A Sindrome Miofascial constitui, dentre os quadros de dor crônica, o que leva um número expressivo de pacientes a buscar atendimento médico nos Serviços de Tratamento de Dor. Caracteriza-se pela presença de pontos dolorosos localizados na musculatura, chamados de “pontos de gatilho”, e de espasmo muscular, podendo ocorrer limitação funcional e disfunção do sistema vegetativo (Sola & Bonica, 2001). A dor e a limitação funcional decorrentes da Sindrome Miofascial constituem atualmente uma das grandes causas de falta ou afastamento do trabalho, o que acarreta graves problemas econômicos e sociais (Roth et al, 1998; Sola & Bonica, 2001). Seu tratamento pode ser demorado e requer participação ativa por parte do paciente. O índice de melhora dos pacientes portadores de Síndrome Miofascial parece sofrer influência de diversos fatores tais como sintomas depressivos, ansiedade, ganho com os sintomas, etc. Os objetivos deste trabalho foram estabelecer a incidência de sucesso e insucesso da terapêutica proposta e verificar qual a relação daqueles fatores com o desfecho clínico estabelecido. Efetuou-se um estudo observacional. O delineamento experimental realizado foi um estudo de incidência. Estudaram-se 62 pacientes adultos (mais de 18 anos), de ambos os sexos, com diagnóstico de Síndrome Miofascial, que procuraram atendimento no Serviço de Tratamento de Dor e Medicina Paliativa do HCPA. Os instrumentos de aferição utilizados foram a Escala Análoga Visual de Dor (VAS), Escala de Ansiedade Traço- Estado (IDATE), Escala para Depressão de Montegomery-Äsberg, Self-Reporting Questionnaire (OMS), Questionário sobre Expectativa de Futuro e questionário estruturado. Os testes psicológicos e de avaliação da dor foram aplicados em dois momentos, na primeira consulta e ao final do tratamento proposto. Foi considerado como desfecho clínico o insucesso terapêutico, avaliado ao final do estudo. Observaram-se incidência de 71% e 29%, respectivamente, para sucesso e insucesso terapêutico. Os pacientes que estavam afastados do trabalho apresentaram aproximadamente 9 vezes mais chances de insucesso terapêutico. Aqueles que obtiveram ganho com os sintomas apresentaram em torno de 7 vezes mais chances de manutenção da dor ao final do tratamento. Maior ansiedade-estado (razão de chances ou RC = 3,4), expectativa negativa de futuro (RC = 22), sintomas depressivos moderados a intensos (RC =4,5) e presença de distúrbios psiquiátricos menores (RC = 3,6) associaram-se com maiores chances de insucesso terapêutico. Características demográficas, familiares, de ocupação e clínicas não se associaram ao desfecho clínico avaliado. Dos pacientes analisados, 29% permaneceram sem alívio da dor, com dificuldades para dormir e afastados de suas atividades, caracterizando o insucesso terapêutico. Observou-se a associação entre insucesso e presença de distúrbios psiquiátricos menores, sintomas depressivos moderados a intensos, maior estado de ansiedade, ganho com os sintomas e afastamento do trabalho. A abordagem multidisciplinar destes pacientes se faz necessária para elevar os índices de sucesso terapêutico no tratamento da SMF.

To determine whether there is familiar aggregation of severe preeclampsia in a Brazilian population from Rio Grande do Norte and to characterize the maternal and perinatal outcomes in the studied population. Methods: A case control study was performed with 412 participants who were admitted at Maternidade Escola Januário Cicco (MEJC) for medical care. Of these, 264 subjects presented normal blood pressure and 148 were cases. Cases were composed of eclampsia (n=47), HELLP Syndrome (n=85) and Eclampsia associated with HELLP syndrome (n=16). The diagnosis of these illness were based on the citeria developed by National High Blood Pressure Education Program Working (2000). An interview was performed with each subject and questions related to personal and familiar history of hypertension, preeclampsia, HELLP syndrome and eclampsia. Statistical analysis was performed and comparison of median and mean between cases and controls were performed, with the level of significance of 5%. The Odds-Ratio was determined to estimate the risk of preeclampsia within the families. Results: There were no difference in the demographic data between cases and controls. Previous history of chronic hypertension and preeclampsia was more frequent in the case group. Headaches were more frequent in eclampsia and epigastric pain in the HELLP syndrome cases. Bleeding and oliguria were more frequently found in the eclampsia associated with HELLP syndrome cases. Acute Renal insufficiency was a common complication in the case group, but these cases did not evolve to chronic renal insufficiency. The maternal mortality was 0.4% and the perinatal mortality was high, 223 per 1,000 live births. The 111 risk of a woman to develop preeclampsia whose mother has hypertension or had preeclampsia was respectively 2.5 and 3.5. This risk was increased 5 times, when a sibling has hypertension and 6 times when both sibling and mother had previous history of preeclampsia. Conclusions: This study confirms that there is familiar aggregation of preeclampsia in this Brazilian population. The potential for cardiovascular complications due to development of chronic hypertension indicates the need of closely follow up of women who develop preeclampsia

