O mundo vive uma mudança de época. A comunidade internacional, respondendo aos desequilíbrios econômicos, distributivos e ambientais do estilo de desenvolvimento dominante, aprovou recentemente a Agenda 2030 para o Desenvolvimento Sustentável e seus 17 Objetivos. Este documento, que a Comissão Econômica para a América Latina e o Caribe (CEPAL) apresenta aos Estados membros no trigésimo sexto período de sessões, complementa analiticamente essa Agenda com base na perspectiva estruturalista do desenvolvimento e sob o ponto de vista dos países da América Latina e do Caribe. Suas propostas se concentram na necessidade de impulsionar uma mudança estrutural progressiva que aumente a incorporação de conhecimento na produção, garanta a inclusão social e combata os efeitos negativos da mudança climática. As reflexões e propostas para avançar rumo a um novo estilo de desenvolvimento mantêm seu foco no impulso à igualdade e à sustentabilidade ambiental. A criação de bens públicos globais e de seus correlatos no âmbito regional e de políticas nacionais é o núcleo a partir do qual se expande a visão estruturalista para um keynesianismo global e uma estratégia de desenvolvimento concentrada num grande impulso ambiental.

Le monde traverse un changement d’époque. Face aux déséquilibres économiques, distributifs et environnementaux du mode de développement prédominant, la communauté internationale vient d’adopter le Programme de développement durable à l’horizon 2030 et ses 17 objectifs. Le document que la Commission économique pour l’Amérique latine et les Caraïbes (CEPALC) présente aux états membres à l’occasion de sa trente-sixième session a pour but d’apporter un complément analytique à ce Programme dans la perspective structuraliste du développement et de l’optique des pays d’Amérique latine et des Caraïbes. Les propositions contenues dans ce document sont centrées sur la nécessité de promouvoir un changement structurel progressif qui favorise l’incorporation du savoir à la production, garantisse l’inclusion sociale et combatte les effets néfastes du changement climatique. Les réflexions et les propositions visant à avancer sur la voie d’un nouveau mode de développement sont axées sur la promotion de l’égalité et de la pérennité de l’environnement. La création de biens publics mondiaux et leurs équivalents à l’échelle régionale, ainsi que l’application de politiques nationales sont au coeur d’une vision structuraliste qui est appelée à évoluer vers un keynésianisme mondial et une stratégie de développement centrée sur un élan majeur pour l’environnement.

La Agenda 2030 para el Desarrollo Sostenible, aprobada en septiembre de 2015 por la Asamblea General de las Naciones Unidas, establece una visión transformadora hacia la sostenibilidad económica, social y ambiental de los 193 Estados Miembros que la suscribieron y será la guía de referencia para el trabajo de la institución en pos de esta visión durante los próximos 15 años. Esta nueva hoja de ruta presenta una oportunidad histórica para América Latina y el Caribe, ya que incluye temas altamente prioritarios para la región, como la reducción de la desigualdad en todas sus dimensiones, un crecimiento económico inclusivo con trabajo decente para todos, ciudades sostenibles y cambio climático, entre otros. El conocimiento de los 17 Objetivos de Desarrollo Sostenible (ODS) asociados a esta Agenda ayuda a evaluar el punto de partida de los países de la región y a analizar y formular los medios para alcanzar esta nueva visión del desarrollo sostenible, que se expresó de manera colectiva y quedó plasmada en la Agenda 2030. Los ODS también son una herramienta de planificación para los países, tanto a nivel nacional como local. Gracias a su visión a largo plazo, constituirán un apoyo para cada país en su senda hacia un desarrollo sostenido, inclusivo y en armonía con el medio ambiente, a través de políticas públicas e instrumentos de presupuesto, monitoreo y evaluación. La Agenda 2030 es una agenda civilizatoria, que pone la dignidad y la igualdad de las personas en el centro. Al ser ambiciosa y visionaria, requiere de la participación de todos los sectores de la sociedad y del Estado para su implementación. Por lo tanto, se invita a los representantes de los Gobiernos, la sociedad civil, el ámbito académico y el sector privado a apropiarse de esta ambiciosa agenda, a debatirla y a utilizarla como una herramienta para la creación de sociedades inclusivas y justas, al servicio de las personas de hoy y de futuras generaciones.

