The phosphosulfomannan 1 (PI-88) is a mixture of highly sulfated oligosaccharides that is currently undergoing clinical evaluation in cancer patients. As well as it's anticancer properties, 1 displays a number of other interesting biological activities. A series of analogues of 1 were synthesized with a single carbon (pentasaccharide) backbone to facilitate structural characterization and interpretation of biological results. In a fashion similar to 1, all compounds were able to inhibit heparanase and to bind tightly to the proangiogenic growth factors FGF-1, FGF-2, and VEGF. The compounds also inhibited the infection of cells and cell-to-cell spread of herpes simplex virus (HSV-1). Preliminary pharmacokinetic data indicated that the compounds displayed different pharmacokinetic behavior compared with 1. Of particular note was the n-octyl derivative, which was cleared 3 times less rapidly than 1 and may provide increased systemic exposure.

The mechanisms responsible for the immunosuppression associated with sepsis or some chronic blood infections remain poorly understood. Here we show that infection with a malaria parasite (Plasmodium berghei) or simple systemic exposure to bacterial or viral Toll-like receptor ligands inhibited cross-priming. Reduced cross-priming was a consequence of downregulation of cross-presentation by activated dendritic cells due to systemic activation that did not otherwise globally inhibit T cell proliferation. Although activated dendritic cells retained their capacity to present viral antigens via the endogenous major histocompatibility complex class I processing pathway, antiviral responses were greatly impaired in mice exposed to Toll-like receptor ligands. This is consistent with a key function for cross-presentation in antiviral immunity and helps explain the immunosuppressive effects of systemic infection. Moreover, inhibition of cross-presentation was overcome by injection of dendritic cells bearing antigen, which provides a new strategy for generating immunity during immunosuppressive blood infections.

Recent technological advances have resulted in the production of safe subunit and synthetic small peptide vaccines. Unfortunately, these vaccines are weakly or non-immunogenic in the absence of an immunological adjuvant (agents that can induce strong immunity to antigens). In addition, in order to prevent and/or control infection at the mucosal surface, stimulation of the mucosal immune system is essential. This may be achieved via the common mucosal immune system by exposure to antigen at a mucosal surface remote from the area of infection. Initial studies investigated the potential of multiple emulsions in effecting oral absorption and the subsequent immune responses to a lipopolysaccharide vaccine (LPS) after immunisation. Nasal delivery of LPS was carried out in parallel work using either aqueous solution or gel formulations. Tetanus toxoid vaccine in simple solution was delivered to guinea pigs as free antigen or entrapped in DSPC liposomes. In addition, adsorbed tetanus toxoid vaccine was delivered nasally free or in an aerosil gel formulation. This work was extended to investigate guinea pigs immunised by various mucosal routes with a herpes simplex virus subunit vaccine prepared from virus infected cells and delivered in gels, multiple emulsions and liposomes. Comparable serum antibody responses resulted but failed to produce enhanced protection against vaginal challenge when compared to subcutaneous immunisation with alhydrogel adjuvanted vaccine. Thus, immunisation of the mucosal surface by these methods may have been inadequate. These studies were extended in an attempt to protect against HSV genital challenge by construction of an attenuated Salmonella typhimurium HWSH aroA mutant expressing a cloned glycoprotein D-l gene fused to the Es-cherichia coli lac z promoter. Preliminary work on the colonisation of guinea pigs with S. typhimurium HWSH aroA mutants were carried out, with the aim of using the guinea pig HSV vaginal model to investigate protection.

The herpes simplex virus (HSV) UL31 gene encodes a conserved member of the herpesvirus nuclear egress complex that not only functions in the egress of DNA-containing capsids from the nucleus, but is also required for optimal viral genome expression, replication and packaging into capsids. Here, we report that the UL31 protein from HSV-2 and the orthologous protein, ORF69, from Kaposi's sarcoma-associated herpesvirus (KSHV) are recruited to sites of DNA damage. Recruitment of UL31 to sites of DNA damage occurred in HSV-2 infected cells, but did not require other viral proteins. The N-terminus of UL31 contains sequences resembling a poly(ADP-ribose) (PAR) binding motif. As protein poly-ADP ribosylation (PARylation) is a hallmark of the DNA damage response we examined the relationship between PARylation and UL31 recruitment to DNA damage. While the PAR polymerase (PARP)1/2 inhibitor, olaparib, prevented UL31 recruitment to damaged DNA, KU55933 inhibition of signaling through the ataxia telangiectasia mutated (ATM) DNA damage response pathway had no effect. These findings were further supported by experiments demonstrating direct and specific interaction between HSV-2 UL31 and PAR using purified components. Co-transfection with the viral kinase Us3, known to phosphorylate UL31, inhibited UL31 recruitment to DNA damage but also prevented the recruitment of other proteins recruited to DNA damage sites. The viral E3 ubiquitin ligase ICP0 was observed to co-localize with UL31 in transfected cells in a manner that is independent of the PAR-binding ability of UL31. However, inhibition of PARP1/2/3 did not reduce the ability of HSV-2 to replicate and we observed reduced PAR levels in the nuclei of infected cells. This study reveals a previously unrecognized function for UL31 orthologs and may suggest that the recognition of PAR by UL31 is coupled to the nuclear egress of herpesvirus capsids, influences viral DNA replication and packaging, or possibly modulates the DNA damage response mounted by virally infected cells.

