Os produtos agrícolas originários dos travessões e das margens ao longo da rodovia Transamazônica são expostos e vendidos, em Altamira, na Feira Livre Municipal, Feira do Produtor e Feira da Brasília. As duas primeiras são contíguas e dificilmente distinguíveis uma da outra, localizadas no centro comercial da cidade e conhecidas na linguagem corrente como Mercado Municipal. Têm o seu ponto alto no sábado, com fraca movimentação durante a semana. A terceira, localizada no bairro Brasília, espalhase por uma das ruas principais de Altamira, a Abel Figueiredo, e oferece produtos variados ao público, nas manhãs de domingo. Mais do que pontos de venda, são espaços de vida social e de expressão de gestão dos recursos naturais, seja pelo extrativismo, seja por processos produtivos instalados em contextos definidos. Neste artigo descrevemos suas características estruturais, exercício que pressupõe a análise das relações sociais e econômicas que ali se desenvolvem.

As a result of the 20th-century industrial development, and particularly after World War II, cars have become an important consumer good, and this has led to a large number of circulating vehicles. To this day, the number of cars has increased exponentially worldwide, which results in a high number of traffic accidents (TAs) every day. TAs are responsible for 1.2 million deaths worldwide each year, and in Brazil, they are considered to be a public health problem, considering that its traffic is regarded as one of the worst and most dangerous in the world. In addition to temporary or permanent physical disabilities that bring great harm to individuals, accidents also cause psychological disorders, such as the post-traumatic stress disorder. Hence, considering the importance of TAs in the country and their outcomes, this study aimed at investigating the epidemiological aspects of car accidents occurring in 2010 who were assisted in the emergency room of a university hospital in São Paulo state. By means of records made by the nursing team and medical records analysis, it was possible to extract a stratified sample of 88 cases involving adults and eight cases involving children. The sample was analyzed by the SAS software for Windows, version 9.2. As regards the accidents involving adults, there was a predominance of males (67%), with an approximate 2:1 ratio in relation to females. The victims’ most frequent age range was 20-29 years (39%), and they were mostly single (56%). The month showing the largest number of occurrences was July, with 17%, and the weekday was Sunday, with 25%. Showing 32%, the time period when most accidents occurred was from 06:00 o’clock p.m. to 11:59 p.m. As regards vehicles types involved, motorcycles were in first place, with 47% of cases, and the most common accident type was crashing between vehicles, with 43%... Objective: to describe the profile of non-fatal cases related to interpersonal violence treated in an emergency care unit of reference that serves seven municipalities of the state of São Paulo, Brazil, from 2008 to 2010. Methods: the study data came from the cases reported from the Epidemiological Vigilance in Penápolis-SP to the Brazilian Information System for Notifiable Diseases; variables were shown according to the Notification/Investigation Individual Formulary of Domestic, Sexual, and/or other Types of Violences. Results: 109 occurrences were studied; most of the victims were young and female (93.6%); and the aggressors, mostly were men (57.8%), partners or relatives/acquaintances of victims. Physical violence was the main form of aggression (93.6%), principally in the home (67.9%), on Sunday (16.5%), between 6:01pm and 12:00pm (57.8%). Conclusion: the cases reported had a consistent profile of domestic family violence against women, different from other studies about interpersonal violence in large cities and metropolitan regions

