BACKGROUND: No studies to date have investigated cumulative anticholinergic exposure and its effects in adults with intellectual disabilities. AIMS: To determine the cumulative exposure to anticholinergics and the factors associated with high exposure. METHOD: A modified Anticholinergic Cognitive Burden (ACB) scale score was calculated for a representative cohort of 736 people over 40 years old with intellectual disabilities, and associations with demographic and clinical factors assessed. RESULTS: Age over 65 years was associated with higher exposure (ACB 1-4 odds ratio (OR) = 3.28, 95% CI 1.49-7.28, ACB 5+ OR = 3.08, 95% CI 1.20-7.63), as was a mental health condition (ACB 1-4 OR = 9.79, 95% CI 5.63-17.02, ACB 5+ OR = 23.74, 95% CI 12.29-45.83). Daytime drowsiness was associated with higher ACB (P<0.001) and chronic constipation reported more frequently (26.6% ACB 5+ v. 7.5% ACB 0, P<0.001). CONCLUSIONS: Older people with intellectual disabilities and with mental health conditions were exposed to high anticholinergic burden. This was associated with daytime dozing and constipation.

Background: Compared to existing literature on childhood attention deficit hyperactivity disorder (ADHD), little published adult data are available, particularly outside of the United States. Using General Practitioner (GP) questionnaires from the United Kingdom, this study aimed to examine a number of issues related to ADHD in adults, across three cohorts of patients, adults who received ADHD drug treatment in childhood/adolescence but stopped prior to adulthood; adults who received ADHD drug treatment in childhood/adolescence and continued treatment into adulthood and adults who started ADHD drug treatment in adulthood.Methods: Patients with a diagnosis of ADHD and prescribed methylphenidate, dexamfetamine or atomoxetine were identified using data from The Health Improvement Network (THIN). Dates when these drugs started and stopped were used to classify patients into the three cohorts. From each cohort, 50 patients were randomly selected and questionnaires were sent via THIN to their GPs.GPs returned completed questionnaires to THIN who forwarded anonymised copies to the researchers. Datasets were analysed using descriptive statistics.Results: Overall response rate was 89% (133/150). GPs stated that in 19 cases, the patient did not meet the criteria of that group; the number of valid questionnaires returned was 114 (76%). The following broad trends were observed: 1) GPs were not aware of the reason for treatment cessation in 43% of cases, 2) patient choice was the most common reason for discontinuation (56%), 3) 7% of patients who stopped pharmacological treatment subsequently reported experiencing ADHD symptoms, 4) 58% of patients who started pharmacological treatment for ADHD in adulthood received pharmacological treatment for other mental health conditions prior to the ADHD being diagnosed.Conclusion: This study presents some key findings relating to ADHD; GPs were often not aware of the reason for patients stopping ADHD treatment in childhood or adolescence. Patient choice was identified as the most common reason for treatment cessation. For patients who started pharmacological treatment in adulthood, many patients received pharmacological treatment for comorbidities before a diagnosis of ADHD was made.

TRPV4 ion channels represent osmo-mechano-TRP channels with pleiotropic function and wide-spread expression. One of the critical functions of TRPV4 in this spectrum is its involvement in pain and inflammation. However, few small-molecule inhibitors of TRPV4 are available. Here we developed TRPV4-inhibitory molecules based on modifications of a known TRPV4-selective tool-compound, GSK205. We not only increased TRPV4-inhibitory potency, but surprisingly also generated two compounds that potently co-inhibit TRPA1, known to function as chemical sensor of noxious and irritant signaling. We demonstrate TRPV4 inhibition by these compounds in primary cells with known TRPV4 expression - articular chondrocytes and astrocytes. Importantly, our novel compounds attenuate pain behavior in a trigeminal irritant pain model that is known to rely on TRPV4 and TRPA1. Furthermore, our novel dual-channel blocker inhibited inflammation and pain-associated behavior in a model of acute pancreatitis - known to also rely on TRPV4 and TRPA1. Our results illustrate proof of a novel concept inherent in our prototype compounds of a drug that targets two functionally-related TRP channels, and thus can be used to combat isoforms of pain and inflammation in-vivo that involve more than one TRP channel. This approach could provide a novel paradigm for treating other relevant health conditions.

