The progressive myoclonic epilepsies (PMEs) are a clinically and etiologically heterogeneous group of symptomatic epilepsies characterized by myoclonus, tonic-clonic seizures, psychomotor regression and ataxia. Different disorders have been classified as PMEs. Of these, the group of neuronal ceroid lipofuscinoses (NCLs) comprise an entity that has onset in childhood, being the most common cause of neurodegeneration in children. The primary aim of this thesis was to dissect the molecular genetic background of patients with childhood onset PME by studying candidate genes and attempting to identify novel PME-associated genes. Another specific aim was to study the primary protein properties of the most recently identified member of the NCL-causing proteins, MFSD8. To dissect the genetic background of a cohort of Turkish patients with childhood onset PME, a screen of the NCL-associated genes PPT1, TPP1, CLN3, CLN5, CLN6, MFSD8, CLN8 and CTSD was performed. Altogether 49 novel mutations were identified, which together with 56 mutations found by collaborators raised the total number of known NCL mutations to 364. Fourteen of the novel mutations affect the recently identified MFSD8 gene, which had originally been identified in a subset of mainly Turkish patients as the underlying cause of CLN7 disease. To investigate the distribution of MFSD8 defects, a total of 211 patients of different ethnic origins were evaluated for mutations in the gene. Altogether 45 patients from nine different countries were provided with a CLN7 molecular diagnosis, denoting the wide geographical occurrence of MFSD8 defects. The mutations are private with only one having been established by a founder-effect in the Roma population from the former Czechoslovakia. All mutations identified except one are associated with the typical clinical picture of variant late-infantile NCL. To address the trafficking properties of MFSD8, lysosomal targeting of the protein was confirmed in both neuronal and non-neuronal cells. The major determinant for this lysosomal sorting was identified to be an N-terminal dileucine based signal (9-EQEPLL-14), recognized by heterotetrameric AP-1 adaptor proteins, suggesting that MFSD8 takes the direct trafficking pathway en route to the lysosomes. Expression studies revealed the neurons as the primary cell-type and the hippocampus and cerebellar granular cell layer as the predominant regions in which MFSD8 is expressed. To identify novel genes associated with childhood onset PME, a single nucleotide polymorphism (SNP) genomewide scan was performed in three small families and 18 sporadic patients followed by homozygosity mapping to determine the candidate loci. One of the families and a sporadic patient were positive for mutations in PLA2G6, a gene that had previously been shown to cause infantile neuroaxonal dystrophy. Application of next-generation sequencing of candidate regions in the remaining two families led to identification of a homozygous missense mutation in USP19 for the first and TXNDC6 for the second family. Analysis of the 18 sporadic cases mapped the best candidate interval in a 1.5 Mb region on chromosome 7q21. Screening of the positional candidate KCTD7 revealed six mutations in seven unrelated families. All patients with mutations in KCTD7 were reported to have early onset PME, rapid disease progression leading to dementia and no pathologic hallmarks. The identification of KCTD7 mutations in nine patients and the clinical delineation of their phenotype establish KCTD7 as a gene for early onset PME. The findings presented in this thesis denote MFSD8 and KCTD7 as genes commonly associated with childhood onset symptomatic epilepsy. The disease-associated role of TXNDC6 awaits verification through identification of additional mutations in patients with similar phenotypes. Completion of the genetic spectrum underlying childhood onset PMEs and understanding of the gene products functions will comprise important steps towards understanding the underlying pathogenetic mechanisms, and will possibly shed light on the general processes of neurodegeneration and nervous system regulation, facilitating the diagnosis, classification and possibly treatment of the affected cases.

