Aim: To assess if the intake of levodopa in patients with Parkinson’s Disease (PD) changes cerebral connectivity, as revealed by simultaneous recording of hemodynamic (functional MRI, or fMRI) and electric (electroencephalogram, EEG) signals. Particularly, we hypothesize that the strongest changes in FC will involve the motor network, which is the most impaired in PD. Methods: Eight patients with diagnosis of PD “probable”, therapy with levodopa exclusively, normal cognitive and affective status, were included. Exclusion criteria were: moderate-severe rest tremor, levodopa induced dyskinesia, evidence of gray or white matter abnormalities on structural MRI. Scalp EEG (64 channels) were acquired inside the scanner (1.5 Tesla) before and after the intake of levodopa. fMRI functional connectivity was computed from four regions of interest: right and left supplementary motor area (SMA) and right and left precentral gyrus (primary motor cortex). Weighted partial directed coherence (w-PDC) was computed in the inverse space after the removal of EEG gradient and cardioballistic artifacts. Results and discussion: fMRI group analysis shows that the intake of levodopa increases hemodynamic functional connectivity among the SMAs / primary motor cortex and: sensory-motor network itself, attention network and default mode network. w-PDC analysis shows that EEG connectivity among regions of the motor network has the tendency to decrease after the intake the levodopa; furthermore, regions belonging to the DMN have the tendency to increase their outflow toward the rest of the brain. These findings, even if in a small sample of patients, suggest that other resting state physiological functional networks, beyond the motor one, are affected in patients with PD. The behavioral and cognitive tasks corresponding to the affected networks could benefit from the intake of levodopa.

Nel presente lavoro di tesi, in seguito ad acquisizioni di dati effettuate nella sala prove del "Laboratorio di Macchine e Propulsione" della Scuola di Ingegneria e Architettura di Forlì sul turboshaft Allison 250 C18, in una prima fase sono state ricavate le mappe prestazionali dei singoli componenti del motore, elaborando i dati sperimentali in ambiente MatLab. Le acquisizioni sono state effettuate mediante l'utilizzo di sensori di pressione, temperatura e velocità installati in precedenza sul motore e opportunamente calibrati. In seguito alla realizzazione delle mappe prestazionali, si è passati all'allestimento completo di un modello dinamico in ambiente Simulink, del motore Allison 250 C18. Tale modello riproduce, in opportuni blocchi, ciascun componente facente parte della motorizzazione. Ogni blocco riceve in ingresso le caratteristiche fisiche di interesse del flusso (temperatura, pressione, calore specifico a pressione costante e gamma) provenienti dal blocco precedente tramite un "filo", le rielabora al suo interno risolvendo le equazioni tipiche di ciascun componente e interpolando le mappe di prestazione ricavate precedentemente in MatLab, e le invia come input al blocco successivo o in retroazione ai blocchi precedenti. In ogni blocco è stato realizzato un sistema di dinamica di pressione che, ad ogni istante, risolve un'equazione differenziale dipendente dalla differenza di portata a monte e a valle di un componente e dal volume di controllo, che restituisce il valore di pressione in uscita proporzionale alla variazione di portata stessa. Nel presente lavoro di tesi si è cercato di stabilizzare questo complesso sistema in una condizione di progetto, fissata a 30000 rpm del gruppo gas generator. Un sistema di controllo del numero di giri tramite variazione di portata di combustibile è stato realizzato al fine di poter, in futuro, far funzionare il modello anche fuori dalla condizione di progetto e riuscire a simulare l'andamento delle prove sperimentali reali.

