Background: Between 1998 and 1999, a burden of disease assessment was carried out in Victoria, Australia applying and improving on the methods of the Global Burden of Disease Study. This paper describes the methods and results of the calculations of the burden due to 22 mental disorders, adding 14 conditions not included in previous burden of disease estimates, Methods: The National Survey of Mental Health and Wellbeing provided recent data on the occurrence of the major adult mental disorders in Australia. Data from international studies and expert advice further contributed to the construction of disease models, describing each condition in terms of incidence, average duration and level of severity, with adjustments for comorbidity with other mental disorders. Disability weights for the time spent in different states of mental ill health were borrowed mainly from a study in the Netherlands, supplemented by weights derived in a local extrapolation exercise. Results: Mental disorders were the third largest group of conditions contributing to the burden of disease in Victoria, ranking behind cancers and cardiovascular diseases. Depression was the greatest cause of disability in both men and women. Eight other mental disorders in men and seven in women ranked among the top twenty causes of disability. Conclusions: Insufficient information on the natural history of many of the mental disorders, the limited information on the validity of mental disorder diagnoses in community surveys and considerable differences between ICD-10 and DSM-IV defined diagnoses were the main concerns about the accuracy of the estimates. Similar and often greater concerns have been raised in relation to the estimation of the burden from common non-fatal physical conditions such as asthma, diabetes and osteoarthritis. In comparison, psychiatric epidemiology can boast greater scientific rigour in setting standards for population surveys.

Injection drug use (involving the injection of illicit opiates) poses serious public health problems in many countries. Research has indicated that injection drug users are at higher risk for morbidity in the form of HIV/AIDS and Hepatitis B and C, and drug-related mortality, as well as increased criminal activity. Methadone maintenance treatment is the most prominent form of pharmacotherapy treatment for illicit opiate dependence in several countries, and its application varies internationally with respect to treatment regulations and delivery modes. In order to effectively treat those patients who have previously been resistant to methadone maintenance treatment, several countries have been studying and/or considering heroin-assisted treatment as a complementary form of opiate pharmacotherapy treatment. This paper provides an overview of the prevalence of injection drug use and the opiate dependence problem internationally, the current opiate dependence treatment landscape in several countries, and the status of ongoing or planned heroin-assisted treatment trials in Australia, Canada and certain European countries.

A methodology and framework for discipline-specific curriculum development in a local context is described. These activities, as part of the Thailand-Australia Science and Engineering Assistance Project, were in response to a needs analysis for curriculum assistance to a number of publicly-funded Thai universities in the engineering priority area of Materials Processing and Manufacturing. The paper outlines a strategy for the delivery of a centralised curriculum development workshop for academic staff follow-up visits and local curriculum activities with participating universities, and the presentation of technical short courses as guidance for such activity in other settings and/or discipline areas. This paper is part of a process of documentation so that others can apply the developed methodology and framework for curriculum development. While the paper is a report on curriculum activities in a particular setting, it is written in a manner that allows application of the methodology to other settings. The reader is advised that each curriculum activity needs to adopt a methodology and strategy to fit the particular circumstances being considered To assist in applying this approach elsewhere, a description of the various steps in the curriculum process, and typical responses to some of the more global issues, have been presented. Full details are available in the various TASEAP reports prepared by the authors. Specific detail has been omitted where this detail does not provide any information for generalized consumption.

