varying from 47% to 82%, while specificity remained statistically unchanged. CONCLUSION: Although the injection of a microbubble agent improved the sensitivity of the method detecting vascularization of tumors, a positive finding for vascularization by this method was not clinically useful in the differentiation of benign and malignant ovarian tumors.

PURPOSE: Williams-Beuren syndrome is a rare multiple anomalies/mental retardation syndrome caused by deletion of contiguous genes at chromosome region 7q11.23. The aim of this work was to determine the frequency and the types of renal and urinary tract anomalies in 20 patients with Williams-Beuren syndrome. METHODS: The fluorescence in situ hybridization test using a LSI Williams syndrome region DNA probe was performed for all 20 patients to confirm the diagnosis of Williams-Beuren syndrome. A prospective study was performed in order to investigate renal and urinary aspects using laboratory assays to check renal function, ultrasonography of the kidneys and urinary tract, voiding cystourethrogram and urodynamics. RESULTS: Deletion of the elastin gene (positive fluorescence in situ hybridization test) was found in 17 out of 20 patients. Renal alterations were diagnosed in 5 of 17 (29%) the patients with the deletion and in 1 of 3 patients without the deletion. Fourteen patients with the deletion presented dysfunctional voiding. Arterial hypertension was diagnosed in 3 patients with deletions and 1 of these presented bilateral stenosis of the renal arteries. CONCLUSIONS: Due to the high incidence of renal and urinary abnormalities in Williams-Beuren syndrome, performing a systematic laboratory and sonographic evaluation of the patients is recommended.

OBJETIVO: Investigar a presença de transtornos depressivos em crianças portadoras de leucemia linfoide aguda (LLA) e insuficiência renal crônica terminal (IRCT) atendidas no IMIP. MÉTODO: Estudo descritivo do tipo série de casos, composto por 52 crianças entre 8 e 15 anos portadoras de LLA e de IRCT. RESULTADOS: Três (5,8%) casos preenchiam os critérios para episódio depressivo maior (EDM), sendo dois portadores de IRCT e um portador de LLA. Oito (15,4%) preenchiam os critérios para transtorno distímico (TD), todos eles portadores de IRCT. A associação entre faixa etária e EDM não foi significativa (p=0,327). Entretanto, a faixa etária foi significante em relação ao TD (p=0,014), todos os seus portadores tinham entre 12 e 15 anos de idade. A associação entre os transtornos depressivos e o tempo de evolução da doença de base não foi significante. Contudo, observou-se uma tendência a quanto maior o tempo de evolução da doença de base, maior a associação com o TD. CONCLUSÃO: A frequência de EDM ficou dentro da faixa encontrada na literatura para escolares saudáveis, entretanto, a de TD foi mais alta. Não foram encontradas diferenças significantes entre as faixas etárias no diagnóstico de EDM. Porém, corroborando a literatura, a faixa etária maior prevaleceu em relação ao TD.

Renal cell tumors (RCTs) are the most lethal of the common urological cancers. The widespread use of imaging entailed an increased detection of small renal masses, emphasizing the need for accurate distinction between benign and malignant RCTs, which is critical for adequate therapeutic management. Histone methylation has been implicated in renal tumorigenesis, but its potential clinical value as RCT biomarker remains mostly unexplored. Hence, the main goal of this study was to identify differentially expressed histone methyltransferases (HMTs) and histone demethylases (HDMs) that might prove useful for RCT diagnosis and prognostication, emphasizing the discrimination between oncocytoma (a benign tumor) and renal cell carcinoma (RCC), especially the chromophobe subtype (chRCC). We found that the expression levels of three genes-SMYD2, SETD3, and NO66-was significantly altered in a set of RCTs, which was further validated in a large independent cohort. Higher expression levels were found in RCTs compared to normal renal tissues (RNTs) and in chRCCs comparatively to oncocytomas. SMYD2 and SETD3 mRNA levels correlated with protein expression assessed by immunohistochemistry. SMYD2 transcript levels discriminated RCTs from RNT, with 82.1% sensitivity and 100% specificity (AUC=0.959), and distinguished chRCCs from oncocytomas, with 71.0% sensitivity and 73.3% specificity (AUC: 0.784). Low expression levels of SMYD2, SETD3, and NO66 were significantly associated with shorter disease-specific and disease-free survival, especially in patients with non-organ confined tumors. We conclude that expression of selected HMTs and HDMs might constitute novel biomarkers to assist in RCT diagnosis and assessment of tumor aggressiveness.