Over the last three decades, there has been a precipitous rise in curiosity regarding the clinical use of mindfulness meditation for the self-management of a broad range of chronic health conditions. Despite the ever-growing body of evidence supporting the use of mindfulness-based therapies for both medical and psychological concerns, data on the active ingredients of these mind-body interventions are relatively scarce. Regular engagement in formal mindfulness practice is considered by many to be requisite for generating therapeutic change; however, previous investigations of at-home practice in MBIs have produced mixed results. The equivocal nature of these findings has been attributed to significant methodological limitations, including the lack of standardized, systematic practice monitoring tools, and a singular focus on practice time, with little attention paid to the nature and quality of one’s practice. The present study used a prospective, observational design to assess the effects of home-based practice on dispositional mindfulness, self-compassion, and psychological functioning in twenty-eight people enrolled in an MBSR or MBCT program. To address some of the aforementioned limitations, the present study collected detailed weekly accounts of participants’ home-based practice engagement, including information about practice time (i.e., frequency and duration), exercise type, perceived effort and barriers to participation, and practice quality. Hierarchical multiple regression was used to examine the relative contribution of practice time and practice quality on treatment outcomes, and to explore possible predictors of adherence to at-home practice recommendations. As anticipated, practice quality and perceived effort improved with time; however, rather unexpectedly, practice quality was not a significant predictor of treatment-related improvements in psychological health. Home practice engagement, however, was predictive of change in dispositional mindfulness, in the expected direction. Results of our secondary analyses demonstrated that employment status was predictive of home practice engagement, with those who were unemployed completing more at-home practice on average. Mindfulness self-efficacy at baseline and previous experience with meditation or other contemplative practices were independently predictive of mean practice quality. The results of this study suggest that home practice helps generate meaningful change in dispositional mindfulness, which is purportedly a key mechanism of action in mindfulness-based interventions.

The skin is home to trillions of microbes, many of which are recently implicated in immune system regulation and various health conditions (33). The skin is continuously exposed to the outside environment, inviting microbial transfer between human skin and the people, animals, and surfaces with which an individual comes into contact. Thus, the aim of this study is to assess how different environmental exposures influence skin microbe communities, as this can strengthen our understanding of how microbial variation relates to health outcomes. This study investigated the skin microbial communities of humans and domesticated cattle living in rural Madagascar. The V3 region of the 16S rRNA gene was sequenced from samples of zebu (the domesticated cattle of Madagascar), zebu owners, and non-zebu owners. Overall, human armpits were the least diverse sample site, while ankles were the most diverse. The diversity of zebu samples was significantly different from armpits, irrespective of zebu ownership (one-way ANOVA and Tukey’s HSD, p<0.05). However, zebu owner samples (from the armpit, ankle forearm, and hand) were more similar to other zebu owner samples than they were to zebu, yet no more similar to other zebu owner samples than they were to non-zebu owner samples (unweighted UniFrac distances, p<0.05). These data suggest a lack of a microbial signature shared by zebu owners and zebu, though further taxonomic analysis is required to explain the role of additional environmental variables in dictating the microbial communities of various samples sites. Understanding the magnitude and directionality of microbial sharing has implications for a breadth of microbe-related health outcomes, with the potential to explain mosquito host preference and mitigate the threats of vector-borne diseases.

Background: As the global population is ageing, studying cognitive impairments including dementia, one of the leading causes of disability in old age worldwide, is of fundamental importance to public health. As a major transition in older age, a focus on the complex impacts of the duration, timing, and voluntariness of retirement on health is important for policy changes in the future. Longer retirement periods, as well as leaving the workforce early, have been associated with poorer health, including reduced cognitive functioning. These associations are hypothesized to differ based on gender, as well as on pre-retirement educational and occupational experiences, and on post-retirement social factors and health conditions. Methods: A cross-sectional study is conducted to determine the relationship between duration and timing of retirement and cognitive function, using data from the five sites of International Mobility in Aging Study (IMIAS). Cognitive function is assessed using the Leganes Cognitive Test (LCT) scores in 2012. Data are analyzed using multiple linear regressions. Analyses are also done by site/region separately (Canada, Latin America, and Albania). Robustness checks are done with an analysis of cognitive change from 2012 to 2014, the effect of voluntariness of retirement on cognitive function. An instrumental variable (IV) approach is also applied to the cross-sectional and longitudinal analyses as a robustness check to address the potential endogeneity of the retirement variable. Results: Descriptive statistics highlight differences between men and women, as well as between sites. In linear regression analysis, there was no relationship between timing or duration of retirement and cognitive function in 2012, when adjusting for site/region. There was no association between retirement characteristics and cognitive function in site/region/stratified analyses. In IV analysis, longer retirement and on time or late retirement was associated with lower cognitive function among men. In IV analysis, there is no relationship between retirement characteristics and cognitive function among women. Conclusions: While results of the thesis suggest a negative effect of retirement on cognitive function, especially among men, the relationship remains uncertain. A lack of power results in the inability to draw conclusions for site/region-specific analysis and site-adjusted analysis in both linear and IV regressions.