The concern with infant mortality has been a priority in public policies, especially for Brazil to achieve the Millennium Development Goal number four: reducing child mortality by 75% by the year 2015. It is known that prematurity has an intimate relationship with mortality of children under one year and therefore it is necessary an effective intervention in risk factors linked to premature births. To evaluate the profile of mothers and newborn babies living in Botucatu-SP, in the period 2001 to 2009, focusing on prematurity. A quantitative study, retrospective, descriptive, epidemiological, from the Information System Newborn Alive. The prevalence of preterm births was 15.1%, and low birth weight 14.7%. Among infants, the Apgar score below 7 at 1 and at 5 minutes was 13.1% and 2.4% respectively. Considering the total of pregnant women studied, 20.3% were adolescents and 10.3% were aged 35 years or more and most (63.2%) had eight or more years of school approval. The profile of mothers of premature infants shows that 23.6% were teenagers, 14.1% had at least 35 years and 60.4% had at least primary education. With regard to premature deliveries, caesarean sections and 58.4% were between the total newborns, this type of delivery was achieved in 46.6% of cases. Prematurity was associated with an Apgar score below seven in the first and fifth minutes of life, presence of malformation and multiple pregnancy. Among maternal variables, prematurity was associated with extremes of maternal age, maternal education, history of stillbirth, primiparas, white and presence of the mother's partner. With regard to delivery was associated with cesarean section and outside the hospital. This study assessed maternal characteristics associated with prematurity and point out how relevant the pregnancies at the extremes of age and those that occur in women with low education level, indicating the interference... Small businesses are considered important engines for job growth and economic development by policy makers worldwide. One of the most commonly cited constraints of small businesses is a lack of access to capital. To address this constraint, small business loan guarantee programs have been established in over 100 countries. There are a variety of types of guarantee funds, with the most significant differences being which borrowers are eligible for guarantees, and how borrowers are approved for guarantees. There is currently no clear delineation between types of programs and the economic conditions they operate in, though some trends are becoming apparent. However, these trends may not be leading to the best economic outcomes possible. By better matching the structure of the guarantee fund to the economic conditions it operates in, the program’s success in meeting economic development goals may be greatly improved. Many programs in developing countries may not be taking advantage of bank expertise and may be limiting the scope of their effectiveness. At the same time, programs in developed countries may be wasting resources by scattering their efforts too thinly and subsidizing less competitive firms to the detriment of local economic development.

One of the main problems recognized in sustainable development goals and sustainable agricultural objectives is Climate change. Farming contributes significantly to the overall Greenhouse gases (GHG) in the atmosphere, which is approximately 10-12 percent of total GHG emissions, but when taking in consideration also land-use change, including deforestation driven by agricultural expansion for food, fiber and fuel the number rises to approximately 30 percent (Smith et. al., 2007). There are two distinct methodological approaches for environmental impact assessment; Life Cycle Assessment (a bottom up approach) and Input-Output Analysis (a top down approach). The two methodologies differ significantly but there is not an immediate choice between them if the scope of the study is on a sectorial level. Instead, as an alternative, hybrid approaches which combine these two approaches have emerged. The aim of this study is to analyze in a greater detail the agricultural sectors contribution to Climate change caused by the consumption of food products. Hence, to identify the food products that have the greatest impact through their life cycle, identifying their hotspots and evaluating the mitigation possibilities for the same. At the same time evaluating methodological possibilities and models to be applied for this purpose both on a EU level and on a country level (Italy).