La transplantation de sang de cordon ombilical (TSCO) constitue un traitement de choix pour une multitude de pathologies hématologiques malignes et non malignes chez l’enfant et dans certains cas l’adulte. La TSCO est associée à certaines complications, dont une reconstitution immunitaire plus lente et une incidence élevée d’infections opportunistes, notamment celles reliées au cytomégalovirus (CMV) et au virus varicella-zoster (VZV). Dans le cadre de ce travail, nous nous sommes intéressés dans un premier temps à la caractérisation de la reconstitution immunitaire spécifique au CMV et au VZV. Nos résultats ont démontré que la reconstitution de l’immunité cellulaire ne requiert ni un statut séropositif pré-transplantation ni le développement de la maladie. De plus, des reconstitutions spontanées ont été détectées chez certains patients séronégatifs vis-à-vis du CMV ou du VZV. Outre le fait qu’elle se manifeste surtout à partir de 6 mois post-transplantation, ladite reconstitution mérite le qualificatif de « protectrice » en termes de réactivations virales et du développement de signes cliniques lorsqu’une fréquence de 150 cellules produisant l’IFN-γ/million est dépassée. Toutefois, moins de 5% des patients développent une réponse T anti-VZV et anti-CMV au cours 100 premiers jours suivant la TSCO. Il est donc possible que les lymphocytes CD8+ T provenant du SCO, comparativement à leurs homologues provenant de la moelle osseuse (MO), présentent un défaut de fonctionnalité, communément appelé « épuisement clonal ». La caractérisation du répertoire de récepteurs inhibiteurs exprimés par les cellules T CD8+ suivant la TSCO ou la transplantation de moelle osseuse (TMO) a révélé une augmentation significative de la fréquence des cellules exprimant PD-1 tôt suivant la transplantation. Cette population, caractérisée majoritairement par un phénotype effecteur-mémoire (EM), démontre une perte significative de la capacité proliférative et exprime moins d'IFN-γ, d'IL-2, de TNF-α et de CD107a. Une meilleure caractérisation de la reconstitution immunitaire après TSCO permettrait, d'une part de sélectionner des biomarqueurs en vue d’une meilleure gestion des patients à risques de développer des infections virales et/ou de rechuter, et d'autre part d'améliorer leur pronostic.

