Size-selective fishing is expected to affect traits such as individual growth rate, but the relationship between the fishery-linked selection differentials and the corresponding phenotypic changes is not well understood. We analysed a 25-year monitoring survey of sympatric populations of the two Alpine whitefish Coregonus albellus and C. fatioi. We determined the fishing-induced selection differentials on growth rates, the actual change of growth rates over time, and potential indicators of reproductive strategies that may change over time. We found marked declines in adult growth rate and significant selection differentials that may partly explain the observed declines. However, when comparing the two sympatric species, the selection differentials on adult growth were stronger in C. albellus while the decline in adult growth rate seemed more pronounced in C. fatioi. Moreover, the selection differential on juvenile growth was significant in C. albellus but not in C. fatioi, while a significant reduction in juvenile growth over the last 25 years was only found in C. fatioi. Our results suggest that size-selective fishing affects the genetics for individual growth in these whitefish, and that the link between selection differentials and phenotypic changes is influenced by species-specific factors.

BACKGROUND: Pulse wave velocity (PWV), an index of arterial wall stiffness, is modulated by blood pressure (BP). Whether heart rate (HR) is also a modulator of PWV is controversial. Recent research involving mainly patients with high aortic PWV have found either no change or a positive correlation between the two. Given that PWV is increasingly being measured in cardiovascular studies, the relationship between HR and PWV should be known in patients with preserved arterial wall elasticity. OBJECTIVE: The aim of this study was to evaluate the importance of HR as a determinant of the variability in PWV in patients with a low degree of atherosclerosis. DESIGN AND METHODS: Fourteen patients (five female, nine male; aged 68 +/- 8 years) were evaluated post pacemaker implantation due to sick sinus or carotid hypersensitivity syndromes. Carotid-femoral PWV was measured at rest and during atrial pacing at 80, 90 and 100 bpm (paced HR). Arterial femoral blood flow (AFBF) was measured by echodoppler. RESULTS: PWV increased from 6.2 +/- 1.5 m/s (mean +/- SD) during resting sinus rhythm (HR 62 +/- 8 bpm; mean +/- SD) to 6.8 +/- 1.0, 7.0 +/- 0.9, and 7.6 +/- 1.1 m/s at pacing rates of 80, 90 and 100 bpm, respectively (P < 0.0001). Systolic (SBP) and mean blood pressure (MBP) remained constant at all HR levels, whereas AFBF increased in a linear fashion. CONCLUSIONS: These results demonstrate that even in patients with a low degree of atherosclerosis, HR is a potential modulator of carotid-femoral PWV.

Previous studies have demonstrated the difference between the basal metabolic rate (BMR) and the sleeping metabolic rate (SMR): however, the difference in the Japanese population has not yet been explored. This study examined the relationship between the BMR and SMR in ninety-four healthy Japanese subjects (37 males and 57 females, 39 +/- 12 y of age and 22.0 +/- 7.4% body fat) in a respiratory chamber. The SMR was significantly lower than the BMR (1416 +/- 245 vs. 1492 +/- 256 kcal/d): however, there was a highly significant correlation between the two (r = 0.867; p < 0.001). The ratio of SMR/BMR largely varied among individuals (0.95 +/-0.08, 8.4% of the coefficient of variation). The ratio was significantly lower in males than in females (0.93 +/- 0.10 vs. 0.97 +/- 0.06, p < 0.05). None of the anthropometric measures (age, weight, body mass index, body surface area or percent body fat) correlated with the ratio. These results showed that SMR was 95%, of BMR on average in a healthy Japanese group. However, when applied over a longer time period (24 h or more), the difference tends to become negligible for most analyses in a group. Although the difference between SMR and BMR will induce a 5% gap of physical activity level defined as the total energy expenditure divided by the BMR or SMR, this factor seems to have little practical importance in epidemiological research.