Background: The domestic animals heart is a conical hollow viscera, surrounded by pericardium, laterally compressed, accompanying the thorax shape. Atriums constituted the heart basis and their auricles partially bound the initial portion of the aorta and pulmonary trunk. In mammals, heart is kept suspended in the thoracic cavity and the pericardic sac is fixed dorsally by great veins and arteries roots, and ventrally fixed to the sternum, although its fixation to the diaphragm varies among species. This paper aimed to describe morphological aspects of the heart of the paca, the second biggest Brazilian rodent.Materials, Methods & Results: There were used 12 hearts of adult pacas for this study, obtained from the UNESP, Jaboticabal, SP, which died due fights or anesthesia during bandages or radiograph exams. The thoracic aorta was filled with colored latex and the animal was set in a 10% formaldehyde solution for at least 72 hours. The thoracic cavity was dissected and hearts individualized and measured with a paquimeter, lateromedially, craniocaudally and dorsoventrally. The paca heart is placed between the first and fifth intercostal space (ICS), in a craniocaudal oblique position; its basis is craniodorsally positioned, on the middle third between the first and second ICS and its apex is located near the sternodiaphragmatic joint, on the fifth ICS, tilted to the left antimere. The heart is surrounded by pericardium, which from ventrocaudally is originated the sternopericardic ligament, that continues as phrenopericardic ligament. At the heart basis, the rising of the pulmonary trunk was observed and the conus arteriosus formed a typical projection. The aorta also rised from the heart basis and its arch, which was caudally curved, crossed dorsally the pulmonary trunk; the right cranial and caudal cava veins drained to the right atrium. There is a left cranial cava vein, which surrounded the left atrium and joined the right caudal cava vein on the right atrium. The azygos vein joins the right cranial cava vein and four pulmonary veins drained to the left atrium. At palpation, a hard structure on the rising of the aorta was observed, similarly to a cartilaginous tissue, which would be part of the cardiac skeleton. The left and right coronary arteries were observed in all hearts.Discussion: The paca heart is anatomica and topographically similar to those of domestic mammals, differing from them for being placed one intercostal space more cranial and due to the presence of two cranial cava veins, the left and the right ones, besides the presence of the caudal cava vein. This vascular description is similar to that of small rodents, as rats and mice. In paca heart, the sinus venous, the terminal crest, the oval fossa, the atrioventricular valvae, the papillary muscles and tendinous cords, besides smooth atriums and auricles covered by pectinate muscles, were observed. The sternopericardic ligament, which is dorsally elongated as phrenopericardic ligament, is similar to the one present in humans, pigs, castors, and different from the one observed in carnivorous, that presents the phrenopericardic ligament and from the one of horses and ruminants, which present the sternopericardic ligament.

This paper presents the master plan and geoenvironmental zoning natures as instruments of environmental planning and management. The discussion of territory environmental planning is guided by two directions: at first the environmental elements involved in planning and the another is the implementation of these instruments at the municipal territory. To analyze the planning directed of the municipal territory we consider the fragments of its, represented by country and urban. The master plan inside of the Estatuto da Cidade (City Statute) and the geoenvironmental zoning are directed to territorial environmental planning. Regarding of the master plan the first challenge has been the spacial area that the plan can cover. It is necessary to prepare master plans that could include all the territory. The environmental zoning are directed for the territory totality.In this sense, the geoenvironmental zoning of the Currais Novos was done in the totality of the municipal territory and guided by the environmental physics variables. The geoenvironmental zoning sets in a planning and ordering of the territory instrument based in the landscape analysis. Therefore grounded in the Geosystems‟s Theory this work has like a main objective to propose a geoenvironmental zoning for the Currais Novos Municipality in RN. So, was used an analysis technique suggested for Bardin (2010) and the Currais Novos‟s physical environment characterization through of the fieldwork and cartographic data vectorization, beyond the image‟s treatment SRTM. The geoenvironmental systems definitions were based in the suggestion of Cestaro, et al. (2007) support in Bertrand (1968). For both were identified five geoenvironmental systems: Borborema Plateau, Residual plateau, Chapada da Serra de Santana, semiarid river valley and lagoon valley and eleven geoenvironmental subsystems: Borborema Plateau Western Slope, Isolated Massif of the Borborema Plateau, Residual Crest, Residual Massif, Erosional Scarp of the Chapada, flat top plateau, fluvial plains, temporary river of the semiarid and ornamental water or sluice