This report examines the religious beliefs and practices of American Protestant teenagers using new, nationally representative survey data from the National Study of Youth and Religion (NSYR). The NSYR is a major study of the religious and spiritual lives of contemporary American teens, which recently produced a book on its major findings entitled, Soul Searching: the Religious and Spiritual Lives of American Teenagers (by Christian Smith with Melinda Lundquist Denton, Oxford University Press, 2005). In addition to broadly describing the religious outlook of Protestant teenagers today — and as a more detailed, descriptive follow-up to that book — this report highlights several positive and negative experiences and evaluations of teenagers in different Protestant denominations and groupings of denominations. In brief, this report presents the following findings in these areas of interest: ♦ Religious Participation: Protestant teenagers are relatively active in religious organizations and activities, both within and beyond their churches. About one-half of all Protestant teens attend church weekly, participate in Sunday school or in a religious youth group, pray and attend a religious summer camp or retreat, though less than one-third read the Bible each week. This also means, however, that substantial numbers of Protestant teens are not actively participating in their religious traditions. Teens from conservative denominations such as Southern Baptist Convention and Assemblies of God are especially likely to regularly attend church and participate in other religious activities. ♦ Theological Beliefs: Protestant teenagers are likely to hold many traditional Christian religious beliefs. The majority of Protestant teens say they believe in God, the afterlife, angels, demons, miracles, judgment day and they view God as a personal being involved in the lives of people today. Sizable numbers of Protestant teens, on the other hand, do not hold these traditional Christian religious beliefs. Teens from conservative and black Protestant denominations are more likely than mainline Protestant teens to hold these religious beliefs. ♦ Importance of Faith: The majority of Protestant teenagers report that their religious faith is very important in their lives. Most of them also say that their families talk about religion together, that they have shared their faith with someone not of their faith and that they have had a powerful worship experience. A large minority of all Protestant teenagers, and in the case of some denominations a majority of teenagers, do not report that religious faith is very important in their lives. Teens from conservative and black Protestant denominations are particularly likely to report that faith is important in their lives. ♦ Evaluations of Churches: The majority of Protestant teenagers express relatively positive views of their churches and fellow church members. They typically report that they would continue to attend church if it were totally up to them, that they would attend a similar church if given the choice and that their current church is generally warm and welcoming. Protestant adolescents, however, do have some reservations about and problems with their churches and fellow church attendees, as spelled out in the following pages, particularly with other teenage attendees.

Compliance lebertransplantierter Patienten mit der immunsuppressiven Therapie ist unerlässlich für den lang-fristigen Erfolg der Lebertransplantation. Aus Non-Compliance mit der immunsuppressiven Therapie können Abstoßungsreaktionen, Organverlust oder sogar Tod resultieren. Hauptziel der vorliegenden Studie war die erstmalige Evaluation der Compliance bei Einnahme von Prograf® (zweimal tägliche Einnahme von Tacrolimus) im Vergleich zur Einnahme von Advagraf® (einmal tägliche Einnahme von Tacrolimus). Von Interesse war außerdem die Fragestellung, ob sich die Compliance bezüglich der immunsuppressiven Therapie mit dem Zeitabstand zur Transplantation verändert. rnDie Compliancemessung wurde offen mittels MEMS® (Aardex Ltd., Schweiz) durchgeführt, der Patient war also über die Compliancekontrolle informiert. Mittels MEMS® konnten Datum und Uhrzeit der Dosisentnahme dokumentiert und damit zuverlässig das gesamte Compliancemuster über im Durchschnitt 176 Tage mit der zweimal täglichen Einnahme und 188 Tage mit der einmal täglichen Einnahme pro Patient erfasst werden. 