Another additional peculiarity in Leishmania will be discussed about of the amino acid divergence rate of three structural proteins: acidic ribosomal P1 and P2b proteins, and histone H3 by using multiple sequence alignment and dendrograms. These structural proteins present a high rate of divergence regarding to their homologous protein in Trypanosoma cruzi. At this regard, L. (V.) peruviana P1 and T. cruzi P1 showed 57.4% of divergence rate. Likewise, L. (V.) braziliensis histone H3 and acidic ribosomal P2 protein exhibited 31.8% and 41.7% respectively of rate of divergence in comparison with their homologous in T. cruzi.

QUESTIONS UNDER STUDY AND PRINCIPLES: Estimating glomerular filtration rate (GFR) in hospitalised patients with chronic kidney disease (CKD) is important for drug prescription but it remains a difficult task. The purpose of this study was to investigate the reliability of selected algorithms based on serum creatinine, cystatin C and beta-trace protein to estimate GFR and the potential added advantage of measuring muscle mass by bioimpedance. In a prospective unselected group of patients hospitalised in a general internal medicine ward with CKD, GFR was evaluated using inulin clearance as the gold standard and the algorithms of Cockcroft, MDRD, Larsson (cystatin C), White (beta-trace) and MacDonald (creatinine and muscle mass by bioimpedance). 69 patients were included in the study. Median age (interquartile range) was 80 years (73-83); weight 74.7 kg (67.0-85.6), appendicular lean mass 19.1 kg (14.9-22.3), serum creatinine 126 μmol/l (100-149), cystatin C 1.45 mg/l (1.19-1.90), beta-trace protein 1.17 mg/l (0.99-1.53) and GFR measured by inulin 30.9 ml/min (22.0-43.3). The errors in the estimation of GFR and the area under the ROC curves (95% confidence interval) relative to inulin were respectively: Cockcroft 14.3 ml/min (5.55-23.2) and 0.68 (0.55-0.81), MDRD 16.3 ml/min (6.4-27.5) and 0.76 (0.64-0.87), Larsson 12.8 ml/min (4.50-25.3) and 0.82 (0.72-0.92), White 17.6 ml/min (11.5-31.5) and 0.75 (0.63-0.87), MacDonald 32.2 ml/min (13.9-45.4) and 0.65 (0.52-0.78). Currently used algorithms overestimate GFR in hospitalised patients with CKD. As a consequence eGFR targeted prescriptions of renal-cleared drugs, might expose patients to overdosing. The best results were obtained with the Larsson algorithm. The determination of muscle mass by bioimpedance did not provide significant contributions.

BACKGROUND: Estimated glomerular filtration rate (eGFR) is an important diagnostic instrument in clinical practice. The National Kidney Foundation-Kidney Disease Quality Initiative (NKF-KDOQI) guidelines do not recommend using formulas developed for adults to estimate GFR in children; however, studies confirming these recommendations are scarce. The aim of our study was to evaluate the accuracy of the new Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) formula, the Modification of Diet in Renal Disease (MDRD) formula, and the Cockcroft-Gault formula in children with various stages of chronic kidney disease (CKD). METHODS: A total of 550 inulin clearance (iGFR) measurements for 391 children were analyzed. The cohort was divided into three groups: group 1, with iGFR >90 ml/min/1.73 m(2); group 2, with iGFR between 60 and 90 ml/min/1.73 m(2); group 3, with iGFR of <60 ml/min/1.73 m(2). RESULTS: All formulas overestimate iGFR with a significant bias (p < 0.001), present poor accuracies, and have poor Spearman correlations. For an accuracy of 10 %, only 11, 6, and 27 % of the eGFRs are accurate when using the MDRD, CKD-EPI, and Cockcroft-Gault formulas, respectively. For an accuracy of 30 %, these formulas do not reach the NKF-KDOQI guidelines for validation, with only 25, 20, and 70 % of the eGFRs, respectively, being accurate. CONCLUSIONS: Based on our results, the performances of all of these formulas are unreliable for eGFR in children across all CKD stages and cannot therefore be applied in the pediatric population group.

