Previous research has demonstrated covariation of physiological responding with judgments of valence and arousal. However, until now links between these affective dimensions and respiratory measures have not been extensively investigated. In this study, eight picture series of different affective valence and arousal level were shown to 30 subjects, while respiration, skin conductance level (SCL), heart rate (HR) and affective judgments were measured. With increasing pleasantness, inspiratory time lengthened, mean inspiratory flow decreased and thoracic breathing increased. With increasing arousal, inspiratory time and total breath duration shortened and mean inspiratory flow, minute ventilation, thoracic breathing and electrodermal activity increased. These findings confirm the importance of arousal in respiratory responding, but also indicate a modulatory role of affective valence.We propose that the arousal effects reflect energy mobilization in preparation to act, and thatthe valence effects might be a manifestation of an attention bias toward negative stimuli. The Western Alpine Are has been created during the Cretaceous and the Tertiary orogenies. The interference patterns of the Tertiary structures suggest their formation during continental collision of the European and the Adriatic Plates, with an accompanying anticlockwise rotation of the Adriatic indenter. Extensional structures are mainly related to ductile deformation by simple shear. These structures developed at a deep tectonic level, in granitic crustal rocks, at depths in excess of 10 km. In the early Palaeogene period of the Tertiary Orogeny, the main Tertiary nappe emplacement resulted from a NW-thrusting of the Austroalpine, Penninic and Helvetic nappes. Heating of the deep zone of the Upper Cretaceous and Tertiary nappe stack by geothermal heat flow is responsible for the Tertiary regional metamorphism, reaching amphibolite-facies conditions in the Lepontine Gneiss Dome (geothermal gradient 25 degrees C/ km). The Tertiary thrusting occurred mainly during prograde metamorphic conditions with creation of a penetrative NW-SE-oriented stretching lineation, X(1) (finite extension), parallel to the direction of simple shear. Earliest cooling after the culmination of the Tertiary metamorphism, some 38 Ma ago, is recorded by the cooling curves of the Monte Rosa and Mischabel nappes to the west and the Suretta Nappe to the east of the Lepontine Gneiss Dome. The onset of dextral transpression, with a strong extension parallel to the mountain belt, and the oldest S-vergent `'backfolding'' took place some 35 to 30 Ma ago during retrograde amphibolite-facies conditions and before the intrusion of the Oligocene dikes north of the Periadriatic Line. The main updoming of the Lepontine Gneiss Dome started some 32-30 Ma ago with the intrusion of the Bergell tonalites and granodiorites, concomitant with S-vergent backfolding and backthrusting and dextral strike-slip movements along the Tonale and Canavese Lines (Argand's Insubric phase). Subsequently, the center of main updoming migrated slowly to the west, reaching the Simplon region some 20 Ma ago. This was contemporaneous with the westward migration of the Adriatic indenter. Between 20 Ma and the present, the Western Aar Massif-Toce culmination was the center of strong uplift. The youngest S-vergent backfolds, the Glishorn anticline and the Berisal syncline fold the 12 Ma Rb/Sr biotite isochron and are cut by the 11 Ma old Rhone-Simplon Line. The discrete Rhone-Simplon Line represents a late retrograde manifestation in the preexisting ductile Simplon Shear Zone. This fault zone is still active today. The Oligocene-Neogene dextral transpression and extension in the Simplon area were concurrent with thrusting to the northwest of the Helvetic nappes, the Prealpes (35-15 Ma) and with the Jura thin-skinned thrust (11-3 Ma). It was also contemporaneous with thrusting to the south of the Bergamasc (> 35-5 Ma) and Milan thrusts (16-5 Ma).