As water quality interventions are scaled up to meet the Millennium Development Goal of halving the proportion of the population without access to safe drinking water by 2015 there has been much discussion on the merits of household- and source-level interventions. This study furthers the discussion by examining specific interventions through the use of embodied human and material energy. Embodied energy quantifies the total energy required to produce and use an intervention, including all upstream energy transactions. This model uses material quantities and prices to calculate embodied energy using national economic input/output-based models from China, the United States and Mali. Embodied energy is a measure of aggregate environmental impacts of the interventions. Human energy quantifies the caloric expenditure associated with the installation and operation of an intervention is calculated using the physical activity ratios (PARs) and basal metabolic rates (BMRs). Human energy is a measure of aggregate social impacts of an intervention. A total of four household treatment interventions – biosand filtration, chlorination, ceramic filtration and boiling – and four water source-level interventions – an improved well, a rope pump, a hand pump and a solar pump – are evaluated in the context of Mali, West Africa. Source-level interventions slightly out-perform household-level interventions in terms of having less total embodied energy. Human energy, typically assumed to be a negligible portion of total embodied energy, is shown to be significant to all eight interventions, and contributing over half of total embodied energy in four of the interventions. Traditional gender roles in Mali dictate the types of work performed by men and women. When the human energy is disaggregated by gender, it is seen that women perform over 99% of the work associated with seven of the eight interventions. This has profound implications for gender equality in the context of water quality interventions, and may justify investment in interventions that reduce human energy burdens.

Purpose This paper aims to provide conceptual clarity by distinguishing self‐initiated expatriates (SIEs) from company‐assigned expatriates (AEs), and skilled migrants; most importantly, it introduces an overarching conceptual framework based on career capital theory to explain SIEs’ career success. Design/methodology/approach This conceptual framework is based on a review of the relevant literature on SIE, expatriation, career studies, cross‐cultural studies, migration, and other related areas. Findings Protean career attitude, career networks, and cultural intelligence are identified as three major types of career capital influencing SIEs career success positively; the predicting relationships between these are mediated by cultural adjustment in the host country. Cultural distance acts as the moderator, which highlights the influence of macro‐contextual factors on SIEs’ career development. Research limitations/implications The current paper applied career capital theory and did not integrate the impact of family and labour market situation on SIEs’ career development. Further research should test the proposed framework empirically, and integrate the impact of family‐ and career‐related factors into a holistic approach. Practical implications When constructing international talent acquisition and retention strategies, organizations and receiving countries should understand the different career development needs and provide SIEs with opportunities to increase career capital during expatriation. Furthermore, the current framework suggests how to adjust to the host country in order to meet career development goals. Originality/value The multi‐level and sequential framework adds value by identifying specific types of career capital for SIEs and providing a conceptual underpinning for explaining how they interact and foster SIEs’ career success. Moreover, the framework embraces SIEs from both developed and developing economies.

There is a general consensus that healthy soils are pivotal for food security. Food production is one of the main ecosystem services provided by and thus dependent on well-functioning soils. There are also intrinsic connections between the four pillars of food security: food availability, access, utilization, and stability; with how soils are managed, accessed and secured, in particular by food insecure and vulnerable populations. On the other hand, socio-political and economic processes that precipitate inequalities and heighten vulnerabilities among poor populations often increase pressure on soils due to unsustainable forms of land use and poor agricultural practises. This has often led to scenarios that can be described as: ‘poor soils, empty stomachs (hungry people) and poor livelihoods.' In 2015, in particular, as we head towards approval of the ‘Sustainable Development Goals' (SDGs), the role of Financing for Development is debated and agreed upon and a new climate pact is signed – these three political dimensions define how a new post-2015 agenda needs to be people-smart as well as resource-smart. For proposed SDG 2 (Food Security and Hunger), there can be so resolution without addressing people, policies and institutions.

