FUNDAMENTO: Alterações cardíacas são muito frequentes nos indivíduos portadores de doença renal crônica terminal e estão associadas com a morbimortalidade. OBJETIVOS: Avaliar as alterações evolutivas cardíacas após o transplante renal. MÉTODOS: Foram avaliados prospectivamente 40 pacientes com doença renal crônica terminal, imediatamente antes e com um mês, três meses e seis meses após o transplante renal, por meio de estudo ecocardiográfico com Doppler tecidual. Os parâmetros de massa ventricular, função sistólica e diastólica foram analisados. RESULTADOS: A média da idade foi 31,6 anos e 40% eram do sexo feminino. Observamos redução do diâmetro diastólico do VE (52,23 para 49,95 mm, p = 0,021) e do índice de massa do VE (131,48 para 113,039 g/m², p = 0,002) após o transplante renal. A razão E/e' média reduziu no terceiro e no sexto meses após o transplante renal em relação ao exame basal (8,13 e 7,85 vs 9,79, p < 0,05). A fração de ejeção aumentou a partir do primeiro mês do transplante renal em relação a exame basal (69,72% vs 65,68%, p < 0,05). A prevalência de disfunção diastólica reduziu 43% no período avaliado. A fração de ejeção e a razão E/e' média basais estiveram associadas com redução do índice de massa do VE após o transplante renal. O índice de massa do VE basal, o sexo feminino e a redução do fósforo sérico apresentaram associação com a redução da razão E/e' média após o transplante renal. CONCLUSÃO: O transplante renal promoveu alterações significativas nos parâmetros ecodopplercardiográficos de massa do VE, função sistólica e função diastólica nos pacientes com doença renal crônica terminal.

FUNDAMENTO: Dados sobre a avaliação não invasiva vascular e suas relações com variáveis de risco cardiovascular são escassos em jovens. OBJETIVO: Avaliar a relação entre a velocidade de onda de pulso e a pressão arterial,variáveis antropométricas e metabólicas, incluindo as adipocitocinas, em indivíduos adultos jovens. MÉTODOS: Foram avaliados 96 indivíduos (51 homens) do estudo do Rio de Janeiro, de 26 a 35 anos (média 30,09 ± 1,92). Foram obtidos a velocidade de onda de pulso (método Complior), pressão arterial, índice de massa corporal, glicose, perfil lipídico, leptina, insulina, adiponectina e o índice de resistência à insulina HOMA-IR. Os indivíduos foram estratificados em três grupos segundo o tercil da VOP para cada sexo. RESULTADOS: O grupo com maior tercil de VOP mostrou maiores médias de pressão arterial sistólica, pressão arterial diastólica, pressão arterial média, índice de massa corporal, insulina, HOMA-IR e menores médias de adiponectina, além de maiores prevalências de diabetes mellitus/intolerância à glicose e hiperinsulinemia. Houve correlação significativa e positiva da velocidade da onda de pulso com pressão arterial sistólica, pressão arterial diastólica, pressão de pulso e pressão arterial média, índice de massa corporal, e LDL-colesterol e negativa com HDL-colesterol e adiponectina. Em modelo de regressão múltipla, após ajuste do HDL-colesterol, LDL-colesterol e adiponectina para sexo, idade, índice de massa corporal e pressão arterial média, apenas o sexo masculino e a pressão arterial média mantiveram correlação significativa com a velocidade de onda de pulso. CONCLUSÃO: A velocidade de onda de pulso em adultos jovens mostrou relação significativa com variáveis de risco cardiovascular, destacando-se o sexo masculino e a pressão arterial média como importantes variáveis no seu determinismo. Os achados sugerem que a medida da VOP pode ser útil para a identificação do acometimento vascular nessa faixa etária.