Background: Metabolic syndrome (MS) is a clustering of cardiometabolic risk factors that is considered a predictor of cardiovascular disease, type 2 diabetes and mortality. There is no consistent evidence on whether the MS construct works in the same way in different populations and at different stages in life. Methods: We used confirmatory factor analysis to examine if a single-factor-model including waist circumference, triglycerides/HDL-c, insulin and mean arterial pressure underlies metabolic syndrome from the childhood to adolescence in a 6-years follow-up study in 174 Swedish and 460 Estonian children aged 9 years at baseline. Indeed, we analyze the tracking of a previously validated MS index over this 6-years period. Results: The estimates of goodness-of-fit for the single-factor-model underlying MS were acceptable both in children and adolescents. The construct stability of a new model including the differences from baseline to the end of the follow-up in the components of the proposed model displayed good fit indexes for the change, supporting the hypothesis of a single factor underlying MS component trends. Conclusions: A single-factor-model underlying MS is stable across the puberty in both Estonian and Swedish young people. The MS index tracks acceptably from childhood to adolescence.

Background: To know how moderate-to-vigorous physical activity (MVPA) and sedentary time change across lifespan periods is needed for designing successful lifestyle interventions. We aimed to study changes in objectively measured (accelerometry) MVPA and sedentary time from childhood to adolescence and from adolescence to young adulthood. Methods: Estonian and Swedish participants from the European Youth Heart Study aged 9 and 15 years at baseline (N = 2312) were asked to participate in a second examination 6 (Sweden) to 9/10 (Estonia) years later. 1800 participants with valid accelerometer data were analyzed. Results: MVPA decreased from childhood to adolescence (21 to 22.5 min/d per year of follow-up, P = 0.01 and ,0.001, for girls and boys respectively) and also from adolescence to young adulthood (20.8 to 22.2 min/d per year, P = 0.02 and ,0.001 for girls and boys, respectively). Sedentary time increased from childhood to adolescence (+15 and +20 min/d per year, for girls and boys respectively, P,0.001), with no substantial change from adolescence to young adulthood. Changes in both MVPA and sedentary time were greater in Swedish than in Estonian participants and in boys than in girls. The magnitude of the change observed in sedentary time was 3–6 time larger than the change observed in MVPA. Conclusions: The decline in MVPA (overall change = 30 min/d) and increase sedentary time (overall change = 2:45 h/d)observed from childhood to adolescence are of concern and might increase the risk of developing obesity and other chronic diseases later in life. These findings substantially contribute to understand how key health-related behaviors (physical activity and sedentary) change across important periods of life.

The evidence collected concerning the biocentric judgment that young children express when evaluating human actions on the environment leads some scholars to suggest that an essential understanding of the notion of living beings should appear earlier than previously believed. This research project aims to study that assumption. To this end, young children’s choice when they are put in situation of having to compare and choose the most negative option between environmentally harmful actions and the breaking of social conventions are examined. Afterwards, the results are categorized in relation to those obtained from the study of children’s grasp of the distinction between living beings and inanimate entities. The data is analysed according to the individuals’ age and overall, it suggests a lack of relationship between environmental judgment and the understanding of the concept of living beings. The final results are discussed in keeping with recent research in the field of moral development that underscores the role that unconscious emotional processing plays in the individual’s normative judgment.