A 49-year-old woman, without known cardiovascular risk factors. Hoarseness of voice caused by a paralysis of left vocal cord. She was admitted to hospital because of acute coronary syndrome, associated to resuscitated cardiac arrest (asystolia documented) without later neurology sequels. Physical examination was anodyne. Echocardiographic study demonstrated a compatible image with a large left sinus of Valsalva aneurysm (SVA) (Panel A) and mild aortic regurgitation. Cardiac catheterization confirmed the presence of left SVA (Panel B) that produced extrinsic compression of the left main coronary artery (Panels C and D). Repair surgery was made by means of closing the aneurysmal orifice with a patch of dacron. Intra-operatory echocardiographic control study found severe aortic regurgitation, so valvular replacement with 19 mm mechanical prosthesis and extension of the valve annulus with patch of dacron was performed, associated with bypass with safena vein graft to left coronary artery. SVA is a very infrequent cardiac anomaly, generally with silent clinical course until it ruptures. Myocardial ischaemia caused by coronary artery compression is unusual. We described the case of a patient diagnosed of left SVA, whose initial clinical manifestation was the appearance of resuscitated sudden cardiac death in the context of an acute coronary syndrome.

BACKGROUND While pain is frequently associated with unipolar depression, few studies have investigated the link between pain and bipolar depression. In the present study we estimated the prevalence and characteristics of pain among patients with bipolar depression treated by psychiatrists in their regular clinical practice. The study was designed to identify factors associated with the manifestation of pain in these patients. METHODS Patients diagnosed with bipolar disorder (n=121) were selected to participate in a cross-sectional study in which DSM-IV-TR criteria were employed to identify depressive episodes. The patients were asked to describe any pain experienced during the study, and in the 6 weeks beforehand, by means of a Visual Analogical Scale (VAS). RESULTS Over half of the bipolar depressed patients (51.2%, 95% CI: 41.9%-60.6%), and 2/3 of the female experienced concomitant pain. The pain was of moderate to severe intensity and prolonged duration, and it occurred at multiple sites, significantly limiting the patient's everyday activities. The most important factors associated with the presence of pain were older age, sleep disorders and delayed diagnosis of bipolar disorder. CONCLUSIONS Chronic pain is common in bipolar depressed patients, and it is related to sleep disorders and delayed diagnosis of their disorder. More attention should be paid to study the presence of pain in bipolar depressed patients, in order to achieve more accurate diagnoses and to provide better treatment options.

Obscure gastrointestinal bleeding is still a clinical challenge for gastroenterologists. The recent development of novel technologies for the diagnosis and treatment of different bleeding causes has allowed a better management of patients, but it also determines the need of a deeper comprehension of pathophysiology and the analysis of local expertise in order to develop a rational management algorithm. Obscure gastrointestinal bleeding can be divided in occult, when a positive occult blood fecal test is the main manifestation, and overt, when external sings of bleeding are visible. In this paper we are going to focus on overt gastrointestinal bleeding, describing the physiopathology of the most usual causes, analyzing the diagnostic procedures available, from the most classical to the novel ones, and establishing a standard algorithm which can be adapted depending on the local expertise or availability. Finally, we will review the main therapeutic options for this complex and not so uncommon clinical problem.

INTRODUCTION: Fabry disease is an X-linked recessive abnormality of glycosphingolipid metabolism that is due to deficiency of the lysosomal enzyme alpha-galactosidase A. CURRENT KNOWLEDGE AND KEY POINTS: A majority of hemizygous men develop severe multisystemic disease (classic form), dominated by renal failure, progressive neurological and cardiac involvement. Nevertheless, some affected men retain sufficient enzyme activity and long remain asymptomatic (atypical form); their main manifestation is hypertrophic cardiomyopathy. Female heterozygous carriers are usually asymptomatic; 15% of them, however, have severe involvement of one or several organs. Laboratory, histologic and molecular diagnosis identifies 100% of hemizygous and over 80% of heterozygous subjects. FUTURE PROSPECTS AND PROJECTS: With developments in molecular genetics, it is now possible to produce the human recombinant enzyme alpha-galactosidase A. Two recent studies had proven that this therapeutic approach was able to be clinically and histologically effective in men. In addition, the results of a trial of gene therapy in a Fabry gene knocked-out mouse appear promising.

