Blastic plasmacytoid dendritic cell neoplasm is a rare and aggressive hematodermic neoplasia with frequent cutaneous involvement and leukemic dissemination. We report the case of a 76-year-old man with a 2 month history of violaceous nodules and a tumor with stony consistency, located on the head, and mandibular, cervical and supraclavicular lymphadenopathies. Multiple thoracic and abdominal adenopathies were identified on computerized tomography. Flow cytometry analysis of the skin, lymph node and bone marrow biopsies demonstrated the presence of plasmocytoid dendritic cell neoplastic precursor cells (CD4+, CD45+, CD56+ and CD123+ phenotype). After initial clinical and laboratorial complete remission with chemotherapy, the patient died due to relapse of the disease associated with the appearance of a cervical mass with medullary compromise.

The hypoxia inducible factor 1 alpha (HIF1a) is a key regulator of tumour cell response to hypoxia, orchestrating mechanisms known to be involved in cancer aggressiveness and metastatic behaviour. In this study we sought to evaluate the association of a functional genetic polymorphism in HIF1A with overall and metastatic prostate cancer (PCa) risk and with response to androgen deprivation therapy (ADT). The HIF1A +1772 C>T (rs11549465) polymorphism was genotyped, using DNA isolated from peripheral blood, in 1490 male subjects (754 with prostate cancer and 736 controls cancer-free) through Real-Time PCR. A nested group of cancer patients who were eligible for androgen deprivation therapy was followed up. Univariate and multivariate models were used to analyse the response to hormonal treatment and the risk for developing distant metastasis. Age-adjusted odds ratios were calculated to evaluate prostate cancer risk. Our results showed that patients under ADT carrying the HIF1A +1772 T-allele have increased risk for developing distant metastasis (OR, 2.0; 95%CI, 1.1-3.9) and an independent 6-fold increased risk for resistance to ADT after multivariate analysis (OR, 6.0; 95%CI, 2.2-16.8). This polymorphism was not associated with increased risk for being diagnosed with prostate cancer (OR, 0.9; 95%CI, 0.7-1.2). The HIF1A +1772 genetic polymorphism predicts a more aggressive prostate cancer behaviour, supporting the involvement of HIF1a in prostate cancer biological progression and ADT resistance. Molecular profiles using hypoxia markers may help predict clinically relevant prostate cancer and response to ADT.

BACKGROUND: Wireless capsule endoscopy has been introduced as an innovative, non-invasive diagnostic technique for evaluation of the gastrointestinal tract, reaching places where conventional endoscopy is unable to. However, the output of this technique is an 8 hours video, whose analysis by the expert physician is very time consuming. Thus, a computer assisted diagnosis tool to help the physicians to evaluate CE exams faster and more accurately is an important technical challenge and an excellent economical opportunity. METHOD: The set of features proposed in this paper to code textural information is based on statistical modeling of second order textural measures extracted from co-occurrence matrices. To cope with both joint and marginal non-Gaussianity of second order textural measures, higher order moments are used. These statistical moments are taken from the two-dimensional color-scale feature space, where two different scales are considered. Second and higher order moments of textural measures are computed from the co-occurrence matrices computed from images synthesized by the inverse wavelet transform of the wavelet transform containing only the selected scales for the three color channels. The dimensionality of the data is reduced by using Principal Component Analysis. RESULTS: The proposed textural features are then used as the input of a classifier based on artificial neural networks. Classification performances of 93.1% specificity and 93.9% sensitivity are achieved on real data. These promising results open the path towards a deeper study regarding the applicability of this algorithm in computer aided diagnosis systems to assist physicians in their clinical practice.

Little is known of cancer rehabilitation needs in Europe. EUROCHIP-3 organised a group of experts to propose a list of population-based indicators used for describing cancer rehabilitation across Europe. The aim of this study is to present and discuss these indicators. A EUROCHIP-3 expert panel reached agreement on two types of indicators. (a) Cancer prevalence indicators. These were proposed as a means of characterising the burden of cancer rehabilitation needs by time from diagnosis and patient health status. These indicators can be estimated from cancer registry data or by collecting data on follow-up and treatments for samples of cases archived in cancer registries. (b) Indicators of rehabilitation success. These include: return to work, quality of life, and satisfaction of specific rehabilitation needs. Studies can be performed to estimate these indicators in individual countries, but to obtain comparable data across European countries it will be necessary to administer a questionnaire to randomly selected samples of patients from population-based cancer registry databases. However, three factors complicate questionnaire studies: patients may not be aware that they have cancer; incomplete participation in surveys could lead to bias; and national confidentiality laws in some cases prohibit cancer registries from approaching patients. Although these studies are expensive and difficult to perform, but as the number of cancer survivors increases, it is important to document their needs in order to provide information on cancer control.

The aims of this retrospective review were to determine the frequency of malignant endometrial polyps diagnosed with ambulatory hysteroscopy in the Obstetrics/Gynaecology Department of HDE, Lisbon, between January 2001 and December 2005 and to characterize these cases according to risk factors, sonographic and endoscopic findings tumoral histology, and tumor stage.We found seven cases of malignant endometrial polyps in a total of 1333 polyps initially diagnosed: an incidence rate of 0. 53%. These seven patients had a mean age of 68 years (55–82 years), and all were postmenopausal, with five having one risk factor each for endometrial cancer. Metrorrhagia was present in six of the seven patients(85.7%). Ultrasonography was abnormal in all seven patients, with a mean endometrial thickness of 26 mm(range: 12–44 mm). The hysteroscopy images suggested malignancy in all cases. All except one patient had a single polyp. The polyps had volumes between 1.5 and 3 cm; two were removed completely and five were biopsed. The histological subtype was: mixed endometrioid/serous papillary or clear cell (2), adenocarcinoma with squamous differentiation (2), carcinosarcoma (2), and clear cell carcinoma (1). Malignancy inside polyps is rare, but diagnostic hysteroscopy with visual guided biopsies can identify these cases in the earlier stages. The risk factors are not different from those of other endometrial carcinomas, but the histological subtype seems to point to more aggressive cancers.

