Introdução: A Anorexia Nervosa (AN) é a perturbação do comportamento alimentar (PCA) com a maior taxa de mortalidade de todos os transtornos psiquiátricos. Carateriza-se pela recusa em manter um peso corporal normal mínimo, pela distorção da imagem corporal e por um obsessivo medo de ganhar peso. Os comportamentos patológicos a ela associados podem levar a uma semi-inanição que necessita de cuidados médicos pluridisciplinares, muitas vezes, em regime de internamento. Vários ensais clínicos avaliaram a eficácia da Terapia Cognitivo- Comportamental (TCC), indicando que ela favorece a remissão ou a diminuição da frequência de episódios de compulsão alimentar, dos comportamentos purgativos e da restrição alimentar. Objetivo: Combinar os resultados da melhor evidência científica de forma a avaliar a eficácia da TCC em comparação com outras terapias utilizadas no tratamento da AN. Métodos: A pesquisa realizou-se nas bases de dados eletrónicas da MEDLINE, Psyc-Info, Embase, CCTR e de forma manual, incluindo ensaios clínicos controlados randomizados que comparam a TCC com qualquer outro tipo de intervenção no tratamento da AN. Resultados: Foram incluídos 10 estudos que envolveram 957 pacientes: dos quais 571 (59,7%) foram submetidos a tratamento com Terapia cognitivo comportamental e 556 (49,3%) a outras terapias. Não se registaram diferenças significativas nos resultados obtidos em diversos outcomes, exceto nas subescalas Restrições (z=3,03; p=0,02), Preocupações alimentares (z=2,98; p=0,002) e Preocupações com a forma (z=1,71; p=0,09) do EDE e nos scores da escala GAF (z=1,87; p=0,06). Registaram-se diferenças estatisticamente significativas no número de episódios bulímicos (z=2,61; p=0,009), número de episódios de indução de vómito (z=2,11; p=0,03) e no número de episódios de uso indevido de laxantes (z=3,04; p=0,002). Conclusão: A utilização da Terapia Cognitivo-Comportamental no tratamento de doentes com AN parece melhorar bastante os sintomas da doença, revelando-se particularmente eficaz nos resultados obtidos na Eating Disorder Examination Scale. A sua utilização parece levar a uma melhoria no scores da GAF, evidenciando uma melhoria geral do estado de saúde dos pacientes (redução dos episódios de vómito, bulimia e uso de laxantes). / Página | viii ABSTRACT Background: Anorexia Nervosa is an eating disorder with the highest mortality rate of all psychiatric disorders. It is characterized by refusal to maintain a minimally normal body weight, the distortion of body image and obsessive fear of gaining weight. The pathological behaviors associated with it can lead to semi-starvation, requiring medical treatment and multidisciplinary inpatient care. Several clinical trials evaluated the efficacy of Cognitive Behavioral Therapy (CBT) in lead to remission or reduction of the frequency of bingue eating episodes, purgative behaviors and food restriction. Objective: Combining the results of the best scientific evidence to assess the efficacy of CBT in comparison with other therapies used in the treatment of AN. Methods: The research was carried out in electronic databases of MEDLINE, Psyc- Info, Embase, CCTR and manually, including randomized controlled trials that compared CBT with any other type of intervention in the treatment of AN. Results: Of which 571 (59.7%) were treated with cognitive behavioral therapy and 556 (49.3%) to other therapies: 10 studies involving 957 patients were included. No significant differences in the results obtained in different outcomes, except subscales Restrictions (z = 3.03, p = 0.02), Eating Concerns (z = 2.98, p = 0.002) and Shape Concerns (z = 1.71, p = 0.09) in the scores of EDE and the GAF scale (z = 1.87, p = 0.06). There were statistically significant differences in the number of bulimic episodes (z = 2.61, p = 0.009), number of episodes of induced vomiting (z = 2.11, p = 0.03) and the number of occurrences of use misuse of laxatives (z = 3.04, p = 0.002). Conclusion: The use of cognitive-behavioral therapy in the treatment of patients with AN seems to greatly improve the symptoms of the disease, revealing particularly effective results in the Eating Disorder Examination Scale. Its use seems to lead to an improvement in the GAF scores, showing a general improvement of the health status of patients (reduction of episodes of vomiting, bulimia and laxative use).