Agricultural ecosystems and their associated business and government systems are diverse and varied. They range from farms, to input supply businesses, to marketing and government policy systems, among others. These systems are dynamic and responsive to fluctuations in climate. Skill in climate prediction offers considerable opportunities to managers via its potential to realise system improvements (i.e. increased food production and profit and/or reduced risks). Realising these opportunities, however, is not straightforward as the forecasting skill is imperfect and approaches to applying the existing skill to management issues have not been developed and tested extensively. While there has been much written about impacts of climate variability, there has been relatively little done in relation to applying knowledge of climate predictions to modify actions ahead of likely impacts. However, a considerable body of effort in various parts of the world is now being focused on this issue of applying climate predictions to improve agricultural systems. In this paper, we outline the basis for climate prediction, with emphasis on the El Nino-Southern Oscillation phenomenon, and catalogue experiences at field, national and global scales in applying climate predictions to agriculture. These diverse experiences are synthesised to derive general lessons about approaches to applying climate prediction in agriculture. The case studies have been selected to represent a diversity of agricultural systems and scales of operation. They also represent the on-going activities of some of the key research and development groups in this field around the world. The case studies include applications at field/farm scale to dryland cropping systems in Australia, Zimbabwe, and Argentina. This spectrum covers resource-rich and resource-poor farming with motivations ranging from profit to food security. At national and global scale we consider possible applications of climate prediction in commodity forecasting (wheat in Australia) and examine implications on global wheat trade and price associated with global consequences of climate prediction. In cataloguing these experiences we note some general lessons. Foremost is the value of an interdisciplinary systems approach in connecting disciplinary Knowledge in a manner most suited to decision-makers. This approach often includes scenario analysis based oil simulation with credible models as a key aspect of the learning process. Interaction among researchers, analysts and decision-makers is vital in the development of effective applications all of the players learn. Issues associated with balance between information demand and supply as well as appreciation of awareness limitations of decision-makers, analysts, and scientists are highlighted. It is argued that understanding and communicating decision risks is one of the keys to successful applications of climate prediction. We consider that advances of the future will be made by better connecting agricultural scientists and practitioners with the science of climate prediction. Professions involved in decision making must take a proactive role in the development of climate forecasts if the design and use of climate predictions are to reach their full potential. RESUMO - Âmbito e objectivos: O presente trabalho incide sobre a área dos cuidados paliativos, nomeadamente, visa contribuir para uma maior efectividade do processo de aquisição e acompanhamento deste tipo de cuidados. Neste contexto, e como objectivo principal do estudo, foram identificados os indicadores chave, na vertente da qualidade, indispensáveis para a conceptualização de um modelo de contratualização e, consequentemente, para assegurar a monitorização, acompanhamento e avaliação da actividade realizada. Método e metodologias: Numa primeira fase, foi conduzida uma pesquisa documental tendente ao levantamento dos indicadores, na dimensão da qualidade, em utilização nos cuidados paliativos em vários países, tendo sido escolhidos aqueles que apresentavam indicadores já validados, designadamente, o Canadá, Reino Unido, Austrália, Espanha e EUA. Desta forma, foram seleccionados os indicadores comuns aos vários países estudados, para efeitos de discussão e consensualização, no âmbito de uma Técnica de Grupo Nominal, passíveis de integrarem um modelo de contratualização nesta área em Portugal. Apresentação e discussão de resultados: Da pesquisa documental resultaram 188 indicadores na dimensão qualidade, em utilização nos países estudados, tendo sido identificados 19 indicadores comuns. Estes indicadores foram submetidos a discussão e consensualização em reunião de peritos, tendo sido hierarquizados os sete indicadores mais adequados para a contratualização em cuidados paliativos: composição de uma equipa interdisciplinar; capacidade de resposta doentes tratados/pedido de acesso; avaliação inicial e periódica das necessidades (oito domínios); acessibilidade farmacológica; congruência entre os cuidados prestados e os desejados; prática regular de reuniões multidisciplinares; conferência familiar, pelo menos 2 vezes. Conclusões: No panorama nacional, não se conhecem indicadores unanimemente ratificados como adequados à avaliação da efectividade, qualidade e eficiência dos cuidados paliativos. Por outro lado, atento o objectivo do presente trabalho, a informação recolhida a nível internacional devolveu indicadores aplicáveis maioritariamente num contexto da avaliação da prestação dos cuidados paliativos, e Indicadores de Qualidade para a Contratualização de Cuidados Paliativos em Portugal x/ 93 não tanto num contexto de um processo de contratualização efectivo. Assim, partindo destes indicadores, em utilização nos vários países estudados, o estudo permitiu identificar sete indicadores considerados como adequados para a contratualização destes cuidados, em Portugal. -------------- ABSTRACT - Scope and objectives: This work focuses on the palliative care area, in particular, aims to contribute to greater effectiveness of the monitoring and procurement process of this type of care. In this context, as the main objective of the study, key indicators were identified regarding the quality issue, crucial to the conceptualization of a contracting model, and consequently to ensure the monitoring, supervising and evaluating of the undertaken activities. Method and methodology: Initially, a documentary research was held. That research aimed to surveying the indicators in their dimension of quality, regarding their use in palliative care throughout several countries. The chosen countries already had validated indicators, namely, Canada, United Kingdom , Australia, Spain and the USA. Thus, the indicators selected were common to all the above mentioned countries, for discussion purposes and agreement, as part of a Nominal Group Technique, that could integrate a model of contracting in this area in Portugal. Presentation and result discussion: The documentary research resulted in 188 quality indicators in use in the referred countries, 19 of those indicators were common to all countries. These indicators were submitted for discussion and agreement at a meeting of experts, having been ranked the seven most appropriate indicators for palliative care contracting: composition of an interdisciplinary team; response ability to the patients / access requests, initial and periodic assessment of the needs (eight areas), pharmacology accessibility; congruence between the desired care and its effective provision and regular practice of multidisciplinary meetings, family conferences, at least two times. Conclusions: In the national scene, unanimously ratified as suitable indicators to assess the effectiveness, efficiency and quality of palliative care are unknown. On other hand, given the objective of this work, the international collection of information has given applicable indicators mostly in the context of assessing the provision of palliative care, rather than an effective contracting process context. Thus, based on these indicators in use in the studied countries, the study itself identified seven indicators considered appropriate for the contracting of such care in Portugal.