Haur-obesitatea gorputzeko gantz kantitatea gehiegizkoa izateari deritzo. Gaixotasun epidemiko kontsideratu den arren, urtero, prebalentzia goraka doa. Eritasun honek ondorio latzak ekar ditzake, besteak beste, beste hainbat gaixotasun eragin baititzake. Hori dela eta, prebentzioan lan egitea da biderik onena, eta horretarako, haur eta gurasoak arrisku faktoreei buruz informatzea da bide zuzena. Lan honen bitartez, gaixotasun honen prebentziorako suertatu diren interbentzio eraginkorrak aztertu eta horietan erizainaren funtzioa zehaztu nahi dugu. Horretarako, 2010 urteaz geroztik argitaratuta dauden ikerlanetan, eraginkortasuna baieztatuta duten prebentzio-programak bilatu dira Pubmed eta Scopus datu baseak erabilita. Bilaketan, entseiu klinikoak eta berrikuspen sistematikoak kontuan hartu dira eta horietan burutatuko interbentzio eraginkorrak aztertu dira. Behin hori edukita, horietan erizainak duen funtzioa aztertu eta prebentzioan duen garrantzia analizatu da. Horretaz aparte, frogatu gabe dauden interbentzio-programak ere aztertu dira, nahiz eta daturik eraginkortzat eman ez den, erizainak barnebiltzen dituen betebeharrak zehazteko eta prebentzioan duen erabilgarritasuna aztertzeko. Hainbat entseiu kliniko aztertu eta gero, haur-obesitatearen aurkako programak eraginkorrak direla ondorioztatu da. Gainera, erizainak hauetan paper garrantzitsua du, kontrolen segimendua eta obesitatearen inguruko heziketa azaltzen baitu. Honek konfidantza eta segurtasun nahikoa ematen die guraso zein umeei eta obesitatea pairatzeko probabilitatea gutxitzen du. [EN] Studies have reported a negative association between dairy product consumption and weight status. However, not as much research has focused on cheese; therefore, the aim of this study was to study the association between cheese intake and overweight and obesity in a representative Basque adult population. A food frequency questionnaire (FFQ) was obtained from a random sample of 1081 adults (530 males and 551 females, 17–96 years old). Cheese consumption data were expressed as g/1000 kcal/day. The prevalence of overweight/obesity was higher in men (55.1%) than in women (35.4%) (p50.001). Participants with low or moderate intake of fresh and processed cheese demonstrated a higher prevalence of excess weight, compared with those with higher consumption. The confounding variables selected in multivariate analysis were occupational status and age in both genders; and place of residence in men. In conclusion, negative associations were found between consumption of some types of cheese and overweight and obesity in this population.

Alterações nutricionais, hormonais e ambientais nos períodos críticos do desenvolvimento como a gestação e/ou lactação podem influenciar a estrutura e a fisiologia de órgãos e tecidos, predispondo ao aparecimento de doenças na vida adulta. Esse fenômeno é conhecido como programação metabólica. O fumo materno na gestação/lactação tem sido associado ao sobrepeso/obesidade na infância e na vida adulta em ambos os sexos. Porém, estudos evidenciam diferenças entre os gêneros em resposta a exposição à nicotina. Já foi demonstrado que muitas mulheres param de fumar na gestação, mas a maioria destas volta a fumar na lactação. Anteriormente, mostramos que machos adultos cujas mães foram expostas à nicotina na lactação, desenvolveram obesidade central, hiperleptinemia e hipotireoidismo. Como a nicotina afeta a função adrenal e como catecolaminas e glicocorticóides têm efeitos bem conhecidos sobre o tecido adiposo, avaliamos a função da medula adrenal e o conteúdo de leptina no tecido adiposo e músculo de machos e fêmeas cujas mães foram expostas à nicotina na lactação. Dois dias pós-parto, implantamos minibombas osmóticas nas ratas lactantes dividas em: NIC infusão de nicotina (6mg/Kg/dia s.c.) por 14 dias, e C infusão de salina pelo mesmo período. Estas lactantes foram divididas de acordo com o sexo das proles. O sacrifício das proles de ambos os sexos ocorreu aos 15 (fim da exposição à nicotina) e 180 dias de vida. Aos 15 dias, os machos da prole NIC apresentaram aumento de MGV absoluta e relativa ao peso corporal (+72% e +73% respectivamente), hiperleptinemia (+35%), hipercorticosteronemia (+67%), maior peso adrenal (+39%), conteúdo de catecolaminas totais (absoluto: +69% e relativo: +41%), embora diminuição da enzima TH (-33%). Quando adultos, os machos programados exibiram maior massa corporal (+10%), MGV absoluta (+47%) e relativa (+33%), além de hiperleptinemia (+41%) e maior conteúdo de leptina no TAV (+23%). Esses animais também apresentaram hipercorticosteronemia (+77%), maior conteúdo de catecolaminas totais absoluto e relativo (+79% e +89% respectivamente) e de TH (+38%) embora tenham menor secreção de catecolaminas in vitro estimulada por cafeína (-19%) e maior expressão do ADRB3 no TAV (+59%). Em relação as fêmeas da prole NIC aos 15 dias de vida, estas apresentaram menor massa corporal (-6%) e hiperleptinemia (+41%) embora sem alteração da MGV. Aos 180 dias, as fêmeas da prole NIC apresentaram menor conteúdo de leptina no TAS (-46%) e maior conteúdo de leptina no músculo solear (+22%) e diminuição da expressão do ADRB3 no TAV (-39%). Concluímos que a nicotina materna afeta ambos, medula adrenal e tecido adiposo de forma gênero dependente, tanto em curto prazo (quando a nicotina está presente no leite materno), quanto em longo prazo (repercussões na vida adulta). De forma geral, as fêmeas da prole NIC apresentam alterações mais discretas do que os machos em ambos os períodos estudados.