Os sintomas psicológicos nos indivíduos consumidores de álcool é o tema investigado neste trabalho, que se dedicou a analisar as diferenças entre grupos de indivíduos alcoólicos e não alcoólicos homens e mulheres. A oportunidade desta investigação se originou no trabalho de psicologia clínica com pacientes alcoólicos e, principalmente, frente às carências bibliográficas no mercado sobre o assunto. O presente trabalho, buscou, portanto, investigar-se, a partir das hipóteses, a intensidade de problemas psicológicos e sintomas psicopatológicos em sujeitos alcoólicos de ambos os sexos. Para tal, foram realizadas entrevistas orientadas para a aplicação do instrumento. O instrumento utilizado na pesquisa foi o SCL 90-R (Symptom Check List 90 - Revised) (DEROGATIS, 1983) que se propõe a medir a intensidade dos sintomas, especificamente em casos de alcoolismo, sendo este instrumento validado no Brasil (LALONI, 2001). Os sintomas psicológicos avaliados foram: Psicose, Relações Interpessoais, Ansiedade, Ideação Paranóide, Hostilidade, Depressão, Fobia e Obsessivo Compulsivo. A população investigada abrangeu pessoas não consumidoras de álcool da comunidade e pessoas alcoólicas pertencentes aos AA (Alcoólicos Anônimos). Os dois grupos pertencem à Região Metropolitana de Porto Alegre no Estado do Rio Grande do Sul, Brasil. A comparação foi abrangente em relação aos aspectos de gênero, pois além de comparar o grupo de alcoólicos e não alcoólicos verificou as relações de gênero, analisando as diferenças entre o grupo de homens e mulheres alcoólicos e não alcoólicos. Os resultados encontrados demonstram que os alcoólicos são mais sintomáticos que os não alcoólicos e as mulheres apresentam-se em relação aos sintomas avaliados com médias mais altas que os homens. Os alcoólicos homens e mulheres não diferem de forma estatisticamente significativa em seus sintomas psicológicos. Da mesma forma homens e mulheres não alcoólicos não diferem em seus sintomas psicológicos. / The subject of this research concerns about psychological symptoms on alcohol consuming individuals, analyzing the differences between groups of alcoholic and non alcoholic individuals, men and women. This investigation arose from the psychological clinic work with alcoholic patients in face of the lack of literature about this subject. Based on the hypothesis we investigated the intensity of psychological problems and psychopathological symptoms in alcoholic individuals of both sexes. We also evaluated the presence and intensity of psychological problems and psychopathological symptoms in non alcoholic individuals of both sexes. For this research we performed interviews oriented to the application of the instrument. The instrument employed in the research was SCL 90-R (Symptom Check List 90 - Revised) (DEROGATIS, 1983) which measures the intensity of the symptoms. The instrument was validated in Brazil (LALONI, 2001) and is specific to evaluate symptoms in case of alcoholism. The psychological symptoms we evaluated were: Psychosis, Interpersonal Relations, Anxiety, Paranoid Ideation, Hostility, Depression, Phobia, and Obsessive Compulsive. The population investigated is formed by non alcohol consuming people and alcoholics belonging to the Anonymous Alcoholics (AA). Both groups are from the Metropolitan Region of Porto Alegre in the southern sate of Rio Grande do Sul, Brazil. The comparison was comprehensive regarding the aspects of gender, since besides comparing the group of alcoholics and non alcoholics, it compared alcoholic men and women among themselves, and non alcoholic men and women as well. The results that were found demonstrate that alcoholics are more symptomatic than non alcoholics and women, regarding the evaluated symptoms, present themselves with higher averages than men. Alcoholic men and women do not expressively differ in their psychological symptoms, as non alcoholic men and women do not expressively differ in their psychological symptoms as well.

To compile data on Tourette's syndrome (TS), tics and associated disorders. METHODS: A systematic review of the literature was conducted using the 5S levels of organization of healthcare research evidence (systems, summaries, synopses, syntheses, studies), based on the model described by Haynes. The search keywords were Tourette, tics and comorbidity, which were cross-referenced. Studies provided by publishers and articles being processed on July 31, 2013, were also included. RESULTS: Of all studies retrieved during the search, 64 were selected because they analyzed the epidemiology, clinical features and etiopathogenesis of TS and its comorbidities. TS is classified as a hyperkinetic movement disorder, and at least 90% of the patients have neuropsychiatric comorbidities, of which attention deficit hyperactivity and obsessive-compulsive disorders are the most common. The syndrome is clinically heterogeneous and has been associated with a dysfunction of cortico-striatal-thalamic-cortical circuits involving various neurotransmitters. Although its genetic etiology has been widely studied, other factors may be important to understand this syndrome and its associated disorders. CONCLUSIONS: TS is a neurodevelopmental disorder that results from the impact of stress factors on a vulnerable biological substrate during the critical periods of neurodevelopment. The study of TS and its comorbidities may contribute, at different levels, to the understanding of several neuropsychiatric disorders of clinical and therapeutic relevance.