Diversos estudos relataram os benefícios dos betabloqueadores (BB) para pacientes com insuficiência cardíaca sistólica. No entanto, muitos pacientes hospitalizados em decorrência de insuficiência cardíaca aguda já estão usando os BB e requerem dobutaminas para hipotensão arterial e baixo débito cardíaco. Portanto, deve-se tomar uma decisão a respeito de se o BB deve ser mantido ou até mesmo iniciado nesses casos. O objetivo deste estudo foi determinar se há provas que sustentem a segurança e a eficácia dos BB junto com a dobutamina para pacientes com insuficiência cardíaca aguda descompensada (ICAD). Foi realizada uma pesquisa na literatura de língua inglesa nas bases de dados MEDLINE, ISI Web of Science, Biblioteca Virtual em Saúde, Cochrane Library e o Portal de Revistas Científicas do Capes para identificar estudos relacionados. Literatura adicional foi obtida mediante a análise das respectivas referências encontradas nos artigos identificados. Os resultados esperados incluíram informações sobre o prognóstico (intra-hospitalar e na mortalidade no acompanhamento, número de dias de internação e reinternação), eficácia e segurança (agravamento dos sintomas, choque, intolerância) do uso concomitante desses medicamentos em pacientes hospitalizados com ICAD e baixo débito cardíaco. Esta análise incluiu nove estudos. No entanto, não foram encontrados ensaios clínicos randomizados sobre o assunto. A maioria dos estudos inclui baixo número de pacientes, e não foram encontrados estudos que abordem a segurança do uso concomitante desses medicamentos. Os dados resultantes sugerem que uma cuidadosa revisão da literatura não forneceu evidências para o uso sistemático de BB em pacientes com síndrome de baixo débito cardíaco que necessitam de suporte inotrópico com dobutamina.

Hipertensão resistente (HAR) é definida como a pressão arterial que permanece acima da meta pressórica, apesar do uso de três classes de anti-hipertensivos em doses otimizadas, sendo um deles um diurético. Além disso, são considerados também hipertensos resistentes os pacientes que usam quatro ou mais classes e possuem suas pressões controladas. Embora essa definição seja útil na categorização de um maior grupo de resistentes, visto esses dois subgrupos compartilharem alto risco cardiovascular, algumas importantes particularidades clínicas e fisiopatológicas necessitam ser mais bem avaliadas antes de resistentes controlados e não controlados pertencerem ao mesmo grupo. Foram comparadas algumas características cardiovasculares nesses dois subgrupos de hipertensos resistentes. Embora algumas semelhanças, o subgrupo HRNC apresenta fenótipos cardiovasculares com pior prognóstico como maior rigidez vascular e hipertrofia ventricular esquerda, além de função endotelial mais prejudicada e menor queda de pressão arterial no período noturno, entre outras. Frente às diferenças, o subgrupo HRNC está associado à maior risco cardiovascular, podendo ser considerado mais resistente ao tratamento anti-hipertensivo. Além da importância de melhor prevenção e tratamento da HAR com medidas de identificar precocemente os fatores de risco e otimizar a terapia farmacológica, algumas implicações clínicas devem ser consideradas na abordagem de pacientes controlados e não controlados como semelhantes ao grupo de resistentes.

Os autores revisam o conceito de hipertensão arterial resistente e o envolvimento do sistema nervoso simpático na hipertensão arterial como base racional para a técnica de desnervação simpática renal (DSR), realizada por via percutânea. A presente revisão é resultado de uma busca ativa de artigos científicos com o nome "renal denervation" na base de dados Medline e PubMed. A técnica e os dispositivos empregados no procedimento são apresentados, bem como os resultados clínicos em seis, 12 e 24 meses após a intervenção com o cateter Symplicity. Observam-se reduções expressivas e progressivamente maiores da PAS e PAD após a DSR. A taxa de complicações foi mínima. Novos dispositivos para a DSR e respectivos estudos clínicos em andamento são citados. Em conclusão, a DSR apresenta-se como um procedimento efetivo e seguro para a abordagem da hipertensão arterial resistente. Resultados de estudos em andamento e tempo maior de seguimento desses pacientes são aguardados para confirmar os resultados iniciais e colocar em perspectiva a ampliação da utilização do procedimento na abordagem da hipertensão arterial.

