The characteristics of neurological, psychiatric, developmental and substance-use disorders in low-and middle-income countries are unique and the burden that they have will be different from country to country. Many of the differences are explained by the wide variation in population demographics and size, poverty, conflict, culture, land area and quality, and genetics. Neurological, psychiatric, developmental and substance-use disorders that result from, or are worsened by, a lack of adequate nutrition and infectious disease still afflict much of sub-Saharan Africa, although disorders related to increasing longevity, such as stroke, are on the rise. In the Middle East and North Africa, major depressive disorders and post-traumatic stress disorder are a primary concern because of the conflict-ridden environment. Consanguinity is a serious concern that leads to the high prevalence of recessive disorders in the Middle East and North Africa and possibly other regions. The burden of these disorders in Latin American and Asian countries largely surrounds stroke and vascular disease, dementia and lifestyle factors that are influenced by genetics. Although much knowledge has been gained over the past 10 years, the epidemiology of the conditions in low-and middle-income countries still needs more research. Prevention and treatments could be better informed with more longitudinal studies of risk factors. Challenges and opportunities for ameliorating nervous-system disorders can benefit from both local and regional research collaborations. The lack of resources and infrastructure for health-care and related research, both in terms of personnel and equipment, along with the stigma associated with the physical or behavioural manifestations of some disorders have hampered progress in understanding the disease burden and improving brain health. Individual countries, and regions within countries, have specific needs in terms of research priorities.

Background Jumping to conclusions (JTC) is associated with psychotic disorder and psychotic symptoms. If JTC represents a trait, the rate should be (i) increased in people with elevated levels of psychosis proneness such as individuals diagnosed with borderline personality disorder (BPD), and (ii) show a degree of stability over time. Methods The JTC rate was examined in 3 groups: patients with first episode psychosis (FEP), BPD patients and controls, using the Beads Task. PANSS, SIS-R and CAPE scales were used to assess positive psychotic symptoms. Four WAIS III subtests were used to assess IQ. Results A total of 61 FEP, 26 BPD and 150 controls were evaluated. 29 FEP were revaluated after one year. 44% of FEP (OR = 8.4, 95% CI: 3.9-17.9) displayed a JTC reasoning bias versus 19% of BPD (OR = 2.5, 95% CI: 0.8-7.8) and 9% of controls. JTC was not associated with level of psychotic symptoms or specifically delusionality across the different groups. Differences between FEP and controls were independent of sex, educational level, cannabis use and IQ. After one year, 47.8% of FEP with JTC at baseline again displayed JTC. Conclusions JTC in part reflects trait vulnerability to develop disorders with expression of psychotic symptoms.

Ao contrário da maioria dos transtornos psiquiátricos, o transtorno de estresse pós-traumático (TEPT) apresenta uma fator causal necessário, embora não-suficiente: a exposição a um evento traumático (EPT). Em consequência deste evento desenvolvem-se três dimensões de sintomas: revivescência, esquiva/ entorpecimento emocional e hiperexcitabilidade. Um dos achados mais relevantes das pesquisas epidemiológicas de TEPT é que embora a maioria dos indivíduos seja exposta a um evento traumático em algum momento de sua vida, apenas uma minoria destes vai desenvolver o transtorno. Desta forma, a maior parte dos indivíduos expostos pode ser considerada resiliente. A resiliência consiste, portanto, na capacidade de adaptação eficaz diante de um distúrbio, estresse ou adversidade. Foi realizada uma revisão sistemática com metanálise de estudos longitudinais que investigaram fatores preditores de resiliência ao desenvolvimento de TEPT. A ausência de TEPT foi considerada proxy de resiliência. Em função do grande número de estudos identificados pela estratégia de busca, decidimos post hoc restringir as variáveis preditoras a apoio social (AS), personalidade, autoestima e eventos de vida potencialmente estressantes (EVPE). Apenas vinte artigos preencheram os critérios de elegibilidade. Treze estudos avaliaram apoio social, nove avaliaram personalidade e dois avaliaram EVPE. Nenhum dos trabalhos que pesquisou autoestima era elegível. Dezesseis dos vinte estudos incluídos nesta revisão avaliaram a associação de interesse na população geral. A maioria dos trabalhos avaliou a exposição de interesse após o EPT. Ainda que alguns destes tenham tentado captar a informação sobre a exposição antes do ocorrido, devido à natureza retrospectiva desta aferição, não há como se isentar o potencial efeito do trauma sobre resultado obtido. Além disso, foi observada grande heterogeneidade entre as pesquisas, limitando o número de estudos incluídos nas metanálises. Neuroticismo foi a única dimensão de personalidade avaliada por mais de um estudo. As medidas sumárias resultantes da combinação destes trabalhos revelaram que maior apoio social positivo prediz resiliência ao TEPT enquanto neuroticismo reduz a chance de resiliência. Os dois estudos que investigaram EVPE não puderam ser combinados. Um deles foi inconclusivo e o outro demonstrou associação entre menor número de EVPE e resiliência. Ressaltamos que as medidas sumárias devem ser interpretadas com cautela devido à grande heterogeneidade entre os estudos. Heterogeneidade na forma de avaliação dos fatores preditores de resiliência ao TEPT é compreensível devido à complexidade dos construtos avaliados. Todavia, a falta de padronização do método de operacionalização reduz a comparabilidade dos resultados.