OBJECTIVE: Connective tissue diseases (CTDs) are associated with several interstitial lung diseases. The aim of this study was to describe the recently individualized syndrome of combined pulmonary fibrosis and emphysema (CPFE) in a population of patients with CTD. METHODS: In this multicenter study, we retrospectively investigated data from patients with CTD who also have CPFE. The demographic characteristics of the patients, the results of pulmonary function testing, high-resolution computed tomography, lung biopsy, and treatment, and the outcomes of the patients were analyzed. RESULTS: Data from 34 patients with CTD who were followed up for a mean±SD duration of 8.3±7.0 years were analyzed. Eighteen of the patients had rheumatoid arthritis (RA), 10 had systemic sclerosis (SSc), 4 had mixed or overlap CTD, and 2 had other CTDs. The mean±SD age of the patients was 57±11 years, 23 were men, and 30 were current or former smokers. High-resolution computed tomography revealed emphysema of the upper lung zones and pulmonary fibrosis of the lower zones in all patients, and all patients exhibited dyspnea during exercise. Moderately impaired pulmonary function test results and markedly reduced carbon monoxide transfer capacity were observed. Five patients with SSc exhibited pulmonary hypertension. Four patients died during followup. Patients with CTD and CPFE were significantly younger than an historical control group of patients with idiopathic CPFE and more frequently were female. In addition, patients with CTD and CPFE had higher lung volumes, lower diffusion capacity, higher pulmonary pressures, and more frequently were male than those with CTD and lung fibrosis without emphysema. CONCLUSION: CPFE warrants inclusion as a novel, distinct pulmonary manifestation within the spectrum of CTD-associated lung diseases in smokers or former smokers, especially in patients with RA or SSc.

Ce travail s'appuie sur une recherche qualitative s'articulant autour d'études de cas. Il s'articule autour de travaux de psychologie clinique d'orientation psychanalytique européens et de travaux de criminologie clinique belges et nord-américains. Il vise à interroger les facteurs de l'engagement, du renforcement et du renoncement aux agirs transgressifs à la période de latence d'une part puis au cours du processus adolescent d'autre part. La proposition du terme agir transgressif permettra d'explorer les contours de la problématique des pathologies de l'agir dans les registres psychodynamique, intrapsychique et intersubjectif, compréhension psychodynamique de la place qu'occupent ces agirs transgressifs dans le processus adolescent. Le temps du renoncement n'a, à notre connaissance, jamais été investigué dans le temps de l'adolescence et à deux reprises en psychologie clinique - psychocriminologie - (Born, Boët, 2001 ; Villerbu, Winter, Laurent, 2012). Le terme de desistance apparaît en 1934 aux Etats-Unis. Il est peu utilisé et est repris à partir de 1982 et 1983, respectivement par Meisenhelder et Shover. Feld et Strauss (1989) et plus récemment Laroche (2003) utilisent le terme de desistance pour désigner l'absence de nouvelle manifestation de violence psychologique ou physique d'un des deux partenaires d'un couple (cas des violences conjugales) sur une durée d'au moins un an. Ce travail de recherche a donc pour ambition d'enrichir une quasi-absence de recherches en psychologie clinique dans ce domaine.