Background: The classification or index of heart failure severity in patients with acute myocardial infarction (AMI) was proposed by Killip and Kimball aiming at assessing the risk of in-hospital death and the potential benefit of specific management of care provided in Coronary Care Units (CCU) during the decade of 60. Objective: To validate the risk stratification of Killip classification in the long-term mortality and compare the prognostic value in patients with non-ST-segment elevation MI (NSTEMI) relative to patients with ST-segment elevation MI (STEMI), in the era of reperfusion and modern antithrombotic therapies. Methods: We evaluated 1906 patients with documented AMI and admitted to the CCU, from 1995 to 2011, with a mean follow-up of 05 years to assess total mortality. Kaplan-Meier (KM) curves were developed for comparison between survival distributions according to Killip class and NSTEMI versus STEMI. Cox proportional regression models were developed to determine the independent association between Killip class and mortality, with sensitivity analyses based on type of AMI. Results: The proportions of deaths and the KM survival distributions were significantly different across Killip class >1 (p <0.001) and with a similar pattern between patients with NSTEMI and STEMI. Cox models identified the Killip classification as a significant, sustained, consistent predictor and independent of relevant covariables (Wald χ2 16.5 [p = 0.001], NSTEMI) and (Wald χ2 11.9 [p = 0.008], STEMI). Conclusion: The Killip and Kimball classification performs relevant prognostic role in mortality at mean follow-up of 05 years post-AMI, with a similar pattern between NSTEMI and STEMI patients.

Background: Studies on atrial fibrillation (AF) in decompensated heart failure (DHF) are scarce in Brazil. Objectives: To determine AF prevalence, its types and associated factors in patients hospitalized due to DHF; to assess their thromboembolic risk profile and anticoagulation rate; and to assess the impact of AF on in-hospital mortality and hospital length of stay. Methods: Retrospective, observational, cross-sectional study of incident cases including 659 consecutive hospitalizations due to DHF, from 01/01/2006 to 12/31/2011. The thromboembolic risk was assessed by using CHADSVASc score. On univariate analysis, the chi-square, Student t and Mann Whitney tests were used. On multivariate analysis, logistic regression was used. Results: The prevalence of AF was 40%, and the permanent type predominated (73.5%). On multivariate model, AF associated with advanced age (p < 0.0001), non-ischemic etiology (p = 0.02), right ventricular dysfunction (p = 0.03), lower systolic blood pressure (SBP) (p = 0.02), higher ejection fraction (EF) (p < 0.0001) and enlarged left atrium (LA) (p < 0.0001). The median CHADSVASc score was 4, and 90% of the cases had it ≥ 2. The anticoagulation rate was 52.8% on admission and 66.8% on discharge, being lower for higher scores. The group with AF had higher in-hospital mortality (11.0% versus 8.1%, p = 0.21) and longer hospital length of stay (20.5 ± 16 versus 16.3 ± 12, p = 0.001). Conclusions: Atrial fibrillation is frequent in DHF, the most prevalent type being permanent AF. Atrial fibrillation is associated with more advanced age, non-ischemic etiology, right ventricular dysfunction, lower SBP, higher EF and enlarged LA. Despite the high thromboembolic risk profile, anticoagulation is underutilized. The presence of AF is associated with longer hospital length of stay and high mortality.

Background:Studies show an association between changes in apolipoprotein E (ApoE) and LDLR receptor with the occurrence of dyslipidemia.Objectives:To investigate the association between polymorphisms of the APOE (ε2, ε3, ε4) and LDLR (A370T) genes with the persistence of abnormal serum lipid levels in young individuals followed up for 17 years in the Rio de Janeiro Study.Methods:The study included 56 individuals (35 males) who underwent three assessments at different ages: A1 (mean age 13.30 ± 1.53 years), A2 (22.09 ± 1.91 years) and A3 (31.23 ± 1.99 years). Clinical evaluation with measurement of blood pressure (BP) and body mass index (BMI) was conducted at all three assessments. Measurement of waist circumference (WC) and serum lipids, and analysis of genetic polymorphisms by PCR-RFLP were performed at A2 and A3. Based on dyslipidemia tracking, three groups were established: 0 (no abnormal lipid value at A2 and A3), 1 (up to one abnormal lipid value at A2 or A3) and 2 (one or more abnormal lipid values at A2 and A3).Results:Compared with groups 0 and 1, group 2 presented higher mean values of BP, BMI, WC, LDL-c and TG (p < 0.01) and lower mean values of HDL-c (p = 0.001). Across the assessments, all individuals with APOE genotypes ε2/ε4 and ε4/ε4 maintained at least one abnormal lipid variable, whereas those with genotype ε2/ε3 did not show abnormal values (χ2 = 16.848, p = 0.032). For the LDLR genotypes, there was no significant difference among the groups.Conclusions:APOE gene polymorphisms were associated with dyslipidemia in young individuals followed up longitudinally from childhood.

