A Reabilitação Vestibular visa melhorar o equilíbrio global, a qualidade de vida e orientação espacial dos pacientes com tontura. OBJETIVOS: Traçar o perfil dos pacientes atendidos no Ambulatório de Reabilitação Vestibular do Setor de Otoneurologia de um hospital universitário e verificar os resultados obtidos no período de novembro/2000 a dezembro/2004. MATERIAL E MÉTODO: Levantamento de dados contidos nas fichas dos 93 pacientes submetidos à Reabilitação Vestibular no período. FORMA DE ESTUDO: Clínico retrospectivo. RESULTADOS: A média etária dos pacientes foi de 52,82 anos, 56 do sexo feminino e 37 do sexo masculino. O número médio de atendimentos foi 4,3, sendo maior para os pacientes com distúrbios otoneurológicos centrais (média de 5,9). Dentre os pacientes que concluíram o tratamento proposto, 37 (60,7%) obtiveram melhora significativa, 14 (22,9%) tiveram melhora parcial e 10 (16,4%) não referiram benefícios significativos. Os pacientes que mais se beneficiaram com a Reabilitação Vestibular tinham distúrbios otoneurológicos periféricos. CONCLUSÃO: A maior parte dos pacientes era do sexo feminino, com idade média de 52,8 anos. Cinqüenta e um pacientes (83,6%) tiveram benefício com a terapia confirmando a eficácia do tratamento.

Aperda auditiva é mais prevalente que outros distúrbios já rastreados ao nascimento. Esforços têm sido feitos para identificação e tratamento precoces de perdas auditivas por meio de programas de triagem auditiva neonatal. OBJETIVO: Estudo prospectivo com objetivo caracterizar o processo de implantação do Programa de Triagem Auditiva Neonatal (PTAN) num Hospital Universitário. Analisar a investigação diagnóstica de perda auditiva em recém-nascidos. Apresentar propostas para aprimoramento do PTAN. MATERIAIS E MÉTODOS: Foram estudados recém-nascidos (RNs) submetidos à TAN por emissões otoacústicas transientes (EOAT), reflexo cócleo-palpebral (RCP) e Potencial Evocado Auditivo de Tronco Encefálico (PEATE). RESULTADOS: Foram testadas 625 crianças. Na primeira etapa passaram 458 RNs e falharam 155. Retornaram na segunda etapa 122 RNs, sendo que 8 o fizeram por apresentar fator de alto risco para PA. Encaminhados para investigação diagnóstica 12 RNs (1,9%). Dos 5 que retornaram para PEATE, observou-se PA em dois RNs. CONCLUSÃO: O programa testou 81,7% dos candidatos. O índice de adesão ao programa foi 68,2%. Na primeira etapa falharam 26,7% dos RNs. A implantação do programa está em andamento e necessita constantemente de análise das dificuldades, visando solucioná-las a fim de tornar a Triagem Auditiva Neonatal Universal uma realidade.

This paper aims to describe the processes of teaching illustration and animation, together, in the context of a masters degree program. In Portugal, until very recently, illustration and animation higher education courses, were very scarce and only provided by a few private universities, which offered separated programs - either illustration or animation. The MA in Illustration and Animation (MIA) based in the Instituto Politécnico do Cávado e Ave in Portugal, dared to join these two creative areas in a common learning model and is already starting it’s third edition with encouraging results and will be supported by the first international conference on illustration and animation (CONFIA). This masters program integrates several approaches and techniques (in illustration and animation) and integrates and encourages creative writing and critique writing. This paper describes the iterative process of construction, and implementation of the program as well as the results obtained on the initial years of existence in terms of pedagogic and learning conclusions. In summary, we aim to compare pedagogic models of animation or illustration teaching in higher education opposed to a more contemporary and multidisciplinary model approach that integrates the two - on an earlier stage - and allows them to be developed separately – on the second part of the program. This is based on the differences and specificities of animation (from classic techniques to 3D) and illustration (drawing the illustration) and the intersection area of these two subjects within the program structure focused on the students learning and competencies acquired to use in professional or authorial projects.