Background: Glutathione (GSH), a major cellular redox regulator and antioxidant, is decreased in cerebrospinal fluid and prefrontal cortex of schizophrenia patients. The gene of the key GSH-synthesizing enzyme, glutamate-cysteine ligase, modifier (GCLM) subunit, is associated with schizophrenia, suggesting that the deficit in the GSH system is of genetic origin. Using the GCLM knock-out (KO) mouse as model system with 60% decreased brain GSH levels and, thus, strong vulnerability to oxidative stress, we have shown that GSH dysregulation results in abnormal mouse brain morphology (e.g., reduced parvalbumin, PV, immuno-reactivity in frontal areas) and function. Additional oxidative stress, induced by GBR12909 (a dopamine re-uptake inhibitor), enhances morphological changes even further. Aim: In the present study we use the GCLM KO mouse model system, asking now, whether GSH dysregulation also compromises mouse behaviour and cognition. Methods: Male and female wildtype (WT) and GCLM-KO mice are treated with GBR12909 or phosphate buffered saline (PBS) from postnatal day (P) 5 to 10, and are behaviourally tested at P 60 and older. Results: In comparison to WT, KO animals of both sexes are hyperactive in the open field, display more frequent open arm entries on the elevated plus maze, longer float latencies in the Porsolt swim test, and more frequent contacts of novel and familiar objects. Contrary to other reports of animal models with reduced PV immuno-reactivity, GCLM-KO mice display normal rule learning capacity and perform normally on a spatial recognition task. GCLM-KO mice do, however, show a strong deficit in object-recognition after a 15 minutes retention delay. GBR12909 treatment exerts no additional effect. Conclusions: The results suggest that animals with impaired regulation of brain oxidative stress are impulsive and have reduced behavioural control in novel, unpredictable contexts. Moreover, GSH dysregulation seems to induce a selective attentional or stimulus-encoding deficit: despite intensive object exploration, GCLM-KO mice cannot discriminate between novel and familiar objects. In conclusion, the present data indicate that GSH dysregulation may contribute to the manifestation of behavioural and cognitive anomalies that are associated with schizophrenia.

Over the past few years, the control of pain exerted by glial cells has emerged as a promising target against pathological pain. Indeed, changes in glial phenotypes have been reported throughout the entire nociceptive pathway, from peripheral nerves to higher integrative brain regions, and pharmacological inhibition of such glial reactions reduces the manifestation of pain in animal models. This complex interplay between glia and neurons relies on various mechanisms depending both on glial cell types considered (astrocytes, microglia, satellite cells, or Schwann cells), the anatomical location of the regulatory process (peripheral nerve, spinal cord, or brain), and the nature of the chronic pain paradigm. Intracellularly, recent advances have pointed to the activation of specific cascades, such as mitogen-associated protein kinases (MAPKs) in the underlying processes behind glial activation. In addition, given the large number of functions accomplished by glial cells, various mechanisms might sensitize nociceptive neurons including a release of pronociceptive cytokines and neurotrophins or changes in neurotransmitter-scavenging capacity. The authors review the conceptual advances made in the recent years about the implication of central and peripheral glia in animal models of chronic pain and discuss the possibility to translate it into human therapies in the future.

Anaphylactic shock is an unexpected, sudden and sometimes deadly event that attacks the patient in 75% of the cases without pre-existent history of allergy. In general, drugs, hymenopteric poisons and nutrients (according to the recent concept) are responsible. Beside the classical IgE-mediated anaphylactic shock there exists another anaphylactic shock, identical in its clinical picture and treatment but not in the mechanism of development. Epinephrine is the only effective drug in case of respiratory (bronchial asthma, laryngeal edema) or cardiovascular (hypotension, arrhythmias, hypovolemic shock) manifestation. It has to be administered as rapidly as possible

RESUME En faisant référence à la notion de préjugé idéologique, ce travail s'intéresse à la manifestation d'une croyance qui oppose la culture à la nature lors de la classification et l'évaluation des individus. Nous proposons que cette croyance se manifeste par l'attribution de traits spécifiques aux groupes (traits culturels et naturels) et que sa fonction est de justifier la suprématie de l'homme bourgeois blanc occidental sur autrui. Ainsi, nous abordons la perception de plusieurs groupes ethniques de la part d'individus suisses. Notre travail est organisé en trois parties. La première partie présente une étude exploratoire dont l'objectif est de cerner les phénomènes étudiés. Les résultats mettent en évidence que l'attribution de traits culturels .positifs aux groupes ethniques est relativement indépendante de l'attribution de traits naturels positifs àceux-ci: les groupes perçus comme les plus culturels sont également perçus comme les plus naturels. De plus, l'attribution de traits naturels positifs semble sous-tendre une attitude favorable envers les groupes. La deuxième partie reprend les critères qu'identifient les notions de culture et de nature. Les études 2, 3 et 4 ont mis en évidence qu'il y au continuum dans la signification des traits par rapport à .l'être humain et à l'animal. Cela nous a amené sélectionner des traits attribués uniquement à l' être humain (culture) et des traits attribués davantage à l' animal qu'à l'être humain (nature). Les études 5 et 6 de la troisième partie montrent que, lorsqu'il est question de groupes ethniques, l'endogroupe dominant et ses alliés sont associés à la culture positive, alors que des exogroupes spécifiques sont associés à la nature positive (des exogroupes sujets au paternalisme). L'étude 7 confirme les résultats concernant l'endogroupe dominant et ses alliés avec des groupes fictifs et il met en évidence que les membres du groupe dominant utilisent la notion de culture positive pour hiérarchiser les groupes. L'attribution de nature positive n'est pas prise en compte pour hiérarchiser des groupes fictifs. Pour conclure, les études montrent qu'il n'y a pas d'opposition entre la culture et la nature (positives): les membres du groupe ethnique dominant utilisent la notion de culture pour classifier et évaluer les individus sur une hiérarchie de valeurs. La notion de nature n'est pas utilisée pour hiérarchiser les groupes, mais elle identifie des exogroupes spécifiques.