AbstractIntroduction:Coronary computed tomography angiography (CCTA) allows for non-invasive coronary artery disease (CAD) phenotyping. There are still some uncertainties regarding the impact this knowledge has on the clinical care of patients.Objective:To determine whether CAD phenotyping by CCTA influences clinical decision making by the prescription of cardiovascular drugs and their impact on non-LDL cholesterol (NLDLC) levels.Methods:We analysed consecutive patients from 2008 to 2011 submitted to CCTA without previous diagnosis of CAD that had two serial measures of NLDLC, one up to 3 months before CCTA and the second from 3 to 6 months after.Results:A total of 97 patients were included, of which 69% were men, mean age 64 ± 12 years. CCTA revealed that 18 (18%) patients had no CAD, 38 (39%) had non-obstructive (< 50%) lesions and 41 (42%) had at least one obstructive ≥ 50% lesion. NLDLC was similar at baseline between the grups (138 ± 52 mg/dL vs. 135 ± 42 mg/dL vs. 131 ± 44 mg/dL, respectively, p = 0.32). We found significative reduction in NLDLC among patients with obstrctive lesions (-18%, p = 0.001). We also found a positive relationship between clinical treatment intensification with aspirin and cholesterol reducing drugs and the severity of CAD.Conclusion:Our data suggest that CCTA results were used for cardiovascular clinical treatment titration, with especial intensification seen in patients with obstructive ≥50% CAD.

Abstract Background: Functional tests have limited accuracy for identifying myocardial ischemia in patients with left bundle branch block (LBBB). Objective: To assess the diagnostic accuracy of dipyridamole-stress myocardial computed tomography perfusion (CTP) by 320-detector CT in patients with LBBB using invasive quantitative coronary angiography (QCA) (stenosis ≥ 70%) as reference; to investigate the advantage of adding CTP to coronary computed tomography angiography (CTA) and compare the results with those of single photon emission computed tomography (SPECT) myocardial perfusion scintigraphy. Methods: Thirty patients with LBBB who had undergone SPECT for the investigation of coronary artery disease were referred for stress tomography. Independent examiners performed per-patient and per-coronary territory assessments. All patients gave written informed consent to participate in the study that was approved by the institution’s ethics committee. Results: The patients’ mean age was 62 ± 10 years. The mean dose of radiation for the tomography protocol was 9.3 ± 4.6 mSv. With regard to CTP, the per-patient values for sensitivity, specificity, positive and negative predictive values, and accuracy were 86%, 81%, 80%, 87%, and 83%, respectively (p = 0.001). The per-territory values were 63%, 86%, 65%, 84%, and 79%, respectively (p < 0.001). In both analyses, the addition of CTP to CTA achieved higher diagnostic accuracy for detecting myocardial ischemia than SPECT (p < 0.001). Conclusion: The use of the stress tomography protocol is feasible and has good diagnostic accuracy for assessing myocardial ischemia in patients with LBBB.

Background: Heart failure prediction after acute myocardial infarction may have important clinical implications. Objective: To analyze the functional echocardiographic variables associated with heart failure in an infarction model in rats. Methods: The animals were divided into two groups: control and infarction. Subsequently, the infarcted animals were divided into groups: with and without heart failure. The predictive values were assessed by logistic regression. The cutoff values predictive of heart failure were determined using ROC curves. Results: Six months after surgery, 88 infarcted animals and 43 control animals were included in the study. Myocardial infarction increased left cavity diameters and the mass and wall thickness of the left ventricle. Additionally, myocardial infarction resulted in systolic and diastolic dysfunction, characterized by lower area variation fraction values, posterior wall shortening velocity, E-wave deceleration time, associated with higher values of E / A ratio and isovolumic relaxation time adjusted by heart rate. Among the infarcted animals, 54 (61%) developed heart failure. Rats with heart failure have higher left cavity mass index and diameter, associated with worsening of functional variables. The area variation fraction, the E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate were functional variables predictors of heart failure. The cutoff values of functional variables associated with heart failure were: area variation fraction < 31.18%; E / A > 3.077; E-wave deceleration time < 42.11 and isovolumic relaxation time adjusted by heart rate < 69.08. Conclusion: In rats followed for 6 months after myocardial infarction, the area variation fraction, E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate are predictors of heart failure onset.