O excesso de peso tem sido cada vez mais reconhecido como um importante problema de saúde entre os adolescentes. Para identificar seus fatores de risco, recentemente, alguns pesquisadores têm incorporando fatores psicossociais em modelos explicativos, incluindo a violência familiar na infância. No entanto, a questão ainda é pouco explorada na literatura, sendo este o primeiro estudo nacional sobre o assunto. O objetivo deste estudo foi avaliar se a violência na infância é um fator de risco para o excesso de peso na adolescência. Para isso, foi realizado, em 2010, um estudo transversal (linha de base do Estudo Longitudinal de Avaliação Nutricional de Adolescentes / ELANA) com 1014 adolescentes entre 13 a 19 anos de idade pertencentes ao 1 ano do ensino médio de duas escolas públicas e quatro escolas particulares, localizadas na cidade e na região metropolitana do Rio de Janeiro. O estado nutricional foi avaliado pelo índice de massa corporal (IMC) e a violência familiar na infância por meio do Childhood Trauma Questionnaire (CTQ). Modelos hierarquizados de regressão linear múltipla foram utilizados nas análises. Dos 1014 adolescentes, 53,4% eram do sexo feminino e 50,2% estudavam em escola pública. A prevalência de excesso de peso foi de 26,7%. De acordo com o modelo multivariado, houve uma menor tendência ao excesso de peso na adolescência entre os meninos que sofreram violência do tipo negligência física na infância (β=0,196, 95% CI: 0,346; 0,045, p < 0,011), e de acordo com o aumento da idade para todas as dimensões da violência aferida pelo CTQ (estimativas variaram de 0,136 a 0,126, p < 0.002). O risco de excesso de peso foi maior entre os adolescentes cujos pais apresentavam excesso de peso. Estimativas das variáveis de IMC para mães e pais variaram de 0,065 a 0,066 (p < 0,001) e 0,051 a 0,053 (p < 0,001), respectivamente. Conclui-se que a exposição à violência na infância parece não estar associada com o excesso de peso na adolescência. Foi notada uma tendência de redução do índice de massa corporal em adolescentes do sexo masculino que foram vítimas de negligência física na infância.