RESUMO - Objectivos: Este estudo pretende caracterizar os casos de Tuberculose notificados em Portugal Continental, em função das suas características sociodemográficas e clínicas, aferir a sua evolução através da comparação dos biénios 2000/2001 e 2008/2009 e a possibilidade de existência de diferenças entre os casos de Tuberculose na população imigrante e não imigrante. Métodos: Foi desenvolvido um estudo descritivo de casos notificados nos biénios 2000/2001 (n= 9182) e 2008/2009 (n= 6087). A fonte de dados foi o Sistema De Vigilância Intrínseco ao Programa Nacional de Luta contra a Tuberculose (SVIG-TB). Para calcular as incidências foram utilizados os dados disponibilizados pelo Instituto Nacional de Estatística e pelo Serviço de Estrangeiros e Fronteiras referentes à população não imigrante e imigrante residente em Portugal. Foi utilizado o teste do Qui-quadrado e o teste exacto de Fisher para analisar possíveis relações entre variáveis qualitativas. O teste de Mann-Whitney foi utilizado para comparação da distribuição entre variáveis quantitativas nos dois grupos. A odds ratio foi determinada para aferir a força de associação entre as variáveis e a condição de imigrante. Resultados: No biénio 2000/2001, 10,2% dos casos notificados eram de imigrantes e no biénio 2008/2009 aumentou para 13,3%. Relativamente às características sociodemográficas dos pacientes, observaram-se diferenças estatisticamente significativas apenas na idade. Nas características clínicas observaram-se diferenças significativas em algumas patologias associadas, nomeadamente na infecção VIH (p <0,001: OR=1,696).Quanto aos factores de risco, observaram-se diferenças no consumo do tabaco (p <0,001: OR= 0,499) e drogas (p <0,001; OR= 0,582). Relativamente à situação final de tratamento existem diferenças significativas nas categorias em tratamento (p <0,001;OR=0,661), tratamento completo (p <0,001;OR =1,293) e morte (p <0,001; OR=1,806). Conclusões: O número de casos de Tuberculose em Portugal, bem como a taxa de incidência, diminuíram do biénio 2000/2001 para o biénio 2008/2009 em ambos os grupos. Os imigrantes são mais jovens, têm maior risco de infecção VIH e menor risco de patologias crónicas.

Infraventus – Internationalizing a Renewables Business to Kenya Infraventus is a renewable energy development company, with a consolidated experience, since 1988, in developing, constructing and operating projects mainly in Portugal, but also in other countries: Spain, Poland, France, Australia, Finland and Panama, and is now regarding Kenya as a possible destiny. Kenya is the biggest economy of East Africa, with a growing GDP around 5% and a low level of electric grid coverage thru its territory, 33% of national access to electricity. Kenya energy policy is intended to promote the investment in renewables; wind and sun are existing resources in good measures. Kenya is an interesting destiny to Infraventus, but has many risks. The objective of this dissertation is to evaluate the possible advantages of this investment and risks and propose recommendations about how to mitigate them considering alternative entry mode

A história de um casal de emigrantes portugueses em França bateu, no verão de 2013, recordes de audiência nas salas de cinema. O filme “A Gaiola Dourada”, de Ruben Alves, recuperou a temática da emigração portuguesa, numa altura em que esta atingiu o boom registado nos anos 1960. Recorrendo ao seu percurso, o realizador refere-se à ‘portugalidade’ como alegada ‘pertença a Portugal’, que assume como um cliché, e utiliza de forma humorística vários estereótipos associados aos emigrantes portugueses, através dos quais é mostrada alguma vergonha que os filhos dos emigrantes sentem em relação ao comportamento dos pais, trazendo ao de cima os contrastes com a sociedade onde vivem. A grande ficção reside no regresso ao país de origem, concretizando o sonho da grande maioria dos emigrantes, mas subvertendo a lógica: no filme, não são os pais que voltam a Portugal, mas os filhos, que aparentemente pouco se identificam com o país dos progenitores. “A Gaiola Dourada” reintroduziu o debate sobre a emigração em Portugal, aproveitando a crise económica para refletir sobre o seu regresso em força, bem como traçar o perfil dos novos emigrantes. Será que os portugueses na ‘diáspora’ ainda reavivam a chama ‘lusitana’ (Gonçalves, 2009)? E será que a partilha do nome ‘Portugal’ basta para sublinhar uma alegada identidade nacional (Sobral, 2012)?

