O estudo propõe-se a analisar as dificuldades e facilidades dos Agentes Comunitários de Saúde (ACS) frente ao trabalho em equipe. A análise pautou-se na perspectiva hermenêutica-dialética, tendo como referência os princípios do método de interpretação dos sentidos. As dificuldades e facilidades apontadas por eles revelam que trabalhar em equipe demanda relações efetivas, com ênfase na comunicação, respeito e cooperação, sendo as reuniões de equipe estratégia importante para isso. Depreende-se a necessidade de constantes investimentos nas relações entre os membros da equipe.

TEMA: eficácia da reabilitação em disfagia orofaríngea. A atuação fonoaudiológica com disfagia orofaríngea em nosso País alcançou proporções significativas e merece neste momento atenção para que esta atuação esteja baseada em evidências científicas. As técnicas terapêuticas e a eficácia da reabilitação em disfagia orofaríngea têm sido estudadas desde a década de 70, alcançando seu ápice na década de 80 e 90. Poucos estudos têm relatado a eficácia da reabilitação em disfagia orofaríngea, sendo mais freqüente aqueles que têm se preocupado em provar os efeitos da técnica terapêutica na dinâmica da deglutição. No Brasil, as pesquisas em disfagia orofaríngea têm valorizado os procedimentos de avaliação, sendo poucos os trabalhos que tratam da reabilitação. OBJETIVO: apresentar uma análise crítica sobre a eficácia da reabilitação em disfagia orofaríngea. CONCLUSÃO: este artigo de revisão aponta que estudos não randomizados têm comprometido os resultados, uma vez que a casuística das pesquisas têm utilizado amostras muito heterogêneas, que incluem disfagias orofaríngeas mecânicas e neurogênicas ocasionadas por distintas etiologias. Além disto, os programas terapêuticos empregados são pouco descritivos comprometendo a reprodução por parte de outros pesquisadores. Tais achados sugerem a necessidade de estudos mais randomizados, talvez inicialmente por meio de estudos de casos que possam excluir as variáveis do controle da eficácia terapêutica. Outra sugestão seria empregar, assim como as pesquisas atuais têm proposto, escalas que possam medir o impacto do treinamento de deglutição nas condições nutricionais e pulmonares do indivíduo disfágico. Uma importante área da pesquisa, relacionada ao controle da eficiência e eficácia terapêutica, está nos estudos que objetivam estabelecer o grau de redução de custos hospitalares e em empresas de home care, mediante a atuação do fonoaudiólogo com a disfagia orofaríngea.

OBJETIVO: O Programa Saúde da Família propõe uma nova dinâmica para estruturação dos serviços de saúde, assim como para a relação com a comunidade e para diversos níveis de assistência. O objetivo do estudo foi analisar o significado da experiência do trabalho em equipe para os profissionais do Programa Saúde da Família. MÉTODOS: O estudo foi realizado no município de Conchas, Estado de São Paulo no ano de 2004. A abordagem utilizada foi qualitativa, na vertente da fenomenologia, buscando a essência da realidade vivenciada por oito profissionais de duas equipes do Programa Saúde da Família. RESULTADOS: Os temas revelaram que o trabalho em equipe se caracteriza por dedicação durante as atividades diárias. É necessário haver interação entre todos os membros para ações integrais, embora haja diferenças de ideologias e condutas entre os profissionais. O contato próximo com as famílias permitiu melhor intervenção nos problemas e o trabalho integrado é fundamental para atuação eficaz e de qualidade. CONCLUSÕES: O fenômeno desvelado engendra nova perspectiva de atuação para os profissionais e possibilita a compreensão do trabalho em equipe multiprofissional.