Methadone is administered as a chiral mixture of (R,S)-methadone. The opioid effect is mainly mediated by (R)-methadone, whereas (S)-methadone blocks the human ether-à-go-go-related gene (hERG) voltage-gated potassium channel more potently, which can cause drug-induced long QT syndrome, leading to potentially lethal ventricular tachyarrhythmias. To investigate whether substitution of (R,S)-methadone by (R)-methadone could reduce the corrected QT (QTc) interval, (R,S)-methadone was replaced by (R)-methadone (half-dose) in 39 opioid-dependent patients receiving maintenance treatment for 14 days. (R)-methadone was then replaced by the initial dose of (R,S)-methadone for 14 days (n = 29). Trough (R)-methadone and (S)-methadone plasma levels and electrocardiogram measurements were taken. The Fridericia-corrected QT (QTcF) interval decreased when (R,S)-methadone was replaced by a half-dose of (R)-methadone; the median (interquartile range [IQR]) values were 423 (398-440) milliseconds (ms) and 412 (395-431) ms (P = .06) at days 0 and 14, respectively. Using a univariate mixed-effect linear model, the QTcF value decreased by a mean of -3.9 ms (95% confidence interval [CI], -7.7 to -0.2) per week (P = .04). The QTcF value increased when (R)-methadone was replaced by the initial dose of (R,S)-methadone for 14 days; median (IQR) values were 424 (398-436) ms and 424 (412-443) ms (P = .01) at days 14 and 28, respectively. The univariate model showed that the QTcF value increased by a mean of 4.7 ms (95% CI, 1.3-8.1) per week (P = .006). Substitution of (R,S)-methadone by (R)-methadone reduces the QTc interval value. A safer cardiac profile of (R)-methadone is in agreement with previous in vitro and pharmacogenetic studies. If the present results are confirmed by larger studies, (R)-methadone should be prescribed instead of (R,S)-methadone to reduce the risk of cardiac toxic effects and sudden death.

Whole-body (WB) planar imaging has long been one of the staple methods of dosimetry, and its quantification has been formalized by the MIRD Committee in pamphlet no 16. One of the issues not specifically addressed in the formalism occurs when the count rates reaching the detector are sufficiently high to result in camera count saturation. Camera dead-time effects have been extensively studied, but all of the developed correction methods assume static acquisitions. However, during WB planar (sweep) imaging, a variable amount of imaged activity exists in the detector's field of view as a function of time and therefore the camera saturation is time dependent. A new time-dependent algorithm was developed to correct for dead-time effects during WB planar acquisitions that accounts for relative motion between detector heads and imaged object. Static camera dead-time parameters were acquired by imaging decaying activity in a phantom and obtaining a saturation curve. Using these parameters, an iterative algorithm akin to Newton's method was developed, which takes into account the variable count rate seen by the detector as a function of time. The algorithm was tested on simulated data as well as on a whole-body scan of high activity Samarium-153 in an ellipsoid phantom. A complete set of parameters from unsaturated phantom data necessary for count rate to activity conversion was also obtained, including build-up and attenuation coefficients, in order to convert corrected count rate values to activity. The algorithm proved successful in accounting for motion- and time-dependent saturation effects in both the simulated and measured data and converged to any desired degree of precision. The clearance half-life calculated from the ellipsoid phantom data was calculated to be 45.1 h after dead-time correction and 51.4 h with no correction; the physical decay half-life of Samarium-153 is 46.3 h. Accurate WB planar dosimetry of high activities relies on successfully compensating for camera saturation which takes into account the variable activity in the field of view, i.e. time-dependent dead-time effects. The algorithm presented here accomplishes this task.

