The characteristic finding of autoantibodies in patients with vasculitis has raised the possibility that these antibodies play a role in the pathogenesis of the disease. The expression of adhesion molecules (AM) on leucocytes and endothelial cells is believed to be integral to the development of vasculitis. We therefore investigated the effect of sera, positive for anti-neutrophil cytoplasmic antibodies (ANCA) or anti-nuclear antibodies (ANA) from patients with vasculitis, on granulocyte expression of the adhesion molecule Mac-1 (CD11b). Autoantibody-positive sera from 15 out of 35 patients with vasculitis stimulated an up-regulation of Mac-1 on granulocytes. In most cases this effect was reproduced by the autoantibody-positive purified IgG fraction. Autoantibody-negative samples did not stimulate AM up-regulation. Of interest, preincubation of sera with purified antigens did not inhibit AM up-regulation by the autoantibody samples. Blocking the Fc receptors on granulocytes did result in a decrease of Mac-1 up-regulation, but this trend was not statistically significant. These results suggest that both ANCA and ANA have the capacity to up-regulate granulocyte AM expression, and that while Fc interaction with granulocyte Fc receptors is important, it is not the only mechanism whereby such autoantibodies activate cells.

Framings provide a way to construct Quillen functors from simplicial sets to any given model category. A more structured set- up studies stable frames giving Quillen functors from spectra to stable model categories. We will investigate how this is compatible with Bousfield localisation to gain insight into the deeper structure of the stable homotopy category. We further show how these techniques relate to rigidity questions and how they can be used to study algebraic model categories.

Aim: The purpose of this study was to evaluate the patient-centredness of community palliative care from the perspective of family members who were responsible for the care of a terminally ill family member. Method: A survey questionnaire was mailed to families of a deceased family member who had been designated as palliative and had received formal home care services in the central west region of the Province of Ontario, Canada. Respondents reported on service use in the last four weeks of life; the Client-Centred Care Questionnaire (CCCQ) was used to evaluate the extent to which care was patient-centred. The accessibility instrument was used to assess respondent perception of access to care. Descriptive and inferential statistics were used for data analyses. Results: Of the 243 potential participants, 111 (46.0%) family caregivers completed the survey questionnaire. On average, respondents reported that they used five different services during the last four weeks of the care recipient's life. When asked about programme accessibility, care was also perceived as largely accessible and responsive to patients' changing needs (M=4.3 (SD=1.04)]. Most respondents also reported that they knew what service provider to contact if they experienced any problems concerning the care of their family member. However, this service provider was not consistent among respondents. Most respondents were relatively positive about the patient-centred care they received. There were however considerable differences between some items on the CCCQ. Respondents tended to provide more negative ratings concerning practical arrangement and the organization of care: who was coming, how often and when. They also rated more negatively the observation that service providers were quick to say something was possible when it was not the case. Bivariate analyses found no significant differences in CCCQ or accessibility domain scores by caregiver age, care recipient age, income, education and caregiver sex. Conclusions: Patient-centred care represents a service attribute that should be recognized as an important outcome to assess the quality of service delivery. Age-related macular degeneration is the most common cause of blindness in older adults in western countries, and is likely to become the largest cause of irreversible sight loss in the developing world. Treatments such as ranibizumab and bevacizumab that inhibit VEGF have improved visual outcomes markedly. Controlled trials and clinical experience have shown that the best outcomes are achieved when monthly treatment has been administered over 2 years. This poses a significant burden on health providers and patients. A novel inhibitor of VEGF, VEGF Trap-Eye, which allows less frequent dosing without loss of efficacy, has emerged as a potential treatment. CLEAR-IT 2 was a prospective randomized Phase II trial designed to assess the safety, tolerability and the anatomic and visual effects of repeated treatments with a range of doses of VEGF Trap-Eye. Impressive anatomic and visual improvements were noted with no safety concerns. © 2011 Expert Reviews Ltd.