La dialisi è una terapia essenziale alla sopravvivenza delle persone colpite da insufficienza renale. Senza questo trattamento periodico l’aspettativa di vita si riduce a pochi giorni. Il fine di questo trattamento è il ripristino dell’equilibrio idro-elettrolitico e acido-base del paziente e la rimozione del fluido in eccesso che l’organismo non è in grado di espellere autonomamente. La parte di liquido che passa attraverso il filtro dal lato sangue, alla parte di bagno dialisi prende il nome di ultrafiltrato. Ciò comporta che nelle macchine per emodialisi ci sia un sistema di controllo dedicato all’ultrafiltrazione. La durata della seduta è variabile, è finalizzata a riportare il paziente al peso ottimale in un tempo limitato. Il seguente elaborato è stato svolto presso il laboratorio della start-up IBD Italian Biomedical Devices, uno dei progetti in via di sviluppo è la realizzazione di una macchina per emodialisi a basso costo. Obiettivo della tesi è stato quello di confrontare due misuratori di portata e valutarne l’adeguatezza per la gestione del sistema di ultrafiltrazione. È stato esplorato tutto il range di portate a cui potrebbero essere sottoposti durante l’utilizzo del macchinario. I sensori oggetto di studio sono l’Oval Gears Flowmeters ed il flussimetro Series 800, entrambi adatti all’impiego in ambito biomedicale. Appartengono a due diverse classi: il primo è un contatore volumetrico, registra il numero di volumi base lo che attraversano. Il secondo è un misuratore a turbina, genera impulsi verso l’esterno in numero proporzionale alla velocità del fluido. Lo studio necessita anche di una parte di acquisizione dei segnali provenienti dai sensori, è stata implementata interamente sulla piattaforma Arduino. I dati acquisiti sono stati elaborati ed analizzati al fine di avere un confronto tra i misuratori e con il gold-standard. Per poter confrontare le prestazioni è stato realizzato e seguito un protocollo sperimentale.

Il Lago di Cavazzo (o dei Tre Comuni) è il più esteso lago naturale del Friuli Venezia Giulia. Situato nelle Prealpi Carniche, in provincia di Udine, è ubicato in un’antica valle scavata in epoca pre – glaciale dal fiume Tagliamento, che oggi scorre circa 3 km ad Est. A partire dagli anni ’40, la S.A.D.E. (ora Edipower) ottenne le concessioni per la costruzione di una serie di impianti idroelettrici in tutto il Friuli Venezia Giulia che nel 1954 portò alla realizzazione della Centrale idroelettrica di Somplago, costruita in caverna lungo la sponda Nord – occidentale del lago. La Centrale turbina le acque di scarico provenienti dai bacini di accumulo superiori («Lumiei», «Ambiesta») e da altre prese minori sul Tagliamento, immettendo a sua volta le acque turbinate all’interno del lago di Cavazzo tramite galleria. Dai dati disponibili in letteratura, dalle cronache e dai resoconti riportati dalla popolazione locale, l’attività della Centrale ha notevolmente influenzato l’equilibrio di questo ambiente, in termini geologici, biologici ed idrologici, soprattutto a causa dell’enorme volume di acqua fredda (e relativi sedimenti) scaricata, delle continue variazioni di livello dell’acqua per regolarne il volume di invaso e dello scavo del canale emissario, localizzato nell’estremità meridionale. Nel Maggio 2015 l’ISMAR – CNR di Bologna ha effettuato un rilievo geofisico del lago, tramite tecniche non distruttive di ecografia e sismica a riflessione, in grado di analizzare la stratigrafia superficiale e la distribuzione degli apporti sedimentari, con lo scopo di quantificare da questo punto di vista l’impatto della Centrale sul lago. I dati acquisiti, che comprendono profili sismici ad alta risoluzione, profili batimetrici single/multi – beam ed immagini side – scan sonar, sono stati successivamente elaborati per realizzare varie mappe tematiche (morfobatimetria, riflettività, gradiente topografico e spessore dei sedimenti penetrabili dal segnale sismico) che hanno permesso di descrivere l’attuale assetto deposizionale del lago. Sono stati inoltre effettuati alcuni carotaggi in vari punti della conca lacustre, al fine di quantificare il tasso di sedimentazione e le caratteristiche fisiche dei depositi. Scopo di questo lavoro di Tesi è stato analizzare, interpretare e discutere in una prospettiva di evoluzione ambientale del lago i dati geofisici e geologici raccolti nell’ambito della campagna del Maggio 2015 e reperiti in bibliografia.