Purpose – The purpose of this paper is to explore the process of organizational change undergone by a large Portuguese business group within the context of the environmental agenda and the role of accounting as a mechanism for change. Design/methodology/approach – The paper reports the results of a case study conducted between 2006 and 2009. Information was obtained from semi-structured interviews and secondary sources. Organizational changes were analyzed using Laughlin’s model in order to identify which category reflected most of the changes introduced to address environmental matters. Findings – This paper offers evidence that change is not a homogeneous phenomenon. Additionally, it confirms previous studies’ findings which found that accounting did not play a significant role in the process of organizational change within the context of the environmental agenda. Originality/value – This paper seeks to complement the research in this area by integrating observations from a case study into an existing model of levels of organizational change according to how a Portuguese business group incorporated environmental issues into its processes, policies and corporate culture.

O objetivo deste estudo é relatar os tipos de obstrução do trato digestório, após ingestão de corpos estranhos, em animais de companhia, atendidos no Hospital Veterinário da Universidade Federal de Viçosa (HVT-UFV), durante o ano de 2010; a predisposição racial e por faixa etária; a localização da obstrução; os sintomas relacionados; os achados laboratoriais; o tratamento proposto e a evolução clínica dos pacientes. Foi observada predisposição para animais de raça pequena e de faixas etárias de até um ano e após cinco anos, com predileção pela raça Teckel. O maior número de corpos estranhos foi encontrado no estômago e intestinos, com maior gravidade e elevada taxa de mortalidade, em relação aos encontrados no esôfago. O maior período de evolução do quadro clínico, desde a observação inicial, contribuiu para maiores taxas de mortalidade. A sintomatologia clínica dos animais foi variada, mas quadros de apatia e de anorexia ocorreram na maioria dos casos. Regurgitação e vômito foram frequentes, quando a localização do corpo estranho foi esofágica ou gastrointestinal, respectivamente. Os resultados dos exames laboratoriais foram inespecíficos.

A procura incessante de novas ferramentas de gestão como forma de solucionar dificuldades financeiras sem a perda do nível de qualidade esperado da população, tem sido uma constante nas instituições que prestam cuidados de saúde. Entre os referidos instrumentos de gestão distinguimos o Balanced Scorecard que tem tido um relevante foco de interesse para diversos tipos de organizações, incluindo as prestadoras de cuidados de saúde. Neste sentido, pretendemos com este trabalho analisar o âmbito da aplicação do BSC nas instituições hospitalares, procurando, através da realização de um estudo de caso, desenvolver um esboço de um BSC no Serviço de Urgência de um hospital classificado como Entidade Pública Empresarial.

A procura incessante de novas ferramentas e técnicas de gestão como forma de solucionar dificuldades financeiras sem a perda do nível de qualidade esperado da população, tem sido uma constante nas instituições que prestam cuidados de saúde em todo o mundo. A empresarialização dos hospitais visa, essencialmente, a implementação e o aperfeiçoamento de instrumentos de gestão já amplamente utilizados no sector privado. Entre os referidos instrumentos de gestão, distinguimos o Balanced Scorecard (BSC), que tem tido um relevante foco de interesse para diversos tipos de organizações, incluindo as prestadoras de cuidados de saúde. Neste sentido, pretendemos com este trabalho analisar o âmbito da aplicação do BSC nas instituições prestadoras de cuidados de saúde, procurando, através da realização de um estudo de caso, desenvolver um esboço de um BSC num hospital EPE.

In this work, we consider the numerical solution of a large eigenvalue problem resulting from a finite rank discretization of an integral operator. We are interested in computing a few eigenpairs, with an iterative method, so a matrix representation that allows for fast matrix-vector products is required. Hierarchical matrices are appropriate for this setting, and also provide cheap LU decompositions required in the spectral transformation technique. We illustrate the use of freely available software tools to address the problem, in particular SLEPc for the eigensolvers and HLib for the construction of H-matrices. The numerical tests are performed using an astrophysics application. Results show the benefits of the data-sparse representation compared to standard storage schemes, in terms of computational cost as well as memory requirements.

