Objetivo: relatar a evolução de uma série de casos de gestação em mulheres previamente submetidas à cirurgia de bypass gástrico para tratamento de obesidade grave. Métodos: cinco casos consecutivos de gravidez após gastroplastia ocorridos entre 2001 e 2004 foram avaliados. As pacientes tinham idade entre 30 e 34 anos e todas haviam sido submetidas à cirurgia de Capella. Aspectos clínicos, laboratoriais e do acompanhamento materno e fetal foram considerados, durante o período gestacional e após o parto. Foi realizada revisão da literatura internacional, por meio das bases de dados MEDLINE e Web of Science, utilizando os seguintes unitermos: gastroplasty, gastric bypass surgery, bariatric surgery e pregnancy. Resultados: todas as gestações observadas foram únicas e não ocorreram complicações obstétricas, durante o seguimento pré-natal e parto. Também não houve registro de recém-nascidos prematuros ou de baixo peso ao nascimento. Conclusão: nossos dados sugerem que a gravidez após gastroplastia é segura para a mãe e feto. Entretanto, em virtude do limitado volume de informação disponível sobre o tema, investigações adicionais são necessárias para estabelecer recomendações apropriadas com relação ao seguimento dessas gestações _________________________________________________ABSTRACT Purpose: we report a small series of pregnant women who underwent gastric bypass surgery for severe obesity, with a review of the literature on this topic. Bakgrund: Flera studier har visat att personer med bipolär sjukdom har en sämre livskvalitet jämfört med övriga befolkningen. Som sjuksköterska inom psykiatrisk vård är det viktigt att ha kunskap om sjukdomen för att ge bästa möjliga behandling med mål att stabilisera symtom, förebygga återfall och främja livskvalitet. Syfte: Att utifrån sjuksköterskans uppfattning beskriva vilka omvårdnadsinsatser som främjar livskvalitet vid bipolär sjukdom. Metod: En kvalitativ innehållsanalys som baserades på åtta semistrukturerade intervjuer. Resultat: Resultatet visade tre huvudteman: Kunskap om sjukdomen: betydelsen av att både patient och anhöriga fick kunskap om sjukdomen; Trygghet och kontinuitet i vården: veta var och till vem man vänder sig i vården hade stor betydelse samt Personcentrerad vård: att bli sedd och bemött som en unik person. Slutsats: Patientutbildning där man tar upp vikten av regelbundna levnadsvanor, att undvika stress samt att bli medveten om sina tidiga tecken kan hjälpa att stabilisera symtom och förebygga återfall.

We worked toward developing a core outcome set for clinical research studies in polymyalgia rheumatica (PMR) by conducting (1) patient consultations using modified nominal group technique; (2) a systematic literature review of outcome measures in PMR; (3) a pilot observational study of patients presenting with untreated PMR, and further discussion with patient research partners; and (4) a qualitative focus group study of patients with PMR on the meaning of stiffness, using thematic analysis. (1) Consultations included 104 patients at 4 centers. Symptoms of PMR included pain, stiffness, fatigue, and sleep disturbance. Function, anxiety, and depression were also often mentioned. Participants expressed concerns about diagnostic delay, adverse effects of glucocorticoids, and fear of relapse. (2) In the systematic review, outcome measures previously used for PMR include pain visual analog scores (VAS), morning stiffness, blood markers, function, and quality of life; standardized effect sizes posttreatment were large. (3) Findings from the observational study indicated that asking about symptom severity at 7 AM, or "on waking," appeared more relevant to disease activity than asking about symptom severity "now" (which depended on the time of assessment). (4) Preliminary results were presented from the focus group qualitative study, encompassing broad themes of stiffness, pain, and the effect of PMR on patients' lives. It was concluded that further validation work is required before a core outcome set in PMR can be recommended. Nevertheless, the large standardized effect sizes suggest that pain VAS is likely to be satisfactory as a primary outcome measure for assessing response to initial therapy of PMR. Dissection of between-patient heterogeneity in the subsequent treatment course may require attention to comorbidity as a potential confounding factor.

