Esta dissertação versa sobre o papel da Ouvidoria no Hospital Ophir Loyola a partir das informações contidas nas manifestações dos usuários do serviço. Trata-se de uma pesquisa retrospectiva, realizada entre os anos de 2008 a 2010, de cunho exploratório-descritivo, baseada na análise de documentos oriundos do serviço. Tem por objetivo geral analisar em que medida a Ouvidoria cumpriu as suas atribuições quanto às manifestações dos usuários do HOL. Visa também identificar quais as principais manifestações de insatisfação apontadas pelos usuários da ouvidoria que prejudicam a dinâmica do atendimento, e, ainda, verificar como a gestão do HOL responde as manifestações dos usuários da ouvidoria, no sentido de atender individual ou coletivamente as suas necessidades. O instrumento utilizado foi um roteiro de coleta de dados, inspirado em um modelo desenvolvido por Falcão (2010) em pesquisa semelhante, dentro da seguinte composição: manifestações relacionadas à esfera da gestão hospitalar e manifestações relacionadas à esfera da gestão do Sistema Único de Saúde (SUS). Dentro da esfera da gestão hospitalar, as manifestações foram assim distribuídas: denúncia, reclamação, solicitação, informação, elogio e sugestão, para cada um dos eixos de composição. Para a esfera da gestão do SUS, foram adotadas apenas: a reclamação, a solicitação e a informação, devido não terem tido ocorrências nas demais categorias. A amostra selecionada foi de 398 registros do serviço de ouvidoria. Contudo, no decorrer da pesquisa identificou-se que um registro gerava manifestação nas duas esferas de gestão, hospitalar e do SUS, o que gerou um acréscimo de 46 ocorrências, passando para 444, destas 81% contempla a esfera da gestão hospitalar e apenas 19%, a gestão do SUS. Quanto aos resultados, o estudo constatou que 50% das manifestações não foram atendidas no seu pleito, dessas, 8% estão relacionadas às não respondidas e 42% refere-se às respostas não satisfatórias, no entanto, esse dado chama atenção para o fato de a ouvidoria dar respostas, mesmo que não alcance o pleito desejado, principalmente quando este se relaciona à gestão do SUS, pela ingerência da gestão nesse processo. Com relação aos 50% restantes, 188 equivalem às respostas satisfatórias do pleito, destes 94% contemplam a soluções individuais e somente 6% (11 manifestações), alcançaram a coletividade, e apenas duas destas provocaram mudança de ordem gerencial. Desta forma, conclui-se que a ouvidoria do HOL não conseguiu cumprir as sua atribuições institucionais devido ao comportamento da gestão frente às informações geradas no serviço de ouvidoria mostrar-se pontual, com resoluções individuais e não voltado para a coletividade, no sentido de realinhar os entraves que prejudicam o desenvolvimento da assistência, sinalizados pelo usuário e promover a aprendizagem. Nesse sentido, o objetivo geral de analisar em que medida a Ouvidoria cumpriu as suas atribuições quanto às manifestações dos usuários do HOL foi alcançado.

As micobactérias não tuberculosas (MNT) estão amplamente presentes no ambiente, tendo sido isoladas em águas naturais, sistemas de distribuição de água, solo e animais. Caracterizam-se pela presença de ácido micólico na parede celular. Em geral, são adquiridas através de inalação de gotículas de água contendo micobactérias. Podem causar formas variadas de doença como linfadenite, pulmonar, cutânea e disseminada. São patógenos oportunistas, com patogenicidade variável, que requerem defeitos na imunidade local ou sistêmica, congênitos ou adquiridos para causar doenças em humanos. Foram avaliados aspectos epidemiológicos, clínicos e radiológicos de 44 casos de micobacteriose não tuberculosa na forma pulmonar no Hospital João de Barros Barreto (HUJBB) através de estudo retrospectivo e foram tratados e acompanhados 21/44 (47,7%) pacientes durante um período de seis a dezessete meses através de estudo do tipo coorte prospectivo. Os dados mostraram um incremento de mais de 100% no número de casos a partir do ano de 2010 em relação aos anos anteriores no HUJBB. As micobactérias mais isoladas foram M. intracellulare (22,7%) e M. massiliense (20,5%). As condições mais frequentemente associadas à doença incluíram tratamento prévio para tuberculose (93,2%), bronquiectasias (59%), HIV (11,4%), asma (9,1%) e doença pulmonar obstrutiva crônica (9,1%). Não foram observadas diferenças nos aspectos radiológicos entre as espécies, exceto na análise das radiografias de tórax, onde atelectasias foram mais frequentes nos grupo M. massiliense do que no grupo de M. abscessus. A resposta ao tratamento de acordo com a análise das culturas para micobactérias mostrou que em 58,8% dos casos ocorreu negativação, persistência da positividade em 11,7% e positividade após negativação inicial em 11,7%. Durante o período de acompanhamento, a taxa de óbito foi de 17,7%. Os dados sugerem que a forma pulmonar da micobacteriose não tuberculosa tem se tornado uma doença com importância cada vez maior em nossa região. Adicionalmente, a resposta ao tratamento tem sido bastante satisfatória quando comparada à literatura. Entretanto, é necessário um seguimento desses pacientes por período mais prolongado para estabelecer o real desfecho da nossa abordagem terapêutica.