Background. According to the WHO 2007 country report, Haiti lags behind the Millennium Development Goal of reducing child mortality and maintains the highest under-5 mortality rate in the Western hemisphere. 3 Overall, few studies exist that seek to better grasp barriers in caring for a seriously ill child in a resource-limited setting and only a handful propose sustainable, effective interventions. ^ Objectives. The objectives of this study are to describe the prevalence of serious illnesses among children hospitalized at 2 children's hospitals in Port au Prince, to determine the barriers faced when caring for seriously ill children, and to report hospital outcomes of children admitted with serious illnesses. ^ Methods. Data were gathered from 2 major children's hospitals in Port au Prince, Haiti (Grace Children's Hospital [GCH] and Hopital d l'Universite d'Etat d'Haiti [HUEH]) using a triangulated approach of focus group discussions, physician questionnaires, and retrospective chart review. 23 pediatric physicians participated in focus group discussions and completed a self-administered questionnaire evaluating healthcare provider knowledge, self-efficacy, and perceived barriers relating to the care of seriously ill children in a resource-limited setting. A sample of 240 patient charts meeting eligibility criteria was abstracted for pertinent elements including sociodemographics, documentation, treatment strategies, and outcomes. Factors associated with mortality were analyzed using χ2 test and Fisher exact test [Minitab v.15]. ^ Results. The most common primary diagnoses at admission were gastroenteritis with moderate dehydration (35.5%), severe malnutrition (25.8%), and pneumonia (19.3%) for GCH, and severe malnutrition (32.6%), sepsis (24.7%), and severe respiratory distress (18%) for HUEH. Overall, 12.9% and 27% of seriously ill patients presented with shock to GCH and HUEH, respectively. ^ Shortage of necessary materials and equipment represented the most commonly reported limitation (18/23 respondents). According to chart data, 9.4% of children presenting with shock did not receive a fluid bolus, and only 8% of patients presenting with altered mental status or seizures received a glucose check. 65% of patients with meningitis did not receive a lumbar puncture due to lack of materials. ^ Hospital mortality rates did not differ by gender or by institution. Children who died were more likely to have a history of prematurity (OR 4.97 [95% CI 1.32-18.80]), an incomplete vaccination record (OR 4.05 [95% CI 1.68-9.74]), or a weight for age ≤3rd percentile (OR 6.1 [95% CI 2.49-14.93]. Case-fatality rates were significantly higher among those who presented with signs of shock compared with those who did not (23.1% vs. 10.7%, RR=2.16, p=0.03). Caregivers did not achieve shock reversal in 21% of patients and did not document shock reversal in 50% of patients. ^ Conclusions. Many challenges face those who seek to optimize care for seriously ill children in resource-limited settings. Specifically, in Haiti, qualitative and quantitative data suggest major issues with lack of supplies, pre-hospital factors, including malnutrition as a comorbidity, and early recognition and management of shock. A tailored intervention designed to address these issues is needed in order to prospectively evaluate improvements in child mortality in a high-risk population.^

Background. The United Nations' Millennium Development Goal (MDG) 4 aims for a two-thirds reduction in death rates for children under the age of five by 2015. The greatest risk of death is in the first week of life, yet most of these deaths can be prevented by such simple interventions as improved hygiene, exclusive breastfeeding, and thermal care. The percentage of deaths in Nigeria that occur in the first month of life make up 28% of all deaths under five years, a statistic that has remained unchanged despite various child health policies. This paper will address the challenges of reducing the neonatal mortality rate in Nigeria by examining the literature regarding efficacy of home-based, newborn care interventions and policies that have been implemented successfully in India. ^ Methods. I compared similarities and differences between India and Nigeria using qualitative descriptions and available quantitative data of various health indicators. The analysis included identifying policy-related factors and community approaches contributing to India's newborn survival rates. Databases and reference lists of articles were searched for randomized controlled trials of community health worker interventions shown to reduce neonatal mortality rates. ^ Results. While it appears that Nigeria spends more money than India on health per capita ($136 vs. $132, respectively) and as percent GDP (5.8% vs. 4.2%, respectively), it still lags behind India in its neonatal, infant, and under five mortality rates (40 vs. 32 deaths/1000 live births, 88 vs. 48 deaths/1000 live births, 143 vs. 63 deaths/1000 live births, respectively). Both countries have comparably low numbers of healthcare providers. Unlike their counterparts in Nigeria, Indian community health workers receive training on how to deliver postnatal care in the home setting and are monetarily compensated. Gender-related power differences still play a role in the societal structure of both countries. A search of randomized controlled trials of home-based newborn care strategies yielded three relevant articles. Community health workers trained to educate mothers and provide a preventive package of interventions involving clean cord care, thermal care, breastfeeding promotion, and danger sign recognition during multiple postnatal visits in rural India, Bangladesh, and Pakistan reduced neonatal mortality rates by 54%, 34%, and 15–20%, respectively. ^ Conclusion. Access to advanced technology is not necessary to reduce neonatal mortality rates in resource-limited countries. To address the urgency of neonatal mortality, countries with weak health systems need to start at the community level and invest in cost-effective, evidence-based newborn care interventions that utilize available human resources. While more randomized controlled studies are urgently needed, the current available evidence of models of postnatal care provision demonstrates that home-based care and health education provided by community health workers can reduce neonatal mortality rates in the immediate future.^