This work presents geophysical and geological results obtained in a dunefield located in the east coast of Rio Grande do Norte State, with the aim to recognize the aeolian body depositional geometries to a future geologic modeling of the aeolian petroliferous reservoirs. The research, which was done in blowouts region situated at Nisia Floresta Municipally, included the characterization of external geometries with GPS and internal geometry analysis by GPR. Data was integrated in GoCAD software, where it was possible the three-dimensional characterization and interpretation of the studied deposits. The interpretation of GPR profiling allowed identifying: First-order bounding surfaces that separated the aeolian deposits of the Barreiras Formation rocks; Second-order bounding surfaces, which limit dune generations and Third-order bounding surfaces, a reactivation surface. This classification was based and adapted by the Brookfield (1977) and Kocurek (1996) propose. Four radarfacies was recognized: Radarfacies 1, progradational reflectors correlated to foresets of the dunes, Radarfacies 2, plain parallels reflectors related to sand sheets, Radarfacies 3, plain parallels reflectors associated to reworking of the blowout dune crest and Radarfacies 4, mounded reflectors associated to vegetated mound of sand or objects buried in subsurface. The GPR and GPS methods was also employed to the monitoring of dunefields susceptible to human activities in Buzios Beach, where the constructions along the blowout region and the tourism are changing the natural evolution of the deposits. This fact possibly to cause negative impacts to the coastal zone. Data obtained in Dunas Park, a unit environmental conservation, was compared with information of the Buzios Beach. There is a major tendency of erosion in Buzios, specifically in blowout corridor and blowout dune

The geo-environmental zoning is an important means to plan the management of the territory, once it is the result of integration of different elements of the physical environment. The Municipality of Crato has a diversity of geo-environmental systems due to its complex landscape, which characterization and delimitation will contribute to the most appropriate occupation and the fewer prejudicial to municipal area. This study, which was supported by geo-systemic theory, searched to reach the following objectives: to elaborate a proposal of geo-environmental zoning; to characterize identified and demarcated geo-environmental systems, including their potentiality and limitations of land use and human occupation; and to generate a database on digital maps through GIS Geographic Information System. Four geo-environmental systems were identified (Chapada do Araripe: plateau and hillside, Massif and residual crest, Sertão of peripheric southern depression and fluvial Plains) and ten geo-environmental subsystems (Eastern plateau covered by cerradão/cerrado, Western plateau covered by cerrado/carrasco, Northern-west hillside, central residual Massif, Northern Residual Massifs, Sertaneja depression dominated by sedimentary materials, Sertaneja depression dominated by embasement crystalline material; wet fluvial plains and dry fluvial plains). The identified system and subsystem are similar to those proposed by FUNCEME in spite of some changes due to the more detailed mapping

Aim: To describe the orthodontic, periodontal and prosthetic management of a case with a 3 mm root fracture below the crest of the alveolar bone.Methods: The root was extruded and periodontal surgery carried out to improve aesthetics and dental function.Conclusion: A multidisciplinary approach to the management of dental root fractures is necessary for successful treatment. Background: Very limited information is available from in vivo studies about whether smoking and/or nicotine affect gingival tissues in the absence of plaque. The purpose of this study is to evaluate the effect of the systemic administration of nicotine in the proliferation and counting of fibroblast-like cells in the gingival tissue of rats.Methods: Thirty adult male Wistar rats were randomly assigned into two groups to receive subcutaneous injections of a saline solution (control group = group C) or nicotine solution (group N; 3 mg/kg) twice a day. The animals were euthanized 37, 44, or 51 days after the first subcutaneous injection. Specimens were routinely processed for serial histologic sections. Five fields of view in the connective tissue adjacent to the gingival epithelium and above the alveolar bone crest of the maxillary first molar were selected for the counting of fibroblast-like cells. Data were statistically analyzed (P<0.05).Results: The intergroup analysis detected a lower number of fibroblast-like cells in group N compared to group C on days 37 (2.65 +/- 1.41 and 6.67 +/- 3.25, respectively), 44 (2.70 +/- 1.84 and 8.57 +/- 2.37, respectively), and 51(2.09 +/- 1.41 and 7.49 +/- 2.60, respectively) (P<0.05). The quantification of fibroblast-like cells showed no significant difference (P >0.05) in the intragroup analysis of control and nicotine throughout experimental periods. In the intergroup analysis, group N had reduced proliferating cell nuclear antigen positive fibroblasts compared to group C in all periods (P<0.05).Conclusion: The daily systemic administration of nicotine negatively affected, in vivo, the number and proliferation of fibroblast-like cells in the gingival tissue of rats. Purpose: The implant-abutment connection (microgap) influences the pen-implant bone morphology. However, it is unclear if different microgap configurations additionally modify bone reactions. This preliminary study aimed to radiographically monitor pen-implant bone levels in two different microgap configurations during 3 months of nonsubmerged healing. Materials and Methods: Six dogs received two implants with internal Morse taper connection (INT group) on one side of the mandible and two implants with external-hex connection (EXT group) on the other side. One implant on each side was positioned at bone level (equicrestal); the second implant was inserted 1.5 mm below the bone crest (subcrestal). Healing abutments were attached directly after implant insertion, and the implants were maintained for 3 months without prosthetic loading. At implant placement and 1, 2, and 3 months, standardized radiographs were taken to monitor pen-implant bone levels. Results: All implants osseointegrated. A total bone loss of 0.48 +/- 0.66 mm was measured in the equicrestal INT group, 0.69 +/- 0.43 mm in the equicrestal EXT group, 0.79 +/- 0.93 mm in the subcrestal INT group, and 1.56 +/- 0.53 mm in the subcrestal EXT group (P>.05, paired t tests). Within the four groups, bone loss over time became significantly greater in the EXT groups than in the INT groups. The greatest bone loss was noted in the subcrestal EXT group. Conclusion: Within the limits of this animal study, it seems that even without prosthetic loading, different microgap configurations exhibit different patterns of bone loss during nonsubmerged healing. Subcrestal positioning of an external butt joint microgap may lead to faster radiographic bone loss. Int J Prosthodont 2011;24:445-452.