Abstract Background: Carotid intima-media thickness (CIMT) has been shown to be increased in children and adolescents with traditional cardiovascular risk factors such as obesity, hypertension, and chronic kidney disease, compared with those of healthy children. Objective: To assess the influence of sex, age and body mass index (BMI) on the CIMT in healthy children and adolescents aged 1 to 15 years. Methods: A total of 280 healthy children and adolescents (males, n=175; mean age, 7.49±3.57 years; mean BMI, 17.94±4.1 kg/m2) were screened for CIMT assessment. They were divided into 3 groups according to age: GI, 1 to 5 years [n=93 (33.2%); males, 57; mean BMI, 16±3 kg/m2]; GII, 6 to 10 years [n=127 (45.4%); males, 78; mean BMI, 17.9±3.7 kg/m2], and GIII, 11 to 15 years [n=60 (21.4%); males, 40; mean BMI, 20.9±4.5 kg/m2]. Results: There was no significant difference in CIMT values between male and female children and adolescents (0.43±0.06 mm vs. 0.42±0.05 mm, respectively; p=0.243). CIMT correlated with BMI neither in the total population nor in the 3 age groups according to Pearson correlation coefficient. Subjects aged 11 to 15 years had the highest CIMT values (GI vs. GII, p=0.615; GI vs. GIII, p=0.02; GII vs. GIII, p=0.004). Conclusions: CIMT is constant in healthy children younger than 10 years, regardless of sex or BMI. CIMT increases after the age of 10 years.

Metamysidopsis elongata atlantica (Bacescu, 1968) was reared in the laboratory for 45 days at 20±1°C and salinity of 30ppt. Growth curves (von Bertalanffy model) were calculated for both sexes and for each sex. The daily rate of carapace growth was significantly different between females and males (F test, p <0.05). Before the sexual maturity (14 days), the growth rate of females was higher than that of males (females, 0.0457 mm day-1; males, 0.0448 mm day-1). After the maturity (15 to 45 days), these rates decreased similarly for both sexes (females, 0.0203 mm day-1; males, 0.0174mm day-1). The average growth rate was 0.0207mm day-1 over the 45 days. Twelve molts were observed in a period of 60 days. The first five molts occurred up to 14 days old (age of the sexual differentiation), with a mean intermolt period of 2.9 days. From the 6th molt it increased to 5.6 days. The results suggest that the use of the carapace length is a good measure to calculate the growth and longevity of the organisms.

The present paper aims to describe the temporal and spatial distribution of the composition and abundance of Decapoda larvae in the shallow waters around Arvoredo Marine Biological Reserve. Stomatopod occurrence is also discussed. Plankton samples were collected at five sites around the Arvoredo Island every two months for one year from May, 2002 to April, 2003. Thirty-nine morphotypes, 11 genus and 4 species (Artemesia longinaris Bate, 1888, Hexapanopeus schmitii Rathbun, 1930, Menippe nodifrons Stimpson, 1859 and Pleoticus muelleri Bate, 1888) were identified, among them only two morphotypes of Stomatopoda larvae, and the remainder Decapoda larvae. Brachyuran zoeae were the most abundant group and they were well represented by Portunidae and Xanthidae zoeae. Lucifer sp. and Caridea zoeae were the most abundant non-brachyuran taxa. Decapod larvae were observed to occur at all sampling sites, however the spatial distribution demonstrated a general tendency to greater abundance and diversity at the southern sites of the Island. Decapoda and Stomatopoda larvae occurred throughout the year, showing that reproduction is continuous, but that larval input in planktonic community was significantly higher during autumn and spring.
