En este artículo pretendemos enriquecer el marco explicativo que inquiere en las razones del hiato entre el trazado ideal y la constitución efectiva del sistema público de instrucción en Argentina en el periodo que va desde la sanción de la Ley 1.420 hasta la entrada en la década de 1910. Nos interesará dejar asentado que tal distancia no dependió únicamente de las limitaciones materiales con las que se enfrentaron las autoridades escolares, o de las decisiones de los padres con respecto de la asistencia de sus hijos a la escuela, sino que la misma estuvo enraizada en un elemento más profundo, del orden de las representaciones, que desde muy temprano funcionó como un impedimento para la realización de la vocación educativa pretendidamente ecuménica de las élites argentinas. De hecho, las posturas educativas universalistas convivieron con ostras que rechazaban la posibilidad e incluso la conveniencia de que todos los niños se educaran en escuelas comunes, aparente contradicción que atravesó los discursos y las prácticas de casi todos los miembros de las élites, incluyendo los de los adalides de la "educación popular". A través del análisis y de la puesta en relación de la ley de Educación común (Ley n.1.420/1884) y la Ley de Reglamentación del Trabajo de Mujeres y Niños (Ley n.5.291/1907), exploraremos cómo se fueran estableciendo los criterios que desde temprano determinaron que "niños" podrían convertirse en alumnos, y cuáles no.

BACKGROUND: Present combination antiretroviral therapy (cART) alone does not cure HIV infection and requires lifelong drug treatment. The potential role of HIV therapeutic vaccines as part of an HIV cure is under consideration. Our aim was to assess the efficacy, safety, and immunogenicity of Vacc-4x, a peptide-based HIV-1 therapeutic vaccine targeting conserved domains on p24(Gag), in adults infected with HIV-1. METHODS: Between July, 2008, and June, 2010, we did a multinational double-blind, randomised, phase 2 study comparing Vacc-4x with placebo. Participants were adults infected with HIV-1 who were aged 18-55 years and virologically suppressed on cART (viral load <50 copies per mL) with CD4 cell counts of 400 × 10(6) cells per L or greater. The trial was done at 18 sites in Germany, Italy, Spain, the UK, and the USA. Participants were randomly assigned (2:1) to Vacc-4x or placebo. Group allocation was masked from participants and investigators. Four primary immunisations, weekly for 4 weeks, containing Vacc-4x (or placebo) were given intradermally after administration of adjuvant. Booster immunisations were given at weeks 16 and 18. At week 28, cART was interrupted for up to 24 weeks. The coprimary endpoints were cART resumption and changes in CD4 counts during treatment interruption. Analyses were by modified intention to treat: all participants who received one intervention. Furthermore, safety, viral load, and immunogenicity (as measured by ELISPOT and proliferation assays) were assessed. The 52 week follow-up period was completed in June, 2011. For the coprimary endpoints the proportion of participants who met the criteria for cART resumption was analysed with a logistic regression model with the treatment effect being assessed in a model including country as a covariate. This study is registered with ClinicalTrials.gov, number NCT00659789. FINDINGS: 174 individuals were screened; because of slow recruitment, enrolment stopped with 136 of a planned 345 participants and 93 were randomly assigned to receive Vacc-4x and 43 to receive placebo. There were no differences between the two groups for the primary efficacy endpoints in those participants who stopped cART at week 28. Of the participants who resumed cART, 30 (34%) were in the Vacc-4x group and 11 (29%) in the placebo group, and percentage changes in CD4 counts were not significant (mean treatment difference -5·71, 95% CI -13·01 to 1·59). However, a significant difference in viral load was noted for the Vacc-4x group both at week 48 (median 23 100 copies per mL Vacc-4x vs 71 800 copies per mL placebo; p=0·025) and week 52 (median 19 550 copies per mL vs 51 000 copies per mL; p=0·041). One serious adverse event, exacerbation of multiple sclerosis, was reported as possibly related to study treatment. Vacc-4x was immunogenic, inducing proliferative responses in both CD4 and CD8 T-cell populations. INTERPRETATION: The proportion of participants resuming cART before end of study and change in CD4 counts during the treatment interruption showed no benefit of vaccination. Vacc-4x was safe, well tolerated, immunogenic, seemed to contribute to a viral-load setpoint reduction after cART interruption, and might be worth consideration in future HIV-cure investigative strategies. Pharmacokinetic variability in drug levels represent for some drugs a major determinant of treatment success, since sub-therapeutic concentrations might lead to toxic reactions, treatment discontinuation or inefficacy. This is true for most antiretroviral drugs, which exhibit high inter-patient variability in their pharmacokinetics that has been partially explained by some genetic and non-genetic factors. The population pharmacokinetic approach represents a very useful tool for the description of the dose-concentration relationship, the quantification of variability in the target population of patients and the identification of influencing factors. It can thus be used to make predictions and dosage adjustment optimization based on Bayesian therapeutic drug monitoring (TDM). This approach has been used to characterize the pharmacokinetics of nevirapine (NVP) in 137 HIV-positive patients followed within the frame of a TDM program. Among tested covariates, body weight, co-administration of a cytochrome (CYP) 3A4 inducer or boosted atazanavir as well as elevated aspartate transaminases showed an effect on NVP elimination. In addition, genetic polymorphism in the CYP2B6 was associated with reduced NVP clearance. Altogether, these factors could explain 26% in NVP variability. Model-based simulations were used to compare the adequacy of different dosage regimens in relation to the therapeutic target associated with treatment efficacy. In conclusion, the population approach is very useful to characterize the pharmacokinetic profile of drugs in a population of interest. The quantification and the identification of the sources of variability is a rational approach to making optimal dosage decision for certain drugs administered chronically.

