Integrated care has been introduced as a means of improving health outcomes and access to care, and reducing the cost of healthcare. Despite its importance, the integration of oral health into primary care is still an emerging healthcare pathway. This scoping review protocol has been developed and funded by the Canadian Institutes of Health Research to provide an evidence-based synthesis on a primary oral healthcare approach and its effectiveness in improving oral health outcomes.

Purpose: To evaluate psychometric properties of Quinn’s leadership questionnaire (CFV questionnaire; 1988) to the Portuguese health services. Design: Cross-sectional study, using the Quinn’s leadership questionnaire, administered to registered nurses and physicians in Portuguese health care services (N = 687). Method: Self-administered survey applied to two samples. In the first (of convenience; N = 249 Portuguese health professionals) were performed exploratory factor and reliability analysis to the CFV questionnaire. In the second sample (stratified; N = 50 surgical units of 33 Portuguese hospitals) was performed confirmatory factor analysis using LISREL 8.80. Findings: The first sample supported an eight-factor solution accounting for 65.46% of the variance, in an interpretable factorial structure (loadings> .50), with Cronbach’s α upper than .79. This factorial structure, replicated with the second sample, showed reasonable fit for each of the 8 leadership roles, quadrants, and global model. The models evidenced, generally, nomological validity, with scores between good and acceptable (.235 < x2/df < 2.055 e .00 < RMSEA < .077). Conclusions: Quinn’s leadership questionnaire presented good reliability and validity for the eight leadership roles, showing to be suitable for use in hospital health care context. Background Expenditure on dental and oral health services in Australia is $3.4 billion AUD annually. This is the sixth highest health cost and accounts for 7 % of total national health expenditure. Approximately 49 % of Australian children aged 6 years have caries experience in their deciduous teeth and this is rising. The aetiology of dental caries involves a complex interplay of individual, behavioural, social, economic, political and environmental conditions, and there is increasing interest in genetic predisposition and epigenetic modification. Methods The Oral Health Sub-study; a cross sectional study of a birth cohort began in November 2012 by examining mothers and their children who were six years old by the time of initiation of the study, which is ongoing. Data from detailed questionnaires of families from birth onwards and data on mothers’ knowledge, attitudes and practices towards oral health collected at the time of clinical examination are used. Subjects’ height, weight and mid-waist circumference are taken and Body Mass Index (BMI) computed, using an electronic Bio-Impedance balance. Dental caries experience is scored using the International Caries Detection and Assessment System (ICDAS). Saliva is collected for physiological measures. Salivary Deoxyribose Nucleic Acid (DNA) is extracted for genetic studies including epigenetics using the SeqCap Epi Enrichment Kit. Targets of interest are being confirmed by pyrosequencing to identify potential epigenetic markers of caries risk. Discussion This study will examine a wide range of potential determinants for childhood dental caries and evaluate inter-relationships amongst them. The findings will provide an evidence base to plan and implement improved preventive strategies.

O Ministério da Saúde (MS), através de sua política de incentivos financeiros, vem promovendo um processo de reorganização da atenção à saúde bucal, com a implantação das Equipes de Saúde Bucal (ESB) na Estratégia Saúde da Família (ESF), no âmbito da atenção básica, e dos Centros de Especialização Odontológica (CEO) e Laboratório Regional de Próteses Dentárias (LRPD), na atenção secundária. Nesse contexto, esta dissertação foca o processo de reorganização da atenção à saúde bucal em Cascavel e nos demais municípios pertencentes à 10 Regional de Saúde do Estado do Paraná. Consideramos que, em tese, este processo de regionalização deve ser orientado pelas diretrizes da Política Nacional de Saúde Bucal (PNSB), que priorizam a atenção básica em saúde bucal através da ESF e trazem ainda como proposta a ampliação da média e alta complexidade em saúde bucal. Foi realizada ampla pesquisa nas bases de dados do DATASUS, relativos à produção das ações de saúde bucal dos 25 municípios pertencentes à 10 Regional de Saúde do Estado do Paraná, no período de janeiro de 1998 a dezembro de 2007. Os dados foram cotejados com aqueles disponibilizados pelo Departamento de Atenção Básica (DAB) relativos à implantação das ESB, CEO e LRPD. Os resultados demonstram que, no âmbito da 10 RS, ocorreu ampliação do acesso e da oferta de ações e serviços de saúde bucal. No entanto, a despeito da ampliação do acesso, há um longo caminho a ser percorrido para que haja avanços significativos nas condições de saúde bucal da população, tal como é a proposta da PNSB.