Background: Over the last few years, microdeletions of the 22q11.2 region responsible for DiGeorge syndrome, or velocardiofacial syndrome, have been increasingly related to neuropsychiatric disorders including schizophrenia and bipolar disorders. These signs seem to be related to certain genes located in the hemideleted region as the proline dehydrogenase (PRODH) and the catecholo-methyltransferase (COMT) genes. The PRODH or proline oxidase deficiency is responsible for hyperprolinemia type 1 (HPI) also causing psychiatric manifestations. Case Report: We describe a 17 year old boy with previous mild psychomotor and speech delay, mild cognitive impairment, and obsessive behaviours who started his adolescent psychiatric care presenting irritablemood and aggressive behaviour with schizophrenia symptoms that scored a “severely ill” level PANSS assessment. Symptoms got worse when he was treated with valproic acid and plasma aminoacids showing increase in alanine and proline, suggested a mitochondrial involvement of the proline metabolic pathway. Results: Mild dysmorphia suggested a possible 22q11.2 deletion genetically confirmed involving both the PRODH and COMT regions. HPI that can present with psychiatric features is however a recessive disorder and therefore the symptoms could not be solely explained by this genetic deletion. Additional investigations also showed disclosed a p.L289m (c.1865 T > A) mutation in the PRODH gene. Discussion: We believe that the association of this mutation together with the 22q11.2 deletion would lead to a decrease of functional protein. Although it may be difficult to diagnosis chromosomal abnormalities in patients with no clear malformations and mild dysmorphic features as in this patient we emphasize need to investigate the aetiology in patients with psychiatric symptoms, especially if they have other systemic manifestations such as developmental delay or psychotic symptoms, as it may be important in the management of the patients.

Luke Banas is a young video artist who lives illegally in the disused Domino sugar refinery in Williamsburg, Brooklyn. While his art is an attempt to fully record and share his own life story, developers want to tear down the building where he works; a building that’s a monument to his hip neighborhood’s industrial past. The novel’s narrator, Lila Fairfax, is a journalist writing her first feature article about Luke and the fate of the factory. Observant and astute, she soon realizes that, despite his obsessive self-revelation, Luke is hiding a secret. Lila’s rational, detached approach to life is disrupted as, in the course of her reporting, she falls in love with Luke and as a result, learns far more than she anticipated. Though primarily a love story, The Sugar Factory is also an investigation of art, and art’s interaction with commerce, history, and new technology.

The aim of this paper was to obtain evidence of the validity of the LSB-50 (de Rivera & Abuín, 2012), a screening measure of psychopathology, in Argentinean adolescents. The sample consisted of 1002 individuals (49.7% male; 50.3% female) between 12 and 18 years-old (M = 14.98; SD = 1.99). A cross-validation study and factorial invariance studies were performed in samples divided by sex and age to test if a seven-factor structure that corresponds to seven clinical scales (Hypersensitivity, Obsessive-Compulsive, Anxiety, Hostility, Somatization, Depression, and Sleep disturbance) was adequate for the LSB-50. The seven-factor structure proved to be suitable for all the subsamples. Next, the fit of the seven-factor structure was studied simultaneously? in the aforementioned subsamples through hierarchical models that imposed different constrains of equivalency?. Results indicated the invariance of the seven clinical dimensions of the LSB-50. Ordinal alphas showed good internal consistency for all the scales. Finally, the correlations with a diagnostic measure of psychopathology (PAI-A) indicated moderate convergence. It is concluded that the analyses performed provide robust evidence of construct validity for the LSB-50