This work addresses the study of interdisciplinarity in higher administration education, a topic of great relevance in the present context, due to guidelines issued by Ministry of Education through the National Curriculum Guidelines -NCGs for undergraduate courses in Management. The attention on the subject emerges from the gained experience of a researcher and teacher working as manager of an undergraduate course in Business Administration who set up interdisciplinarity through interdisciplinary projects in a management course pedagogical project at a private university in State of Pará/Brazil. The work rebuilds and reports experiences, and studies the practice of lecturers involved in those interdisciplinary actions. The study aimed to identify changes in pedagogical practices of teachers involved in the searched interdisciplinary experience. To address the questions that directed the work and achieve proposed objectives, from a qualitative approach, a significant bibliographical and documentary work was conducted on the topic. In the survey carried out on secondary sources such as publications of major authors and scientific papers reporting interdisciplinary experiences in higher administration education, it was found that interdisciplinarity for its ambiguous character is still poorly understood by teachers, and reports on its application in administration teaching are incipient. This study also used data collected from primary sources, from dialogues through interviews with educational fellows - teachers and officers of the institution that served as locus of the research, who had the opportunity to experience the studied interdisciplinary experience. Data were processed and analyzed using content analysis technique. Research results showed that teachers of the institution of research have a good understanding of meaning of interdisciplinarity as a link between disciplines; it was also found substantial evidence of changes in teaching practices and actions of such teachers based on their participation in interdisciplinary projects. Although the experience studied can be considered innovative and challenging, much needs to be done in the course management for achievement of interdisciplinary actions in the course, particularly regarding to the removal of institutional, methodological, psychosocial, epistemological obstacles in operationalizing interdisciplinary practices, with emphasis on the need of a process of continuous and specific training aiming at developing skills for interdisciplinary acting, to the extent that these education professionals do not perceive themselves able to act as interdisciplinary lecturers

Can entrepreneurship be taught, or be learned? This is a question that brings a broad debate to the table. Indubitably, education has an essential role in attitude development and competences promotion. This assumption has fostered entrepreneurship initiatives and course proliferation, seeking to teach entrepreneurship to individuals. The increasing importance of the entrepreneurial spirit leads to the creation and promotion of entrepreneurship initiatives and encouragement of youngsters to have entrepreneurial profiles. Today, entrepreneurship education has a crucial role promoting entrepreneurial mindset s in younger people. It stimulates competence and skill development, which extends beyond the business world. The effects of such education depend on strategy and teaching pedagogies, but mostly on its effective implementation. This gives teachers and higher education institutions the challenge of finding nontraditional methodologies and alternative ways to teach the subject. Therefore, this study aims to explore, through the theory of planned behavior, the effects of entrepreneurship education and its relationship with entrepreneurial intention. Additionally, the essential and challenging role of the teacher is explored, contributing to a better understanding of its significance in entrepreneurship education that is remarkably poorly explored in the literature. Findings reinforce the strength of the theory of planned behavior as an intention measure; in addition, students were found to increase their entrepreneurial intentions, entrepreneurial knowledge and institutional context perceptions after taking a higher education course. Concerning teachers, a consensus was found between their education aims and similarities among their pedagogic methods, topics and forms of evaluation, along with time and resource management struggles.