***********Amended Report, May 2, 2016*************** Promoting and protecting the health of Iowans is the mission of the Iowa Department of Public Health (IDPH). Surveillance of notifiable health conditions is essential in establishing what, how, and when events impact the public’s health. Once this information is gathered, public health and health care providers around the state are able to use this data to take steps to prevent illnesses from occurring. Multiple divisions and bureaus are dedicated to accomplishing the goals of surveillance. In 2014, there were more than 86,000 laboratory results of infectious diseases and conditions submitted to IDPH disease surveillance programs. IDPH also investigates non-infectious conditions related to lead, occupational, and environmental hazards like carbon monoxide. In 2014, approximately 71,000 children’s and more than 7,000 adults’ blood tests results were reported to IDPH.

This study aims to understand the changes in the improvement of economic and social conditions of small entrepreneurs who participate in solidarity groups linked to the Non-Governmental Organization (NGO) "X" in Fortaleza city, Ceará, through the use of productive-guided microcredit. It is come a research in exploratory and descriptive nature, with a quantitative and qualitative approach. Data were collected from the small entrepreneurs by applying a questionnaire, as well as through structured interviews with group leaders. They were worked on issues relating to income generation, employment generation, housing and health conditions, dietary pattern and leisure activities of small entrepreneurs and their families before and after the union to solidarity groups and use of productive-guided microcredit. The research showed that the use of microcredit has interfered with social and economic life of these small entrepreneurs, generating positive effects

For over a century, the Canadian state funded a church-run system of residential schools designed to assimilate Aboriginal children into Euro-Canadian culture. In addition to the problems associated with its ethnocentric philosophy, the school system was also characterised by terrible health conditions and physical and sexual abuse of the students was widespread. Recently, the schools have been the object of the most successful struggle for redress in Canadian history. One particularly puzzling aspect about the school system is that it persisted for so long, despite that many of its failings were known very early in its operation. In this article, this puzzle is addressed via a cultural analysis of a political struggle over the residential schools that occurred within Canadian Anglicanism at the outset of the twentieth century. The article concludes that the meaning of the school system as a sacred enterprise contributed to its persistence.

A esperança média de vida tem vindo a aumentar, resultando no envelhecimento da população mundial e, consequentemente, num aumento das populações com idades mais avançadas. Torna-se, por isso, importante estudar as especificidades do envelhecimento para que possamos trazer bem-estar e qualidade de vida a esta população que é cada vez mais numerosa, já que o envelhecimento traz mudanças a nível do corpo humano que se vão repercutir na saúde geral e na saúde oral. Os idosos apresentam, normalmente, pobres condições de saúde oral, sendo as doenças orais que mais acometem a esta população a perda dentária, a experiência de cárie, as altas taxas de prevalência de doença periodontal, a xerostomia e o pré-cancro/cancro oral. Além do envelhecimento populacional, têm sido notadas, ao longo do tempo, mudanças na estrutura familiar. Tudo isto leva a que o número de idosos institucionalizados aumente. Neste estudo foram utilizados dados, ainda não publicados, recolhidos no âmbito do projeto Sorrisos de Porta em Porta, que pertence à Mundo a Sorrir – Associação de Médicos Dentistas Portugueses. Este projeto visa a promoção de hábitos de saúde oral na população idosa e atua através da realização de ações de sensibilização subordinadas à temática da saúde oral no idoso e realização de rastreios de saúde oral aos idosos que se encontrem no âmbito de uma reposta social. Foram observados um total de 3586 idosos com idade igual ou superior a 65 anos, onde 70,3% eram do género masculino. A idade média (desvio padrão) encontrada foi 81,9 (±7,5) e a maioria referiu ser autónoma nos cuidados de higiene oral, no entanto, observou-se que grande parte dos idosos não realizava a escovagem diária e mais de metade destes disse não sentir necessidade de o fazer. Observou-se também, que apenas 13,7% tinham tido a sua última consulta de Medicina Dentária nos últimos 6 meses e a maioria disse visitar o Médico Dentista por razões de dor dentária. A média (desvio padrão) obtida para o Índice CPOd foi 26,3 (±8,4), sendo a componente “Perdidos” a mais significativa. Relativamente ao Índice de Placa, a maioria apresentava um acúmulo de placa bacteriana maior de 1/3 mas menor de 2/3. Quanto ao tipo de desdentação a maior percentagem era a de idosos desdentados parciais sem prótese. Foi também realizada uma pesquisa bibliográfica. Com este estudo concluiu-se que o estado de saúde oral dos idosos é bastante pobre consequência de uma pobre higiene oral e de falta de cuidados de saúde oral. Há uma grande necessidade de se instruir as pessoas relativamente à importância da Medicina Dentária e dos problemas que uma má saúde oral pode trazer para a saúde em geral.