Soft tissue sarcomas (STS) comprise a heterogenenous group of greater than 50 malignancies of putative mesenchymal cell origin and as such they may arise in diverse tissue types in various anatomical locations throughout the whole body. Collectively they account for approximately 1% of all human malignancies yet have a spectrum of aggressive behaviours amongst their subtypes. They thus pose a particular challenge to manage and remain an under investigated group of cancers with no generally applicable new therapies in the past 40 years and an overall 5-year survival rate that remains stagnant at around 50%. From September 2000 to July 2006 I undertook a full time post-doctoral level research fellowship at the MD Anderson Cancer Center, Houston, Texas, USA in the department of Surgical Oncology to investigate the biology of soft tissue sarcoma and test novel anti- sarcoma adenovirus-based therapy in the preclinical nude rat model of isolated limb perfusion against human sarcoma xenografts. This work, in collaboration with colleagues as indicated herein, led to a number of publications in the scientific literature furthering our understanding of the malignant phenotype of sarcoma and reported preclinical studies with wild-type p53, in a replication deficient adenovirus vector, and oncolytic adenoviruses administered by isolated limb perfusion. Additional collaborative and pioneering preclinical studies reported the molecular imaging of sarcoma response to systemically delivered therapeutic phage RGD-4c AAVP. Doxorubicin chemotherapy is the single most active broadly applicable anti-sarcoma chemotherapeutic yet only has an approximate 30% overall response rate with additional breakthrough tumour progression and recurrence after initial chemo-responsiveness further problematic features in STS management. Doxorubicin is a substrate for the multi- drug resistance (mdr) gene product p-glycoprotein drug efflux pump and exerts its main mode of action by induction of DNA double-strand breaks during the S-phase of the cell cycle. Two papers in my thesis characterise different aspects of chemoresistance in sarcoma. The first shows that wild-type p53 suppresses Protein Kinase Calpha (PKCα) phosphorylation (and activation) of p-glycoprotein by transcriptional repression of PKCα through a Sp-1 transcription factor binding site in its -244/-234 promoter region. The second paper demonstrates that Rad51 (a central mediator of homologous recombination repair of double strand breaks) has elevated levels in sarcoma and particularly in the S- G2 phase of the cell cycle. Suppression of Rad51 with small interfering RNA in sarcoma cell culture led to doxorubicin chemosensitisation. Reintroduction of wild-type p53 into STS cell lines resulted in decreased Rad51 protein and mRNA expression via transcriptional repression of the Rad51 promoter through increased AP-2 binding. In light of poor response rates to chemotherapy, escape from local control portends a poor prognosis for patients with sarcoma. Two papers in my thesis characterise aspects of sarcoma angiogenesis, invasion and metastasis. Human sarcoma samples were found to have high levels of matrix metalloproteinase-9 (MMP-9) with expression levels that correlated with p53 mutational status. MMP-9 is known to degrade extracellular collagen, contribute to the control of the angiogenic switch necessary in primary tumour progression and facilitate invasion and metastasis. Reconstitution of wild-type p53 function led to decreased levels of MMP-9 protein and mRNA as well as zymography-assessed MMP-9 proteolytic activity and decreased tumour cell invasiveness. Reintroduction of wild-type p53 into human sarcoma xenografts in-vivo decreased tumour growth and MMP-9 protein expression. Wild-type p53 was found to suppress mmp-9 transcription via decreased binding of NF-κB to its -607/-595 mmp-9 promoter element. Studies on the role of the VEGF165 in sarcoma found that sarcoma cells stably transfected with VEGF165 formed more aggressive xenografted tumours with increased vascularity, growth rate, metastasis, and resistance to chemotherapy. Use of the anti-VEGFR2 antibody DC101 enhanced doxorubicin sensitivity at sub-conventional dosing, inhibited tumour growth, decreased development of metastases, and reduced tumour micro-vessel density while increasing the vessel maturation index. These effects were explained primarily through effects on endothelial cells (e.c.s), rather than the tumour cells per se, where DC101 induced e.c. sensitivity to doxorubicin and suppressed e.c. production of MMPs. The p53 tumour suppressor pathway is the most frequently mutated pathway in sarcoma. Recapitulation of wild-type p53 function in sarcoma exerts a number of anti-cancer outcomes such as growth arrest, resensitisation to chemotherapy, suppression of invasion, and attenuation of angiogenesis. Using a modified nude rat-human sarcoma xenograft model for isolated limb perfusion (ILP) delivery of wild-type p53 in a replication deficient adenovirus vector I showed that functionally competent wild-type p53 could be delivered to and detected in human leiomyosarcoma xenografts confirming preclinical feasibility - although not efficacious due to low transgene expression. Viral fibre modification to express the RGD tripeptide motif led to greater viral uptake by sarcoma cells in vitro (transductional targeting) and changing the transgene promoter to a response element active in cells with active telomerase expression restricted the transgene expression to the tumour intracellular environment (transcriptional targeting). Delivery of the fibre-modified, selectively replication proficient oncolytic adenovirus Ad.hTC.GFP/ E1a.RGD by ILP demonstrated a more robust, and tumour-restricted, transgene expression with evidence of anti-sarcoma effect confirmed microscopically. Collaborative studies using the fibre modified phage RGD-4C AAVP confirmed that systemic delivery specifically, efficiently, and repeatedly targets human sarcoma xenografts, binds to αv integrins in tumours, and demonstrates a durable, though heterogeneous, transgene expression of 1-4 weeks. Incorporation of the Herpes Simplex Virus thymidine kinase (HSVtk) transgene into RGD-4C AAVP permitted CT-PET spatial and temporal molecular imaging in vivo of transgene expression and allowed quantification of tumour metabolic activity both before and after interval administration of a systemic cytotoxic with predictable and measurable response to treatment before becoming apparent clinically. These papers further the medical and scientific community’s understanding of the biology of soft tissue sarcoma and report preclinical studies with novel and promising anti- sarcoma therapeutics.