65 Patienten mit dem Basisimmunsuppressivum Prograf® wurden in die prospektive, nicht-interventionelle Studie eingeschlossen und nach Per Protokoll-Analyse konnten die Daten von 63 in Mainz lebertransplantierten Patienten ausgewertet werden (Prograf®: Gruppe 1: 15 Patienten (Pat.), Gruppe 2: 23 Pat., Gruppe 3: 22 Pat., Drop-outs: 3 Pat.; Advagraf®: Gruppe 1: 16 Pat., Gruppe 2: 23 Pat., Gruppe 3: 23 Pat., Drop-outs: 1 Pat.). Die Dosing Compliance (DC), definiert als Prozent der Tage, an denen der MEMS®-Behälter korrekt geöffnet und die Dosis höchstwahrscheinlich korrekt eingenommen wurde, war der primäre Zielparameter. Weitere Methoden der Compliancemessung, wie der Pill Count, mehrere Fragebögen (Selbsteinschätzung, Patientenwissen-, Morisky-, MESI-, HADS-, SF-36- und Patientenzufriedenheit-Fragebogen) sowie die Blutspiegelmessung wurden eingesetzt, um die Compliance der Patienten umfassend charakterisieren zu können. rnDer Median der DC mit der zweimal täglichen Einnahme betrug 97% bei Pat. > 6 m.p.t. < 2 y.p.t., 97% bei Pat. > 2 y.p.t. < 5 y.p.t. und 98% bei Pat. > 5 y.p.t. (p=0,931; Kruskal-Wallis-Test). Der Median der DC von Tacroli-mus bei einmal täglicher Einnahme (Advagraf®) betrug 99% bei Pat. > 6 m.p.t. < 2 y.p.t., 98% bei Pat. > 2 y.p.t. < 5 y.p.t. und 97% bei Pat. > 5 y.p.t. (p=0,158; Kruskal-Wallis-Test). Insgesamt zeigten die Patienten während des gesamten Beobachtungszeitraums von 12 Monaten eine gute Compliance für die Einnahme ihres Immun-suppressivums. Die Timing Compliance (TiC)-raten lagen auf einem niedrigeren Niveau als die Dosing- und Taking Compliance (TC)-raten. Die Complianceraten der drei Subgruppen unterschieden sich nicht signifikant. Die Patienten mit dem geringsten Abstand zur Transplantation zeigten bei beinahe allen Messmethoden die höchste Compliance im Gegensatz zur etwas geringeren Compliance der Patienten mit größerem Abstand zur Transplantation. Die während der Advagraf®-Phase mittels MEMS® gemessenen DC-, TC- und TiC-raten fielen höher aus als bei Einnahme von Prograf® (p(DC)=0,003; p(TC)=0,077; p(TiC)=0,003; Wilcoxon Vorzeichen-Rang-Test). Dieses Ergebnis untermauert die in anderen Indikationen gefundene Complianceverbesserung durch die einmal tägliche Arzneimittelgabe im Vergleich zur zweimal täglichen Gabe. Die Auswertung der Drug Holidays ergab für die Advagraf®-Phase hingegen niedrigere Complianceraten als für die Prograf®-Phase. Dieses Ergebnis ist auf die Definition des Drug Holidays (keine Arzneimitteleinnahme über 48 h) zurück zu führen. Die Chance Advagraf® einmal pro Tag zu vergessen ist doppelt so hoch, als Prograf® dreimal aufeinander fol-gend zu vergessen. Mit einer verhältnismäßigeren Definition von Drug Holidays (Einnahmepause von 72 Stun-den bei einmal täglicher Einnahme von Advagraf® entsprechend drei ausgelassenen Dosen von Prograf®) ist die Compliancerate 81%. Die Ergebnisse des Pill Counts waren sowohl bei Einnahme von Prograf® als auch von Advagraf® mit der jeweils gemessenen TC vergleichbar, was die Zuverlässigkeit der Messergebnisse bes-tätigt. rnDie zusätzlich eingesetzten Methoden verifizierten das Ergebnis der höheren Compliance mit der einmal tägli-chen Einnahme. Die während der Advagraf®-Phase beantworteten Fragebögen zeigten einen Trend zu besserer Compliance und Lebensqualität. Lediglich die Ergebnisse des MESI-Fragebogens und der Blutspiegelmessungen wichen sowohl während der Prograf®- als auch während der Advagraf®-Phase stark von den Ergebnis-sen der anderen Methoden ab. rnUnter Einbeziehung aller mittels MEMS® und Pill Count objektiv gemessenen Complianceparameter konnten während der Prograf®-Einnahme 54 von 60 Pat. (90%) und während der Advagraf®-Phase 59 von 62 Pat. (95%) als compliant eingestuft werden. Aufgrund subjektiver Compliancemessungen waren 49 von 58 Pat. (84%) während der Prograf®- und 54 von 59 Pat. (92%) während der Advagraf®-Phase als compliant einzustufen. Es wurde beobachtet, dass die zeitlich korrekte Einnahme der Morgendosis einfacher und bei Einmalgabe zu bevorzugen ist. Die wochentagsbezogene Auswertung ergab erwartungsgemäß, dass am Wochenende (Samstag und Sonntag) am häufigsten Dosen ausgelassen wurden. rnDie Umstellung von Prograf® auf Advagraf® stellte kein Problem dar. Beinahe alle Patienten waren dankbar und zufrieden mit der Reduzierung der Dosierungsfrequenz und der größeren Unabhängigkeit durch die entfallene abendliche Einnahme. Der positive Einfluss der geringeren Dosierungshäufigkeit auf die Langzeitcompliance der Patienten, ist ein hinreichender Grund die Entwicklung von Formulierungen zur einmal täglichen Ein-nahme für weitere Immunsuppressiva zu fordern. Insbesondere bei den häufig eingesetzten Kombinationstherapien von Immunsuppressiva würde der Effekt der Complianceverbesserung noch verstärkt werden, wenn alle eingesetzten Immunsuppressiva zur einmal täglichen Gabe geeignet wären.