Eventos ou estímulos no início da vida podem afetar o desenvolvimento do indivíduo; dentre esses o tabagismo materno. A exposição materna isolada à nicotina, principal componente do cigarro, causa na prole alterações metabólicas, em curto e longo prazo, como aumento da adiposidade, resistência à leptina, e disfunção tireoideana e adrenal. Entretanto é sabido que na fumaça de cigarro estão presentes outros componentes com potenciais efeitos tóxicos. Assim propomos comparar o efeito de duas formas de exposição neonatal à fumaça do cigarro sobre o perfil endócrino-metabólico da prole em curto e longo prazo. Para isso, no 3 dia após o nascimento, ratos lactentes foram submetidos a dois modelos: Modelo I (exposição pelo leite materno), ninhadas separadas em: exposição à fumaça (EF; n=8) lactantes expostas à fumaça de cigarros 2R1F (1,7 mg de nicotina/cigarro por 1h, 4 vezes ao dia), separadas de suas proles e grupo controle (C; n=8), onde as mães foram separadas de suas proles e expostas ao ar filtrado; Modelo II (exposição direta à fumaça), ninhadas separadas em: exposição à fumaça (EF; n=8) mães e proles expostas à fumaça de cigarros 2R1F e controle (C; n=8) mães e proles expostas ao ar filtrado. A exposição ao tabaco ocorreu até o desmame. Mães sacrificadas aos desmame e proles aos desmame e aos 180 dias de idade. As mães lactantes expostas à fumaça (EF) apresentaram hipoleptinemia (-46%), hiperprolactinemia (+50%), hipoinsulinemia (-40%) e diminuição de triglicérides (-53%). Quanto a composição bioquímica do leite, as lactantes EF mostraram aumento de lactose (+52%) e triglicérides (+78%). No modelo I, as proles EF apresentaram ao desmame: diminuição da gordura corporal total (-24%), aumento de proteína corporal total (+17%), diminuição da glicemia (-11%), hiperinsulinemia (+28%), hipocorticosteronemia (-40%) e aumento de triglicérides (+34%). Quando adultas, as proles EF apresentaram somente alteração da função adrenal onde observou-se menor conteúdo de catecolaminas (-50%) e da expressão de tirosina hidroxilase na medula adrenal (-56%). No modelo II, as proles EF aos 21 dias exibiram diminuição da MC (-7%), do comprimento nasoanal (-5%), da gordura retroperitoneal (-59%), da área dos adipócitos viscerais (-60%) com maior área dos adipócitos subcutâneos (+95%), aumento do T4 (+59), da corticosterona (+60%), do conteúdo adrenal de catecolaminas (+58%) e hipoinsulinemia (-29%). Aos 180 dias, as proles EF apresentaram aumento da ingestão alimentar (+10%), dos depósitos de gordura visceral (~60%) e conteúdo de gordura corporal total (+50%), menor área dos adipócitos subcutâneos (-24%), aumento da leptina (+85%), glicemia (+11%), adiponectina (+1.4x), T3 (+71%), T4 (+57%) e TSH (+36,5%) com menor corticosterona (-41%) e catecolaminas adrenais (-57%) e aumento dos triglicerídeos (+65%). Somados nossos dados evidenciam o impacto negativo que a exposição à fumaça do cigarro tem sobre o desenvolvimento do neonato com o surgimento de desordens endócrinas futuras, que ocorrem pelo menos em parte devido às alterações observadas nas mães. Portanto, independente da forma de exposição, seja via aleitamento materno ou por inalação direta, é de grande importância alertar a sociedade sobre as possíveis complicações metabólicas em longo prazo decorrente da exposição involuntária do neonato ao tabagismo.