Num primeiro momento a criança se apropria do conhecimento por mediações externas, nas quais um objeto externo a ela, que pode ser um adulto, faz uma mediação entre ela e o conteúdo a ser adquirido. Assim, a hospitalização pode tornar-se um fator de risco no desenvolvimento psicológico da criança, tanto no âmbito cognitivo quanto no afetivo, caso suas potencialidades não sejam mediadas pela equipe de saúde, pois esta tem grande contato com a criança durante a internação. Foi investigado, como objetivo do estudo, o conhecimento do processo de desenvolvimento infantil por parte dos profissionais da saúde envolvidos no cuidado com crianças hospitalizadas, em um hospital-escola público do Estado de São Paulo. Participaram da pesquisa onze profissionais, entre técnicas e auxiliares de enfermagem, e os resultados indicam que estas consideram importantes alguns aspectos no cuidado com a criança, tais como estimulação da linguagem, atenção, brincadeira, vinculação, contato físico, porém não reconhecem estes aspectos como importantes para o desenvolvimento, e elegem profissionais específicos para tratarem de aspectos relacionados ao desenvolvimento infantil na internação, não se percebendo como mediadoras no processo de desenvolvimento psicossocial da criança hospitalizada.

O presente estudo visa a analisar sentidos pessoais e significações sociais das atividades de atenção em saúde mental desenvolvidas por profissionais integrantes de uma equipe de saúde da família. Parte-se, para tal, da perspectiva teórica da psicologia histórico-cultural de Vigotsky (1896-1934). O trabalho é parte de uma pesquisa participante e, portanto, é contextualizado na etapa de inserção no campo. Observou-se que a equipe considera relevante a determinação das condições de vida no processo saúde-doença da população atendida, a necessidade de lançar mão de estratégias diversificadas no cuidado para além da consulta, a importância de se cuidar da saúde mental da própria equipe, bem como dificuldades na abordagem da família. Indica-se a importância, para o trabalho cotidiano das equipes, das possibilidades de superação da exclusividade do núcleo biomédico na determinação do processo saúde-doença apontadas nos princípios operacionais da Estratégia de Saúde da Família, expressas na utilização do acolhimento como recurso de cuidado, a constituição de vínculos e responsabilização e a continuidade da atenção.

Background: Treatment of deep-vein thrombosis (DVT) with a once-daily regimen of enoxaparin, rather than a continuous infusion of unfractionated heparin (UFH) is more convenient and allows for home care in some patients. This study was designed to compare the efficacy and safety of these two regimens for the treatment of patients with proximal lower limb DVT. Methods: 201 patients with proximal lower limb DVT from 13 centers in Brazil were randomized in an open manner to receive either enoxaparin [1.5 mg/kg subcutaneous (s.c.) OD] or intravenous (i.v.) UFH (adjusted to aPTT 1.5-2.5 times control) for 5-10 days. All patients also received warfarin (INR 2-3) for at least 3 months. The primary efficacy endpoint Was recurrent DVT (confirmed by venography or ultrasonography), and safety endpoints included bleeding and serious adverse events. The rate of pulmonary embolism (PE) was also collected. Hospitalization was at the physician's discretion. Results: Baseline patient characteristics were comparable between groups. The duration of hospital stay was significantly shorter with enoxaparin than with UFH (3 versus 7 days). In addition, 36% of patients receiving enoxaparin did not need to be hospitalized, whereas all of the patients receiving UFH were! hospitalized. The treatment duration was slightly longer with enoxaparin (8 versus 7 days). There was a nonsignificant trend toward a reduction in the rate of recurrent DVT with enoxaparin versus UFH, and similar safety. Conclusions: A once-daily regimen of enoxaparin 1.5 mg/kg subcutaneous is at least as effective and safe as conventional treatment with a continuous intravenous infusion of UFH. However, the once daily enoxaparin regimen is easier to administer (subcutaneous versus intravenous), does not require aPTT monitoring, and leads to both a reduced number of hospital admissions and an average 4-day-shorter hospital stay. Intrauterine Dentistry is a highly relevant subject of our time. The use of preventive measures in the intrauterine stage can avoid several diseases, among these, dental caries. The WHO advises that from the 4th month of pregnancy, women should avoid the intake of sugar, so that the fetus, future child, does not develop an exaggerated attraction for these types of foods, thus being susceptible to caries. Through questionnaires sent to gynecologist-obstetricians and dentists, this research investigated the information they have about this subject and how they instruct their patients. Questionnaires were also sent to pregnant women requesting information about the instructions they had received for the prevention of oral diseases of their fetus. Seventy-one percent of the dentists and 80% of the gynecologist-obstetricians reported having instructed the pregnant women to reduce the intake of sugar. However, only 13.6% of the dentists and no gynecologist-obstetrician instructed the reduction of sugar intake between the 12th and 18th week of pregnancy. A total of 42.2% of the pregnant women referred to these instructions, but none received instruction as to the specific period of the 12th and 18th week. An ideal model of treatment for pregnant women must include integrated and multiprofessional treatment, in which general dentists and gynecologist-obstetricians work together with the participation of the patient.

