OBJECTIVE: To extract and to validate a brief version of the DISCERN which could identify mental health-related websites with good content quality. METHOD: The present study is based on the analysis of data issued from six previous studies which used DISCERN and a standardized tool for the evaluation of content quality (evidence-based health information) of 388 mental health-related websites. After extracting the Brief DISCERN, several psychometric properties (content validity through a Factor analysis, internal consistency by the Cronbach's alpha index, predictive validity through the diagnostic tests, concurrent validity by the strength of association between the Brief DISCERN and the original DISCERN scores) were investigated to ascertain its general applicability. RESULTS: A Brief DISCERN composed of two factors and six items was extracted from the original 16 items version of the DISCERN. Cronbach's alpha coefficients were more than acceptable for the complete questionnaire (alpha=0.74) and for the two distinct domains: treatments information (alpha=0.87) and reliability (alpha=0.83). Sensibility and specificity of the Brief DISCERN cut-off score > or =16 in the detection of good content quality websites were 0.357 and 0.945, respectively. Its predictive positive and negative values were 0.98 and 0.83, respectively. A statistically significant linear correlation was found between the total scores of the Brief DISCERN and those of the original DISCERN (r=0.84 and p<0.0005). CONCLUSION: The Brief DISCERN seems to be a reliable and valid instrument able to discriminate between websites with good and poor content quality. PRACTICE IMPLICATIONS: The Brief DISCERN is a simple tool which could facilitate the identification of good information on the web by patients and general consumers.

Ce travail de recherche évalue dans quelle mesure le pharmacien, en collaboration avec les autres professionnels de soins, peut soutenir le patient VIH-positif dans sa prise médicamenteuse en associant de façon originale l'entretien motivationnel et les piluliers électroniques. Les résultats montrent que l'adhésion thérapeutique peut être maintenue à un niveau élevé même dans un groupe de patients à risque de non-adhésion. [Auteurs]

Cette étude a été menée par l'Unité d'évaluation de programmes de prévention de l'Institut de médecine sociale et préventive (IUMSP) de Lausanne. Elle s'inscrit dans le cadre de l'évaluation globale de la stratégie de prévention du VIH/sida pour laquelle l'IUMSP a reçu mandat de l'Office fédéral de la santé publique. Le programme national "VIH et sida : Programme national de 1999 à 2003" a été préparé en 1998 par l'OFSP en collaboration avec des experts et publié en février 1999. Il fait suite au manuel "Prévention du VIH en Suisse : buts, stratégies, mesures" publié en 1993 et couvre la période de 1999 à 2003. Les 14 objectifs qu'il définit, avec leurs jalons, sont répartis dans cinq grands domaines à savoir: promotion de la santé, prévention, thérapie et diagnostic, conseil et soutien, savoir et transfert des compétences. Afin d'en planifier la mise en oeuvre, l'OFSP a organisé 14 plates-formes régionales en août et septembre 1999 qui ont fait l'objet d'un rapport publié en janvier 2000. Les plates-formes ont été suivies d'une "Conférence intercantonale VIH/sida" le 8 septembre 2000 à Berne, organisée conjointement par l'Office fédéral de la santé publique (OFSP)/Section Sida et l'Aide Suisse contre le Sida (ASS), dont le thème général était "Transfert dans la lutte contre le VIH/sida". Faisant suite à la première phase d'évaluation qui, elle, a porté sur l'état des activités de lutte contre le VIH/sida dans les cantons, la deuxième phase de l'évaluation fait le point sur la mise en oeuvre du programme national 1999-2003 au niveau national et dans les cinq cantons pour la période 2000-2001 et porte sur l'implantation du programme national. Y sont analysés le développement des activités dans tous les domaines, la pertinence des objectifs et jalons, les éléments garantissant la pérennité et la qualité des interventions, et le développement des collaborations à différents niveaux.

The present prospective study, with a five-year follow-up, presents an extensive psychiatric and educational assessment of an adolescent population (N = 30) in the age range 14-20, suffering from several psychiatric disorders, though apt to follow a normal academic program. The residential settings where the study took place provide both psychiatric and schooling facilities. In this environment, what is the effectiveness of long-term hospitalization? Are there any criteria for predicting results? After discharge, could social adjustments difficulties be prevented? Assessment instruments are described and the results of one preliminary study are presented. The actual data seems to confirm the impact of the special treatment facilities combining schooling and psychiatric settings on the long term outcome of adolescents.

Health assessment and medical surveillance of workers exposed to combustion nanoparticles are challenging. The aim was to evaluate the feasibility of using exhaled breath condensate (EBC) from healthy volunteers for (1) assessing the lung deposited dose of combustion nanoparticles and (2) determining the resulting oxidative stress by measuring hydrogen peroxide (H2O2) and malondialdehyde (MDA). Methods: Fifteen healthy nonsmoker volunteers were exposed to three different levels of sidestream cigarette smoke under controlled conditions. EBC was repeatedly collected before, during, and 1 and 2 hr after exposure. Exposure variables were measured by direct reading instruments and by active sampling. The different EBC samples were analyzed for particle number concentration (light-scattering-based method) and for selected compounds considered oxidative stress markers. Results: Subjects were exposed to an average airborne concentration up to 4.3×10(5) particles/cm(3) (average geometric size ∼60-80 nm). Up to 10×10(8) particles/mL could be measured in the collected EBC with a broad size distribution (50(th) percentile ∼160 nm), but these biological concentrations were not related to the exposure level of cigarette smoke particles. Although H2O2 and MDA concentrations in EBC increased during exposure, only H2O2 showed a transient normalization 1 hr after exposure and increased afterward. In contrast, MDA levels stayed elevated during the 2 hr post exposure. Conclusions: The use of diffusion light scattering for particle counting proved to be sufficiently sensitive to detect objects in EBC, but lacked the specificity for carbonaceous tobacco smoke particles. Our results suggest two phases of oxidation markers in EBC: first, the initial deposition of particles and gases in the lung lining liquid, and later the start of oxidative stress with associated cell membrane damage. Future studies should extend the follow-up time and should remove gases or particles from the air to allow differentiation between the different sources of H2O2 and MDA.

The R package EasyStrata facilitates the evaluation and visualization of stratified genome-wide association meta-analyses (GWAMAs) results. It provides (i) statistical methods to test and account for between-strata difference as a means to tackle gene-strata interaction effects and (ii) extended graphical features tailored for stratified GWAMA results. The software provides further features also suitable for general GWAMAs including functions to annotate, exclude or highlight specific loci in plots or to extract independent subsets of loci from genome-wide datasets. It is freely available and includes a user-friendly scripting interface that simplifies data handling and allows for combining statistical and graphical functions in a flexible fashion. This book comprises two volumes and builds on the findings of the DISMEVAL project (Developing and validating DISease Management EVALuation methods for European health care systems), funded under the European Union's (EU) Seventh Framework Programme (FP7) (Agreement no. 223277). DISMEVAL was a three-year European collaborative project conducted between 2009 and 2011. It contributed to developing new research methods and generating the evidence base to inform decision-making in the field of chronic disease management evaluation (www.dismeval.eu). In this book, we report on the findings of the project's first phase, capturing the diverse range of contexts in which new approaches to chronic care are being implemented and evaluating the outcomes of these initiatives using an explicit comparative approach and a unified assessment framework. In this first volume, we describe the range of approaches to chronic care adopted in 12 European countries. By reflecting on the facilitators and barriers to implementation, we aim to provide policy-makers and practitioners with a portfolio of options to advance chronic care approaches in a given policy context.