O objetivo deste estudo foi investigar na saliva de crianças pré-púberes obesas, em comparação com crianças eutróficas, a presença de possíveis diferenças na expressão de mediadores proteicos relacionados à metainflamação através de um estudo observacional transversal. Foram selecionadas 105 crianças pré-púberes com idade entre 5 e 9 anos, sem quaisquer outros comprometimentos sistêmicos ou bucais sendo realizada a mensuração do comprimento da circunferência da cintura (CC), além do peso e estatura para cálculo do IMC e seu escore Z (zscore IMC), de forma a compor 3 grupos: EU - eutrofia (-2SD≤ zscore IMC≤1SD), SP - sobrepeso (1SD < zscore IMC < 2SD) e OB obesidade (zscore IMC ≥ 2SD). Após o exame médico e odontológico foi feita a coleta de sangue e de saliva total não estimulada, de forma protocolada. Amostras séricas e salivares foram analisadas, individualmente, para a dosagem de IL1β, IL6, IL8. IL10, IL12p70, TNFα, MCP1, leptina, grelina e insulina, por Multiplex e adiponectina total e de alto peso molecular (adipoHMW), por ELISA. Além disso, as amostras séricas foram utilizadas para o delineamento hemodinâmico dos pacientes. As amostras salivares de EU e OB foram também analisadas em pools por espectrometria de massa (MS) e a presença de algumas proteínas foi validada por imunoblotting, para a comparação do perfil salivar proteico. As concentrações dos analitos foram comparadas tanto entre os grupos (teste de Kruswall-Wallis), como em relação ao zscore IMC e ao CC (Correlação de Spearman ou Pearson). Dos 12 analitos, somente a adipoHMW não foi detectada em nenhuma das amostras salivares. Na comparação OBxEU houve aumento na concentração total de proteínas salivares, da insulina e leptina sérica e salivar e do MCP1, além de diminuição da adiponectina sérica total e de adipoHMW e uma menor relação adiponectina/leptina (A/L) no grupo OB. As principais correlações obtidas com o zscore IMC foram com as concentrações séricas e salivares de insulina e leptina (positivas) e com a relação A/L (negativa), observando-se também a correlação negativa com a adiponectina total e adipoHMW séricas. Na comparação entre as concentrações séricas e salivares, foi possível detectar correlação positiva entre as dosagens de insulina e leptina, assim como com os valores da relação A/L. Na análise proteômica por MS foram identificadas 670 proteínas, sendo 163 delas com expressão diferenciada na saliva de OB em relação a EU. Dentre estas, encontram-se alteradas proteínas relacionadas à resposta inflamatória humoral, em especial com a via alternativa do sistema complemento e do metabolismo redox. Foram selecionadas três destas proteínas para validação por imunoblotting, que confirmou o aumento do Fator H e a diminuição do Fator B e da tioredoxina na saliva de OB em relação a EU. Considerando os resultados, podemos verificar que embora com menores concentrações absolutas dos mediadores, a saliva mostrou praticamente as mesmas associações observadas no sangue, em especial para a insulina, leptina e relação A/L, sendo possível admitir que a análise salivar venha a ser um bom método diagnóstico não invasivo para a obesidade infantil.

Background: U19/EAF2 is a potential tumor suppressor exhibiting frequent down-regulation and allelic loss in advanced human prostate cancer specimens. U 19/EAF2 has also been identified as ELL-associated factor 2 (EAF2) based on its binding to ELL, a fusion partner of MLL in acute myeloid leukemia. U19/EAF2 is a putative transcription factor with a transactivation domain and capability of sequence-specific DNA binding. Methods: Yeast-two-hybrid-screening was used to identify U19/EAF2-binding partners. Co-immunoprecipitation and mammalian 1-hybrid assay were used to characterize a U19/EAF2-binding partner. Results: FB1, an E2A fusion partner in childhood leukemia, was identified as a binding-partner of U19/EAF2. FB1 also binds to EAF1, the only homologue of U19/EAF2. FB1 also interacts and co-localizes with ELL in the nucleus. Interestingly, FB1 inhibited the transcriptional activity of U19/EAF2 but not EAF1. Conclusions: FB1 is an important binding partner and a functional regulator of U19/EAF2, EAF1, and/or ELL. (c) 2007 Elsevier Ireland Ltd. All rights reserved.