PURPOSE: The objective of this study was to evaluate the viability of the microorganism Streptococcus mutans on toothbrushes made of opaque and transparent materials. METHODS: Twenty-eight toothbrushes (14 opaque and 14 transparent) were inoculated in tubes with brain heart infusion (BHI) broth of a standard strain of S. mutans and incubated in candle jars at 37 degrees C for 24 hours. Both the opaque and transparent toothbrushes were removed at T = 0 h (control); T = 0.5 h; T = 1 h; T = 2 h; T = 4 h; T = 8 h; and T = 24 h. Individual toothbrushes were subjected to agitation in a saline solution and samples of the solution were diluted and inoculated in Bacitracin Sucrose Agar--SB-20. RESULTS: After half an hour (T2) there was a significant decrease in the number of microorganisms on the transparent and opaque toothbrushes, respectively 6.0 x 10(5) and 9.4 x 10(5), when compared to the control. After the T3 = 1 hour, T4 = 2 hours, T5 = 4 h, the number of microorganisms decreased from 4.1 x 10(5); 2.1 x 10(5); 1.4 x 10(5); and 9.2 x 10(5); 5.7 x 10(5); 1.2 x 10(5) to zero (0.0) in T6 = 8 h, respectively on the transparent and opaque toothbrushes. The reduction in viable microorganisms was more obvious with the transparent toothbrushes, although the number of viable microorganisms was not significantly different for the two types of toothbrushes at the end of the experiment, T5 = 1.4 x 10(5) (transparent) and T5 = 1.2 x 10(5) (opaque). CONCLUSIONS: With both opaque and transparent toothbrushes, the number of microorganisms decreased with time. A reduction in the number of microorganisms on the transparent toothbrushes was observed following inoculation and incubation. This suggests the transparent toothbrushes inhibit the viability of the S. mutans.

The chronic obstructive lung disease is reviewed with emphasys on its epidemiology and risk factors. The diagnosis, clinical aspects pulmonary, functional alterations and laboratorial findings are discussed. The treatment is also reviewed, based on the actual consensus, considering the following classes of approaches: bronchodilators, inhaled β-agonists, corticoids, methilxanthines, prolonged domiciliar-orygen therapy.

PURPOSE: This review of the literature intends to evaluate the effect of brushes with high frequency motion when compared with manual toothbrushes regarding the indices of plaque and gingival bleeding. METHODS: Patients presenting gingivitis and/or chronic periodontitis were evaluated in addition to patients having osseointegrated implants and fixed orthodontic appliances. Pertinent literature was reviewed to select articles according to previously defined inclusion criteria. RESULTS: In the assessed studies results showed significant decreases in plaque and gingival indices by utilization of both types of brushes. However, in the selected studies where sonic brushes were tested in orthodontic and dental implant patients there was a more significant decrease in the indices. Furthermore, there was no indication of gingival recession attributed to product use. CONCLUSION: Future studies with a more homogeneous methodology and better experiment designs will be needed.