The aim of our study was to access office hysteroscopy results in postmenopausal patients with thickened endometrium. A retrospective descriptive study was carried out on 245 postmenopausal patients submitted to office hysteroscopy after sonographic diagnosis of thickened endometriumin 20 consecutive months.Women were evaluated for age, hormonal therapy, hysteroscopic findings, procedure duration, complications and associated pain, and histological diagnosis. Patients with and without uterine bleeding were considered separately. Symptomatic patients were older and had longer procedure duration. The most frequent hysteroscopic finding was endometrial polyp in both groups. Pain was subjectively assessed in a numeric scale from 0 to 10 and median value was 4. There were no complications reported. Global neoplasia rate was 2.9% for asymptomatic patients and 16.4% for symptomatic ones (p<0.05). Thickened endometrium with postmenopausal metrorrhagia gave patients a significantly higher risk for neoplasia and hyperplasia.

A neurofibromatose é uma doença multissistémica na qual as possíveis múltiplas manifestações clínicas podem ter importantes implicações diagnósticas e prognósticas: se coexiste hipertensão arterial, vários mecanismos patogénicos justificam que se considere a possibilidade da etiologia secundária. Neste trabalho, revemos mecanismos patogénicos e manifestações clínicas, e apontamos estratégias diagnósticas para anomalias e doenças, que embora presentes, podem estar ocultas.

A 34-year-old woman with no known medical history was evaluated for multiple painful brown nodules and papules on the anterior aspect of the trunk. She mentioned a history of similar cutaneous findings on her mother. Biopsies of three lesions revealed piloleiomyomata. Renal and adrenal ultrasound revealed an isolated simple cortical cyst, and pelvic and endovaginal ultrasound revealed two uterine myomata. The clinical diagnosis of hereditary leiomyomatosis and renal cell cancer was corroborated by the identification of a heterozygous variant on exon 5 of the fumarate hydratase gene (c.578C>T p.T193I). Identification of the tumor piloleiomyoma should alert the dermatologist to this rare genodermatosis, which is associated with an increased risk of renal cell tumors, demanding multidisciplinary follow-up, and personal and family counseling.

Os autores apresentam três casos de mielite transversa de instalação aguda em doentes jovens, salientando a possível gravidade do quadro neurológico, a necessidade de excluir uma causa potencialmente tratável e a controvérsia da terapêutica com corticosteroides.

Apresenta-se um caso de linfoma não-Hodgkin primário do duodeno, classificado como linfoma B do tubo digestivo de baixo grau de malignidade. Foi demonstrado por ecografia, tomografia computorizada e estudo baritado intestinal. Discutem-se as formas de apresentação e localização dos linfomas não-Hodgkin primários gastro-intestinais, assim como o contributo dos métodos de imagem na sua caracterização, estadiamento e diagnós tico diferencial.

O aumento da incidência do linfoma cerebral primitivo parece ser real em pessoas imunologicamente normais. Contudo, os linfomas do sistema nervoso central têm uma muito maior incidência em doentes imunodeprimidos tais como os submetidos a transplantes cardíacos ou renais, doentes com deficiência de IgA ou síndrome de Wiskott-Aldrich. Assim, não é surpreendente o aumento da predisposição dos doentes com a síndrome de imunodeficiência adquirida para os linfomas do sistema nervoso central, sendo esta a segunda causa mais frequente de lesão ocupando espaço intracerebral nesta população. Os autores descrevem o caso de uma mulher de 36 anos com síndrome de imunodeficiência adquirida e uma lesão ocupando espaço intracerebral que demonstraram tratar-se de um linfoma cerebral.

Os tumores primitivos do baço são extremamente raros. Descrevemos o 1° caso dos Hospitais Civis de Lisboa, em S. José, numa mulher de 70 anos que apresentava espleno egália. Havia história anterior de irradiação pélvica. A ressonância magnética esclareceu a presença de massa tumoral e a angiografia caracterizou-a como vascular. A doente está bem um ano após esplenectomia. Encontrámos na literatura a descrição de 61 casos deste tumor de etiopatogenia não esclarecida. A esplenectomia precoce deve ser a primeira abordagem terapêutica pelo risco de ruptura. Esta foi a forma de apresentação em 34° o dos casos da série de Autry et al. Por se tratar de um tumor tão raro, não há estudos sobre a eficácia da quimioterapia, mas nas formas disseminadas podem utilizar-se esquemas de tratamentos citostáticos activos em sarcomas de partes moles.

Os autores apresentam o caso clínico de uma mulher de 51 anos, internada por Hipertensão Arterial (H.T.A.) grave, com paroxismos, não controlável médicamente. A investigação clínico laboratorial-Imagiológica, permitiu detectar um feocromocitoma da glândula supra-renal direita, que foi excisado cirurgicamente com êxito. Discutem em seguida o Diagnóstico,diferencial, e terminam com uma breve revisão da literatura dos aspectos clínicos, diagnósticos, preparação préoperatória e de técnica cirúrgica mais relevantes.