Le syndrome des jambes sans repos (SJSR) est une pathologie fréquente dont la prévalence dans la population générale est de 5-15%. Le SJSR se caractérise par un besoin impérieux de bouger les membres inférieurs qui apparaît le plus souvent au repos, le soir et la nuit. Les symptômes sont typiquement atténués par la mobilisation des jambes. Plusieurs études ont montré que le SJSR a d'importantes répercussions sur le quotidien des patients atteints avec une diminution de leur qualité de vie, des troubles du sommeil et une altération des fonctions cognitives. Un lien entre la carence martiale et le SJSR est supposé depuis longtemps. Il a été montré que les symptômes sont aggravés en cas de déficit en fer sans qu'une corrélation directe ait pu être établie, le taux de ferritine étant souvent normal chez les patients avec un SJRS. Des études récentes ont permis de montrer qu'il existe une carence en fer au niveau du système nerveux central des patients avec un SJSR notamment au niveau du liquide céphalo-rachidien et de la substance noire. Il existe une probable dysfonction du système dopaminergique qui explique la symptomatologie des patients et la réponse souvent favorable aux agonistes dopaminergiques. Par ailleurs, des études ont récemment montré qu'il existe des polymorphismes de plusieurs gènes chez les patients avec un SJSR faisant supposer une probable prédisposition génétique. Les femmes donnant leur sang, population préalablement en bonne santé, sont plus à risque de présenter une carence en fer qui pourrait être associée à une augmentation du risque de développer un SJSR. Les études à ce sujet ont montré des résultats contradictoires. Une étude suédoise parue en 2004 montrait une prévalence du SJSR chez les donneuses de sang de presque 25% alors qu'une étude américaine de 2010 décrivait une prévalence de 9% comparable à la population générale. Il est donc d'intérêt de savoir si l'on fait prendre un risque aux donneuses de sang de développer une affection invalidante telle que le SJSR. Le but de ce travail de thèse en médecine était donc de déterminer la prévalence du SJSR chez les donneuses de sang. Notre étude s'est déroulée de 2008 à 2010 et notre collectif comprenait 291 donneuses de sang de 18 à 49 ans. Nos résultats ont montré une prévalence du SJSR de 6.9% chez les donneuses de sang. La prévalence de l'hyperménorrhée était augmentée chez les donneuses avec un SJSR et elles étaient également significativement plus fatiguées. Nous n'avons pas montré d'association entre le SJSR et la capacité aérobique, la fréquence des dons et la qualité de vie. Comme montré dans plusieurs études, nous avons confirmé l'absence de lien entre le SJSR et le taux d'hémoglobine et la ferritine.