Drosophila mutants have played an important role in elucidating the physiologic function of genes. Large-scale projects have succeeded in producing mutations in a large proportion of Drosophila genes. Many mutant fly lines have also been produced through the efforts of individual laboratories over the past century. In an effort to make some of these mutants more useful to the research community, we systematically mapped a large number of mutations affecting genes in the proximal half of chromosome arm 2L to more precisely defined regions, defined by deficiency intervals, and, when possible, by individual complementation groups. To further analyze regions 36 and 39-40, we produced 11 new deficiencies with gamma irradiation, and we constructed 6 new deficiencies in region 30-33, using the DrosDel system. trans-heterozygous combinations of deficiencies revealed 5 additional functions, essential for viability or fertility.

Social stressors at work may result in long-term health impairments if recovery is insufficient. In the present psychophysiological field study, we tested whether the inability to psychologically detach from work issues mediates the negative effect of social stressors at work on sleep during weekends. Sixty full-time employees participated in the study. Daily assessment included diaries on psychological detachment and continuous ambulatory actigraphy to assess psychophysiological indicators of sleep. Hierarchical regression analyses revealed that enduring social stressors at work were negatively related with psychological detachment on Sunday evening and negatively related with various sleep indicators on Sunday night. Furthermore, psychological detachment from work on Sunday evening partially mediated the effect of social stressors at work on two sleep indicators. Social stressors at work may threaten recovery processes just before the working week starts.

BACKGROUND Conflicting data exist of an association between off-hour (weekend, holiday, or night-time) hospital admission and adverse outcome in intracerebral hemorrhage (ICH). We determined the association between off-hour admissions and poor clinical outcome, and of any differential effect of early intensive blood pressure (BP) lowering treatment between off- and on-hour admissions, among participants of the Intensive BP Reduction in Acute Cerebral Hemorrhage Trial (INTERACT2). METHODS Subsidiary analysis of INTERACT2, a multinational, multicenter, clinical trial of patients with spontaneous ICH with elevated systolic BP, randomly assigned to intensive (target systolic BP <140 mm Hg) or guideline-based (<180 mm Hg) BP management. Primary outcome was death or major disability (modified Rankin scale of 3-6) at 90 days. Off-hour admission was defined as night-time (4:30 p.m. to 8:30 a.m.) on weekdays, weekends (Saturday and Sunday), and public holidays in each participating country. RESULTS Of 2,794 patients with information on the primary outcome, 1,770 (63%) were admitted to study centers during off-hours. Off-hour admission was not associated with risk of poor outcome at 90 days (53% off-hour vs. 55% on-hour; p = 0.49), even after adjustment for comorbid risk factors (odds ratio 0.92; 95% CI 0.76-1.12). Consistency exists in the effects of intensive BP lowering between off- and on-hour admission (p = 0.85 for homogeneity). CONCLUSIONS Off-hour admission was not associated with increased risks of death or major disability among trial protocol participants with acute ICH. Intensive BP lowering can provide similar treatment effect irrespective of admission hours.