La presente tesi discute la realizzazione di un carrello appendice sul quale possa essere montato un aerogeneratore della potenza di circa 8500 W facile da trasportare ed installare. Si desidera progettare una macchina che non necessiti di fondamenta e non richieda particolari procedure burocratiche per l’istallazione permettendo così un investimento iniziale ridotto. Il progetto ha il vantaggio di produrre energia con venti di bassa intensità (3 m/s), livelli di rumore contenuti da considerarsi trascurabili e un impatto visivo ambientale accettabile. Uno degli obiettivi è di massimizzare le prestazioni della turbina eolica ed essere in grado di competere sul mercato attuale, installando parallelamente un impianto fotovoltaico per dare una continuità maggiore all'energia generata. Queste tipologie di macchine potranno diffondersi nei settori dell’agricoltura, del turismo e in aree specifiche, ad esempio aree protette, dove l’eolico di grandi dimensioni ha difficoltà di inserimento. L’aerogeneratore potrà alimentare utenze come ad esempio sistemi di telecomunicazioni, stazioni di pompaggio, utenze rurali site in luoghi isolati, ed è possibile lo spostamento del mezzo anche in zone altamente sconnesse grazie al telaio rinforzato e all'inserimento di freni e sospensioni. La produzione di energia elettrica proveniente da fonti rinnovabili viene promossa dal decreto legislativo n. 387 del 29/12/2003, che dà indicazioni affinché tutti gli impianti con potenza non superiore ai 20000W abbiano la possibilità di connettersi alla rete con la modalità di “scambio sul posto”. Questo servizio permette di ridurre l’esborso sostenuto per la bolletta relativamente alle quote dell’energia consumata e di rientrare dell’investimento iniziale; infatti la società distributrice dell’energia elettrica effettuerà ogni anno un conguaglio tra l’energia prelevata e quella immessa in rete dal cliente.

The rat double-SAH model is one of the standard models to simulate delayed cerebral vasospasm (CVS) in humans. However, the proof of delayed ischemic brain damage is missing so far. Our objective was, therefore, to determine histological changes in correlation with the development of symptomatic and perfusion weighted imaging (PWI) proven CVS in this animal model. CVS was induced by injection of autologous blood in the cisterna magna of 22 Sprague-Dawley rats. Histological changes were analyzed on day 3 and day 5. Cerebral blood flow (CBF) was assessed by PWI at 3 tesla magnetic resonance (MR) tomography. Neuronal cell count did not differ between sham operated and SAH rats in the hippocampus and the cerebral cortex on day 3. In contrast, on day 5 after SAH the neuronal cell count was significantly reduced in the hippocampus (p<0.001) and the inner cortical layer (p=0.03). The present investigation provides quantitative data on brain tissue damage in association with delayed CVS for the first time in a rat SAH model. Accordingly, our data suggest that the rat double-SAH model may be suitable to mimic delayed ischemic brain damage due to CVS and to investigate the neuroprotective effects of drugs.

PURPOSE: To compare diagnostic accuracy of multi-station, high-spatial resolution contrast-enhanced MR angiography (CE-MRA) of the lower extremities with digital subtraction angiography (DSA) as the reference standard in patients with symptomatic peripheral arterial occlusive disease. MATERIALS AND METHODS: Of 485 consecutive patients undergoing a run-off CE-MRA, 152 patients (86 male, 66 female; mean age, 71.6 years) with suspected peripheral arterial occlusive disease were included into our Institutional Review Board approved study. All patients underwent MRA and DSA of the lower extremities within 30 days. MRA was performed at 1.5 Tesla with a single bolus of 0.1 mmol/kg body weight of gadobutrol administered at a rate of 2.0 mL/s at three stations. Two readers evaluated the MRA images independently for stenosis grade and image quality. Sensitivity and specificity were derived. RESULTS: Sensitivity and specificity ranged from 73% to 93% and 64% to 89% and were highest in the thigh area. Both readers showed comparable results. Evaluation of good and better quality MRAs resulted in a considerable improvement in diagnostic accuracy. CONCLUSION: Contrast-enhanced MRA demonstrates good sensitivity and specificity in the investigation of the vasculature of the lower extremities. While a minor investigator experience dependence remains, it is standardizable and shows good inter-observer agreement. Our results confirm that the administration of Gadobutrol at a standard dose of 0.1 mmol/kg for contrast-enhanced runoff MRA is able to detect hemodynamically relevant stenoses. Use of contrast-enhanced MRA as an alternative to intra-arterial DSA in the evaluation and therapeutic planning of patients with suspected peripheral arterial occlusive disease is well justified. J. PURPOSE: To perform baseline T(2) mapping of the hips of healthy volunteers, focusing on topographic variation, because no detailed study has involved hips. T(2) mapping is a quantitative magnetic resonance imaging (MRI) technique that evaluates cartilage matrix components. MATERIALS AND METHODS: Hips of 12 healthy adults (six men and six women; mean age = 29.5 +/- 4.9 years) were studied with a 3.0-Tesla MRI system. T(2) measurement in the oblique-coronal plane used a multi-spin-echo (MSE) sequence. Femoral cartilage was divided into 12 radial sections; acetabular cartilage was divided into six radial sections, and each section was divided into two layers representing the superficial and deep halves of the cartilage. T(2) of these sections and layers were measured. RESULTS: Femoral cartilage T(2) was the shortest (-20 degrees to 20 degrees and -10 degrees to 10 degrees , superficial and deep layers), with an increase near the magic angle (54.7 degrees ). Acetabular cartilage T(2) in both layers was shorter in the periphery than the other parts, especially at 20 degrees to 30 degrees . There were no significant differences in T(2) between right and left hips or between men and women. CONCLUSION: Topographic variation exists in hip cartilage T(2) in young, healthy adults. The aim of this prospective trial was to evaluate sensitivity and specificity of bright lumen magnetic resonance colonography (MRC) in comparison with conventional colonoscopy (CC). A total of 120 consecutive patients with clinical indications for CC were prospectively examined using MRC (1.5 Tesla) which was then followed by CC. Prior to MRC, the cleansed colon was filled with a gadolinium-water solution. A 3D GRE sequence was performed with the patient in the prone and supine position, each acquired during one breathhold period. After division of the colon into five segments, interactive data analysis was carried out using three-dimensional post-processing, including a virtual intraluminal view. The results of CC served as a reference standard. In all patients MRC was performed successfully and no complications occurred. Image quality was diagnostic in 92% (574/620 colonic segments). On a per-patient basis, the results of MRC were as follows: sensitivity 84% (95% CI 71.7-92.3%), specificity 97% (95% CI 89.0-99.6%). Five flat adenomas and 6/16 small polyps (< or =5 mm) were not identified by MRC. MRC offers high sensitivity and excellent specificity rates in patients with clinical indications for CC. Improved MRC techniques are needed to detect small polyps and flat adenomas.