Este artículo describe los métodos, técnicas de enseñanza y recursos didácticos, utilizados por una muestra de docentes de I y II Ciclo de la Educación General Básica en la Dirección Regional Educativa de Heredia al enseñar Probabilidad y Estadística, que surge como resultado de una investigación realizada en el 2009, en Costa Rica. Para obtener los datos necesarios se aplicó un cuestionario auto-administrado a dicha muestra de docentes. En relación con los métodos, dentro de los hallazgos más importantes se encontró una preferencia de los educadores entrevistados, hacia estrategias pedagógicas tradicionales (usan con mayor frecuencia los métodos: deductivo, expositivo, estudio de textos, colectivo). En cuanto a técnicas de enseñanza, se señalaron con uso frecuente la lluvia de ideas, las actividades lúdicas y el trabajo en grupos, que si bien son buenas herramientas para propiciar un proceso de enseñanza y aprendizaje en donde el estudiante interactúe con situaciones problemáticas, también pueden emplearse en lecciones tradicionales. La técnica elaboración de proyectos resultó de uso poco frecuente. Además, dentro los principales recursos didácticos elegidos por los docentes, para enseñar Probabilidad y Estadística, se destacan la pizarra y el material fotocopiado; aunque también se mencionaron las actividades lúdicas y el material concreto. Lisääntyvä ikääntyneiden määrä asettaa vaatimuksia terveydenhuollon järjestämiselle niin Suomessa kuin maailmanlaajuisestikin. Ikääntyneiden monialaiset ongelmat yhdessä perussairauksien kanssa muodostavat monimutkaisen kokonaisuuden, joka voi johtaa avun hakemiseen päivystyksestä. Ikääntyneiden hoidossa esiintyvät erityispiirteet, hoidettavien kognitiivisen toimintakyvyn heikkeneminen, lyhentyneet hoitoajat ja hoidon painopisteen siirtyminen kotiin vaativat hoitohenkilökuntaa yhä enenevässä määrin kiinnittämään huomiota potilasohjaukseen. Tutkimuksen tarkoituksena oli kuvata päivystysosastolta kotiutuvien ikääntyneiden ja sairaanhoitajien välisten ohjauskeskusteluiden rakennetta ja sisältöä. Tavoitteena oli tuottaa tietoa siitä, mitä voimavaraistumista tukevia piirteitä ja tiedon osa-alueita esiintyy ohjauskeskusteluissa. Tutkimuksen aineisto kerättiin ei-osallistuvalla havainnoinnilla videoimalla päivystysosastolta kotiutuvien ikääntyneiden ohjaustilanteita (n=22) syksyllä 2015. Osallistuneiden potilaiden keski-ikä oli 79 vuotta. Aineisto analysoitiin deduktiivisella sisällönanalyysilla. Ohjauskeskustelun rakenteesta etsittiin voimavaraistumista tukevia piirteitä ja sisältöä analysoitiin kuuden voimavaraistumista tukevan tiedon osa-alueen pohjalta (biofysiologinen, toiminnallinen, kokemuksellinen, sosiaalinen, eettinen ja taloudellinen). Ohjauskeskustelujen alussa ei asetettu ohjauksen tavoitteita tai arvioitu ikääntyneiden aikaisemman tiedon tasoa. Ohjauskeskustelut etenivät pääosin hoitajien määrittäminä. Ikääntyneet kysyivät vähemmän kysymyksiä ja heidän yksisanaisten puheenvuorojensa määrä oli suurempi kuin hoitajilla. Ikääntyneiden osallistumista ohjauskeskusteluun ei myöskään tuettu. Kysymyksen esitettyään hoitajat antoivat ikääntyneille mahdollisuuden vastata, mutta eivät varmistaneet, olivatko nämä ymmärtäneet ohjauksen. Ohjauskeskusteluiden sisältöä tarkasteltaessa tietoa ikääntyneille välitettiin pääasiassa biofysiologisen tiedon osa-alueelta (tietoa hoidosta ja oireista). Toiseksi eniten tietoa välitettiin ikääntyneille toiminnallisen tiedon osa-alueelta (liikkuminen ja kotona selviytyminen) ja sosiaalisen tiedon osa-alueelta (omaisten osallistuminen hoitoon). Taloudellisen tiedon osa-alue jäi aineistossa pieneksi. Tutkimuksen tulokset osoittavat tarvetta kiinnittää huomiota ohjauskeskusteluiden rakenteeseen ja sisältöön etenkin hoidettaessa ikääntyneitä, joilla on monitahoisia terveysongelmia. Tutkimuksesta saatavaa tietoa voidaan hyödyntää suunniteltaessa ja toteuttaessa ikääntyneiden potilasohjausta päivystyksessä sekä suunniteltaessa sairaanhoitajien potilasohjauskoulutusta.

Background: Parenteral nutrition (PN) is a costly therapy that can also be associated with serious complications. Therefore, efforts are focusing on reducing rate of complications, and costs related to PN. Objective: The aim was to analyze the effect of the implementation of PN standardization on costs and quality criteria. Secondary aim was to assess the use of individualized PN based on patient's clinical condition. Methods: We compare the use of PN before and after the implementation of PN standardization. Demographic, clinical and PN characteristics were collected. Costs analysis was performed to study the costs associated to the two different periods. Quality criteria included were: 1) PN administration; 2) nutrition assessment (energy intake between 20-35 kcal/kg/day; protein contribution according to nitrogen balance); 3) safety and complications (hyperglycemia, hypertriglyceridemia, hepatic complications, catheter-related infection); 4) global efficacy (as serum albumin increase). Chi-square test was used to compare percentages; logistic regression analysis was performed to evaluate the use of customized PN. Results: 296 patients were included with a total of 3,167 PN compounded. During the first period standardized PN use was 47.5% vs 85.7% within the second period (p < 0.05). No differences were found in the quality criteria tested. Use of individualized PN was related to critical care patients, hypertriglyceridemia, renal damage, and long-term PN. Mean costs of the PN decreased a 19.5%. Annual costs savings would be € 86,700. Conclusions: The use of customized or standard PN has shown to be efficient and flexible to specific demands; however customized PN was significantly more expensive.