Only a few studies have examined the efficacy and safety of smoking cessation programmes in patients with mental disorders. The aim of this paper is to describe in detail the methodology used in the study as well as the Multi-component Smoking Cessation Support Programme in terms of pharmacological treatments and psychological interventions. An open-label 9-month follow-up study was conducted in Spain. A total of 82 clinically stable outpatients with schizophrenia, schizoaffective or bipolar disorder were enrolled. Treatment consisted of a programme specifically developed by the research team for individuals with severe mental disorders. The programme consisted of two phases: (1) weekly individual motivational therapy for 4-12 weeks, and (2) a 12-week active treatment phase. During this phase, at each study visit patients received a one- or two-week supply of medication (transdermal nicotine patches, varenicline or bupropion) with instructions on how to take it, in addition to group psychotherapy for smoking cessation. Evaluations were performed: (1) at the time of enrolment in the study, (2) during the 12-week active treatment phase of the study (weekly for the first 4 weeks and then biweekly), and (3) after the end of this phase (two follow-up assessments at weeks 12 and 24). Evaluations included: (1) smoking history, (2) substance use, (3) psychopathology, (4) adverse events, and (5) laboratory tests. The importance of this study lies in addressing a topical issue often ignored by psychiatrists: the unacceptably high rates of tobacco use in patients with severe mental disorders.

In the present work, the nematic glassy state of the non-symmetric LC dimer -(4-cyanobiphenyl-4-yloxy)--(1-pyrenimine-benzylidene-4-oxy) undecane is studied by means of calorimetric and dielectric measurements. The most striking result of the work is the presence of two different glass transition temperatures: one due to the freezing of the flip-flop motions of the bulkier unit of the dimer and the other, at a lower temperature, related to the freezing of the flip-flop and precessional motions of the cyanobiphenyl unit. This result shows the fact that glass transition is the consequence of the freezing of one or more coupled dynamic disorders and not of the disordered phase itself. In order to avoid crystallization when the bulk sample is cooled down, the LC dimer has been confined via the dispersion of -alumina nanoparticles, in several concentrations.