STUDY DESIGN: The structural integrity of the nucleus pulposus (NP) of intervertebral discs was targeted by enzyme-specific degradations to correlate their effects to the magnetic resonance (MR) signal. OBJECTIVE: To develop quantitative MR imaging as an accurate and noninvasive diagnostic tool to better understand and treat disc degeneration. SUMMARY OF BACKGROUND DATA: Quantitative MR analysis has been previously shown to reflect not only the disc matrix composition, but also the structural integrity of the disc matrix. Further work is required to identify the contribution of the structural integrity versus the matrix composition to the MR signal. METHODS: The bovine coccygeal NPs were injected with either enzyme or buffer, incubated at 37 degrees C as static, unloaded and closed 3-disc segments, and analyzed by a 1.5-Tesla MR scanner to measure MR parameters. RESULTS: Collagenase degradation of the NP significantly decreased the relaxation times, slightly decreased the magnetization transfer ratio, and slightly increased the apparent diffusion coefficient. Targeting the proteoglycan and/or hyaluronan integrity by trypsin and hyaluronidase did not significantly affect the MR parameters, except for an increase in the apparent diffusion coefficient of the disc after trypsin treatment. CONCLUSIONS: Our results demonstrate that changes in the structural integrity of matrix proteins can be assessed by quantitative MR.

PURPOSE: To determine the feasibility of using a high resolution isotropic three-dimensional (3D) fast T1 mapping sequence for delayed gadolinium-enhanced MRI of cartilage (dGEMRIC) to assess osteoarthritis in the hip. MATERIALS AND METHODS: T1 maps of the hip were acquired using both low and high resolution techniques following the administration of 0.2 mmol/kg Gd-DTPA(2-) in 35 patients. Both T1 maps were generated from two separate spoiled GRE images. The high resolution T1 map was reconstructed in the anatomically equivalent plane as the low resolution map. T1 values from the equivalent anatomic regions containing femoral and acetabular cartilages were measured on the low and high resolution maps and compared using regression analysis. RESULTS: In vivo T1 measurements showed a statistically significant correlation between the low and high resolution acquisitions at 1.5 Tesla (R(2) = 0.958, P < 0.001). These results demonstrate the feasibility of using a fast two-angle T1 mapping (F2T1) sequence with isotropic spatial resolution (0.8 x 0.8 x 0.8 mm) for quantitative assessment of biochemical status in articular cartilage of the hip. CONCLUSION: The high resolution 3D F2T1 sequence provides accurate T1 measurements in femoral and acetabular cartilages of the hip, which enables the biochemical assessment of articular cartilage in any plane through the joint. It is a powerful tool for researchers and clinicians to acquire high resolution data in a reasonable scan time (< 30 min).

