We evaluated the effect of adjuvant whole brain irradiation (WBI) after surgery or radiosurgery for solitary brain metastases in a Phase III multicentre trial with randomization to 30-36 Gy WBI or observation. The study was closed early due to slow accrual after 19 patients (WBI 10, observation 9). There was no difference in CNS failure-free survival or overall survival between the arms. There was a trend to reduced CNS relapse with WBI (30% versus 78%, P = 0.12). Limited analysis of quality of life and neurocognitive function data revealed no evidence of difference between the arms. Our results are not inconsistent with two larger randomized trials and support the use of upfront WBI to decrease brain recurrence in this setting. (c) 2006 Elsevier Ireland Ltd. Classic identity negative priming (NP) refers to the finding that when an object is ignored, subsequent naming responses to it are slower than when it has not been previously ignored (Tipper, S.P., 1985. The negative priming effect: inhibitory priming by ignored objects. Q. J. Exp. Psychol. 37A, 571-590). It is unclear whether this phenomenon arises due to the involvement of abstract semantic representations that the ignored object accesses automatically. Contemporary connectionist models propose a key role for the anterior temporal cortex in the representation of abstract semantic knowledge (e.g., McClelland, J.L., Rogers, T.T., 2003. The parallel distributed processing approach to semantic cognition. Nat. Rev. Neurosci. 4, 310-322), suggesting that this region should be involved during performance of the classic identity NP task if it involves semantic access. Using high-field (4 T) event-related functional magnetic resonance imaging, we observed increased BOLD responses in the left anterolateral temporal cortex including the temporal pole that was directly related to the magnitude of each individual's NP effect, supporting a semantic locus. Additional signal increases were observed in the supplementary eye fields (SEF) and left inferior parietal lobule (IPL). (c) 2006 Elsevier Inc. All rights reserved.

MRI diffusion tensor imaging (DTI), optimized for measuring the trace of the diffusion tensor, was used to investigate microstructural changes in the brains of 12 individuals with schizophrenia compared with 12 matched control subjects. To control for the effects of anatomic variation between subject groups, all participants' diffusion images were non-linearly registered to standard anatomical space. Significant statistical differences in mean diffusivity (MD) measures between the two groups were determined on a pixel-by-pixel basis, using Gaussian random field theory. We found significantly elevated MD measures within temporal, parietal and prefrontal cortical regions in the schizophrenia group (P > 0.001), especially within the medial frontal gyrus and anterior cingulate. The dorsal medial and anterior nucleus of the thalamus, including the caudate, also exhibited significantly increased MD in the schizophrenia group (P > 0.001). This study has shown for the first time that MD measures offer an alternative strategy for investigating altered prefrontal-thalamic circuitry in schizophrenia. (c) 2006 Elsevier Inc. <p>The naturally occurring radionuclide (<sup>226</sup>Ra,<sup>232</sup>Th and<sup>40</sup>K) content of building Materials (NORM) contributes to the total radiation dose experienced by humans. In this survey 27 clay and 68 red mud samples were surveyed with gamma spectrometry and screened according to European Basic Safety Standards (BSS) I-index. It was found that average I-index of clays was 0.6 (0.40.8) less than the I-index of 1, which makes them suitable for building material production. The average I-index of red mud 2.3 (1.33.0). The maximal mixing ratio of red mud was calculated, varied between 12 and 39%, with 23% average.</p>

<p>The development of new learning models has been of great importance throughout recent years, with a focus on creating advances in the area of deep learning. Deep learning was first noted in 2006, and has since become a major area of research in a number of disciplines. This paper will delve into the area of deep learning to present its current limitations and provide a new idea for a fully integrated deep and dynamic probabilistic system. The new model will be applicable to a vast number of areas initially focusing on applications into medical image analysis with an overall goal of utilising this approach for prediction purposes in computer based medical systems.</p>