Nos últimos anos, temos nos deparado com a difusão maciça e a popularização crescente de descrições biológicas para aspectos outrora pensados como mentais, sociais, ou relacionais. Visível em diversas arenas leigas e científicas, esta tendência freqüentemente elege o cérebro como o órgão privilegiado da sua atenção. A cada semana é divulgada uma nova localização cerebral correlacionada os mais variados aspectos comportamentais e ou de personalidade. Acompanhando este movimento, é notável o esforço intelectual e financeiro despendido nos últimos anos no campo da saúde mental, no sentido de fazer avançar pesquisas cujo foco central é a descoberta das bases neurobiológicas dos transtornos mentais. Esta tendência apontaria na direção de uma fusão entre a psiquiatria e a neurologia em uma disciplina única, de teor fisicalista, chamada por alguns de cerebrologia. Dentre os acontecimentos que serviram de alicerce para a legitimação e a popularização desta tendência, o desenvolvimento nas últimas décadas de novas técnicas e tecnologias de visualização médica, como a tomografia por emissão de pósitrons (PET scan) e a ressonância magnética funcional (fMRI), foi fundamental. Elas permitiram a construção de imagens das mais diversas categorias nosográficas construídas no campo psiquiátrico, veiculando tacitamente uma série de pressupostos e promessas. Malgrado o imaginário cultural sustentado por estas tecnologias e todo o esforço despendido nas últimas décadas no sentido de se tentar localizar os marcadores biológicos dos transtornos psiquiátricos, não há, até o presente momento, nenhum resultado conclusivo que autorize o diagnóstico por imagem de nosografias como a esquizofrenia, a depressão, e muito menos o jogo patológico. Apesar de todo o alarde midiático e dos montantes milionários direcionados para pesquisas nesta área, os resultados concretos obtidos até agora não estão livres das mais ferozes controvérsias. Entretanto, ainda que estejamos muito longe da construção de mapas precisos para as perturbações mentais é espantoso o poder de convencimento que as neuro-imagens comportam na atualidade. Os scans são exibidos como verdades visuais, ou fatos acerca das pessoas e do mundo, numa proporção muito superior aos dados que apresentam. Alguns críticos chamam este aspecto de neurorealismo, ou de retórica da auto-evidência. A intenção deste trabalho é problematizar o poder persuasivo que as neuro-imagens detém na contemporaneidade, especialmente quando utilizadas com a finalidade diagnóstica no campo da saúde mental. Se estas imagens transmitem uma ideia de neutralidade, transparência imediata e auto-evidência, este trabalho almeja inseri-las num contexto sócio-histórico, a partir do qual puderam adquirir sentido, familiaridade e valor de verdade. O ponto de partida é o de que elas estão localizadas no cruzamento de dois movimentos históricos distintos: o das ilustrações médicas, em sua relação com a produção de conhecimento objetivo; e o das pesquisas acerca da localização no córtex cerebral de comportamentos complexos e traços de personalidade. Além de estabelecer algumas condições históricas de possibilidade para a emergência de um neo-localizacionismo cerebral, mediado pelas novas tecnologias de imageamento, pretende-se enfatizar algumas descontinuidades com projetos anteriores e marcar a influência do contexto cultural da atualidade para o sucesso e poder persuasivo deste tipo de tecnologia.