Introduction: Abdominal pain, in etiology sometimes difficult to be defined, is a frequent complaint in childhood. Abdominal epilepsy is a rare cause of abdominal pain. Objectives: In this article, we report on 5 year old girl patient with abdominal epilepsy. Methods: Some investigations (stool investigation, routine blood tests, ultrasonography (USG), electrocardiogram (ECHO) and electrocardiograpy (ECG), holter for 24hr.) were done to understand the origin of these complaints; but no abnormalities were found. Finally an EEG was done during an episode of abdominal pain and it was shown that there were generalized spikes especially precipitated by hyperventilation. The patient did well on valproic acid therapy and EEG was normal 1 month after beginning of the treatment. Discussion: The cause of chronic recurrent paroxymal abdominal pain is difficult for the clinicians to diagnose in childhood. A lot of disease may lead to paroxysmal gastrointestinal symptoms like familial mediterranean fever and porfiria. Abdominal epilepsy is one of the rare but easily treatable cause of abdominal pain. Conclusion: In conclusion, abdominal epilepsy should be suspected in children with recurrent abdominal pain.

A OA é a doença reumática mais comum no ser humano e uma doença crónica com impacto elevado na sociedade. Tem repercussões na saúde, ao nível da funcionalidade, comprometendo a realização das atividades da vida diária e a qualidade de vida dos indivíduos. Uma das articulações mais afetada pela OA é a articulação do joelho. O tratamento eficaz requer a combinação de tratamentos farmacológicos e não farmacológicos. Os tratamentos não farmacológicos, principalmente o exercício e a educação do doente têm vindo a ganhar importância, no que se refere ao controlo dos sintomas. A realização do estágio curricular surge no programa comunitário PLE²NO. O programa é considerado uma opção não farmacológica no tratamento e controlo dos sintomas da Osteoartrose (OA) no joelho. Os principais objetivos do estágio no PLE²NO consistiram: aquisição de conhecimentos teóricos relacionados com OA e práticos com vista ao aperfeiçoamento da prescrição do exercício; aquisição de competências essenciais à ótima liderança e comunicação com as pessoas e instituições envolvidas; prescrição de exercício a indivíduos com OA no joelho; cativar e motivar as pessoas para a prática de exercício; e determinar a eficácia de um programa de 3 meses de educação e exercício nos sintomas, aptidão física e qualidade de vida dos idosos com OA no joelho. Na aptidão física verificaram-se diferenças significativas na capacidade aeróbia, flexibilidade e velocidade da marcha. Nos indicadores de saúde houve melhoria em praticamente todos os parâmetros avaliados: mobilidade, cuidados pessoais, dor/mal-estar e sintomas de ansiedade/depressão. Assim, esta intervenção revelou-se ser uma mais-valia para o tratamento não farmacológico da osteoartrose do joelho a médio prazo.

Introduktion: Runt 250 000 personer i Sverige är drabbade av hjärtsvikt. Det finns olika grader av hjärtsvikt som påverkar patienters dagliga liv på olika nivåer vilket kan påverka patienters livskvalité. Livskvalité är ett brett begrepp som är multidimensionellt. Patientutbildning är en del av sjuksköterskans profession och syftar till att hjälpa patienter förbättra sin egen hälsa. Syfte: Syftet med litteraturstudien var att belysa patientutbildningens betydelse för livskvalitén hos patienter med hjärtsvikt. Metod: Under litteraturstudien har Polit och Beck (2016) niostegsmodell använts. Till litteratursökningen användes databaserna Cinahl och PubMed. Databassökningen resulterade i 10 artiklar samt två artiklar som söktes manuellt. Artiklarna granskades kritiskt och utgjorde därefter materialet för litteraturstudiens resultat. Resultat: Det slutgiltiga resultatet delades in i två huvudteman vilka var En förändrad tillvaro och Kunskap och trygghet med totalt fem underteman. Slutsats: Litteraturstudien påvisar att patientutbildning kan påverka patienter med hjärtsvikt då livskvalitén generellt har förbättrats för de patienter som erhållit patientutbildning. Litteraturstudien visar att patientutbildning kan ge en förändrad fysisk funktion, socialt och emotionellt stöd, förmåga att utföra egenvård samt ge kunskap, förståelse och kontroll över tillståndet.