O presente relatório apresenta e descreve o percurso efetuado nas atividades de estágio no âmbito do 2º ano do Mestrado em Reabilitação Psicomotora na Faculdade de Motricidade Humana – Universidade de Lisboa. O relatório de estágio que se apresenta envolve uma contextualização das perturbações observadas bem como a descrição da intervenção psicomotora efetuada no Fórum Sócio Ocupacional do Grupo de Ação Comunitária, no âmbito da Saúde Mental Comunitária. As atividades desenvolvidas visaram estimular as capacidades de planeamento, avaliação e intervenção psicomotoras por parte da aluna, no contexto de intervenção onde se inseria, visando também a coordenação do seu trabalho com outros profissionais e com a comunidade. Ao longo do estágio foram desenvolvidas sessões de Psicomotricidade em grupo e individuais, bem como outras atividades relacionadas com a psicomotricidade fora das sessões. Durante o estágio foram ainda realizados dois estudos de caso e elaboradas sessões tendo em vista avaliações inicial e final efetuadas. Conclui-se no final deste relatório que a intervenção em Psicomotricidade em saúde mental em contexto comunitário é pertinente tendo em conta os resultados da avaliação efetuada.

The literature concerning obsessive-compulsive disorder (OCD) indicates that obsessions frequently imply negative evaluative beliefs regarding the self. The construct of the feared self has been used to describe the set of harmful attributes an individual worries they may possess. This study aimed to partially replicate previous research that demonstrated a relationship between feared-self beliefs and obsessional doubt in OCD-relevant contexts. The relationship between perceptions of personal responsibility and associated levels of doubt was also examined. Nonclinical participants (N = 221; 155 female; Mage = 26.4, SD = 9.2) were presented with vignettes related to checking and non OCD-relevant themes, which quantified doubt through the presentation of alternating reality-based (i.e., sensory) and possibility-based information. Of the total sample, 112 participants were randomly allocated to a personally relevant condition (in which the action implied in the vignettes was completed by the reader), and 109 were allocated to a second, other-relevant, condition (in which the action implied in the vignettes was completed by a proximal other). The results provided support for reasoning processes implicated in OCD, suggesting that feared-self beliefs may partially contribute to heightened levels of doubt in response to possibility vs. reality-based information in OCD-relevant contexts. Personal relevance contributed to greater baseline levels of doubt, but not to greater responses to the reality- and possibility-based statements accompanying the OCD-relevant vignette. Implications for theory and future research are discussed.

BACKGROUND: Multi-site repetitive transcranial magnetic stimulation (rTMS) has been applied experimentally in the treatment of obsessive compulsive disorder (OCD). NEW METHOD: This study was conducted to systematically evaluate the safety, tolerability and neurocognitive effects of rTMS applied to three cortical regions over a period of three months. NEW METHOD: Twenty healthy participants aged 22-33 years were randomly allocated to receive one session of active or sham stimulation of low and high frequency rTMS applied sequentially to the pre-supplementary motor area, right-dorsolateral prefrontal cortex and left-orbitofrontal cortex totalling 9min. Tolerability and safety was evaluated using a standardised safety questionnaire. Neurocognitive functioning was examined using the Cambridge Neuropsychological Test Automated Battery and measures of verbal fluency from the Delis-Kaplan Executive Functioning Test™ at five time points over three months. RESULTS: The protocol was safe and tolerable. Frequencies of minor adverse effects were higher in active (17 endorsements) than sham (1 endorsement) conditions. No between group differences in neurocognitive functioning were identified over three months. COMPARISON WITH EXISTING METHOD: This study is the first to evaluate the feasibility of low and high frequency parameters applied sequentially in a single session to the three selected cortical regions whilst providing neurocognitive data. CONCLUSIONS: rTMS applied sequentially over three cortical regions was found to be safe and tolerable in healthy individuals with no major neurocognitive effects over three months. Such findings can be used to inform the development of rTMS protocols involving multi-site stimulation for OCD.

The aim of this study was to systematically review and meta-analyze the prevalence of comorbid personality disorders among treatment-seeking problem gamblers. Almost one half (47.9%) of problem gamblers displayed comorbid personality disorders. They were most likely to display Cluster B disorders (17.6%), with smaller proportions reporting Cluster C disorders (12.6%) and Cluster A disorders (6.1%). The most prevalent personality disorders were narcissistic (16.6%), antisocial (14.0%), avoidant (13.4%), obsessive-compulsive (13.4%), and borderline (13.1%) personality disorders. Sensitivity analyses suggested that these prevalence estimates were robust to the inclusion of clinical trials and self-selected samples. Although there was significant variability in reported rates, subgroup analyses revealed no significant differences in estimates of antisocial personality disorder according to problem gambling severity, measure of comorbidity employed, and study jurisdiction. The findings highlight the need for gambling treatment services to conduct routine screening and assessment of co-occurring personality disorders and to provide treatment approaches that adequately address these comorbid conditions.