We accurately determine the fundamental system parameters of the neutron star X-ray transient Cen X-4 solely using phase-resolved high-resolution UV-Visual Echelle Spectrograph spectroscopy. We first determine the radial-velocity curve of the secondary star and then model the shape of the phase-resolved absorption line profiles using an X-ray binary model. The model computes the exact rotationally broadened, phase-resolved spectrum and does not depend on assumptions about the rotation profile, limb-darkening coefficients and the effects of contamination from an accretion disc. We determine the secondary star-to-neutron star binary mass ratio to be 0.1755 ± 0.0025, which is an order of magnitude more accurate than previous estimates. We also constrain the inclination angle to be 32^{+8}_{-2} degrees. Combining these values with the results of the radial-velocity study gives a neutron star mass of 1.94^{+0.37}_{-0.85}M⊙ consistent with previous estimates. Finally, we perform the first Roche tomography reconstruction of the secondary star in an X-ray binary. The tomogram reveals surface inhomogeneities that are due to the presence of cool starspots. A large cool polar spot, similar to that seen in Doppler images of rapidly rotating isolated stars, is present on the Northern hemisphere of the K7 secondary star and we estimate that ~4 percent of the total surface area of the donor star is covered with spots.This evidence for starspots supports the idea that magnetic braking plays an important role in the evolution of low-mass X-ray binaries.

An unusual application of hydrological understanding to a police search is described. The lacustrine search for a missing person provided reports of bottom-water currents in the lake and contradictory indications from cadaver dogs. A hydrological model of the area was developed using pre-existing information from side scan sonar, a desktop hydrogeological study and deployment of water penetrating radar (WPR). These provided a hydrological theory for the initial search involving subaqueous groundwater flow, focused on an area of bedrock surrounded by sediment, on the lake floor. The work shows the value a hydrological explanation has to a police search operation (equally to search and rescue). With hindsight, the desktop study should have preceded the search, allowing better understanding of water conditions. The ultimate reason for lacustrine flow in this location is still not proven, but the hydrological model explained the problems encountered in the initial search.

Aims - To investigate whether young people with Type 1 diabetes have an increased rate of depression andantidepressant use and whether their risk varies by age group, time from diabetes diagnosis, calendar period ofdiagnosis or complications status. Methods - A cohort of incident cases of patients with Type 1 diabetes diagnosed before 35 years of age (n = 5548) wasidentified within the Clinical Practice Research Datalink and individually age and sex matched with up to two controlsubjects without diabetes (n = 10 657). Patients with depression were identified through general practice-recordeddepression codes and antidepressant prescriptions. Cox regression models gave hazard ratios for depression in peoplewith Type 1 diabetes compared with control subjects. Results - People with Type 1 diabetes were twice as likely to have a record of antidepressant use and generalpractice-diagnosed depression as their matched control subjects (hazard ratio 2.08, 95% CI 1.73–2.50, P < 0.001).These associations varied by time from diagnosis, with marked increases observed within the first 5 years of diagnosis(hazard ratio 2.14, 95% CI 1.51–3.03, P < 0.001), and by age at diabetes diagnosis, with excesses noted even in the 10-to 19-year age group (hazard ratio 1.45, 95% CI 1.06–1.98, P = 0.02). Conclusions - This population-based study shows that people with Type 1 diabetes have higher rates of generalpractice-recorded depression and antidepressant use. The excess is present within 5 years of diabetes diagnosis,suggesting psychological input for patients is warranted in the early years of their condition.