A adesão à terapêutica medicamentosa por idosos é um fenômeno dependente de fatores diversificados. Defende-se como tese: A adesão aos medicamentos prescritos em idosos em atendimento ambulatorial apresenta-se relacionada aos fatores referentes às características: demográficas, socioeconômicas; dos serviços de saúde, dos profissionais de saúde; das condições de saúde; da terapêutica medicamentosa; comportamentais. Esses fatores estão relacionados aos principais motivos referidos pelos idosos para aderirem ou não aos medicamentos prescritos. Foram objetivos: identificar na literatura brasileira e estrangeira a prevalência de adesão à terapêutica medicamentosa e os fatores relacionados em idosos; caracterizar os idosos em atendimento ambulatorial em um hospital universitário no Rio Grande/RS, Brasil, quanto às características demográficas, socioeconômicas, condições de saúde e uso de medicamentos; identificar os motivos referidos por estes idosos que levavam à adesão/não adesão à terapêutica medicamentosa; verificar a prevalência de adesão à terapêutica medicamentosa nestes idosos; verificar se há associação entre adesão à terapêutica medicamentosa e fatores demográficos, socioeconômicos, condições de saúde, terapêutica medicamentosa e fatores comportamentais destes idosos. Pesquisa realizada por meio de uma revisão integrativa da literatura e de um estudo quantitativo. Na revisão integrativa selecionaram-se 49 artigos e os fatores identificados foram organizados nas categorias: demográficas e socioeconômicas; sistema e profissionais de saúde; condições de saúde; terapêutica medicamentosa; comportamentais. O estudo quantitativo foi exploratório, descritivo, transversal, realizado em um serviço ambulatorial de um hospital universitário no Rio Grande/RS, Brasil. Participaram 107 idosos que responderam ao instrumento para caracterização do idoso e dos fatores relacionados à adesão à terapêutica medicamentosa; ao Miniexame do Estado Mental; à Escala de Medida de Adesão aos Tratamentos. A coleta de dados foi realizada em novembro de 2013. Realizou-se análise estatística descritiva e inferencial. Verificaram-se mais idosos do sexo feminino, na faixa etária entre 60-69 anos. A doença mais prevalente foi a Hipertensão Arterial e a média de uso de medicamentos por dia foi de 4,8. Querer sentir-se bem/manter a saúde e querer controlar a doença e os sintomas foram os motivos para aderir à terapêutica medicamentosa prescrita. A ocorrência de reação adversa e falta de condições financeiras foram os motivos para não aderir. A prevalência de adesão à terapêutica medicamentosa foi de 86,9%. Houve associação entre a adesão e receber orientações do médico sobre como tomar os medicamentos, ter reação adversa, acreditar que os medicamentos são importantes para manutenção da saúde e ter vontade de não tomar os medicamentos. A tese é confirmada em parte: a adesão aos medicamentos prescritos em idosos em atendimento ambulatorial apresenta-se relacionada aos fatores referentes às características: dos profissionais de saúde; das condições de saúde; da terapêutica medicamentosa; comportamentais. Esses fatores estiveram relacionados aos principais motivos referidos pelos idosos para aderirem ou não aos medicamentos prescritos. Por outro lado, a adesão não apresentou relação com os fatores demográficos e socioeconômicos, embora as condições financeiras tenham sido referidas pelos idosos como um motivo que leva à não adesão.