Objetivo: Medir la incidencia y determinar los factores de riesgo de las infecciones de transmisión sexual (ITS) en la población del Centro Penitenciario de Daroca (Zaragoza). Método: Estudio de cohortes retrospectivo (2005-2013) para medir la incidencia de ITS y estudio transversal para estudiar los factores de riesgo. Resultados: De los 203 internos, 79 desarrollaron una ITS, 37 tenían ITS previas, el 55,2% conocimientos y el 28,9% comportamientos no favorables a la prevención de ITS. La incidencia fue de 6,5 ITS por cada 1000 internos-año. Las de mayor incidencia fueron la hepatitis B (39,7%), la infección por Ureaplasma urealyticum (19,1%), el herpes simple (16,2%) y la infección por el virus de la inmunodeficiencia humana (8,8%). El riesgo (hazard ratio [HR]) de adquirir una nueva ITS fue significativamente mayor en los internos con antecedentes de ITS previa (HR = 2,61; intervalo de confianza del 95% [IC95%]: 1,01-6,69), y en el límite de la significación para los comportamientos no preventivos (HR = 2,10; IC95%: 0,98-4,53), pero no en los conocimientos frente a ITS (HR = 1,33; IC95%: 0,58-3,07). Conclusión: Los factores de riesgo más relevantes en prisión son los comportamientos y los antecedentes de ITS. Otros factores son ser reincidente, el consumo de drogas inyectadas o estar en un programa de metadona. Los/las profesionales sanitarios y la educación por pares pueden facilitar la prevención y el control.

Objectives: Infectious agents triggering haemophagocytic lymphohistiocytosis (HLH) primarily involve the herpes virus group. We report a case of HLH precipitated by Plasmodium falciparum. Materials and methods: Clinical and laboratory findings in a patient presenting with fever were collected. After confirmation of acute malaria, anti-malarial treatment was administered. Results: Despite initial favourable evolution, the patient developed fever again together with a worsening of the haematological parameters and increased ferritin levels. A bone marrow biopsy confirmed the diagnosis of HLH. Conclusion: This case illustrates that HLH should be considered in the differential diagnosis of acute malaria in patients with persisting fever and pancytopenia.

O herpesvírus está disseminado na população canina mundial, com uma seroprevalência de cerca de 40 a 80%. A infeção por herpesvírus canino causa elevadas taxas de mortalidade em cachorros e em cães adultos pode permanecer em estado latente ou ser reativado. A infeção por este vírus em canis de reprodução representa grandes perdas económicas, pelo que se pretende estudar a seroprevalência de herpesvírus em 52 cães pertencentes a dois canis de reprodução, para fins de venda de cães de raça. Foram analisados animais de ambos os sexos e com idades compreendidas entre 1 e 8 anos de idade. Devido a inexistências de terapia para cachorros com sintomatologia de infeção por HVC-1 pretende-se formular um protocolo de cuidados para ninhadas, suspeitas de serem portadoras deste vírus, e para os restantes cães da colónia. Estão descritos estudos que indicam o HVC-1 como um dos agentes indiretamente envolvidos na etiologia do linfoma canino, pelo que se procurou estudar a sua seroprevalência numa amostra de 28 cães com linfoma, sendo um dos objetivos a deteção de anticorpos HVC-1, nestes animais.

Se realizó un estudio cuasiexperimental sin grupo control de conocimientos, actitudes y prácticas sobre las Infecciones de Transmisión Sexual ITS y VIH/SIDA a los estudiantes de la Escuela de Psicología Educativa de La Universidad Estatal de la Ciudad de Cuenca. Se aplicó un cuestionario con dos instrumentos de medición: antes y 2 meses después de la intervención educativa basada en la teoría cognoscitivo-social de Albert Bandura. La muestra estuvo constituida por 77 estudiantes que a la vez constituyeron el grupo intervención y control. El 70% tienen una edad entre 19 a 23 años, con una media de 22.7años. La media de edad de inicio de las relaciones sexuales fue de 17.8 ± 4.3. El número de personas con las que tuvieron relaciones sexuales, en los 3 últimos meses fue de 1,9 ± 2.6. Se obtuvieron como resultados que el grado de conocimientos aumento de 64.90% a 69.20 %, las actitudes buenas se incrementaron de un 70.20% a 78.00%. Las prácticas disminuyeron de un 36.30 % a 35.90%; en el grupo intervenido. En el conocimiento sobre Condiloma se obtuvo un incremento significativo, al igual que sobre Herpes (p menor que 0.05). En relación al nivel de autoeficacia aumentó en negarse a tener relaciones sexuales bajo el efecto del alcohol y/o drogas (p menor que 0.05).