This study describes the utilization of deep eutectic solvents (DESs) based on the mixture of the N-methylacetamide (MAc) with a lithium salt (LiX, with X = bis[(trifluoromethyl)sulfonyl]imide, TFSI; hexafluorophosphate, PF6; or nitrate, NO3) as electrolytes for carbon-based supercapacitors at 80 °C. The investigated DESs were formulated by mixing a LiX with the MAc (at xLi = 0.25). All DESs show the typical eutectic characteristic with eutectic points localized in the temperature range from −85 to −52 °C. Using thermal properties measured by differential scanning calorimetry (DSC), solid–liquid equilibrium phase diagrams of investigated LiX–MAc mixtures were then depicted and also compared with those predicted by using the COSMOThermX software. However, the transport properties of selected DESs (such as the conductivity (σ) and the fluidity (η–1)) are not very interesting at ambient temperature, while by increasing the temperature up to 80 °C, these properties become more favorable for electrochemical applications, as shown by the calculated Walden products: w = ση–1 (mS cm–1 Pa–1 s–1) (7 < w < 16 at 25 °C and 513 < w < 649 at 80 °C). This “superionicity” behavior of selected DESs used as electrolytes explains their good cycling ability, which was determined herein by cyclic voltammetry and galvanostic charge–discharge methods, with high capacities up to 380 F g–1 at elevated voltage and temperature, i.e., ΔE = 2.8 V and 80 °C for the LiTFSI–MAc mixture at xLi = 0.25, for example. The electrochemical resistances ESR (equivalent series resistance) and EDR (equivalent diffusion resistance) evaluated using electrochemical impedance spectroscopy (EIS) measurements clearly demonstrate that according to the nature of anion, the mechanism of ions adsorption can be described by pure double-layer adsorption at the specific surface or by the insertion of desolvated ions into the ultramicropores of the activated carbon material. The insertion of lithium ions is observed by the presence of two reversible peaks in the CVs when the operating voltage exceeds 2 V. Finally, the efficiency and capacitance of symmetric AC/AC systems were then evaluated to show the imbalance carbon electrodes caused by important lithium insertion at the negative and by the saturation of the positive by anions, both mechanisms prevent in fact the system to be operational. Considering the promising properties, especially their cost, hazard, and risks of these DESs series, their introduction as safer electrolytes could represent an important challenge for the realization of environmentally friendly EDLCs operating at high temperature.

A former silver mine in Tynagh, Co. Galway, Ireland is one of the most contaminated mine sites in Europe with maximum concentrations of Zn, As, Pb, Mn, Ni, Cu, and Cd far exceeding guideline values for water and sediment. The aims of this research were to 1) further assess the contamination, particularly metals, in surface water and sediment around the site, and 2) determine if the contamination has increased 10 years after the Environmental Protection Agency Ireland (EPAI) identified off-site contamination. Site pH is alkaline to neutral because CaCO3-rich sediment and rock material buffer the exposed acid generating sulphide-rich ore. When this study was compared to the previous EPAI study conducted 10 years earlier, it appeared that further weathering of exposed surface sediment had increased concentrations of As and other potentially toxic elements. Water samples from the tailings ponds and adjacent Barnacullia Stream had concentrations of Al, Cd, Mn, Zn and Pb above guideline values. Lead and Zn concentrations from the tailings pond sediment were 16 and 5 times higher, respectively, than concentrations reported 10 years earlier. Pb and Zn levels in most sediment samples exceeded the Expert Group (EGS) guidelines of 1000 and 5000 mg/kg, respectively. Arsenic concentrations were as high as 6238 mg/kg in the tailings ponds sediment, which is 62 and 862 times greater than the EGS and Canadian Soil Quality Guidelines (CSQG), respectively. Cadmium, Cu, Fe, Mn, Pb and Zn concentrations in water and sediment were above guideline values downstream of the site. Additionally, Fe, Mn and organic matter (OM) were strongly correlated and correlated to Zn, Pb, As, Cd, Cu and Ni in stream sediment. Therefore, the nearby Barnacullia Stream is also a significant pathway for contaminant transport to downstream areas. Further rehabilitation of the site may decrease the contamination around the area.

We investigated whether “hidden” (or unobserved) social networks were evident in a 2011 physical activity behavior change intervention in Belfast, Northern Ireland. Results showed evidence of unobserved social networks in the intervention and illustrated how the network evolved over short periods and affected behavior. Behavior change interventions should account for the interaction among participants (i.e., social networks) and how such interactions affect intervention outcome.

This article explores employee voice within the specific institutional arrangement of double-breasting. Double-breasting is when multi-plant organizations recognize trade unions in some company sites, with non-union arrangements at other company plants, or where a unionized firm acquires a new site that it then operates on a non-union basis. We examine three research questions in four separate case study organizations that operate employee voice double-breasting arrangements across 16 workplace locations on the island of Ireland. These questions consider employer motives for double-breasting, the practices that characterize double-breasting employee voice, and the micro-political implications of double-breasting. The article contributes to knowledge on the emergence and impact of double-breasting and employee voice systems. We subsequently advance two theoretical propositions: the first theorizing employer motives for double-breasting, and the second explaining the extent to which the practice of double-breasting is durable over time.