Résumé Le trouble de l'adaptation est un diagnostic très fréquent, mais étonnamment peu étudié et controversé. Il est, selon les études, considéré comme une forme mineure d'un trouble psychiatrique spécifique, comme une fragilité psychologique révélée par un événement stressant pour le sujet ou encore comme une forme précoce annonçant un trouble psychiatrique majeur. Ces trois points de vue ramènent en fait tous à la question de fond concernant son étiologie. L'objectif de cette étude est de montrer si le trouble de l'adaptation est un diagnostic clairement différencié dont l'existence est justifiée. Afin de tenter de répondre à cette question, il nous est apparu intéressant de comparer cette catégorie diagnostique à une autre catégorie diagnostique psychiatrique importante, le trouble dépressif majeur. Dans cette étude rétrospective nous avons sélectionné tous les patients avec un diagnostic de trouble de l'adaptation ou un trouble dépressif majeur parmi les patients hospitalisés à l'hôpital psychiatrique de Malévoz en Valais en 1993 (580). Elle est basée sur des diagnostics cliniques. Nous avons comparé leurs données socio-démographiques (âge, sexe, nationalité, état civil, activité professionnelle), leurs antécédents psychiatriques (hospitalisations antérieures, suivi psychiatrique ambulatoire, antécédents de tentamen), leurs hospitalisations ultérieures dans les 5 ans, leur hospitalisation actuelle (durée, tentamens, comorbidité) et les traitements médicamenteux prescrits (leur nombre et leur classe). Notre étude met en évidence certaines distinctions entre le trouble de l'adaptation et le trouble dépressif majeur: les patients souffrant de trouble de l'adaptation diffèrent des troubles dépressifs majeurs par le fait qu'ils sont plus fréquemment des hommes, célibataires et plus jeunes que ceux souffrant de trouble dépressif majeur; leur durée d'hospitalisation est plus courte, leur évolution entre les hospitalisations est meilleure et ils reçoivent moins de psychotropes. Nous ne pouvons cependant pas conclure à une distinction claire de ces deux catégories diagnostiques, ni que le trouble de l'adaptation n'est pas simplement lié à une moindre gravité. Nos résultats confirment par contre que ce diagnostic n'est pas non plus un diagnostic anodin (nombre élevé d'antécédents psychiatriques, de tentamens, d'hospitalisations psychiatriques ultérieures, importance des comorbidités de même que la lourdeur des traitements psychotropes prescrits (notamment la fréquence des neuroleptiques). A notre avis, les trois hypothèses étiologiques (forme mineure, trouble précoce ou fragilité psychologique spécifique révélée par un événement stressant) qui ont été évoquées peuvent être considérées comme plausibles suivant le point de vue que l'on choisit. Le diagnostic de trouble de l'adaptation révèle une des limitations de l'approche du DSM-Ill-R qui se veut athéorique. Le fait que dans sa définition même, le DSM-111-R évoque "qu'il faut souvent se référer au seul jugement clinique" le montre bien, un tel diagnostic renvoie inévitablement à une référence psychopathologique. Nous pensons qu'il est illusoire de vouloir se passer d'une telle référence qui elle seule permet d'appréhender justement la portée symbolique d'un événement donné pour un individu. Summary In this retrospective study we selected all the patients with a diagnosis of adjustment disorder (77) or major depressive disorder (125) among the patients hospitalised in the psychiatric hospital of Malevoz in Valais during the year 1993 (580). It is based on clinical diagnosis. Their social and demographic characteristics (age, sex, nationality, marital status, professional activity), their past psychiatric history (earlier psychiatric hospitalisations, ambulatory treatment and attempted suicide), their hospitalisations during the next 5 years, their index hospitalisation (length, attempted suicide, comorbidity) and their drug treatment (number and class of prescribed drugs) were compared. This survey confirms certain differences be-tween adjustment disorder and major depression disorder: patients suffering from adjustment disorder were more often men, not married, younger than those suffering from major depression; their hospitalisations were shorter with a better evolution between hospitalisations and they received less medication. However, the study does not allow to clearly distinguish between the two diagnoses or to conclude that adjustment disorder is not only a minor form of a specific psychiatric disorder. Yet it confirms that adjustment disorder is not a light diagnosis (importance of the psychiatric past, high number of past attempted suicides, rehospitalisations, number of comorbid disorders and weight of the prescribed psychotropic treatments among which neuroleptics were frequent). The three aetiological hypotheses that have been proposed (minor form of a specific disorder, specific psychological vulnerability revealed by a stress factor or precursor manifestation of a major psychiatric disorder) can still be considered as plausible. The diagnosis of adjustment disorder points to methodological limitations of the atheoretical approach of the DSM-III-R. The fact that, in its DSM-III-R definition, it is stated that the diagnosis of adjustment disorder has often to be based only on clinical judgment shows very well that such a diagnosis inevitably refers to a psychopathological theory. Indeed, the authors consider an approach without such a reference as difficult, a reference which remains the only way to appreciate accurately the symbolic weight of a given event for an individual person.