In Pseudomonas aeruginosa the extracellular metabolite and siderophore pyochelin is synthesized from two major precursors, chorismate and l-cysteine via salicylate as an intermediate. The regulatory role of isochorismate synthase, the first enzyme in the pyochelin biosynthetic pathway, was studied. This enzyme is encoded by pchA, the last gene in the pchDCBA operon. The PchA protein was purified to apparent electrophoretic homogeneity from a PchA-overexpressing P. aeruginosa strain. The native enzyme was a 52-kDa monomer in solution, and its activity strictly depended on Mg(2+). At pH 7.0, the optimum, a K(m) = 4.5 microm and a k(cat) = 43.1 min(-1) were determined for chorismate. No feedback inhibitors or other allosteric effectors were found. The intracellular PchA concentration critically determined the rate of salicylate formation both in vitro and in vivo. In cultures grown in iron-limiting media to high cell densities, overexpression of the pchA gene resulted in overproduction of salicylate as well as in enhanced pyochelin formation. From this work and earlier studies, it is proposed that one important factor influencing the flux through the pyochelin biosynthetic pathway is the PchA concentration, which is determined at a transcriptional level, with pyochelin acting as a positive signal and iron as a negative signal.

In a Walrasian labor market, the labor income share is constant under the assumptions of a Cobb-Douglas production function and perfect competition. Given the observed decline of the labor share in recent decades, this paper relaxes these assumptions, proposes a time-series calculation of the aggregate price mark-up reflecting the degree of imperfect competition in the product market, and provides estimates of the elasticity of substitution under such product market imperfections. We focus on Spain and the U.S. and show that the elasticity of substitution is above one in Spain and below one in the U.S. We also show that the price markup drives the elasticity of substitution away from one, upwards in Spain, downwards in the U.S. These results are used to explain the declining path of the labor income share, common to both economies, and their contrasted patterns in terms of capital deepening.

The magnitude of coffee-induced thermogenesis and the influence of coffee ingestion on substrate oxidation were investigated in 10 lean and 10 obese women, over two 24-h periods in a respiratory chamber. On one occasion the subjects consumed caffeinated coffee and on the other occasion, decaffeinated coffee. The magnitude of thermogenesis was smaller in obese (4.9 +/- 2.0%) than in lean subjects (7.6 +/- 1.3%). The thermogeneic response to caffeine was prolonged during the night in lean women only. The coffee-induced stimulation of energy expenditure was mediated by a concomitant increase in lipid and carbohydrate oxidation. During the next day, in postabsorptive basal conditions, the thermogenic effect of coffee had vanished, but a significant increase in lipid oxidation was observed in both groups. The magnitude of this effect was, however, blunted in obese women (lipid oxidation increased by 29 and 10% in lean and obese women, respectively). Caffeine increased urinary epinephrine excretion. Whereas urinary caffeine excretion was similar in both groups, obese women excreted more theobromine, theophylline, and paraxanthine than lean women. Despite the high levels of urinary methylxanthine excretion, thermogenesis and lipid oxidation were less stimulated in obese than in lean subjects.

The survival and molting incidence in Triatoma infestans, a vector of Chagas disease, were investigated following sequential shocks at 0ºC in fifth instar nymphs under moderate fasting and full nutritional conditions. The shocks were separated by intervals of 8 h and 24 h at 30ºC. The results indicated that in terms of insect survival, T. infestans is tolerant to a single cold shock at 0ºC even for 12 h, or to sequential cold shocks, regardless of the nutritional state of the specimens. In terms of molting rate, fasting enhanced the tolerance to sequential cold shocks, but did not exceed the tolerance acquired by fully-nourished specimens, except when cold shocks were separated by an 8 h interval at 30ºC. The protective action elicited by fasting was assumed to be additive to that induced by a single mild cold shock or sequential cold shocks. The cold-tolerance response of T. infestans may have favoured its survival in areas of South America with low temperatures, even considering that this species is predominantly associated with human habitats.