The primary health care has been recognized as one of the key components of an effective health system. In its most developed form, the primary health care is the first contact with the health system and the site responsible for the organization of health care over time: individuals, their families and the general population; seeks to provide balance between the two goals of a national health system, which are improving the health of the population and provide equitable distribution of resources. Hospitalizations for primary care sensitive conditions (HPCSC) may be associated with deficiencies of service coverage primary health care or its effectiveness. Hospitalization rates can and should represent a warning sign, triggering mechanisms for analysis and search for explanations for these problems. The use of hospitalization data for HPCSC can serve as indicators of inequality in the health system, contributing to the evaluation of the deployment and implementation of health policies.

Abusive consumption of alcohol leads to several negative consequences to health and quality of life, as it increases the frequency of diseases that cause death or functional disabilities. The rates of patients admitted to hospital due to physical problems stemming from alcohol abuse are high. This study aimed at identifying patients according to variables age, gender and education level as well as at evaluating the prevalence of CAGE-positive patients and morbidity due to CID-10 (International Classification of Diseases) by the World Health Organization. It is a cross-sectional, observational, exploratory, descriptive and quantitative study. It was conducted at the Botucatu School of Medicine University Hospital (HC) from July to September 2010, at the Internal Medicine ward. This ward aggregates 5 specialties, namely, Cardiology, Hematology, General Medicine, Nephrology and Gastroclinic. Presently, it has 36 beds, of which four are for Intensive Care Therapy (ICT). Three hundred and ten medical charts were analyzed for identification of morbidities and of variables age, gender and schooling. A closed semistructured questionnaire including, among other questions, the CAGE test was applied to each individual. The CAGE test is a questionnaire that comprises four questions and considers that one who answers two or more of such questions affirmatively is a suspected case of alcoholism. The total population studied included 310 patients. Indexes of 60% (186) male and of 40% (124) female patients were found. As regards age range, a population with a higher prevalence of individuals from 51 to 70 years old (41%) was observed. The main education indexes were: incomplete Elementary School: 23.22% (72), complete Elementary School: 21.61% (67), incomplete Secondary School: 18.40% (57) and Illiteracy: 17.74% (55). The adolescent pregnancy is a public health problem and a challenge to Psychology, that could contribute proposing health prevention actions to this population. The present study intended to investigate the psychological functioning of 10 pregnant adolescents through analysis of Human Figure Drawing (HFD) and Thematic Apperception Test (TAT). It was observed a high level of anxiety experienced by the pregnant adolescents verified in tracing, in shading and in the poverty of details, that can lead to the development of problems related to pregnancy and the relationship between the family, the adolescent and the baby. As for the TAT, the stories, in general constructed by the teenagers, were poorly structured with reduced content and a magical happy ending. It was concluded that the HFD and the TAT are discriminating and adequate to evaluate these adolescents’ psychodynamics and allow to indicate supporting preventive actions in this area.