Objective: The aim of this study was to assess radiographically the effect of photodynamic therapy (PDT) as an adjunctive treatment to scaling and root planing (SRP) on induced periodontitis in dexamethasone-induced immunosuppressed rats. Material and Methods: The animals were divided into 2 groups: ND group (n=60): saline treatment; D group (n=60): dexamethasone treatment. In both ND and D groups, periodontal disease was induced by the placement of a ligature in the left first mandibular molar. After 7 days, ligature was removed and all animals received SRP, being divided according to the following treatments: SRP: saline and PDT: phenothiazinium dye (TBO) plus laser irradiation. Ten animals per treatment were killed at 7, 15 and 30 days. The distance between the cementoenamel junction and the height of the alveolar bone crest in the mesial surface of the mandibular left first molars was determined in millimeters in each radiograph. The radiographic values were analyzed statistically by ANOVA and Tukey's test at a p value <0.05. Results: Intragroup radiographic assessment (ND and D groups) showed that there was statistically significant less bone loss in the animals treated with PDT in all experimental periods compared to those submitted to SRP. Intergroup radiographic analysis (ND and D groups) demonstrated that there was greater bone loss in the ND group treated with SRP compared to the D group treated with PDT at 7 and 30 days. Conclusion: PDT was an effective adjunctive treatment to SRP on induced periodontitis in dexamethasone-induced immunosuppressed rats.

Purpose: It is unknown whether different micro gap configurations can cause different pen-implant bone reactions. Therefore, this study sought to compare the peri-implant bone morphologies of two implant systems with different implant-abutment connections. Materials and Methods: Three months after mandibular tooth extractions in six mongrel dogs, two oxidized screw implants with an external-hex connection were inserted (hexed group) on one side, whereas on the contralateral side two grit-blasted screw implants with an internal Morse-taper connection (Morse group) were placed. on each side, one implant was inserted level with the bone (equicrestal) and the second implant was inserted 1.5 mm below the bony crest (subcrestal). Healing abutments were inserted immediately after implant placement. Three months later, the peri-implant bone levels, the first bone-to-implant contact points, and the width and steepness of the peri-implant bone defects were evaluated histometrically. Results: All 24 implants osseointegrated clinically and histologically. No statistically significant differences between the hexed group and Morse group were detected for either the vertical position for peri-implant bone levels (Morse equicrestal -0.16 mm, hexed equicrestal -0.22 mm, Morse subcrestal 1.50 mm, hexed subcrestal 0.94 mm) or for the first bone-to-implant contact points (Morse equicrestal -2.08 mm, hexed equicrestal -0.98 mm, Morse subcrestal -1.26 mm, hexed subcrestal -0.76 mm). For the parameters width (Morse equicrestal -0.15 mm, hexed equicrestal -0.59 mm, Morse subcrestal 0.28 mm, hexed subcrestal -0.70 mm) and steepness (Morse equicrestal 25.27 degree, hexed equicrestal 57.21 degree, Morse subcrestal 15.35 degree, hexed subcrestal 37.97 degree) of the pen-implant defect, highly significant differences were noted between the Morse group and the hexed group. Conclusion: Within the limits of this experiment, it can be concluded that different microgap configurations influence the size and shape of the peri-implant bone defect in nonsubmerged implants placed both at the crest and subcrestally.