The clinical syndrome of heart failure is one of the leading causes of hospitalisation and mortality in older adults. Due to ageing of the general population and improved survival from cardiac disease the prevalence of heart failure is rising. Despite the fact that the majority of patients with heart failure are aged over 65 years old, many with multiple co-morbidities, the association between cognitive impairment and heart failure has received relatively little research interest compared to other aspects of cardiac disease. The presence of concomitant cognitive impairment has implications for the management of patients with heart failure in the community. There are many evidence based pharmacological therapies used in heart failure management which obviously rely on patient education regarding compliance. Also central to the treatment of heart failure is patient self-monitoring for signs indicative of clinical deterioration which may prompt them to seek medical assistance or initiate a therapeutic intervention e.g. taking additional diuretic. Adherence and self-management may be jeopardised by cognitive impairment. Formal diagnosis of cognitive impairment requires evidence of abnormalities on neuropsychological testing (typically a result ≥1.5 standard deviation below the age-standardised mean) in at least one cognitive domain. Cognitive impairment is associated with an increased risk of dementia and people with mild cognitive impairment develop dementia at a rate of 10-15% per year, compared with a rate of 1-2% per year in healthy controls.1 Cognitive impairment has been reported in a variety of cardiovascular disorders. It is well documented among patients with hypertension, atrial fibrillation and coronary artery disease, especially after coronary artery bypass grafting. This background is relevant to the study of patients with heart failure as many, if not most, have a history of one or more of these co-morbidities. A systematic review of the literature to date has shown a wide variation in the reported prevalence of cognitive impairment in heart failure. This range in variation probably reflects small study sample sizes, differences in the heart failure populations studied (inpatients versus outpatients), neuropsychological tests employed and threshold values used to define cognitive impairment. The main aim of this study was to identify the prevalence of cognitive impairment in a representative sample of heart failure patients and to examine whether this association was due to heart failure per se rather than the common cardiovascular co-morbidities that often accompany it such as atherosclerosis and atrial fibrillation. Of the 817 potential participants screened, 344 were included in this study. The study cohort included 196 patients with HF, 61 patients with ischaemic heart disease and no HF and 87 healthy control participants. The HF cohort consisted of 70 patients with HF and coronary artery disease in sinus rhythm, 51 patients with no coronary artery disease in sinus rhythm and 75 patients with HF and atrial fibrillation. All patients with HF had evidence of HF-REF with a LVEF <45% on transthoracic echocardiography. The majority of the cohort was male and elderly. HF patients with AF were more likely to have multiple co-morbidities. Patients recruited from cardiac rehabilitation clinics had proven coronary artery disease, no clinical HF and a LVEF >55%. The ischaemic heart disease group were relatively well matched to healthy controls who had no previous diagnosis of any chronic illness, prescribed no regular medication and also had a LVEF >55%. All participants underwent the same baseline investigations and there were no obvious differences in baseline demographics between each of the cohorts. All 344 participants attended for 2 study visits. Baseline investigations including physiological measurements, electrocardiography, echocardiography and laboratory testing were all completed at the initial screening visit. Participants were then invited to attend their second study visit within 10 days of the screening visit. 342 participants completed all neuropsychological assessments (2 participants failed to complete 1 questionnaire). A full comprehensive battery of neuropsychological assessment tools were administered in the 90 minute study visit. These included three global cognitive screening assessment tools (mini mental state examination, Montreal cognitive assessment tool and the repeatable battery for the assessment of neuropsychological status) and additional measures of executive function (an area we believe has been understudied to date). In total there were 9 cognitive tests performed. These were generally well tolerated. Data were also collected using quality of life questionnaires and health status measures. In addition to this, carers of the study participant were asked to complete a measure of caregiver strain and an informant questionnaire on cognitive decline. The prevalence of cognitive impairment varied significantly depending on the neuropsychological assessment tool used and cut-off value used to define cognitive impairment. Despite this, all assessment tools showed the same pattern of results with those patients with heart failure and atrial fibrillation having poorer cognitive performance than those with heart failure in sinus rhythm. Cognitive impairment was also more common in patients with cardiac disease (either coronary artery disease or heart failure) than age-, sex- and education-matched healthy controls, even after adjustment for common vascular risk factors.