We report a case of a palatal calcified foreign body simulating an odontogenic lesion. Surgical exploration revealed a calcified mass that was analysed under light microscopy and identified as a vegetal foreign body. Further scanning electron microscopy analysis revealed that the foreign body was a piece of wood. Hard palate foreign bodies have been reported previously, however, it seems that this is the first case of its kind. © 2007 Nature Publishing Group.

Bone spreading technique (BST) is a horizontal augmentation with minimal trauma for simultaneous implant placement and an alternative to Summer's osteotome technique both for its clinical use and for the armamentarium. The foremost advantage of the crest dilation technique is a substantially less invasive method; the buccal wall expands after the medullary bone is compressed against the cortical bone. The lateral dilation and compaction of medullary bone improved primary stability. The vital difference is that the BST used in this case report avoided discomfort of the patient, thus eliminating the need for malleting.

Among the factors that influence the success of treatment of a root perforation, its location and possibility of contamination are determinant because the interaction of these 2 factors may result in significant periodontal injury. The management of cases of hard-to-reach contaminated perforations depends on the choice of an adequate technique. In the case reported in this article, controlled orthodontic tooth extrusion was successfully performed to treat gingival recession secondary to root perforation. The outcomes showed that this technique preserves the zone of attached gingiva, maintains the crown height, and prevents the involvement of the supporting bone tissue. The favorable clinical and radio?graphic conditions after 7 years of follow-up demonstrate the viability of this treatment approach.

The aim of this study was to evaluate alternative methods for the disinfection of toothbrushes considering that most of the previously proposed methods are expensive and cannot be easily implemented. Two-hundred toothbrushes with standardized dimensions and bristles were included in the study. The toothbrushes were divided into 20 experimental groups (n=10), according to microorganism considered and chemical agent used. The toothbrushes were contaminated in vitro by standardized suspensions of Streptococcus mutans, Streptococcus pyogenes, Staphylococcus aureus or Candida albicans. The following disinfectants were tested: 0.12% chlorhexidine digluconate, 50% white vinegar, a triclosan-containing dentifrice solution, and a perborate-based tablet solution. The disinfection method was immersion in the disinfectant for 10min. After the disinfection procedure, the number of remaining microbial cells was evaluated. The values of cfu/toothbrush of each group of microorganism after disinfection were compared by Kruskal-Wallis ANOVA and Dunn's test for multiple comparisons (5%). The chlorhexidine digluconate solution was the most effective disinfectant. The triclosan-based dentifrice solution promoted a significant reduction of all microorganisms' counts in relation to the control group. As to the disinfection with 50% vinegar, a significant reduction was observed for all the microorganisms, except for C. albicans. The sodium perborate solution was the less effective against the tested microorganisms. Solutions based on triclosan-containing dentifrice may be considered effective, nontoxic, cost-effective, and an easily applicable alternative for the disinfection of toothbrushes. The vinegar solution reduced the presence of S. aureus, S. mutans and S. pyogenes on toothbrushes.

The development of all-ceramic systems following metalceramics restorations allowed simulation of natural dentition due to favorable esthetics and resistance. In-Ceram is an alternative when esthetics is primordial as well as resistance required in rehabilitation. However, an ideal smile is associated to not only shape, color, texture and translucency but also harmony with gingival tissue. So, the aim of this study is to report a clinical case based on periodontal and fixed partial dentures principles to perform periodontal plastic surgery followed by esthetic rehabilitation. A female patient, 40-year-old, presented complaint about dental esthetics. After clinical and radiographic exams, metal-ceramics crowns (teeth 11, 12, 13, 21, 22 and 23) were considered unsatisfactory due to marginal leakage, color change in gingival tissue associated to metallic margin, and gummy smile. So, a crown lengthening surgery of anterior teeth was performed followed by rehabilitation of superior teeth with In-Ceram single crowns. Clinical significance: The interaction between periodontics and fixed prosthodontic area is the key of an adequated treatment planning which involves gingival smile to provide function and an esthetic condition in association with an esthetic, resistant and predictable material.