HDLs protect pancreatic beta cells against apoptosis induced by several endoplasmic reticulum (ER) stressors, including thapsigargin, cyclopiazonic acid, palmitate and insulin over-expression. This protection is mediated by the capacity of HDLs to maintain proper ER morphology and ER functions such as protein folding and trafficking. Here, we identified a distinct mode of protection exerted by HDLs in beta cells challenged with tunicamycin (TM), a protein glycosylation inhibitor inducing ER stress. HDLs were found to inhibit apoptosis induced by TM in the MIN6 insulinoma cell line and this correlated with the maintenance of a normal ER morphology. Surprisingly however, this protective response was neither associated with a significant ER stress reduction, nor with restoration of protein folding and trafficking in the ER. These data indicate that HDLs can use at least two mechanisms to protect beta cells against ER stressors. One that relies on the maintenance of ER function and one that operates independently of ER function modulation. The capacity of HDLs to activate several anti-apoptotic pathways in beta cells may explain their ability to efficiently protect these cells against a variety of insults.

A program of A (90 day moist room), B (14 day moist room) and C (7 day moist room and 7 day 50%_humidity) type curing for the R-11-Z program of durability of concrete using the automatic freeze and thaw machine (ASTM C-291) has been used in the Materials Department of the Iowa State Highway Commission since December 6, 1966. A summary of the results obtained from then until March 25, 1968, indicates that the B and C type curing are yielding very little valuable information. However, the A cure exhibits a wide range of durability factors and also groups the aggregates in an order which is related to the service record (there are definite exceptions. The biggest disadvantage to the A cure is the length of time that it takes to complete the test (90 day cure and 38 day test). The Kansas Highway Department has experimented with different cements and aggregates in order to determine which combination offers a concrete with the best durability factor possible. In an experimental test section of highway, concrete made with a Type II cement appeared to have better durability than others made with Type I cements. Because of this, a question has been raised at the Iowa State Highway Commission - Can concrete made with Type II cements, because of a lesser amount of tricalcium aluminate, yield better durability than concrete made with Type I cements?

Although autologous nerve graft is still the first choice strategy in nerve reconstruction, it has the severe disadvantage of the sacrifice of a functional nerve. Cell transplantation in a bioartificial conduit is an alternative strategy to improve nerve regeneration. Nerve fibrin conduits were seeded with various cell types: primary Schwann cells (SC), SC-like differentiated bone marrow-derived mesenchymal stem cells (dMSC), SC-like differentiated adipose-derived stem cells (dASC). Two further control groups were fibrin conduits without cells and autografts. Conduits were used to bridge a 1 cm rat sciatic nerve gap in a long term experiment (16 weeks). Functional and morphological properties of regenerated nerves were investigated. A reduction in muscle atrophy was observed in the autograft and in all cell-seeded groups, when compared with the empty fibrin conduits. SC showed significant improvement in axon myelination and average fiber diameter of the regenerated nerves. dASC were the most effective cell population in terms of improvement of axonal and fiber diameter, evoked potentials at the level of the gastrocnemius muscle and regeneration of motoneurons, similar to the autografts. Given these results and other advantages of adipose derived stem cells such as ease of harvest and relative abundance, dASC could be a clinically translatable route towards new methods to enhance peripheral nerve repair.

O objetivo deste trabalho foi avaliar a massa de forragem e a composição botânica de pastagens de Brachiaria decumbens Stapf em monocultivo e consorciada com Stylosanthes guianensis (Aubl.) Sw. var. vulgaris cv. Mineirão, em diferentes meses do ano. As amostras obtidas em janeiro e maio foram submetidas a ensaio de degradabilidade in situ da matéria seca. Foram utilizados três piquetes (repetições), cada um dividido em duas parcelas (monocultura e consorciação). Após cada amostragem, o pasto foi submetido a pastejo com vacas secas. A massa de forragem de B. decumbens não variou com o sistema de cultivo; foi menor no mês de outubro (291 kg/ha) e não diferiu entre os demais meses (1.571 kg/ha). A massa de forragem da leguminosa decresceu ao longo do ano e sua porcentagem na pastagem foi maior em outubro/2001 e menor em janeiro/2002. A massa de forragem total na pastagem consorciada (2.158 kg/ha) foi maior que a do monocultivo (1.481 kg/ha). A taxa de degradação e a degradabilidade efetiva média de S. guianensis foram, respectivamente, de 6,8%/h e 61,5%, e de B. decumbens foram de 5,3%/h e 49,2%. A leguminosa contribui no aumento da quantidade e na melhoria da qualidade da forragem disponível na pastagem.