Wild berries are fundamental components of traditional diet and medicine for Native American and Alaska Native tribes and contain a diverse array of phytochemicals, including anthocyanins and proanthocyanidins, with known efficacy against metabolic disorders. Bioexploration represents a new paradigm under which bioactive preparations are screened in coordination with indigenous communities, to prepare for subsequent in-depth chemical and biological analysis. The inclusive, participatory philosophical approach utilized in bioexploration has additional benefits that could be realized in seemingly disparate areas, such as education and economics. Five species of wild Alaskan berries (Vaccinium uliginosum, V. ovalifolium, Empetrum nigrum, Rubus chamaemorus, and R. spectabilis) were tested using “Screens-to-Nature” (STN), a community-participatory approach to screen for potential bioactivity, in partnership with tribal members from three geographically distinct Alaskan villages: Akutan, Seldovia, and Point Hope. Berries were subsequently evaluated via HPLC and LC-MS2, yielding significant species and location-based variation in anthocyanins (0.9-438.6 mg eq /100g fw) and proanthocyanins (73.7-625.2 mg eq /100g fw). A-type proanthocyanidin dimers through tetramers were identified in all species tested. Berries were analyzed for in vitro and in vivo activity related to diabetes and obesity. R. spectabilis samples increased preadipocyte-factor-1 levels by 82% over control, and proanthocyanidin-rich fractions from multiple species reduced lipid accumulation in 3T3-L1 adipocytes. Furthermore, extracts of V. uliginosum and E. nigrum (Point Hope) reduced serum glucose levels in C57bl/6j mice up to 45%. The same precepts of bioexploration, especially the inclusion of indigenous community perspectives and knowledge, have relevance in other areas of study, such as education and economics. Studies have established the apathetic, low-motivational environment characteristic of many introductory science laboratory classes is detrimental to student interest, learning, and continuation in scientific education. A primary means of arresting this decline and stimulating the students’ attention and excitement is via engagement in hands-on experimentation and research. Using field workshops, the STN system is investigated as to its potential as a novel participatory educational tool, using assays centered around bioexploration and bioactive plant compounds that hold the potential to offset human health conditions. This evaluation of the STN system provided ample evidence as to its ability to augment and improve science education. Furthermore, Strengths, Weaknesses, Opportunities, and Threats (SWOT) analysis was employed as a theoretical framework to review the potential benefits and hurdles associated with developing a wild Alaskan berry commodity. Synthesizing various sources of information – including logistics and harvest costs, sources of initial capital, opportunities in the current superfruit industry, and socioeconomic factors – the development of a berry commodity proves to be a complex amalgam of competing factors which would require a delicate balance before proceeding.

This condition can ultimately lead to the development of several health conditions. This study aimed to characterize the occupational exposure to fungal burden in three different settings: swine feed unit, swine units and slaughterhouse.

Objetivo: Investigar o impacto da saúde bucal em relação à qualidade de vida de adolescentes escolares, associando-o às condições sociodemográficas. Métodos: Estudo de campo transversal e quantitativo desenvolvido em 2012, no município de Sumé-PB, com 184 adolescentes na faixa etária de 15 a 19 anos. Para avaliar o impacto, aplicou-se o questionário Oral Health Impact Profile (OHIP-14) aos participantes, enquanto para a obtenção dos dados referentes às condições sociodemográficas, os pais ou responsáveis responderam a um segundo questionário. Utilizou-se o teste Qui-quadrado para associar o impacto da saúde bucal sobre a qualidade de vida e as variáveis sociodemográficas pesquisadas, sendo considerados significativos com p<0,05. Resultados: Em geral, o impacto foi considerado fraco em 167 pesquisados (90,8%). “Dor física” foi a dimensão na qualidade de vida mais afetada pelas questões bucais entre aquelas que resultaram em impacto médio (22,8%; n=42). Apenas as variáveis “Situação do imóvel” e “Acomodação” associaram-se ao impacto geral (p<0,05). Os reduzidos percentuais de impacto geral forte (1,1%; n=2) relacionaram-se aos adolescentes cujas mães só estudaram até o ensino fundamental, ou às famílias que vivem com um salário mínimo ou menos (1,1%). Conclusão: Observou-se que as condições de saúde bucal apresentaram um impacto negativo fraco na qualidade de vida dos adolescentes investigados. As análises das condições sociodemográficas dos indivíduos relacionadas ao impacto geral da qualidade de vida relacionada à saúde oral associaram-se as variáveis “Situação do imóvel” e “Acomodação”.