Cadre de l'étude La part des dépenses de santé dans les budgets des pays occidentaux est importante et tend à croître depuis plusieurs décennies. Une des conséquences, notamment pour la Suisse, est une augmentation régulière des primes d'assurance maladie, dont l'impopularité incite les responsables politiques à trouver des stratégies de frein à la croissance des coûts de la santé. Les médicaments, qui contribuent notablement à l'augmentation de ces coûts, constituent l'une des cibles classiques d'interventions visant à une réduction des dépenses de santé. Le niveau de coûts des médicaments représente plus de 20% des dépenses de soins du domaine ambulatoire à la charge de l'assurance obligatoire des soins (AOS), soit une dépense annuelle de plus de 4 milliards de francs. Différentes mesures peuvent être utilisées par le gouvernement pour contenir cette facture à charge de la collectivité : baisse du prix des médicaments, limitation du nombre de produits remboursés par l'assurance de base et incitations à la concurrence sur les prix en autorisant les importations parallèles. Depuis que de plus en plus de brevets pour des médicaments sont arrivés à échéance, un autre angle d'attaque s'est concrétisé avec l'incitation à la prescription de médicaments génériques. Les génériques sont définis comme des produits thérapeutiques chimiquement identiques à des composés déjà utilisés, appelés médicaments originaux. En principe, une économie substantielle devrait pouvoir être réalisée sur les coûts totaux des médicaments si tous les génériques disponibles sur le marché étaient systématiquement prescrits par les professionnels et acceptés par les patients. Les résultats publiés par les caisses maladie et les offices fédéraux sont des estimations globales et les montants publiés par l'industrie pharmaceutique concernent l'ensemble du marché, incluant les médicaments utilisés dans les séjours hospitaliers, sont par ailleurs exprimés en prix de fabrique. De plus, aucune étude n'a tenu compte de la substituabilité des produits d'un point de vue pharmaceutique. L'objectif général de la thèse est d'évaluer aussi précisément que possible l'économie pouvant être encore réalisée dans le secteur ambulatoire en substituant aux médicaments originaux les produits génériques présents sur le marché et de caractériser plusieurs facteurs qui influencent la substitution générique. Pour cette étude, nous avons pu disposer de l'ensemble des factures pharmaceutiques de la caisse maladie CSS, pour tous ses assurés des cantons d'Argovie, du Tessin et de Vaud, soit plus de 169'000 assurés représentant les trois régions linguistiques de la Suisse. Les 1'341'197 prescriptions médicales qui ont été analysées concernent l'année 2003. C'est un moment critique dans l'histoire des génériques en Suisse, approprié pour établir un premier bilan après l'entrée en vigueur de la première mesure relative au droit de substituer octroyé en 2001 aux pharmaciens et, pour identifier idéalement les principaux déterminants de la substitution par les praticiens avant l'introduction de la quote-part différenciée en 2006. La présence d'un même principe actif n'est pas une condition suffisante pour permettre une substitution et pour ce travail des critères tenant compte des caractéristiques pharmaceutiques des produits ont été établis et appliqués pour valider la substituabilité des originaux par les génériques disponibles sur le marché. Ces critères concernent notamment le mode d'administration, le dosage et le nombre de doses dans l'emballage. L'étude a été réalisée selon deux approches, d'abord par une analyse descriptive sur l'ensemble de la population source pour estimer le marché des génériques et ensuite par une analyse statistique (régression logit multivariée) sur 173'212 prescriptions agrégées, qui concernent spécifiquement soit un générique soit un original substituable, pour caractériser les facteurs déterminants de la substitution générique. Résultats Dans l'ensemble de la population source, les génériques et les originaux substituables représentent 17,4% du marché en termes de coûts facturés, avec 3,4% de génériques et 14,0% d'originaux substituables ce qui correspond à un taux de substitution de 19,5%. En termes de dépenses, les substitutions génériques réalisées représentent une économie de 1,3% du total du marché étudié alors qu'il reste un potentiel notable d'économie par la substitution de 4,6%. Les taux de substitution sont très variables selon les cantons : 10,1% au Tessin, 29,0% pour le canton de Vaud et 35,8% pour