Con una modalidad antes y después se realizó un estudio cuasi experimental con la inclusión de 84 estudiantes de la Escuela de Comunicación Social de la Facultad de Filosofía a quienes se aplicó en el Pre-CAP un cuestionario para medir conocimientos, actitudes y prácticas sobre ITS y después de tres meses de una intervención educativa se midió la modificación Post-CAP. Resultados: todos respondieron el formulario Pre y Post-CAP. El promedio de edad de la muestra fue de 21,3 ± 2,2 años (18 a 23), con un 56% de mujeres y un 86,9% de solteros. El 75% fueron católicos, el 89,3% viven en la zona urbana. La edad de inicio de vida sexual activa del 70,2% del grupo fue entre 15 y 19 años de edad. Se obtuvo un significativo aumento en conocimientos sobre ITS: condiloma, herpes, sífilis, gonorrea, tricomona y clamidiasis, y sobre signos y síntomas (P menor que 0,05) aunque no sobre las vías de transmisión. En las actitudes hubo un incremento significativo en la recomendación a donde acudir en caso de portar una ITS (P = 0,002), sobre uso del preservativo (P = 0,006) y la recomendación de lavarse en caso de mantener relaciones con desconocidos (P = 0,023). En las prácticas hubo aumento significativo en conversar con la pareja (P = 0,003), disminución de relaciones en estado de embriaguez (P = 0,023) y aumento del uso del preservativo en las relaciones fortuitas (P = 0,026).

Se realizó un estudio cuasiexperimental de conocimientos, actitudes y prácticas sobre las ITS/VIH-SIDA a los estudiantes de la Facultad de Filosofía, Letras y Ciencias de la Educación en la Escuela de Lengua y Literatura Inglesa de la ciudad de Cuenca, 2009. Se aplicó un cuestionario con dos instrumentos de medición: Pre-CAPs (antes) y 3 meses después Post-CAPs de la intervención educativa basada en la teoría cognoscitivo-social de Albert Bandura. La muestra estuvo constituida por 80 estudiantes universitarios de entre 20 a 25 años (86.9%), género femenino (83.1%), solteros (78.1%), católicos (75.6%) y del área urbana (7%). Los resultados indican, una elevación significativa de los conocimientos sobre condilomas del 3.8% al 28.8%, herpes del 80% al 91.3% , SIDA del 96.3% al 98.8%, tricomoniasis del 11.3% al 33.8%, y clamidiasis del 6.3% al 22.5% después de la intervención educativa. Sobre los conocimiento sobre las formas de trasmisión de ITS se obtuvo una elevación significativa del 81.3% al 92.5%. en la forma de contagio sin penetración; y de los síntomas de las ITS, se observó una elevación significativa: dolor en el vientre bajo del 13.8% al 52.5%, secreción por los genitales del 68.8% al 87.55%, verrugas en los genitales del 66.35% al 83.8% , manchas en la piel del 56.3% al 63.8% e inflamación en los ganglios del 50% al 62%. Se observaron actitudes favorables de los estudiantes con respecto a las ITS, como el hecho de acudir a un hospital del 66.3% al 88.8%, acudir a un médico del 66.8% al 83.8%.

The influence of different infectious agents and their association with human papillomavirus (HPV) in cervical carcinogenesis have not been completely elucidated. This study describes the association between cytological changes in cervical epithelium and the detection of the most relevant aetiological agents of sexually transmitted diseases. Samples collected from 169 patients were evaluated by conventional cytology followed by molecular analysis to detect HPV DNA, Chlamydia trachomatis , herpes simplex virus 1 and 2, Neisseria gonorrhoeae , Mycoplasma genitalium , Trichomonas vaginalis, and Treponema pallidum , besides genotyping for most common high-risk HPV. An association between cytological lesions and different behavioural habits such as smoking and sedentariness was observed. Intraepithelial lesions were also associated with HPV and C. trachomatis detection. An association was also found between both simple and multiple genotype infection and cytological changes. The investigation of HPV and C. trachomatis proved its importance and may be considered in the future for including in screening programs, since these factors are linked to the early diagnosis of patients with precursor lesions of cervical cancer.