Objetivos Comparar a ocorrência de sintomas depressivos entre dois grupos homogêneos de pacientes em diálise, um em hemodiálise (HD) e outro em diálise peritoneal (DP), verificando o possível papel preditor do método, assim como avaliar a influência de variáveis sociodemográficas e clínicas no diagnóstico deste transtorno. Métodos Amostra envolveu dois grupos homogêneos de pacientes em TRS, 30 em HD e 30 em DP. Aplicou-se o Beck Depression Inventory (BDI), escala que avalia a presença e intensidade de sintomas depressivos. Resultados Os grupos apresentavam características sociodemográficas e clínicas semelhantes, exceto quanto à escolaridade. A média de pontos do BDI no grupo HD foi maior que no grupo DP: 12,53 versus 11,13 (p = 0,352). A presença de sintomas depressivos no grupo em HD ocorreu em 36,7% dos pacientes contra apenas 23,3% do grupo DP (OR 1,9 [IC 0,61 – 5,86]). Não houve diferenças quanto às variáveis, exceto escolaridade. Dos pacientes com ensino fundamental I, 52,9% apresentaram sintomas. Nos pacientes com ensino médio ou superior, a ocorrência de sintomas depressivos foi 20% (p = 0,051). Conclusão Houve tendência à ocorrência de sintomas depressivos em pacientes em HD quando comparados aos pacientes em DP. O risco dessa ocorrência foi quase duas vezes maior nos em HD. Menor escolaridade associou-se à ocorrência de sintomas. As médias da pontuação do BDI nos dois grupos não foram significantes.

OBJETIVO: Avaliar a estrutura e função do ventrículo esquerdo (VE) e a rigidez arterial em portadores de diabetes mellitus tipo II. MÉTODOS: Foram estudados 13 doentes diabéticos de ambos os sexos (55±8 anos) sem outras doenças. A estrutura e função do VE foram avaliadas por meio de ecodopplercardiografia associada à monitorização não invasiva da pressão arterial (PA). Os resultados foram comparados aos obtidos em grupo de indivíduos normais de mesma idade (n=12). RESULTADOS: Não houve diferenças entre os grupos quanto a PA diastólica, dimensões das câmaras esquerdas e índices de função sistólica e diastólica. Os pacientes diabéticos apresentaram índice de massa do VE (101±10 vs 80±14g/m²; p<0,001) e índice de rigidez arterial sistêmica (0,86±0,26 vs 0,69±0,19mmHg/mL; p<0,05) significantemente maiores que os controles. CONCLUSÃO: O diabetes mellitus está associado a aumento da rigidez arterial sistêmica e esse fator poderia contribuir para seus efeitos adversos sobre o VE.