BACKGROUND: Hypoxia and ischemia induce neuronal damage, decreased neuronal numbers and synaptophysin levels, and deficits in learning and memory functions. Previous studies have shown that lycium barbarum polysaccharide, the most effective component of barbary wolfberry fruit, has protective effects on neural cells in hypoxia-ischemia. OBJECTIVE: To investigate the effects of Naotan Pill on glutamate-treated neural cells and on cognitive function in juvenile rats following hypoxia-ischemia. DESIGN, TIME AND SETTING: The randomized, controlled, in vivo study was performed at the Cell Laboratory of Lanzhou University, Lanzhou Institute of Modern Physics of Chinese Academy of Sciences, and Department of Traditional Chinese Medicine of Gansu Provincial Rehabilitation Center Hospital, China from December 2005 to August 2006. The cellular neurobiology, in vitro experiment was conducted at the Institute of Human Anatomy, Histology, Embryology and Neuroscience, School of Basic Medical Sciences, Lanzhou University, and Department of Traditional Chinese Medicine of Gansu Provincial Rehabilitation Center Hospital, China from March 2007 to January 2008. MATERIALS: Naotan Pill, composed of barbary wolfberry fruit, danshen root, grassleaf sweetflag rhizome, and glossy privet fruit, was prepared by Gansu Provincial Rehabilitation Center, China. Rabbit anti-synaptophysin, choline acetyl transferase polyclonal antibody, streptavidin-biotin complex kit and diaminobenzidine kit (Boster, Wuhan, China), as well as glutamate (Hualian, Shanghai, China) were used in this study. METHODS: Cortical neural cells were isolated from neonatal Wistar rats. Neural cell damage models were induced using glutamate, and administered Naotan Pill prior to and following damage. A total of 54 juvenile Wistar rats were equally and randomly assigned into model, Naotan Pill, and sham operation groups. The left common carotid artery was ligated, and then rat models of hypoxic-ischemic injury were assigned to the model and Naotan Pill groups. At 2 days following model induction, rats in the Naotan Pill group were administered Naotan Pill suspension for 21 days. In the model and sham operation groups, rats received an equal volume of saline. MAIN OUTCOME MEASURES: Neural cell morphology was observed using an inverted phase contrast microscope. Survival rate of neural cells was measured by MTT assay. Synaptophysin and choline acetyl transferase expression was observed in the hippocampal CA1 region of juvenile rats using immunohistochemistry. Cognitive function was tested by the Morris water maze. RESULTS: Pathological changes were detected in glutamate-treated neural cells. Neural cell morphology remained normal after Naotan Pill intervention. Absorbance and survival rate of neural cells were significantly greater following Naotan Pill intervention, compared to glutamate-treated neural cells (P < 0.05). Synaptophysin and choline acetyl transferase expression was lowest in the hippocampal CA1 region in the model group and highest in the sham operation group. Significant differences among groups were observed (P < 0.05). Escape latency and swimming distance were significantly longer in the model group compared to the Naotan Pill group (P < 0.05). CONCLUSION: Naotan Pill exhibited protective and repair effects on glutamate-treated neural cells. Naotan Pill upregulated synaptophysin and choline acetyl transferase expression in the hippocampus and improved cognitive function in rats following hypoxia-ischemia.

Depression is a major medical and social problem. Here we review current body of knowledge on the benefits of exercise as an effective strategy for both the prevention and treatment of this condition. We also analyze the biological pathways involved in such potential benefits, which include changes in neurotrophic factors, oxidative stress and inflammation, telomere length, brain volume and microvessels, neurotransmitters or hormones. We also identify major caveats in this field of research: further studies are needed to identify which are the most appropriate types of exercise interventions (intensity, duration, or frequency) to treat and prevent depression.

Childhood asthma, allergic rhinitis and eczema are complex heterogenic chronic inflammatory allergic disorders which constitute a major burden to children, their families. The prevalence of childhood allergic disorders is increasing worldwide and merely rudimentary understanding exists regarding causality, or the influence of the environment on disease expression. Phase Three of the International Study of Asthma and Allergy in Childhood (ISAAC) reported that Irish adolescents had the 4th highest eczema and rhinoconjunctivitis prevalence and 3rd highest asthma prevalence in the world. There are no ISAAC data pertaining to young Irish children. In 2002, Sturley reported a high prevalence of current asthma in Cork primary school children aged 6-9 years. This thesis comprises of three cross-sectional studies which examined the prevalence of and associations with childhood allergy and a quasi-retrospective cohort study which observed the natural history of allergy from 6-9 until 11-13 years. Although not part of ISAAC, data was attained by parentally completed ISAAC-based questionnaires, using the ISAAC protocol. The prevalence, natural history and risk factors of childhood allergy in Ireland, as described in this thesis, echo those in worldwide allergy research. The variations of prevalence in different populations worldwide and the recurring themes of associations between childhood allergy and microbial exposures, from farming environments and/or gastrointestinal infections, as shown in this thesis, strengthen the mounting evidence that microbial exposure on GALT may hold the key to the mechanisms of allergy development. In this regard, probiotics may be an area of particular interest in allergy modification. Although their effects in relation to allergy, have been investigated now for several years, our knowledge of their diversity, complex functions and interactions with gut microflora, remain rudimentary. Birth cohort studies which include genomic and microbiomic research are recommended in order to examine the underlying mechanisms and the natural course of allergic diseases.