Argovie. L'analyse univariée des 173'212 prescriptions de génériques ou d'originaux substituables, montre des taux de substitution plus élevés chez les patients jeunes et lorsqu'il y a d'importantes différences de prix entre les originaux et les génériques. Des taux de substitution peu élevés sont observés chez les patients les plus âgés et pour ceux qui ont des traitements médicamenteux complexes. Les patients ayant plus de 10 médicaments différents durant la même année, présentent une probabilité relative de substituer inférieure (-24%) par rapport aux patients ayant 6 à 10 médicaments différents dans l'année. Cependant, l'analyse multivariée montre que l'effet négatif sur le taux de substitution de l'âge combiné à la complexité des traitements n'excède pas 3%. Bien que le niveau de franchises et la participation financière à la quote-part soient liées à une augmentation de la prescription de génériques, leurs effets sont modérés pour les patients avec des franchises supérieures à 300 francs (effet marginal de 1%) et pour les patients qui n'ont pas atteint le plafond de participation (effet marginal de 2%). La différence de taux substitution entre les médecins hospitaliers et les spécialistes est diminuée de façon notable (effet marginal de -13%) et elle est cependant moins marquée avec les médecins généralistes (effet marginal de -3%). Les facteurs associés au marché ont une influence notable sur la substitution générique et des effets positifs sont observés avec l'augmentation de la taille du marché, du nombre de génériques pour un même original substituable et de l'économie relative entre l'original et le générique. Par contre, la diversification des formes galéniques et des tailles d'emballages au niveau de l'offre des médicaments originaux a des effets fortement négatifs sur la substitution générique (-7%). Le canton de domicile a aussi un impact notable sur la substitution et le canton du Tessin présente un taux plus bas (-26%) que le canton d'Argovie. Conclusion et perspectives Ce travail a montré qu'il y a encore un important potentiel d'économies à réaliser par la substitution générique, calculé à plus de 4% des dépenses pharmaceutiques prises en charge par l'AOS en ambulatoires. Une extrapolation à l'ensemble du marché suisse, qui doit être faite avec prudence, fait apparaître un potentiel d'économies de 127 millions pour les médicaments délivrés par les pharmacies en 2003. L'étude a mis en évidence un certain nombre de déterminants qui freinent la substitution générique, notamment la prescription par un médecin hospitalier. Sur ce point la prescription en DCI (dénomination commune internationale) pourrait favoriser la dispensation de génériques moins chers. Un taux de substitution plus faible est observé chez les patients âgés avec des traitements complexes. Ce constat peut être mis en relation avec la crainte d'avoir un traitement moins efficace ou moins bien supporté et les risques de confusion lors du passage d'un original substituable à un générique ou d'un générique à un autre générique. Sur ces éléments, l'indication claire et précise du nom de la substance, aussi bien sur les emballages des originaux substituables que sur ceux des génériques, pourrait rassurer les patients et diminuer les risques d'erreurs dans la prise des médicaments. Certaines précautions à prendre lors de la prescription de génériques sont reconnues, notamment pour les médicaments à faible marge thérapeutique, et des informations sur la bioéquivalence, régulièrement mises à jour et à disposition des professionnels, pourraient augmenter la confiance dans l'utilisation des génériques. Les industries pharmaceutiques préservent par différentes tactiques leurs parts de marché et notamment avec succès en introduisant de nouvelles formes galéniques juste avant l'expiration des brevets. Des directives complémentaires sur la fixation des prix pour le remboursement, en particulier l'introduction d'un prix de référence quelle que soit la forme galénique, pourraient diminuer l'effet de barrage des médicaments originaux. Les incitations économiques, telles que la franchise et les participations sont efficaces si l'on considère l'impact sur les taux de substitution. Leur effet global reste toutefois modeste et il serait nécessaire de mesurer concrètement l'impact de l'introduction en 2006 de la quote-part différenciée. Les différences de prix entre les originaux et les génériques exigées à 50% pour les gros marchés, c'est-à-dire de plus de 16 millions, devraient aussi avoir un impact qu'il serait opportun de mesurer.