BACKGROUND: Cryptomarkets are digital platforms that use anonymising software (e.g. Tor) and cryptocurrencies (e.g. Bitcoin) to facilitate peer-to-peer (P2P) trade of goods and services. Their emergence has facilitated access to a wide range of high-quality psychoactive substances, according to surveys of users. In this paper, we ask the question 'How does changing access to drugs through cryptomarkets affect the drug use and harm trajectories of their users?'

METHODS: We conducted a digital ethnography spanning 2012-2014, a period that included the seizure of the original Silk Road marketplace and forum by law enforcement. Using encrypted online chat, we interviewed 17 people who reported using Silk Road to purchase illicit drugs. The interviews were in-depth and unstructured, and also involved the use of life history timelines to trace trajectories. Transcripts were analysed thematically using NVivo.

RESULTS: For some, Silk Road facilitated initiation into drug use or a return to drug use after cessation. Typically, participants reported experiencing a glut of drug consumption in their first months using Silk Road, described by one participant as akin to 'kids in a candy store'. There was evidence that very high availability reduced the need for drug hoarding which helped some respondents to moderate use and feel more in control of purchases made online. Cryptomarket access also appeared to affect solitary and social drug users differently. Most participants described using other cryptomarkets after the closure of Silk Road, albeit with less confidence.

CONCLUSION: In the context of high levels of drug access, supply and diversity occurring within a community regulated environment online, the impacts of cryptomarkets upon drug use trajectories are complex, often posing new challenges for self-control, yet not always leading to harmful outcomes. A major policy challenge is how to provide support for harm reduction in these highly volatile settings.

OBJECTIVE: We aimed to describe the prevalence and age distribution of personality disorders and their comorbidity with other psychiatric disorders in an age-stratified sample of Australian women aged ⩾25 years. METHODS: Individual personality disorders (paranoid, schizoid, schizotypal, histrionic, narcissistic, borderline, antisocial, avoidant, dependent, obsessive-compulsive), lifetime mood, anxiety, eating and substance misuse disorders were diagnosed utilising validated semi-structured clinical interviews (Structured Clinical Interview for DSM-IV-TR Axis I Disorders, Research Version, Non-patient Edition and Structured Clinical Interview for DSM-IV Axis II Personality Disorders). The prevalence of personality disorders and Clusters were determined from the study population (n = 768), and standardised to the Australian population using the 2011 Australian Bureau of Statistics census data. Prevalence by age and the association with mood, anxiety, eating and substance misuse disorders was also examined. RESULTS: The overall prevalence of personality disorders in women was 21.8% (95% confidence interval [CI]: 18.7, 24.9). Cluster C personality disorders (17.5%, 95% CI: 16.0, 18.9) were more common than Cluster A (5.3%, 95% CI: 3.5, 7.0) and Cluster B personality disorders (3.2%, 95% CI: 1.8, 4.6). Of the individual personality disorders, obsessive-compulsive (10.3%, 95% CI: 8.0, 12.6), avoidant (9.3%, 95% CI: 7.1, 11.5), paranoid (3.9%, 95% CI: 3.1, 4.7) and borderline (2.7%, 95% CI: 1.4, 4.0) were among the most prevalent. The prevalence of other personality disorders was low (⩽1.7%). Being younger (25-34 years) was predictive of having any personality disorder (odds ratio: 2.36, 95% CI: 1.18, 4.74), as was being middle-aged (odds ratio: 2.41, 95% CI: 1.23, 4.72). Among the strongest predictors of having any personality disorder was having a lifetime history of psychiatric disorders (odds ratio: 4.29, 95% CI: 2.90, 6.33). Mood and anxiety disorders were the most common comorbid lifetime psychiatric disorders. CONCLUSIONS: Approximately one in five women was identified with a personality disorder, emphasising that personality disorders are relatively common in the population. A more thorough understanding of the distribution of personality disorders and psychiatric comorbidity in the general population is crucial to assist allocation of health care resources to individuals living with these disorders.