The introduction of more efficient diagnostic methods, new techniques in surgery and transplantation, antibiotics and chemotherapeutics more potent and novel materials for prostheses, catheters and probes significantly increased the life expectancy and quality of life of critically ill patients, on the other hand, hospital-acquired infections emerged as important iatrogenic complications. Invasive infections are a growing problem in public health hospitals in Brazil and worldwide. Among the various etiological agents found in the hospital environment, the genus Candida has been the third most frequently isolated pathogen. In general, invasive fungal infections are associated with high morbidity and mortality, difficulties in diagnosis, antimicrobial resistance, length of hospital stay and increased hospital costs. OBJECTIVES: To describe noninvasive positive-pressure ventilation use in intensive care unit clinical practice, factors associated with NPPV failure and the associated prognosis. METHODS: A prospective cohort study. RESULTS: Medical disorders (59%) and elective surgery (21%) were the main causes for admission to the intensive care unit. The main indications for the initiation of noninvasive positive-pressure ventilation were the following: post-extubation, acute respiratory failure and use as an adjunctive technique to chest physiotherapy. The noninvasive positive-pressure ventilation failure group was older and had a higher Simplified Acute Physiology Score II score. The noninvasive positive-pressure ventilation failure rate was 35%. The main reasons for intubation were acute respiratory failure (55%) and a decreased level of consciousness (20%). The noninvasive positive-pressure ventilation failure group presented a shorter period of noninvasive positive-pressure ventilation use than the successful group [three (2-5) versus four (3-7) days]; they had lower levels of pH, HCO3 and base excess, and the FiO(2) level was higher. These patients also presented lower PaO2:FiO2 ratios; on the last day of support, the inspiratory positive airway pressure and expiratory positive airway pressure were higher. The failure group also had a longer average duration of stay in the intensive care unit [17 (10-26) days vs. 8 (5-14) days], as well as a higher mortality rate (9 vs. 51%). There was an association between failure and mortality, which had an odds ratio (95% CI) of 10.6 (5.93 - 19.07). The multiple logistic regression analysis using noninvasive positive pressure ventilation failure as a dependent variable found that treatment tended to fail in patients with a Simplified Acute Physiology Score II >= 34, an inspiratory positive airway pressure level >= 15 cmH2O and pH<7.40. CONCLUSION: The indications for noninvasive positive-pressure ventilation were quite varied. The failure group had a longer intensive care unit stay and higher mortality. Simplified Acute Physiology Score II >= 34, pH<7.40 and higher inspiratory positive airway pressure levels were associated with failure.

OBJECTIVES: This prospective, randomized, experimental study with rats aimed to investigate the influence of general treatment strategies on the motor recovery of Wistar rats with moderate contusive spinal cord injury. METHODS: A total of 51 Wistar rats were randomized into five groups: control, maze, ramp, runway, and sham (laminectomy only). The rats underwent spinal cord injury at the T9-T10 levels using the NYU-Impactor. Each group was trained for 12 minutes twice a week for two weeks before and five weeks after the spinal cord injury, except for the control group. Functional motor recovery was assessed with the Basso, Beattie, and Bresnahan Scale on the first postoperative day and then once a week for five weeks. The animals were euthanized, and the spinal cords were collected for histological analysis. RESULTS: Ramp and maze groups showed an earlier and greater functional improvement effect than the control and runway groups. However, over time, unexpectedly, all of the groups showed similar effects as the control group, with spontaneous recovery. There were no histological differences in the injured area between the trained and control groups. CONCLUSION: Short-term benefits can be associated with a specific training regime; however, the same training was ineffective at maintaining superior long-term recovery. These results might support new considerations before hospital discharge of patients with spinal cord injuries.

Objective: to determine the ability of the reduced form of a screening instrument, the Patient Health Questionnaire-2 (PHQ-2), to assess the presence of depressive disorders in patients admitted to a general hospital. Method: A sample of 227 patients admitted to the clinical wards of a Brazilian general university hospital were assessed with Module A of the Diagnostic Structured Interview for the DSM-IV (SCID-IV) and filled out the PHQ-9 and PHQ-2. Results: The PHQ-2 demonstrated an area under the ROC curve of 0.89 (p < 0.0001), with a cutoff point of three or more being the one that best equilibrated the sensitivity (0.86) and specificity (0.75) values. The agreement index between the PHQ-2 and module A of SCID-W was 78.4% and the Kappa value was 0.51. Regarding reliability, the Cronbach alpha value obtained was 0.64 and the intraclass correlation coefficient was 0.52. Conclusion: PHQ-2 proved to be an instrument with good psychometric properties comparable to those of PHQ-9, being superior to the latter regarding the rate of false-positive results. In addition, it is a brief instrument that elicits little resistance on the part of the patient, being inexpensive and requiring little time, thus being of important help to the treatment teams for the detection of depressive disorder, being suitable for incorporation into hospital admission protocols and thus possibly favoring more immediate interventions. This exploratory-descriptive quantitative study aimed to evaluate the protocol for identifying newborns admitted to the Neonatal Intensive and Semi-intensive Therapy Unit of a private hospital. The case series was made up of 540 observation opportunities, selected by simple random probability sampling. The data was collected between May and August 2010 according to a form and analyzed by descriptive statistic. The protocol's general performance had a conformity index of 82.2%. There were three stages to the protocol: identification components, the identification wristbands' condition and the number of identification wristbands. The highest percentage of conformity (93%) was attributed to the second stage and the lowest (89.3%) to the third, presenting a statistically significant difference of p=0.046. In the group of 'special' neonates, 88.5% conformity was achieved. These results will make it possible to restructure the protocol for identifying newborns and to establish care and managerial goals so as to improve the quality of care and the patients' safety.