Aunque en el contexto internacional sea muy estudiada, en Portugal la utilización de la información económico-financiera en el hospital público es un tema de investigación reciente. Nuestro trabajo está basado en un estudio de caso y tiene como objetivo aclarar si los profesionales que tienen poder de decisión sobre los recursos dentro del hospital, en particular los médicos y los cargos de jefe, usan la información económico-financiera disponible y cuáles son los factores que condicionan su utilización de esta información. Los resultados indican que la consulta de la información económico-financiera no es una práctica común para la generalidad de los médicos y jefes dentro del hospital, aunque un porcentaje importante de profesionales ya la integró en su rutina. El análisis estadístico confirmó solo dos de nuestras hipótesis. Por consiguiente, creemos que además de las variables explicativas identificadas en nuestro estudio, hay espacio para que futuras investigaciones identifiquen otros factores que explican la utilización de la información contable por parte de los profesionales de la salud.

This paper presents a catalog of smells in the context of interactive applications. These so-called usability smells are indicators of poor design on an application’s user interface, with the potential to hinder not only its usability but also its maintenance and evolution. To eliminate such usability smells we discuss a set of program/usability refactorings. In order to validate the presented usability smells catalog, and the associated refactorings, we present a preliminary empirical study with software developers in the context of a real open source hospital management application. Moreover, a tool that computes graphical user interface behavior models, giving the applications’ source code, is used to automatically detect usability smells at the model level.

Work-related musculoskeletal disorders (WMSDs) are among the most costly health problems that society is facing today. Prevention involves investments and it is important for organizations to make a cost ebenefit analysis of ergonomic projects. Return on prevention is a recent concern in the domain of occupational safety and health (OSH). There are many studies concerning the return on the prevention of WMSDs, in terms of the benefits for the organization in which the preventive measures are implemented. However, it is also important to perform an analysis of the impact of each measure on society (externalities). A model to perform a financial and economic costebenefit analysis related to OSH projects was developed and it was applied in the case of the prevention of WMSDs in a Portuguese hospital. An analysis of the accidents and corresponding costs has been made in six of the services of the hospital. Financial and an economic costebenefit analysis have been made and the benefitecost ratio (B/C) has been calculated. While the B/C financial ratio, considering only the benefits to the hospital, is around 2, the economic B/C ratio, taking into account all the external benefits that have been quantified, is higher than 14. Relevance to industry: Both the economic and the financial B/C ratio are important support tools for decision makers in public and private organizations, helping them to define which preventive measures should be implemented, taking into account the costs involved and the resulting quantified benefits, for the organization, for the workers and for the society.

BACKGROUND: When an organization performs an integrated analysis of risks through its Occupational Health and Safety Management System, several steps are suggested to address the implications of the identified risks. Namely, the organization should make a detailed analysis of the monetary impact for the organization of each of the preventive measures considered. However, it is also important to perform an analysis of the impact of each measure on society (externalities). OBJECTIVE: The aim of this paper is to present a case study related to the application of the proposed economic evaluation methodology. METHODS: An analysis of the work accidents in a hospital has been made. Three of the major types of accidents have been selected: needle stings, falls and excessive strain. Following the risk assessment, some preventive measures have been designed. Subsequently, the Benefit/Cost ratio (B/C) of these measures has been calculated, both in financial terms (from the organization’s perspective) and in economic terms (including the benefits for the worker and for the Society). RESULTS: While the financial ratio is only advantageous in some cases, when the externalities are taken into account, the B/C ratio increases significantly. CONCLUSIONS: It is important to consider external benefits to make decisions concerning the implementation of preventive measures in Occupational Health and Safety projects

This paper seeks to investigate the use of performance information by politicians and whether the institutional reforms on performance management (PM) have been operationalized by local politicians. Differences on the policy field and the organizational context have been analyzed. Our goal is contribute to knowledge on PM in the political sphere and understand the different responses of politicians to government change initiatives (mainly coercive pressures). Our findings show that local politicians support the notion that greater attention should be devoted to the use of performance information on the evaluation process. Nevertheless they are very skeptic in relation to effective execution of government reforms. There is an internal culture where agencies are embedded, strongly influenced by the high degree of politicisation among senior managers, that lead politicians to be more concerned about personal opinions and informal performance information rather than to use more sophisticated information (output and outcome measures). The institutional approach helps us to identify political responses to institutional pressures and understand the reasons for a reduced use in the Portuguese context.