Esta dissertação foca o processo de reorganização das ações e serviços de saúde bucal no estado do Rio de Janeiro. Este processo, em tese, deve ser orientado pelas diretrizes da Política Nacional de Saúde Bucal (PNSB) que priorizam a atenção básica em saúde bucal através da Estratégia Saúde da Família e ampliam a atenção em saúde bucal na média e alta complexidades. O Ministério da Saúde lança mão de incentivos financeiros para induzir, sob condições específicas, a adesão às diretrizes formuladas pela Política Nacional de Saúde Bucal (PNSB) e assim promover um processo de reorganização da atenção à saúde bucal nas esferas subnacionais. O núcleo deste trabalho tem por base uma pesquisa que analisa um amplo espectro de dados sobre os serviços e as ações de saúde bucal realizadas pelo conjunto dos municípios do estado do Rio de Janeiro no período de janeiro de 1998 a dezembro de 2007. As principais conclusões da pesquisa apontam para o caráter inconcluso do processo de reorganização da atenção em saúde bucal na grande maioria dos municípios estudados e para a necessidade de uma ação conjunta entre as autoridades sanitárias das esferas federal, estadual e municipal orientada para promover uma efetiva melhoria das condições de saúde da população tal como é a proposta da PNSB.

As a result of the financial incentive provided by the GM / MS 1.444, since 2000, Brazil has experienced a substantial increase in the number of oral health services at the Family Health Strategy. There is, however, evidence that these teams have produced qualitatively different experiences which do not translate necessarily into improved quality of life and health. Thus, evaluative research of great importance. This study aims to assess the impact of the Family Health Strategy in oral health in a longitudinal perspective natalense the years 2006 and 2009. This is an intervention study whose design is a community trial in parallel, nearly randomized. The sample consisted of census tracts covered by oral health teams in the Family Health and the Traditional Model (Basic Health Units and non-FHS Program of Community Health Agents and areas not covered.) The sample was determined by drawing ten census tracts to form the experimental group and ten other sectors for the control group by pairing intentional based on socio-economic and geographic. To check the net effect of the intervention was performed multivariate analysis by Poisson regression. As a result of cross-sectional analysis of year 2009, it was found that the effects of the ESF in Natal were satisfactory only for the variables of injuries and for other purposes without and with negative impact on stock coverage reclaimers. However, the longitudinal analysis revealed that the ESB / ESF improved their performance in dealing with grievances, access and coverage of the type of actions and this fact is independent of age, sex and social and economic conditions. In other employees' words are related to the presence of the Family Health Strategy in the region. However it does not say that both models under study (the Family Health Strategy Model and Traditional) are different in terms of performance and it is pertinent to reflect on the need for further development of evaluation studies that use other approaches able to clarify the dynamics of the process whose results can come to the knowledge of the actors responsible for leading the ESF and encourage them to incorporate the assessment in their routine

Due to the fact of oral health sector reestructuration within Brazilian public health politics, this work had the object of evaluating, under users point of view, the accessibility to oral health services in Santa Cruz (RN), focusing on the organizational aspect. To achieve this, questionnaires were given to users in their homes, considering the censitary sector. Nine sectors of the urban zone were evaluated, selected by chance, and one from the rural zone, selected by convenience. The sample was composed by 194 users, calculated considering the estimated prevalence of event represented by the indicator I ve never been to the dentist + I ve been to the dentist more than three years ago from the Projeto SB Brasil Report . To complement the results, interviews were made with others actors involved in the process of oral health care: professionals (dentists) and manager (Health Municipal Secretary). From the data obtained it was possible to identify that 12,9% of the population had never visited the dentist, and that the search for the service was not influenced by the users individual and socioeconomics characteristics, excepting the gender. It was verified that 36,1% of the users went to the dentist in less than one year, with the youngests (p<0,05) being among those who went to the dentist more frequently. 63,3% of the interviewed related that they found some kind of difficulty when they search for dentistry services, with the difficulties for schedule, the queues and the long wait among the most cited. It was identified that 43,2% of the users wait three weeks or more for the appointment. It is still pointed out that 71,4% of the interviewed find difficulties to get urgency appointment, the long wait to be attended by the dentist was the most found. 92,9% and 94,1% of the interviewed had never been headed to especialized appointments and complementary exams, respectively. Due to the data founded, it was possible to verify that the accessibility to oral health services in Santa Cruz (RN) is damaged by factors related to the organization of public polices developed, mainly in relation to the working process

O presente estudo teve como objetivo a análise das intervenções em saúde bucal, registradas em atas de reuniões, de 15 Conselhos Municipais de Saúde, próprios de municípios pertencentes à 17ª Regional de Saúde do Estado do Paraná. A análise documental deu-se a partir da identificação das temáticas em saúde, com ênfase na categorização por assunto das intervenções em saúde bucal. Os resultados evidenciaram os registros relativos à programação e organização da prestação de serviços, seguida pelo orçamento em saúde, como sendo os mais freqüentes do conjunto de temáticas analisadas. Pôde-se identificar, em 90 atas das 591 estudadas, o total de 134 registros de intervenções em saúde bucal. Por meio da análise desses últimos, percebeu-se que as intervenções em saúde bucal eram relatos de ações já concretizadas, desprovidas de características propositivas quando analisadas sob a dimensão do planejamento em saúde. Sinaliza-se para a necessidade da categoria odontológica de adquirir um maior padrão de representatividade nesses espaços, de forma a possibilitar vínculos importantes no processo de planejamento e de fortalecimento da saúde bucal enquanto direito de cidadania.