The purpose of this article was to evaluate the potential of in vivo zonal T2-mapping as a noninvasive tool in the longitudinal visualization of cartilage repair tissue maturation after matrix-associated autologous chondrocyte transplantation (MACT). Fifteen patients were treated with MACT and evaluated cross-sectionally, with a baseline MRI at a follow-up of 19.7 +/- 12.1 months after cartilage transplantation surgery of the knee. In the same 15 patients, 12 months later (31.7 +/- 12.0 months after surgery), a longitudinal 1-year follow-up MRI was obtained. MRI was performed on a 3 Tesla MR scanner; morphological evaluation was performed using a double-echo steady-state sequence; T2 maps were calculated from a multiecho, spin-echo sequence. Quantitative mean (full-thickness) and zonal (deep and superficial) T2 values were calculated in the cartilage repair area and in control cartilage sites. A statistical analysis of variance was performed. Full-tickness T2 values showed no significant difference between sites of healthy cartilage and cartilage repair tissue (p < 0.05). Using zonal T2 evaluation, healthy cartilage showed a significant increase from the deep to superficial cartilage layers (p < 0.05). Cartilage repair tissue after MACT showed no significant zonal increase from deep to superficial cartilage areas during baseline MRI (p > 0.05); however, during the 1-year follow-up, a significant zonal stratification could be observed (p < 0.05). Morphological evaluation showed no significant difference between the baseline and the 1-year follow-up MRI. T2 mapping seems to be more sensitive in revealing changes in the repair tissue compared to morphological MRI. In vivo zonal T2 assessment may be sensitive enough to characterize the maturation of cartilage repair tissue.

The comprehension of stories requires the reader to imagine the cognitive and affective states of the characters. The content of many stories is unpleasant, as they often deal with conflict, disturbance or crisis. Nevertheless, unpleasant stories can be liked and enjoyed. In this fMRI study, we used a parametric approach to examine (1) the capacity of increasing negative valence of story contents to activate the mentalizing network (cognitive and affective theory of mind, ToM), and (2) the neural substrate of liking negatively valenced narratives. A set of 80 short narratives was compiled, ranging from neutral to negative emotional valence. For each story mean rating values on valence and liking were obtained from a group of 32 participants in a prestudy, and later included as parametric regressors in the fMRI analysis. Another group of 24 participants passively read the narratives in a three Tesla MRI scanner. Results revealed a stronger engagement of affective ToM-related brain areas with increasingly negative story valence. Stories that were unpleasant, but simultaneously liked, engaged the medial prefrontal cortex (mPFC), which might reflect the moral exploration of the story content. Further analysis showed that the more the mPFC becomes engaged during the reading of negatively valenced stories, the more coactivation can be observed in other brain areas related to the neural processing of affective ToM and empathy.

Background Conventional cross-sectional imaging with computed tomography and magnetic resonance imaging (MRI) has limited accuracy for lymph node (LN) staging in bladder and prostate cancer patients. Objective To prospectively assess the diagnostic accuracy of combined ultrasmall superparamagnetic particles of iron oxide (USPIO) MRI and diffusion-weighted (DW) MRI in staging of normal-sized pelvic LNs in bladder and/or prostate cancer patients. Design, setting, and participants Examinations with 3-Tesla MRI 24–36 h after administration of USPIO using conventional MRI sequences combined with DW-MRI (USPIO-DW-MRI) were performed in 75 patients with clinically localised bladder and/or prostate cancer staged previously as N0 by conventional cross-sectional imaging. Combined USPIO-DW-MRI findings were analysed by three independent readers and correlated with histopathologic LN findings after extended pelvic LN dissection (PLND) and resection of primary tumours. Outcome measurements and statistical analysis Sensitivity and specificity for LN status of combined USPIO-DW-MRI versus histopathologic findings were evaluated per patient (primary end point) and per pelvic side (secondary end point). Time required for combined USPIO-DW-MRI reading was assessed. Results and limitations At histopathologic analysis, 2993 LNs (median: 39 LNs; range: 17–68 LNs per patient) with 54 LN metastases (1.8%) were found in 20 of 75 (27%) patients. Per-patient sensitivity and specificity for detection of LN metastases by the three readers ranged from 65% to 75% and 93% to 96%, respectively; sensitivity and specificity per pelvic side ranged from 58% to 67% and 94% to 97%, respectively. Median reading time for the combined USPIO-DW-MRI images was 9 min (range: 3–26 min). A potential limitation is the absence of a node-to-node correlation of combined USPIO-DW-MRI and histopathologic analysis. Conclusions Combined USPIO-DW-MRI improves detection of metastases in normal-sized pelvic LNs of bladder and/or prostate cancer patients in a short reading time.