Le présent volume réunit les conférences plénières et une sélection de contributions choisies parmi les plus intéressantes et les plus représentatives des quatre axes de recherche proposés pour le XIIe Congrès de l'International Courtly Literature Society qui s'est tenu aux Universités de Lausanne et de Genève du 29 juillet au 4 août 2007 : - Mythologie courtoise convie à une interrogation sur les implications mythiques de la passion amoureuse, qu'elle marque les aventures de Tristan, de Merlin, de Morgane ou d'Enée ; - Mises en scène du pouvoir examine la capacité à l'autoreprésentation mythique du prince et de la cour, du XIIe au XVe siècle, en Italie aussi bien qu'en France ou en Allemagne ; - Figures exemplaires étudie les modèles culturels actifs dans la construction de héros romanesques dont l'exemplarité est volontiers mise au service d'une utilitas politique ; - Débattre d'amour met en lumière l'importance du débat dans la littérature du Moyen Age finissant, aussi bien dans le sillage du Roman de la Rose qu'à travers les échos suscités par l'oeuvre d'Alain Chartier. Rassemblant des chercheurs d'horizons intellectuels divers, réunis par leur intérêt commun pour la culture courtoise, le volume permet de confronter, dans leur actualité la plus immédiate, les différentes approches de la recherche sur l'art, la littérature et l'histoire du Moyen Age. Son propos, résolument interdisciplinaire, consiste à identifier les problèmes que suscite la notion de mythe, que l'on comprend ici comme une manifestation artistique ou politique, par laquelle une personne ou un événement sont arrachés à la contingence historique pour être chargé d'une valeur symbolique qui leur confère le statut de modèle. Il rend aussi compte de la dimension européenne d'une civilisation en mouvement dont les échos se font entendre jusqu'au XVIe siècle. Enfin, les éditeurs entendent nourrir le débat en l'affranchissant des frontières qui séparent habituellement les domaines des études médiévales.