The aim os this study is to evaluate and compare the utility of 18F-fluorocholine (18F-CH) PET/CT versus 3-Tesla multiparametric MRI (mpMRI) without endorectal coil to detect tumor recurrences in patients with biochemical relapse following radical prostatectomy (RP). Secondarily, to identify possible prognostic variables associated with mpMRI and 18F-CH PET/CT findings. Retrospective study of 38 patients who developed biochemical recurrence after RP between the years 2011 and 2015 at our institution. PET/CT and mpMRI were both performed within 30 days of each other in all patients. The PET/CT was reviewed by a nuclear medicine specialist while the mpMRI was assessed by a radiologist, both of whom were blinded to outcomes. The median prostate-specific antigen (PSA) value pre-MRI/PET-CT was 0.9 ng/mL (interquartile range 0.42.2 ng/mL). There were no differences in the detection rate between 18F-CH PET/CT and mpMRI for local recurrence (LR), lymph node recurrence (LNR) and bone metastases (BM). Separately, mpMRI and 18F-CH PET/CT were positive for recurrence in 55.2% and 52.6% of cases, respectively, and in 65.7% of cases when findings from both modalities were considered together. The detection of LR was better with combined mpMRI and choline PET/CT versus choline PET/CT alone (34.2% vs 18.4%, p=0.04). Salvage treatment was modified in 22 patients (57.8%) based on the imaging findings. PSA values on the day of biochemical failure were significantly associated with mpMRI positivity (adjusted odds ratio (OR): 30.9; 95% confidence interval (CI): 1.5635.8). Gleason score>7 was significantly associated with PET/CT positivity (OR: 13.9; 95% CI: 1.5125.6). A significant association was found between PSA doubling time (PSADT) (OR: 1.3; 95% CI: 1.01.7), T stage (OR: 21.1; 95% CI: 1.6272.1), and LR. Multiparametric MRI and 18F-CH PET/CT yield similar detection rates for LR, LNR and pelvic BM. The combination of both imaging techniques provides a better LR detection versus choline PET/CT alone. The initially planned salvage treatment was modified in 57.8% of patients due to imaging findings. In addition to PSA values, Gleason score, T stage, and PSADT may provide valuable data to identify those patients that are most likely to benefit from undergoing both imaging procedures.

Neuroimaging research involves analyses of huge amounts of biological data that might or might not be related with cognition. This relationship is usually approached using univariate methods, and, therefore, correction methods are mandatory for reducing false positives. Nevertheless, the probability of false negatives is also increased. Multivariate frameworks have been proposed for helping to alleviate this balance. Here we apply multivariate distance matrix regression for the simultaneous analysis of biological and cognitive data, namely, structural connections among 82 brain regions and several latent factors estimating cognitive performance. We tested whether cognitive differences predict distances among individuals regarding their connectivity pattern. Beginning with 3,321 connections among regions, the 36 edges better predicted by the individuals' cognitive scores were selected. Cognitive scores were related to connectivity distances in both the full (3,321) and reduced (36) connectivity patterns. The selected edges connect regions distributed across the entire brain and the network defined by these edges supports high-order cognitive processes such as (a) (fluid) executive control, (b) (crystallized) recognition, learning, and language processing, and (c) visuospatial processing. This multivariate study suggests that one widespread, but limited number, of regions in the human brain, supports high-level cognitive ability differences. After the discovery of ionizing radiation, its applications in various fields of science began to take significant proportions. In the case of medicine, there are the application areas in radiotherapy, diagnostic radiology and nuclear medicine. It was then necessary to create the field of radiological protection to establish the conditions necessary for the safe use of such ionizing radiation. Apply knowledge obtained during the graduation stage and in the practice of radiological protection in the areas of nuclear medicine and diagnostic radiology. In the area of nuclear medicine, tests were made in the Geiger-Muller counters (GM) and the dose calibrator (curiometer), the monitoring tests of radiation, waste management, clean of the Therapeutic room and testing the quality control of gamma-chambers. In the area of radiology, were performed tests of quality control equipment for conventional X-ray equipment and x-ray fluoroscopy, all following the rules of the National Health Surveillance Agency (ANVISA), and reporting of tests. The routine developed in the fields of nuclear medicine in hospitals has proved very useful, since the quality control of GM counters contribute to the values of possible contamination are more reliable. The control of dose calibrator enables the patient not to receive different doses of the recommended amounts, which prevents the repetition of tests and unnecessary exposure to radiation. The management of waste following the rules and laws established and required for its management. Tests for quality control of gamma chambers help to evaluate its medical performance through image. In part of diagnostic radiology, tests for quality control are performed in order to verify that the equipment is acceptable for usage or if repairs are needed. Rationale and Objectives. The authors attempt to provide a set of objectives for medical student training in radiology for contemporary medical practice. Materials and Methods. A questionnaire containing a list of educational objectives was sent to 32 radiologists in charge of medical student training in radiology at accredited residency programs in Australia and New Zealand. The importance of including each preselected objective in the curriculum was measured by respondents' agreement or disagreement on a scale of 1-6. Opportunity also was given to respondents to suggest objectives other than those presented on the questionnaire. Results. Twenty of the 32 questionnaires were returned, and a set of educational objectives was established based on the responses. The objectives were ranked in importance according to the mean score assigned to each objective by the respondents. Conclusion. This new set of educational objectives for medical student radiology training reflects recent changes in radiologic and medical practice and points to potential future developments.