Modified nucleosides have been characterized as tumor markers for a number of malignant diseases. In order to use these markers in children, the age-dependence of the nucleoside levels in healthy children has to be established and taken into account in diagnostic decisions. In this study, the levels of 12 normal and modified nucleosides in urine of 166 healthy children and adolescents with an age between 1 day and 19 years are determined by reversed-phase HPLC, and age-dependent reference ranges are defined. The urinary nucleoside concentrations are related to the creatinine concentrations, which allows the use of randomly collected urine samples. All nucleoside levels in urine of children decrease with age, most pronounced during the first 4 years of life, and the age-dependence of the reference values of the individual nucleosides can be approximated by a mathematical function y = b(0) + b(1) (1/x) with the regression coefficients b(0) and b(1), the nucleoside levels y and the age x between 1 year and 19 years. In the very young children, the shifts in the nucleoside concentrations are more differentiated. Starting with low levels on the first day of life, the concentrations of all studied nucleosides rise up to an age of 1-2 months, when they reach their absolute maximum for all age periods, and then decrease. Introduction: Brazil is experiencing a nutritional transition characterized by a reduction in the prevalence of nutritional deficits and an increase in overweight and obesity, not only in adults but also in children and adolescents.Objectives: This study was designed to evaluate the factors associated with overweight and obesity in Brazilian 5-year-old preschoolers.Methods: A cross-sectional study of a cohort of 232 preschoolers born in Diamantina/Minas Gerais, Brazil, was undertaken. the data, including socioeconomic status, anthropometry, diet, previous history of the preschoolers and family history, were collected between July of 2009 and July of 2010. To identify the factors associated with overweight and obesity, a logistic regression and a hierarchical model were undertaken.Results: Overweight and obesity occurred in 17.2% of the preschoolers. After adjusting for mother's obesity, per capita income, protective food intake, weight gain at age 0-4 months and time spent playing, the factors associated with overweight and obesity that reached statistical significance were mother's obesity [OR = 3.12 (95% CI 1.41-6.91), P = 0.01], weight gain of more than 0.85 kg/month in the first four months of life [OR = 2.16 (95% CI 1.01-4.64), P = 0.041 and lower per capita income [OR = 0.32 (95 %CI 0.13-0.79), P = 0.01].Conclusion: the results show that more weight gain during the first four months of life and being born of mothers with obesity increased the odds of overweight/obesity in the preschoolers, while lower per capita income was a protective factor.

Background Increasing prevalence of overweight and obesity represents a global pandemic. As the largest occupational group in international healthcare systems nurses are at the forefront of health promotion to address this pandemic. However, nurses own health behaviours are known to influence the extent to which they engage in health promotion and the public's confidence in advice offered. Estimating the prevalence of overweight and obesity among nurses is therefore important. However, to date, prevalence estimates have been based on non-representative samples and internationally no studies have compared prevalence of overweight and obesity among nurses to other healthcare professionals using representative data. Objectives To estimate overweight and obesity prevalence among nurses in Scotland, and compare to other healthcare professionals and those working in non-heath related occupations. Design Cross-sectional study using a nationally representative sample of five aggregated annual rounds (2008-2012) of the Scottish Health Survey. Setting Scotland. Participants: 13,483 adults aged 17 to 65 indicating they had worked in the past 4 weeks, classified in four occupational groups: nurses (n = 411), other healthcare professionals (n = 320), unqualified care staff (n = 685), and individuals employed in non-health related occupations (n = 12,067). Main outcome measures: Prevalence of overweight and obesity defined as Body Mass Index ≥ 25.0. Methods Estimates of overweight and obesity prevalence in each occupational group were calculated with 95% confidence intervals (CI). A logistic regression model was then built to compare the odds of being overweight or obese with not being overweight or obese for nurses in comparison to the other occupational categories. Data were analysed using SAS 9.1.3. Results 69.1% (95% CI 64.6,73.6) of Scottish nurses were overweight or obese. Prevalence of overweight and obesity was higher in nurses than other healthcare professionals (51.3%, CI 45.8,56.7), unqualified care staff (68.5%, CI 65.0,72.0) and those in non-health related occupations (68.9%, CI 68.1,69.7). A logistic regression model adjusted for socio-demographic composition indicated that, compared to nurses, the odds of being overweight or obese was statistically significantly lower for other healthcare professionals (Odds Ratio [OR] 0.45, CI 0.33,0.61) and those in non-health related occupations (OR 0.78, CI 0.62,0.97). Conclusions Prevalence of overweight and obesity among Scottish nurses is worryingly high, and significantly higher than those in other healthcare professionals and non-health related occupations. High prevalence of overweight and obesity potentially harms nurses’ own health and hampers the effectiveness of nurses’ health promotion role. Interventions are therefore urgently required to address overweight and obesity among the Scottish nursing workforce.