OBJETIVO: Avaliar pelo Doppler-ecocardiograma alterações cardíacas, estrutural e/ou funcionais, que podem surgir nos jovens normotensos com história familiar de hipertensão arterial sistêmica (HAS). MÉTODOS: Estudo prospectivo realizado com 62 jovens normotensos entre 15 e 30 anos, sendo 32 filhos de hipertensos (grupo 1) e 30 filhos de normotensos (grupo 2) comparáveis quanto à pressão arterial, superfície corporal, freqüência cardíaca, idade e sexo. Após exame clínico, todos realizaram Doppler-ecocardiograma para avaliar as estruturas cardíacas e a função ventricular esquerda (sistólico e diastólica). RESULTADOS: A fração de encurtamento sistólico do ventrículo esquerdo (VE) foi significativamente maior no grupo 1 - valor médio de 38,03±4,95% - do que no grupo 2 - 34,7±4,48 % (p<0,01). O tempo de desaceleração mitral (TD) variou de 85 a 160ms - médio de 116,47±16,99ms - no grupo 1 e de 100 a 220ms - médio de 126,73±26,66ms - no grupo 2 (p<0,05). Houve correlação entre o índice de massa do VE e o diâmetro do átrio esquerdo (AE) no grupo 1 (r= 0,514, p<0,01). CONCLUSÃO: Os filhos de hipertensos, quando comparados com filhos de normotensos, exibem exacerbação da função sistólica do VE, semelhante ao que ocorre na fase inicial da HAS ou na HAS "borderline", mesmo que não haja hipertrofia do VE ou aumento dos níveis pressóricos. O TD mitral (mais curto no grupo 1) foi o único parâmetro de função diastólica do VE que diferiu nos 2 grupos analisados. A correlação entre o índice de massa do VE e o diâmetro do AE nos filhos de hipertensos sugere que o AE se modifica de acordo com as alterações funcionais e hemodinâmicas do VE.

OBJETIVE: To evaluate by Doppler echocardiography (DE) early abnormalities of ventricular function in HIV-positive patients, as well as other cardiac abnormalities that can be detected by this method, with special emphasis on mitral valve flow. METHODS: 84 HIV- positive patients, 59 with CD4 cell count >500/mm³ (Group A) and 25 with CD4 cell count <500/mm³ (Group B), were analyzed. CD4 cells were counted and matched with structural data and systolic and diastolic function of the left ventricle (LV), as analyzed by DE. The results were compared with those obtained in 47 healthy individuals (Group C). RESULTS: 8% of patients in Group B had mild pericardial effusion; 31.5% showed decreased systolic function of the LV, and 12% had moderate mitral regurgitation. A wave velocity from the mitral inflow was different among the 3 groups, being higher in Group B, where the deceleration time of the E wave of the mitral inflow and the E/A ratio were significantly lower with a normal value of the isovolumic relaxation time (IVRT). CONCLUSION: HIV-positive patients with a CD4 cell count >500/mm³ had no abnormalities by DE. Patients with a more advanced infection (those with a CD4 cell count <500/mm³), had a significantly abnormal LV systolic function and a higher incidence of pericardial effusion and mitral regurgitation. Mitral valve inflow by Doppler did not indicate diastolic dysfunction.

OBJECTIVE: Anatomical and functional assessment of the heart through Doppler and echocardiography in patients with cell anemia (SCA). METHODS: Twenty-five patients with SCA and ages ranging from 14 to 45 years were prospectively studied in a comparison with 25 healthy volunteers. All of them underwent clinical and laboratory evaluation and Doppler echocardiography as well.The measurements were converted into body surface indices. RESULTS: There were increases in all chamber diameters and left ventricle (LV) mass of the SCA patients. It was characterised an eccentric hypertrophy of the left ventricle. The preload was increased (left ventricle end-diastolic volume) and the afterload was decreased (diastolic blood pressure, peripheral vascular resistance and end-systolic parietal stress ESPS). The cardiac index was increased due to the stroke volume. The ejection fraction and the percentage of the systolic shortening , as well as the systolic time intervals of the LV were equivalent. The isovolumetric contraction period of the LV was increased. The mitral E-septum distance and the end-systolic volume index (ESVi) were increased. The ESPS/ESVi ratio,a loading independent parameter, was decreased in SCA, suggesting systolic dysfunction. No significant differences in the diastolic function or in the pulmonary pressure occurred. CONCLUSION: Chamber dilations, eccentric hypertrophy and systolic dysfunction confirm the evidence of the literature in characterizing a sickle cell anemia cardiomyopathy.

We report here a case of coronary artery fistula in a neonate with clinical signs of heart failure. The electrocardiogram showed signs of left ventricular hypertrophy and diffuse alterations in ventricular repolarization. Chest X-ray showed an enlargement of the cardiac silhouette with an increase in pulmonary flow. After echocardiographic diagnosis and angiographic confirmation, closure of the fistulous trajectory was performed with a detachable balloon with an early and late successful outcome.