In 2012 we were awarded an Erasmus Intensive Programme grant to facilitate OPTIMAX 2013, a three week duration residential summer school held within the UK during August 2013. The summer school helped to further develop student radiographer skills in optimising x-radiation dose and image quality. With a major emphasis on visual techniques to determine image quality, lesion visibility, lesion detection performance and physical measures of image quality (eg signal to noise ratio (SNR)) we conducted controlled laboratory experiments on phantoms using Computed Radiography, CT and Full Field Digital Mammography. Mathematical modelling was used for radiation dose estimation. Sixty seven people from 5 European countries participated. This included 49 PhD, MSc and BSc students. Discipline areas included radiography, physics, biomedical science and nuclear medicine.

Resumo - O avano tecnolgico e cientfico no campo da Medicina tem favorecido a aplicao das radiaes ionizantes nas reas da Medicina Nuclear, Radiologia, Radioterapia e noutras especialidades relacionadas com a medicina de interveno, como a Cardiologia. As Organizaes de Sade, com profissionais com risco de exposio a radiaes ionizantes, so responsveis, legislativamente, por assegurar a sua vigilncia. Certificando a observao controlada das doses de exposio, os profissionais so monitorizados com dosmetros individuais e submetidos a vigilncia mdica. As leituras de dosmetria individual destes profissionais, originam por vezes, nos prprios, desconfianas e incertezas quanto aos valores medidos. Objectivo: avaliar o grau de confiana dos profissionais de sade, expostos a radiaes ionizantes, no sistema de monitorizao individual. Metodologia: estudo observacional do tipo analtico- transversal, por questionrio. Amostra de 190 Tcnicos de Diagnstico e Teraputica de Cardiopneumologia, Medicina Nuclear, Radiologia, Radioterapia, expostos a radiaes ionizantes, que exercem a sua actividade profissional em hospitais do concelho de Lisboa. Resultados: 51,1% dos Tcnicos de Diagnstico e Teraputica no confiam nas leituras dos dosmetros, no se determinou uma relao estatisticamente significativa entre essa confiana, o tipo de dosmetro e a empresa que realiza as leituras. A confiana maior nos que exercem em Organizaes de Sade pblicas. 40,2% das Organizaes de Sade apresentam plano de vigilncia mdica, constatando-se uma falta de conformidade entre os Tcnicos de Diagnstico e Teraputica, nalguns hospitais, relativamente existncia desse plano no respectivo hospital. A margem de erro associado ao estudo de 5,09%, com um nvel de confiana de 95%. -----------Abstract - The technological and scientific advances in the field of medicine has encouraged the application of ionizing radiation with a considerable positive contribution in the areas of Nuclear Medicine, Radiology, Radiotherapy and other specialties related to medical intervention like Cardiology. The Health Organizations that arise the risk of occupational exposure to ionizing radiation and is their responsibility to ensure monitoring, according the law. To ensure the monitoring of controlled doses of exposure, the exposed works use an individual dosimeter and undergo medical supervision. The individual dosimetry readings of health-care professionals, is sometimes questionable for themselves, arising distrust and uncertainty about the values measured. Objective: evaluate the degree of confidence of health professionals, exposed to radiation, on the system of individual monitoring. Methods: Observational study of cross-type analysis by questionnaire. Sample of 190 of technologists of Cardiopneumology, Nuclear Medicine, Radiology, Radiotherapy, exposed to ionizing radiation, which work in hospitals on the region of Lisbon. Results: 51.1% of technologists does not rely on the readings of their dosimeters, and wasnt determined a statistically significant relationship between this trust, the type of dosimeter and the company that makes the readings(s) of their(s) dosimeter(s). The confidence in the readings is higher in public Health Organizations. 40.2% of the Health Organizations have medical monitoring plan and there is a disagreement between technologists, in some hospitals, relatively to the existence of this plan in their hospital. The Institute of Radiation Physics (IRA) is attached to the Department of Medical Radiology at the Vaud University Hospital Center (CHUV) in Lausanne. The Institute's main tasks are strongly linked to the medical activities of the Department: radiotherapy, radiodiagnostics, interventional radiology and nuclear medicine. The Institute also works in the fields of operational radiation protection, radiation metrology and radioecology. In the case of an accident involving radioactive materials, the emergency services are able to call on the assistance of radiation protection specialists. In order to avoid having to create and maintain a specific structure, both burdensome and rarely needed, Switzerland decided to unite all existing emergency services for such events. Thus, the IRA was invited to participate in this network. The challenge is therefore to integrate a university structure, used to academic collaborations and the scientific approach, to an interventional organization accustomed to strict policies, a military-style command structure and "drilled" procedures. The IRA's solution entails mobilizing existing resources and the expertise developed through professional experience. The main asset of this solution is that it involves the participation of committed collaborators who remain in a familiar environment, and are able to use proven materials and mastered procedures, even if the atmosphere of an accident situation differs greatly from regular laboratory routines. However, this solution requires both a commitment to education and training in emergency situations, and a commitment in terms of discipline by each collaborator in order to be integrated into a response plan supervised by an operational command center.