A literatura evidencia que a má oclusão pode se manifestar desde os estágios mais precoces do desenvolvimento oclusal, e, na maioria dos tipos morfológicos, não apresenta auto-correção nos estágios posteriores do desenvolvimento oclusal. Para que políticas públicas possam ser adotadas no sentido de incluir a ortodontia preventiva e interceptativa nos programa de saúde bucal, visando beneficiar principalmente a parte da população menos favorecida, este trabalho teve como objetivo avaliar a prevalência da má oclusão na dentição decídua na cidade de Belém-PA, examinando crianças de escolas públicas e privadas, verificando a influência do nível sócio-econômico na manifestação da má oclusão. Para tanto, foram examinadas 652 crianças, de ambos os sexos, de 3 a 6 anos de idade, na fase de dentição decídua completa, matriculadas em préescolas da rede privada e da rede pública do ensino de Belém- Pará. Foi observada uma prevalência de 81,44% de má oclusão, sendo mais frequente nas crianças do gênero feminino da rede privada (p=0.01). A má- oclusão de Classe II foi mais prevalente, seguida da Classe I e da Classe III. Dentre os diferentes tipos morfológicos de má oclusão, a sobremordida e a mordida cruzada posterior foram mais prevalentes nas crianças da rede privada, em ambos os gêneros, enquanto o trespasse horizontal aumentado e a classe II, apresentaram uma maior ocorrência nas crianças da rede privada no gênero feminino. A má oclusão de Classe I e III foram mais freqüentes nas crianças do gênero feminino na rede pública. A mordida aberta anterior e a mordida cruzada anterior, mostraram comportamento semelhante nos dois grupos de escola, em ambos os gêneros. A perda precoce de dentes decíduos foi mais prevalente (p=0.000) nas crianças da rede pública em ambos os gêneros. Assim, pode-se concluir que a ocorrência da má oclusão na dentição decídua na cidade de Belém é muito alta; que o nível sócio-econômico exerce influência sobre a manifestação das más oclusões, pois essas foram mais freqüente nas crianças da rede privada do gênero feminino e que a ocorrência da perda precoce de dentes decíduos nas crianças da rede pública ainda apresenta um índice alarmante, decorrente provavelmente de uma política de saúde pública ineficiente, que atue em nível preventivo e interceptativo.

The implement of Specialized’s Dentistry Centers in Brazil is, on present-day, one of the prime faces of Oral Health Brazilian Political, of which is to amplify and to qualify the specialized’s dentistry services proffer. The aim of this study, done with indirect dates, was to analyze the implantation and geographical distribution of these Centers in Brazil and try to make a relationship between this information and social and oral health factors. The results obtained showed the presence of 339 Centers already implanted in Brazil, homogeneously allocated in 283 cities, by a total of 5.560 brazilian cities, presented in the five Greater Regions. The most of the Centers were presented in large load cities. The data showed Southeast, Central West and Northeast regions with the major percentage of cities carrying Specialized’s Dentistry Centers (7,6%; 6,5%; 6,1%, respectively), while North and South regions present the minor covering taxes (4,0% and 4,5%, respectively). Relation to population’s social and buccal needs, North and Northeast Regions have the most adverse conditions, on the contrary to South, Southeast and Central West Regions who show the best one. Although the finded results were not so expressive, reflecting the brazilian geographical and populational characteristics and the essential development of specialized dentistry services, this initiative presents a real progress over the reorganization of dentistry’s practice.

AIM: The aim of this survey was to assess the prevalence of AC in riverine population in countryside of Amazonas, northern Brazil. MATERIAL AND METHODS: Patients answered a questionnaire and were examined between January and December of 2008. Data were gathered on the following participant’s characteristics: 1) age group; 2) gender; 3) ethnicity 4) outdoor activities (sunlight exposure); 5) smoking habits; 6) drinking habits; and 7) access to oral health services (the last dental visit). Clinical observation of the lips for determination of AC presence was used. Patients who presented clinical manifestation of moderate and severe AC were submitted to incisional biopsy to confirm the diagnosis. In case of a positive result after histopathological examination, patients were advised and appropriate treatment was offered. All patients received information about AC and its'prevention. RESULTS: Among the 200 participants that were examined, the prevalence of AC was 2% (4 cases). Of all patients surveyed, women were the majority totalizing 124 patients (72.0%). According to age, 48 (24.0%) people were 20g34 yeargold; 42 (21.0%) were 35g44gyeargold; 50 (25.0%) were 45g60gyeargold; and 60 (30.0%) were 61 or older. CONCLUSION: Even though AC was present in a low prevalence rate, an epidemiological variety is expected, once geographic and ethnic differences should be considered.