The infant mortality rate (IMR) is considered to be one of the most important indices of a country's well-being. Countries around the world and other health organizations like the World Health Organization are dedicating their resources, knowledge and energy to reduce the infant mortality rates. The well-known Millennium Development Goal 4 (MDG 4), whose aim is to archive a two thirds reduction of the under-five mortality rate between 1990 and 2015, is an example of the commitment. ^ In this study our goal is to model the trends of IMR between the 1950s to 2010s for selected countries. We would like to know how the IMR is changing overtime and how it differs across countries. ^ IMR data collected over time forms a time series. The repeated observations of IMR time series are not statistically independent. So in modeling the trend of IMR, it is necessary to account for these correlations. We proposed to use the generalized least squares method in general linear models setting to deal with the variance-covariance structure in our model. In order to estimate the variance-covariance matrix, we referred to the time-series models, especially the autoregressive and moving average models. Furthermore, we will compared results from general linear model with correlation structure to that from ordinary least squares method without taking into account the correlation structure to check how significantly the estimates change.^

En septiembre del año 2000, los gobiernos de 189 países adoptaron en Naciones Unidas un acuerdo global para combatir la desigualdad y mejorar el desarrollo humano en el mundo, con un horizonte puesto en el año 2015. Una de sus metas en este compromiso, llamado ?Los Objetivos de Desarrollo del Milenio?, consiste en reducir la pobreza extrema a la mitad. Específicamente, las autoridades de la República Argentina acordaron ?erradicar la indigencia y el hambre? entre 1990 y 2015, con una meta adicional consistente en ?reducir la pobreza a menos del 20 ?. En tiempos de grandes diferencias entre las estadísticas oficiales ?que están indicando mejores condiciones de vida en estos últimos años??y los informes de la oposición, de las consultoras privadas y de la Iglesia ?los cuales exhiben una posición contraria?, se desprende que la preocupación por erradicar el hambre está implícitamente presente en todos los sectores. Dada la cercanía de la fecha establecida como límite, se considera de interés la posibilidad de construir un nuevo instrumento de medición: un indicador del hambre para el próximo quinquenio. Por la complejidad del tema, esta propuesta innovadora presenta lineamientos básicos tendientes a la elaboración de un indicador que pueda brindar un seguimiento del fenómeno del hambre en la región, como una alternativa ante la inexistencia de estadísticas oficiales actualizadas que lo midan. Para ello se sugiere un marco teórico inexistente, hasta donde se conoce, obligando a definir aspectos conceptuales de variables relevantes. Tener un conocimiento más riguroso sobre los hogares argentinos que padecen el flagelo del hambre y poder identificarlos permitirá una mejor distribución de los recursos y procurar, a no muy largo plazo, que ningún habitante se encuentre en situación de pobreza extrema