This article addresses the normative dilemma located within the application of `securitization,’ as a method of understanding the social construction of threats and security policies. Securitization as a theoretical and practical undertaking is being increasingly used by scholars and practitioners. This scholarly endeavour wishes to provide those wishing to engage with securitization with an alternative application of this theory; one which is sensitive to and self-reflective of the possible normative consequences of its employment. This article argues that discussing and analyzing securitization processes have normative implications, which is understood here to be the negative securitization of a referent. The negative securitization of a referent is asserted to be carried out through the unchallenged analysis of securitization processes which have emerged through relations of exclusion and power. It then offers a critical understanding and application of securitization studies as a way of overcoming the identified normative dilemma. First, it examines how the Copenhagen School’s formation of securitization theory gives rise to a normative dilemma, which is situated in the performative and symbolic power of security as a political invocation and theoretical concept. Second, it evaluates previous attempts to overcome the normative dilemma of securitization studies, outlining the obstacles that each individual proposal faces. Third, this article argues that the normative dilemma of applying securitization can be avoided by firstly, deconstructing the institutional power of security actors and dominant security subjectivities and secondly, by addressing countering or alternative approaches to security and incorporating different security subjectivities. Examples of the securitization of international terrorism and immigration are prominent throughout.

Blood transfusion is the second most common transmission route of Chagas disease in many Latin American countries. In Mexico, the prevalence of Chagas disease and impact of transfusion of Trypanosoma cruzi-contaminated blood is not clear. We determined the seropositivity to T. cruzi in a representative random sample, of 2,140 blood donors (1,423 men and 647 women, aged 19-65 years), from a non-endemic state of almost 5 millions of inhabitants by the indirect hemagglutination (IHA) and enzyme linked immunosorbent assay (ELISA) tests using one autochthonous antigen from T. cruzi parasites, which were genetically characterized like TBAR/ME/1997/RyC-V1 (T. cruzi I) isolated from a Triatoma barberi specimen collected in the same locality. The seropositivity was up to 8.5% and 9% with IHA and ELISA tests, respectively, and up to 7.7% using both tests in common. We found high seroprevalence in a non-endemic area of Mexico, comparable to endemic countries where the disease occurs, e.g. Brazil (0.7%), Bolivia (13.7%) and Argentina (3.5%). The highest values observed in samples from urban areas, associated to continuous rural emigration and the absence of control in blood donors, suggest unsuspected high risk of transmission of T. cruzi, higher than those reported for infections by blood e.g. hepatitis (0.1%) and AIDS (0.1%) in the same region.

Investing in Global Relationships: Ireland’s International Education Strategy 2010 – 2015 was published in September 2010. Significant progress has been made in implementing this strategy, to complement and support the work undertaken at institutional level: - A strong national brand is now in place: Education in Ireland, managed by Enterprise Ireland. - There is much closer alignment between education and immigration policies. - A new Government of Ireland Scholarship has been established, managed by the Higher Education Authority. - An International Education Marketing Fund has been developed which allows institutions to collaborate in Enterprise Ireland- led national marketing initiatives. - Promotional efforts are taking place in the priority markets of the USA, Brazil, China, India, SE Asia and the Gulf. Ministers have visited each of the priority markets in the last two years. - Ireland is participating at national level in major international scholarship schemes such as Science Without Borders (Brazil) and the King Abdullah Scholarship Programme (Saudi Arabia). - Statutory provision is now in place for an international education mark and code of practice, which will be rolled out in 2014 by Qualifications and Quality Assurance Ireland (QQI). - International student numbers have risen in priority sectors: 14% growth in the English language sector and 9% growth in higher education since 2010. - Ireland’s international education offering has also diversified, including with increasingly significant levels of cross- border provision. However, global demand for education continues to change rapidly. There continues to be a massive expansion in demand for education around the world, particularly in emerging markets with growing middle classes.
